1Safe procedure in endoscopic submucosal dissection for colorectal tumors focused on preventing complications显示文摘Endoscopic submucosal dissection (ESD) is efficient for en bloc resection of large colorectal tumors. However, it has several technical difficulties, because the wall of the colon is thin and due to the winding nature of the colon. The main complications of ESD comprise postoperative perforation and hemorrhage, similar to endoscopic mucosal resection (EMR). In particular, the rate of perforation in ESD is higher than that in EMR. Perforation of the colon can cause fatal peritonitis. Endoscopic clipping is reported to be an efficient therapy for perforation. Most cases with perforation are treated conservatively without urgent surgical intervention. However, the rate of postoperative hemorrhage in ESD is similar to that in EMR. Endoscopic therapy including endoscopic clipping is performed and most of the cases are treated conservatively without blood transfusion. In blood examination, some degree of inflammation is detected after ESD. For the standardization of ESD, it is most important to decrease the rate of perforation. Adopting a safe strategy for ESD and a suitable choice of knife are both important waysof preventing perforation. Moreover, appropriate training and increasing experience can improve the endoscopic technique and can decrease the rate of perforation. 2Enhanced recovery program is safe and improves postoperative insulin resistance in gastrectomy显示文摘AIM: To assess the safety of enhanced recovery after surgery(ERAS) program in gastrectomy and influences on nutrition state and insulin-resistance. METHODS: Our ERAS program involved shortening the fasting periods and preoperative carbohydrate loading. Eighty gastrectomy patients were randomly assigned to either the conventional group(CG) or ERAS group(EG). We assessed the clinical characteristics and postoperative outcomes prospectively. The primary endpoint was noninferiority in timely discharge from the hospital within 12 d. Secondary endpoints were the incidence of aspiration at anesthesia induction, incidence of postoperative complications, health related quality of life(HRQOL) using the SF8 Health Survey questionnaire, nutrition state [e.g., albumin, transthyretin(TTR), retinal-binding protein(RBP), and transferrin(Tf)], the homeostasis model assessment-insulin resistance(HOMA-R) index, postoperative urine volume,postoperative weight change, and postoperative oral intake.RESULTS: The ERAS program was noninferior to the conventional program in achieving discharge from the hospital within 12 d(95.0% vs 92.5% respectively; 95%CI:-10.0%-16.0%). There was no significant difference in postoperative morbidity between the two groups. Adverse events such as vomiting and aspiration associated with the induction of general anesthesia were not observed. There were no significant differences with respect to postoperative urine volume, weight change, and oral intake between the two groups. EG patients with preoperative HOMA-R scores above 2.5 experienced significant attenuation of their HOMA-R scores on postoperative day 1 compared to CG patients(P = 0.014). 3Research on thermal shock resistance of mullite-bauxite-silicon carbide castable refractory显示文摘Effects of different silicon carbide contents on thermal shock resistance properties of mullite-bauxite castable refractory were analyzed respectively by taking the mullite,bauxite as main raw materials and taking calcium aluminate cement as a binding system.After 24 h curing in mould and another 24 h curing at 110℃ after demoulding,the samples were heated at 1000,1300 and 1500℃for 3 h in air,respectively.The bulk density,thermal expansion coefficient and thermal shock resistance were examined.The results showed that there was a great effect of content of SiC on bulk density,thermal expansion coefficient and thermal shock resistance of mullite-bauxite castable.Under the experimental conditions,the castable performed the best thermal shock resistance when w(SiC)=5%-10%.Nobuaki Chiyoda 2012Chinese Journal Of Geochemistry2012,31,2:15
