| 1 | Management and therapy of vasovagal syncope: A review显示文摘Vasovagal syncope is a common cause of recurrent syncope. Clinically, these episodes may present as an isolated event with an identifiable trigger, or manifest as a cluster of recurrent episodes warranting intensive evaluation. The mechanism of vasovagal syncope is incompletely understood. Diagnostic tools such as implantable loop recorders may facilitate the identification of patients with arrhythmia mimicking benign vasovagal syncope. This review focuses on the management of vasovagal syncope and discusses the non-pharmacological and pharmacological treatment options, especially the use of midodrine and selective serotonin reuptake inhibitors. | 2 | Comparison of non-cycloplegic photorefraction,cycloplegic photorefraction and cycloplegic retinoscopy in children显示文摘AIM: To compare the results of noncycloplegic photorefraction, cycloplegic photorefraction and cycloplegic refraction in preschool and non-verbal children.METHODS: One hundred and ninety-six eyes of 98children(50 females, 48 males) were included in the study. Firstly, non-cycloplegic photorefraction was achieved with Plusoptix A09; secondly, cycloplegic photorefraction was carried out with Plusoptix A09 after10 min cyclopentolate. Finally, 30 min after instillation of twice cyclopentolate, cycloplegic refraction was obtained with autorefraction and/or standard retinoscopy. Spheric equivalent, spheric power, cylindric power and cylindrical axis measurements were statistically compared.RESULTS: The mean age was 28.8±18.5mo(range12-72mo). The differences in spherical equivalent, spheric power and cylindrical power measured by the three methods were found statistically significant(P <0.05).The spherical equivalent and spheric power measured by cycloplegic photorefraction were statistically higher than the measurements of the other methods(P <0.05). The cylindrical power measured by cycloplegic refraction was statistically lower than the measurements of the photorefraction methods(P <0.05). There was no significant difference in cylindrical axis measurements between three methods(P >0.05).CONCLUSION: For the determination of refractive errors in children, the Plusoptix A09 measurements give incorrect results after instillation of cyclopentolate.Additionally, the cylindrical power measured by Plusoptix A09 with or without cycloplegia is higher. | 4 | Evolution of Microstructure, Residual Stress, and Tensile Properties of Mg–Zn–Y–La–Zr Magnesium Alloy Processed by Extrusion显示文摘The microstructure, texture, residual stress, and tensile properties of Mg–6 Zn–2 Y–1 La–0.5 Zr(wt%) magnesium alloy were investigated before and after extrusion process, which performed at 300 °C and 400 °C. The microstructural characterizations indicated that the as-cast alloy was comprised of α-Mg, Mg–Zn, Mg–Zn–La, and Mg–Zn–Y phases. During homogenization at 400 °C for 24 h, most of the secondary phases exhibited partial dissolution. Extrusion process led to a remarkable grain refi nement due to dynamic recrystallization(DRX). The degree of DRX and the DRXed grain size increased with increasing extrusion temperature. The homogenized alloy did not show a preferential crystallographic orientation, whereas the extruded alloys showed strong basal texture. The extrusion process led to a signifi cant improvement on the compressive residual stress and mechanical properties. | 5 | Open reduction of a total talar dislocation: A case report and review of the literature显示文摘BACKGROUND Total talar dislocation (TTD) is very uncommon for many orthopedic surgeons and emergency/trauma specialists. Scarce cases of TTD have been reported, mainly in the form of open fracture-dislocation injury. CASE SUMMARY We report a very rare injury of closed TTD with a follow-up period of 36 mo. Initial closed reduction was not successful because of a fractured highly unstable medial malleolus displaced into the ankle mortise, blocking the relocation of the talus. The patient was able to walk pain-free after the 3rd month of surgery. At the 36-mo follow-up, there were 10 degrees of flexion loss and 10 degrees of extension loss in the tibiotalar joint. Furthermore, 5 degrees of subtalar joint inversion-eversion loss was present.
