| 1 | 持续性姿势-感知性头晕(PPPD)诊断标准:Bárány学会前庭疾病分类委员会共识显示文摘本文提出被纳入前庭疾病国际分类(International Classification of Vestibular Disorders,ICVD)的持续性姿势-感知性头晕(PPPD)的诊断标准。PPPD虽然是一个新的医学术语,但该病的存在由来已久。前庭疾病分类委员会深入汇总、分析了近30年来恐惧性姿势性眩晕(Phobic Postural Vertigo,PPV)、空间运动不适(Space℃Motion Discomfort,SMD)、视觉性眩晕(Visual Vertigo,VV)和慢性主观性头晕(Chronic Subjective Dizziness,CSD)相关的研究,在此基础上提出了PPPD的诊断标准。临床上,头晕、不稳、非旋转性眩晕为PPPD的3个核心症状。PPPD患者常表现为这3个核心症状中的一个或多个,症状持续时间超过3个月,并且在直立姿势、主动/被动运动、暴露于移动视觉刺激或复杂视觉环境中时症状出现加重。PPPD可能与影响平衡系统或引起眩晕、不稳及头晕的疾病所触发有关,包括周围或中枢前庭病变、其他内科疾病或心理疾病。临床上,PPPD既可单独存在,也可与其他临床疾病共病。PPPD可能的亚型分类尚需进一步识别和验证。迄今为止,PPPD确切的病理生理机制尚不明确。新近研究表明,PPPD可能与躯体姿势控制、多感觉信息处理、威胁评估系统与空间定向系统皮质整合的功能异常相关。因此,PPPD被归类为慢性功能性前庭疾病,并不是结构性前庭疾病或精神心理性前庭疾病。 | 李康之(译) 刘博(译) 顾平(审校) 杨旭(审校) Jeffrey PStaab Annegret Eckhardt-Henn Arata Horii Rolf Jacob Michael Strupp Thomas Brandt Adolfo Bronstein | 2020 | 神经损伤与功能重建2020，15，2： | 20 |

| 5 | Sudden cardiac death caused by myocarditis in persons aged 1-49 years:a nationwide study of 14 294 deaths in Denmark显示文摘Myocarditis is associated with an increased risk of sudden cardiac death (SCD) in the young.However,Information on nationwide burden of SCD caused by myocarditis (SCD-myocarditis) is sparse.For this study all deaths among persons in Denmark aged 1-35years in 2000-2009 and 36-49years in 2007-2009 (27.1 million person-years) were induded.Autopsy reports,death certificates,discharge summaries,and nationwide registries were used to identify all cases of SCD-myocarditis.In the 10-year study period,there were 14294 deaths,of which we identified 1 363 (10%) SCD.Among autopsied SCD (n=753,55%),cause of death was myocarditis in 42 (6%) cases corresponding to an SCD-myocarditis incidence of 0.16 (95%CI: 0.11-0.21) per 100000 person-years.Males had significantly higher incidence rates of SCD-myocarditis compared to females with an incidence rate ratio of 2.2 (95%CI:1.1-4.1).Myocarditis was not registered as cause of death In any of the non-autopsied SCD (n=610,45%).In conclusion,after nationwide unselected Inclusion of 14294 deaths,we found that 6% of all autopsied SCD was caused by myocarditis.No cases of SCD-myocarditis were reported in the non-autopsied SCD,which could reflect underdiagnosing of myocarditis in non-autopsied SCD.Furthermore,our data suggest a female protection towards SCDmyocarditis. | Thomas Hadberg Lynge Trine Skov Nielsen Bo Gregers Winkel Jacob Tfelt-Hansen Jytte Banner | 2019 | Forensic Sciences Research2019，4，3： | 3 |

| 9 | Differentiation of malignant and benign proximal bile duct strictures: The diagnostic dilemma显示文摘AIM: To identify the criteria for the differentiation of hilar cholangiocarcinoma (HCCA) from benign strictures. METHODS: A total of 68 patients underwent resection of lesions suspicious for HCCA between 1998 and 2006. The results of laboratory investigations, imaging studies and brush cytology were collected. These fi ndings were analyzed to obtain the fi nal diagnosis. RESULTS: Histological examination of the resected specimens confirmed HCCA in 58 patients (85%, group Ⅰ) whereas 10 patients (15%, groupⅡ) were diagnosed to have benign strictures. The most common presenting symptom was obstructive jaundice in 77% patients (79% group Ⅰvs 60% group Ⅱ P = 0.23). Laboratory findings showed greater elevation of transaminase levels in group Ⅰcompared to group Ⅱ The various imaging modalities showed vascular involvement exclusively in the malignant group (36%,P < 0.05). Brush cytology was positive for malignant cells in only 50% patients in group Ⅰ whereas none in group Ⅱ showed malignant cells. CONCLUSION: Despite improvements in imaging techniques, 10 patients (15%) with a presumptive diagnosis of HCCA were ultimately found to have benign strictures. Except for vascular involvement which was associated significantly with malignancy, there were no conclusive features of malignancy on regular imaging modalities.