4Usefulness of anti-ulcer drugs for the prevention and treatment of peptic ulcers induced by low doses of aspirin显示文摘AIM:To investigate the usefulness of anti-ulcer drugs for the prevention and treatment of low-dose aspirin- induced peptic ulcer. METHODS:Upper gastrointestinal endoscopy was performed in 68 patients receiving daily low-dose aspirin(81 or 100 mg/day).The endoscopic findings were classified according to the Lanza score,and the scores were compared between groups categorized according to the concomitant use of anti-ulcer drugs and the types of drugs used.In another study,31 hemorrhagic peptic ulcer patients who had been receiving low-dose aspirin were enrolled.The patients were randomly classified into the proton pump inhibitor (PPI)-treated group and the H2 receptor antagonist (H2RA)-treated group.The administration of low-dose aspirin was continued concomitantly,and endoscopic examinations were performed 8 wk later. RESULTS:The Lanza scores(mean±SD)of the gastro-mucosal lesions were 1.0±1.9 and 1.9±2.3 in 8 and 16 patients receiving prevention therapy with a PPI and an H2RA,respectively.Both scores were significantly smaller than the scores in 34 patients who were not receiving prevention therapy(4.7±1.0)and in 10 patients receiving cytoprotective anti-ulcer drugs (4.3±1.6).In the prospective study,18 and 13 patients received a PPI and an H2RA,respectively.Endoscopic examinations revealed that the tissue in the region of the gastro-mucosal lesions had reverted to normal in all patients in the PPI-treated group and in 12 patients (92%)in the H2RA-treated group;no significant differences were observed between the groups. 5Biomarkers for individualized dosage adjustments in immunosuppressive therapy using calcineurin inhibitors after organ transplantation显示文摘Calcineurin inhibitors (CNIs), such as cyclosporine A and tacrolimus, are widely used immunosuppressive agents for the prevention of post-transplantation rejection and have improved 1-year graft survival rates by up to 90%. However, CNIs can induce severe reactions, such as acute or chronic allograft nephropathy, hypertension, and neurotoxicity. Because CNIs have varied bioavailabilities, narrow therapeutic ranges, and individual propensities for toxic effects, therapeutic drug monitoring is necessary for all CNIs. Identifying the genetic polymorphisms in drug-metabolizing enzymes will help to determine personalized dosage regimens for CNIs, as CNIs are substrates for CYP3A5 and P-glycoprotein (P-gp, MDR1). CNIs are often concomitantly administered with voriconazole or proton pump inhibitors (PPIs), giving rise to drug interaction problems. Voriconazole and PPIs can increase the blood concentrations of CNIs, and both are primarily metabolized by CYP2C19. Thus, it is expected that interactions between CNIs and voriconazole or PPI would be affected by CYP2C19 and CYP3A5 polymorphisms. CNI-induced acute kidney injury (AKI) is a serious complication of transplantations. Neutrophil gelatinase-associated lipocalin (NGAL) and kidney injury molecule 1 (KIM-1) are noninvasive urinary biomarkers that are believed to be highly sensitive to CNI-induced AKI. In this article, we review the adverse events and pharmacokinetics of CNIs and the biomarkers related to CNIs, including CYP3A5, CYP2C19, MDR1, NGAL, and KIM-1. 8Effects of length and application rate of rice straw mulch on surface runoff and soil loss under laboratory simulated rainfall显示文摘Forest land affected by deforestation yields high soil and water losses.Suitable management practices need to be found that can reduce these losses and achieve ecological and hydrological sustainability of the deforested areas.Mulch has been found to be effective in reducing soil losses;straw mulch is easy to apply,contributes soil organic matter,and is efficient since the day of application.However,the complex effects of rice straw mulch with different application rates and lengths on surface runoff and soil loss have not been clarified in depth.The current paper evaluates the efficiency of rice straw mulch in reducing the hydrological response of a silty clay loam soil under high intensity and low frequency rainfall events(tap water with total depth of 49 mm and intensity of 98 mm/h)simulated in the laboratory.Surface runoff and soil loss at three lengths of the straw(10,30,and 200 mm)and three application rates(1,2,and 3 Mg/ha)were measured in 50 cm(width)×100 cm(length)×10 cm(depth)plots with disturbed soil