| 1 | Helicobacter pylori-related chronic gastritis as a risk factor for colonic neoplasms显示文摘To summarize the current views and insights on associations between Helicobacter pylori(H. pylori)-related chronic gastritis and colorectal neoplasm, we reviewed recent studies to clarify whether H. pylori infection/H. pylori-related chronic gastritis is associated with an elevated risk of colorectal neoplasm. Recent studies based on large databases with careful control for confounding variables have clearly demonstrated an increased risk of colorectal neoplasm associated with H. pylori infection. The correlation between H. pylori-related chronic atrophic gastritis(CAG) and colorectal neoplasm has only been examined in a limited number of studies. A recent large study using a national histopathological database, and our study based on the stage of H. pylori-related chronic gastritis as determined by serum levels of H. pylori antibody titer and pepsinogen, indicatedthat H. pylori-related CAG confers an increased risk of colorectal neoplasm, and more extensive atrophic gastritis will probably be associated with even higher risk of neoplasm. In addition, our study suggested that the activity of H. pylori-related chronic gastritis is correlated with colorectal neoplasm risk. | 2 | 流血和与内视镜的 submucosal 解剖联系的人工的胃的溃疡的管理显示文摘 Endoscopic submucosal dissection (ESD), an endoscopic procedure for the treatment of gastric epithelial neoplasia without lymph node metastases, spread rapidly, primarily in Japan, starting in the late 1990s. ESD enables en bloc resection of lesions that are difficult to resect using conventional endoscopic mucosal resection (EMR). However, in comparison to EMR, ESD requires a high level of endoscopic competence and a longer resection time. Thus, ESD is associated with a higher risk of adverse events, including intraoperative and postoperative bleeding and gastrointestinal perforation. In particular, because of a higher incidence of intraoperative bleeding with mucosal incision and submucosal dissection, which are distinctive endoscopic procedures in ESD, a strategy for endoscopic hemostasis, mainly by thermo-coagulation hemostasis using hemostatic forceps, is important. | 3 | Radiofrequency ablation as treatment for pulmonary metastasis of colorectal cancer显示文摘Radiofrequency ablation(RFA)causes focal coagulation necrosis in tissue.Its first clinical application was reported in 2000,and RFA has since been commonly used in both primary and metastatic lung cancer.The procedure is typically performed using computed tomography guidance,and the techniques for introducing the electrode to the tumor are simple and resemble those used in percutaneous lung biopsy.The most common complication is pneumothorax,which occurs in up to 50%of procedures;chest tube placement for pneumothorax is required in up to 25%of procedures.Other severe complications,such as pleural effusion requiring chest tube placement,infection,and nerve injury,are rare.The local efficacy depends on tumor size,and local progression after RFA is not rare,occurring in 10%or more of patients.The local progression rate is particularly high for tumors>3 cm.Repeat RFA may be used to treat local progression.Short-to mid-term survival after RFA appears promising and is approximately 85%-95%at 1 year and 45%-55% at 3 years.Long-term survival data are sparse.Better survival may be expected for patients with small metastasis,low carcinoembryonic antigen levels,and/or no extrapulmonary metastasis.The notable advantages of RFA are that it is simple and minimally invasive;preserves pulmonary function;can be repeated;and is applicable regardless of previous treatments.Its most substantial limitation is limited local efficacy.Although surgery is still the method of choice for treatment with curative intent,the ultimate application of RFA may be to replace metastasectomy for small metastases.Randomized trials comparing RFA with surgery are needed. | Takao Hiraki Hideo Gobara Toshihiro Iguchi Hiroyasu Fujiwara Yusuke Matsui Susumu Kanazawa | 2014 | World Journal of Gastroenterology2014，20，4： | 12 |

| 5 | Recent aspects for disseminated carcinomatosis of the bone marrow associated with gastric cancer: What has been done for the past, and what will be needed in future?显示文摘Disseminated carcinomatosis of the bone marrow is characterized by widespread bone metastasis(bone marrow infiltration) from solid tumors with hematological disorders coexisted. This disease is frequently complicated with gastric cancer among solid tumors although its incidence is very rare. In recent years,technological innovations in diagnosis and treatment for cancer have remarkably improved,which made survival rates of various cancers prolonged. Prognosis of disseminated carcinomatosis of the bone marrow associated with gastric cancer,however,is still poor(less than a year),possibly because this disease has not been given attention due to low incidence. | 14 | A case of chronic pancreatitis in which endoscopic ultrasonography was effective in the diagnosis of a pseudoaneurysm显示文摘Endoscopic ultrasonography (EUS) was performed on a patient being treated for chronic pancreatitis because a submucosal tumor was observed in the stomach during gastrointestinal endoscopy. As internal pulsa- tile blood flow on Doppler was present, the diagnosis of an aneurysm was made. The pseudoaneurysm of the left gastric artery was embolized with histoacryl and lipiodol and the splenic artery was embolized with coils at the location of the pseudoaneurysm to prevent hemorrhage. Follow up EUS confirmed the cessation of blood flow from the pseudoaneurysm. | 15 | Intracranial malignant solitary fibrous tumor metastasized to the chest wall:A case report and review of literature显示文摘BACKGROUND Solitary fibrous tumor(SFT)is a rare fibroblastic mesenchymal neoplasm that affects spindle cell soft tissues with broad-spectrum biological behavior;it is predominantly benign,and rarely metastasizes.SFT occurs mainly in the tissue structure of the serosa in the pleura and the thorax,and can be found throughout the body,though extra-thoracic localization,including the cephalic region,is uncommon.We reported the first case of intracranial malignant SFT metastasized to the chest wall.CASE SUMMARY An 81-year-old Japanese man was referred to our hospital due to progressive gait disturbance and appetite loss.His medical history included partial resection due to brain tumor,four times,and 50-Gray radiation therapy at another hospital,starting when he was 74 years old.An unenhanced head computed tomography(CT)scan revealed an 8 cm×5.1 cm×6.5 cm mixed-density mass at the left frontal lobe,accompanying a midline shift,and an unenhanced chest-abdomen CT scan revealed a 6 cm×4.1 cm×6.5 cm low-density mass in the left chest wall.A CT-guided percutaneous lung biopsy was performed,and the pathological findings were SFT corresponding to brain tumor.Finally,the correct diagnosis of his brain tumor in history of past illness revealed to be SFT,and the unremovable tumor,namely present brain lesions enlarged and metastasized to the chest wall.We established a definitive diagnosis of intracranial malignant SFT metastasized to the chest wall.We notified him and his family of the disease,and offered palliative care.He passed away on the 29 th hospital day.CONCLUSION This case suggests the need for careful,detailed examination,and careful followup when encountering patients presenting with a mass. | Daisuke Usuda Shinya Yamada Toshihide Izumida Ryusho Sangen Toshihiro Higashikawa Ken Nakagawa Masaharu Iguchi Yuji Kasamaki | 2020 | World Journal of Clinical Cases2020，8，20： | 2 |
