| 1 | STAT3 deficiency prevents hepatocarcinogenesis and promotes biliary proliferation in thioacetamide-induced liver injury显示文摘AIM To elucidate the role of STAT3 in hepatocarcinogenesis and biliary ductular proliferation following chronic liver injury. METHODS We investigated thioacetamide(TAA)-induced liver injury, compensatory hepatocyte proliferation, and hepatocellular carcinoma(HCC) development in hepatic STAT3-deficient mice. In addition, we evaluated TAAinduced biliary ductular proliferation and analyzed the activation of sex determining region Y-box9(SOX9) and Yes-associated protein(YAP), which regulate the transdifferentiation of hepatocytes to cholangiocytes.RESULTS Both compensatory hepatocyte proliferation and HCC formation were significantly decreased in hepatic STAT3-deficient mice as compared with control mice. STAT3 deficiency resulted in augmentation of hepatic necrosis and fibrosis. On the other hand, biliary ductular proliferation increased in hepatic STAT3-deficient livers as compared with control livers. | 3 | Association of anastomotic leakage with long-term oncologic outcomes of patients with esophagogastric junction cancer显示文摘BACKGROUND Despite improvements in surgical procedures and peri-operative patients management,the postoperative complications in esophagogastric junction(EGJ)cancer remain high because of technical aspects.Several studies have indicated the negative influence of postoperative infectious complications on long-term survival after gastrointestinal surgery.However,no study has shown the association between postoperative complications and long-term survival of patients with EGJ cancer.AIM To elucidate influence of postoperative complications on the long-term outcomes of patients with EGJ cancer.METHODS A total of 122 patients who underwent surgery for EGJ cancer at the Keio University were included in this study.We examined the association between complications and long-term oncologic outcomes.RESULTS In all patients,the 3-year overall survival(OS)rate was 71.9%,and the recurrencefree survival(RFS)rate was 67.5%.Compared with patients without anastomotic leakage,those with anastomotic leakage had poor median OS(8 mo vs not reached,P=0.028)and median RFS(5 mo vs not reached,P=0.055).Among patients with cervical anastomosis,there were not significant differences between patients with and without anastomotic leakage.However,among patients who underwent intrathoracic anastomosis,patients with anastomotic leakage had significantly worse OS(P=0.002)and RFS(P=0.005).CONCLUSION Anastomotic leakage was significantly associated with long-term oncologic outcomes of patients with EGJ cancer,especially those who underwent intrathoracic anastomosis.Cervical anastomosis with subtotal esophagectomy may be an option for the patients who are at high risk for anastomotic leakage. | Masashi Takeuchi Hirofumi Kawakubo Satoru Matsuda Shuhei Mayanagi Tomoyuki Irino Jun Okui Kazumasa Fukuda Rieko Nakamura Norihito Wada Hiroya Takeuchi Yuko Kitagawa | 2022 | World Journal of Gastrointestinal Surgery2022,14,1: | 1 |

| 17 | Mesenchymal stem cells-derived extracellular vesicles as‘natural’drug delivery system for tissue regeneration显示文摘Mesenchymal stem cells(MSCs)have abilities to mediate tissue protection through mechanisms of antiapoptosis,anti-oxidative stress and anti-fibrosis as well as tissue regeneration through mechanisms of cell proliferation,differentiation and angiogenesis.These effects by MSCs are mediated by a variety of factors,including growth factors,cytokines and extracellular vesicles(EVs).Among these factors,EVs,containing proteins,mRNA and microRNAs(miRNA),may carry their contents into distant tissues with high stability.Therefore,the treatment with MSC-derived EVs may be promising as‘natural’drug delivery systems(DDS).Especially,the treatment of MSCderived EVs with the manipulation of specific miRNAs expression has been reported to be beneficial under a variety of diseases and tissue injuries.The overexpression of specific miRNAs in the EVs might be through pre-loading method using the gene editing system by plasmid vector or post-loading method to load miRNA mimics into EVs by electroporation or calcium chloride-mediated transfection.Despite current several challenges for clinical use,it should open the next era of regenerative medicine for a variety of diseases.In this article,we highlight the therapeutic potential of MSC-derived EVs as‘natural’DDS and current challenges. | KENJI TSUJI SHINJI KITAMURA JUN WADA | 2022 | BIOCELL2022,46,4: | 0 |