samples(aggregate soil size<4 mm)collected in a deforested area.Bare soil was used as control experiment.Runoff volume and erosion were significantly(at p<0.05)lower in mulched soils compared to control plots.These reductions were ascribed to the water absorption capacity of the rice straw and the protection cover of the mulch layer.The minimum runoff was observed for a mulch layer of3 Mg/ha of straw with a length of 200 mm.The lowest soil losses were found with straw length of10 mm.The models developed predict runoff and erosion based on simple linear functions of mulch application rate and length,and can be used for a suitable hydrological management of soil.It is concluded that,thanks to rice straw mulch used as an organic soil conditioner,soil erosion and surface runoff are significantly(at p<0.05)reduced,and the mulch protection contributes to reduce the risk of soil degradation.Further research is,however,needed to analyze the upscaling of the hydrological effects of mulching from the plot to the hillslope scale.Misagh Parhizkar Mahmood Shabanpour Manuel Esteban Lucas-Borja Demetrio Antonio Zema Siyue Li Nobuaki Tanaka Artemio Cerdà 2021International Journal of Sediment Research2021,36,4:8
9A minimally invasive multiple percutaneous drainage technique for acute necrotizing pancreatitis显示文摘BACKGROUND: In approximately 20% of patients, necrotizing pancreatitis is complicated with severe acute pancreatitis, with high morbidity and mortality rates. Minimally invasive step-up approach is both safe and effective, but sometimes requires multiple access sites.METHODS: A 62-year-old woman was admitted with diabetic ketoacidosis, and initial computed tomography(CT) revealed no evidence of acute pancreatitis. She was clinically improved with insulin therapy, fl uid administration, and electrolyte replacement. However, on the 14 th day of admission, she developed a high-grade fever, and CT demonstrated evidence of acute necrotizing pancreatitis with a large collection of peripancreatic fl uid. Percutaneous transgastric drainage was performed and a 14 French gauge(Fr) pigtail catheter was placed 1 week later, which drained copious pus. Because of persistent high-grade fever and poor clinical improvement, multiple 8 and 10 Fr pigtail catheters were placed via the initial drainage route, allowing the safe and effective drainage of the extensive necrotic tissue that was occupying the bilateral anterior pararenal space.RESULTS: After drainage, the patient recovered well and the last catheter was removed on day 123 of admission.CONCLUSIONS: Multiple percutaneous drainage requires both careful judgment and specialist skills. The perforation of the colon and small bowel as well as the injury of the kidney and major vessels can occur. 12Prediction of slope failure due to earthquake显示文摘The earthquake-triggered landslides and slope failures are common phenomena during strong earthquakes and have drawn more attention from the world because of severe hazards they induced. These hazards usually cannot be prevented by current mitigating measures, thus, it becomes more and more important to develop a precise technique for the risk assessment of earthquake-induced failures in the mountainous area. The application of discrimination analysis method is proved to be successful and effective in the prediction of earthquake-triggered landslides and slope failures in the region of Imokawa Basin in Japan. Discriminant score can be used to assess the relative risk of slope failures, as the score increases, the possibility of slope failures occurrence increases accordingly. At the same time, the variables in the judgement formula, such as slope gradient, slope curvature and seismic peak ground acceleration, are easy to obtain. This advantage makes this method more practical and manipulable than others at present. 