1CA19-9 level determines therapeutic modality in pancreatic cancer patients with para-aortic lymph node metastasis显示文摘Background: In general, para-aortic lymph node(LN16) metastasis has been considered as a contraindication for pancreatic resection. However, some pancreatic cancer patients with LN16 metastasis have been reported to survive for longer than expected after pancreatectomy. The purpose of this study was to determine whether pancreatic cancer patients with LN16 metastasis might benefit from surgery.Methods: We retrospectively reviewed 201 consecutive patients with invasive pancreatic ductal adenocarcinoma who underwent surgery at Osaka National Hospital between April 2003 and December 2012.These patients included 22 patients with LN16 metastasis who underwent an extended lymphadenectomy and 25 patients who underwent a palliative surgical biliary and gastric bypass. The clinicopathological data and outcomes were evaluated using univariate and multivariate analyses.Results: The overall survival of the patients with LN16 metastasis was poorer than that of the LN16-negative patients(P = 0.0014). An overall survival analysis of the LN16-positive patients stratified according to the preoperative CA19-9 level showed a significant difference between patients with a low preoperative CA19-9 level(≤360 U/mL) and those with a high preoperative CA19-9 level(>360 U/mL)(P = 0.0301). No significant difference in overall survival of patients was observed between those with LN16 positivity and those who underwent bypass surgery. 3Human adipose derived stem cells are superior to human osteoblasts (HOB) in bone tissue engineering on a collagen-fibroin-ELR blend显示文摘The ultrastructure of the bone provides a unique mechanical strength against compressive, torsional andtensional stresses. An elastin-like recombinamer (ELR) with a nucleation sequence for hydroxyapatitewas incorporated into films prepared from a collagen-silk fibroin blend carrying microchannel patternsto stimulate anisotropic osteogenesis. SEM and fluorescence microscopy showed the alignment ofadipose-derived stem cells (ADSCs) and the human osteoblasts (HOBs) on the ridges and in the groovesof microchannel patterned collagen-fibroin-ELR blend films. The Young's modulus and the ultimatetensile strength (UTS) of untreated films were 0.58 ± 0.13 MPa and 0.18 ± 0.05 MPa, respectively. After 28days of cell culture, ADSC seeded film had a Young's modulus of 1.21 ± 0.42 MPa and UTS of0.32 ± 0.15 MPa which were about 3 fold higher than HOB seeded films. The difference in Young'smodulus was statistically significant (p: 0.02). ADSCs attached, proliferated and mineralized better thanthe HOBs. In the light of these results, ADSCs served as a better cell source than HOBs for bone tissueengineering of collagen-fibroin-ELR based constructs used in this study. 4Evaluation of mercury methylation in fresh and pre dried sediments using ^(203)Hg radioisotope显示文摘Mercury methylation rates in fresh and pre dried river sediments were determined in sediment water systems spiked with 203 HgCl 2 at a level of 0.75—0.95 ppm as Hg. At the end of the incubation period (21—38 days), methylmercury production in the fresh sediments was in the order of 3.0% to 13.75%, whereas in the pre dried sediments the methylation was much lower, at 0.6% to 3.9%. The highest methylation levels were generally recorded in the uppermost, 1 cm layer of the sediment column. Total mercury concentration in water in equilibrium with the 203 Hg spiked fresh sediments was in the range of 44.7 to 634 ng/L, whereas in equilibrium with the pre dried sediments the concentration was in the range of 19.5 to 34.5 ng/L. The proportion of methylmercury to total mercury in the water varied from 9.4% to 66.6% over the fresh sediments, and from 7.1% to 10.8% over the pre dried sediments. In the system consisting of water, sediment and sediment inverterbrates the concentrations of methylmercury were 8.61 to 15.69 ng/g in the sediment, 0.011 ng/ml in the water, and 9.22 to 40.69 ng/g in the inverterbrates. Methylmercury bioaccumualtion factors (BAFs) were in the order of 274 to 8087 relative to the water, and 0.25 to 7.31 relative to the sediment.