14Efficient hemostatic method for endoscopic submucosal dissection of colorectal tumors显示文摘AIM:To evaluate a new hemostatic method using hemostatic forceps to prevent perforation and perioperative hemorrhage during colonic endoscopic submucosal dissection(ESD).METHODS:We studied 250 cases,in which ESD for colorectal tumors was performed at the Kyoto Prefectural University of Medicine or Nara City Hospital between 2005 and 2010.We developed a new hemostatic method using hemostatic forceps in December 2008 for the efficient treatment of submucosal thick vessels.ESD was performed on 126 cases after adoption of the new method(the adopted group)and the new method was performed on 102 of these cases.ESD was performed on 124 cases before the adoption of the new method (the unadopted group).The details of the new method are as follows:firstly,a vessel was coagulated using the hemostatic forceps in the soft coagulation mode according to the standard procedure,and the coagulated vessel was removed using the forceps in the'endocut' mode without perioperative hemorrhage.Secondly,the partial surrounding submucosa was dissected using the forceps in the endocut mode.In the current study,we evaluated the efficacy of this method.RESULTS:Coagulated vessels were successfully removed using the hemostatic forceps in all 102 cases without severe perioperative hemorrhage.Moderate perioperative hemorrhage occurred in five cases(4.9%);however,it was stopped by immediately reuse of the hemostatic forceps.The partial surrounding submucosa was dissected using the forceps in all 102 cases.In the adopted group,the median operation time was 105 min.The proportion of endoscopic en bloc resection was 92.8%(P<0.01)compared to 80.6%in the unadopted group.The postoperative hemorrhage and perforation rates were 2.3%and 2.3%.The rate of perforation was significantly lower than that in the unadopted group (9.6%,P<0.01).We evaluated the ease of use of this method by allowing our three trainees to performed ESD on 46 cases,which were accomplished without any severe hemorrhage.CONCLUSION:The new method effectively treated submucosal thick vessels and shows promise for the prevention of perforation and perioperative hemorrhage in colonic ESD.Naohisa Yoshida Yuji Naito Munehiro Kugai Ken Inoue Naoki Wakabayashi Nobuaki Yagi Akio Yanagisawa Toshikazu Yoshikawa 2010World Journal of Gastroenterology2010,16,33:4

16Permeabilities of rebamipide via rat intestinal membranes and its colon specific delivery using chitosan capsule as a carrier显示文摘AIM: To investigate the permeability characteristics of rebamipide across intestinal mucosa, and examine the effects of some absorption enhancers on the permeability across the colonic tissue. Another purpose is to demonstrate the colon-specific delivery of rebamipide with or without absorption enhancers using chitosan capsule as a carrier. METHODS: The permeability of rebamipide was evaluated using an in vitro diffusion chamber system, and the effects of some absorption enhancers on the permeability via colon were further investigated. The release of rebamipide from chitosan or gelatin capsule was studied by Japan Pharmacopoeia rotating basket method. The colonic and plasma concentrations were analyzed by high performance liquid chromatography (HPLC) to evaluate colon-targeting action after oral administration of various dosage forms, and rebamipide with absorption enhancers in chitosan dosage forms. RESULTS: The permeability of rebamipide across the jejunal or ileal membranes was higher than the colonic membranes. Both sodium laurate (C12) and labrasol signifi cantly increased permeability across the colon membranes. On the other hand, the release of rebamipide from chitosan capsule was less than 10%totally within 6 h. The area under concentration-time profile of drug in the colon mucosa using chitosan capsules (AUCLI, 1 6011.2 ng·h/g) was 2.5 times and 4.4 times greater than using gelatin capsules and CMC suspension, respectively. Meanwhile, the area under concentration-time profile of drug in the plasma (AUCPL) was 1016.0 ng·h/mL for chitosan capsule, 1887.9 ng·h/mL for CMC suspension p and 2163.5 ng·h/mL for gelatin capsule. Overall, both AUCLI and AUCPL were increased when C12 was co-administrated, but the increase of AUCLI was much greater; the drug delivery index (DDI) was more than 1 compared with simple chitosan capsule group. CONCLUSION: There was a regional difference in the permeability of Rabamipide across the jejunum, ileum and the colon, and passive diffusion seems to be one of the major transport mechanisms of rebamipide. Absorption enhancers can increase the permeability of rebamipide across the colon tissue signifi cantly. 17Extract of Yokukansan improves anxiety-like behavior and increases serum brain-derived neurotrophic factor in rats with cerebral ischemia combined with amyloid-β42 peptide显示文摘OBJECTIVE: To examine the effects of Yokukansan(YKS) extract on two endogenous modulators of anxiety, brain-derived neurotrophic factor(BDNF)and serotonin (5-HT)_(2A) receptors pharmacologically, in the ischemic rat model of dementia.METHODS: The cerebral ischemia(CI) was induced by bilateral occlusion of the vertebral and common carotid arteries(4-vessel occlusion ischemia). The CI was combined with the amyloid-β42 peptide(A42) injected intracerebroventricularly, and referreβdto as CI+Aβ. Anxiety-like behaviors were assessed by elevated plus maze(enclosed arm), light/dark transition test(dark chamber), and open-field test.Wet-dog shakes were induced by the 5-HT_(2A) receptor agonist 2, 5-dimethoxy-4-iodoamphetamine(DOI). The concentration of BDNF in serum was determined by enzyme-linked immuno sorbent assay.RESULTS: CI + Aβ increased anxiety, as demonstrated by the increase of time spent in the enclosed arms and dark chambers, and locomotion in the outer zone of the open field(thigmotaxis). CI + Adecreased the serum concentration of BDNF. 18Diffuse gastroduodenitis and pouchitis associated with ulcerative colitis显示文摘We experienced a very rare case of ulcerative colitis (UC) accompanied with analogous lesions in the stomach, duodenum, and ileal J -pouch. Ileal J-pouch anal anastomosis was performed on a 29-year old woman in 1996. Six years later, she was admitted again to our hospital because of epigastralgia, nausea, watery diarrhea and low fever. Based on the results of endoscopic examination, we diagnosed it as pouchitis. Moreover, on hypotonic duodenography, expansion of the duodenal bulb and the descending portion were poor. Kerckring folds disappeared with typical lead-pipe appearance. The pathogenesis of the gastric and duodenal lesion in this patient was similar to that of the colonic lesions of UC. For the gastroduodenal lesions in this patient, symptomatic remission was obtained following administration of crushed mesalazine tablets (1500 mg/d) for 14 d with continuous administration of omeprazole. Firstly we used ciprofloxacin to treat pouchitis. On the fifth day, she got a fever because of catheter infection. In the catheter culture, methicillin-resistant Staphylococcus aureus (MRSA) was detected. Therefore we changed ciprofloxacin to vancomycin hydrochloride (Vancomycin?). Vancomycin was very effective, and the stool frequency dramatically improved in three days. 19Image-enhanced endoscopy for diagnosis of colorectal tumors in view of endoscopic treatment显示文摘Recently,image-enhanced endoscopy(IEE) has been used to diagnose gastrointestinal tumors.This method is a change from conventional white-light(WL) endoscopy without dyeing solution,requiring only the push of a button.In IEE,there are many advantages in diagnosis of neoplastic tumors,evaluation of invasion depth for cancerous lesions,and detection of neoplastic lesions.In narrow band imaging(NBI) systems(Olympus Medical Co.,Tokyo,Japan),optical filters that allow narrow-band light to pass at wavelengths of 415 and 540 nm are used.Mucosal surface blood vessels are seen most clearly at 415 nm,which is the wavelength that corresponds to the hemoglobin absorption band,while vessels in the deep layer of the mucosa can be detected at 540 nm.Thus,NBI also can detect pit-like structures named surface pattern.The flexible spectral imaging color enhancement(FICE) system(Fujifilm Medical Co.,Tokyo,Japan) is also an IEE but different to NBI.FICE depends on the use of spectral-estimation technology to reconstruct images at different wavelengths based on WL images.FICE can enhance vascular and surface patterns.The autofluorescence imaging(AFI) video endoscope system(Olympus Medical Co.,Tokyo,Japan) is a new illumination method that uses the difference in intensity of autofluorescence between the normal area and neoplastic lesions.AFI light comprises a blue light for emitting and a green light for hemoglobin absorption.The aim of this review is to highlight the efficacy of IEE for diagnosis of colorectal tumors for endoscopic treatment.Naohisa Yoshida Nobuaki Yagi Akio Yanagisawa Yuji Naito 2012World Journal of Gastrointestinal Endoscopy2012,4,12:3
