| 1 | Micro RNAs in colorectal cancer:Role in metastasis and clinical perspectives显示文摘Colorectal cancer(CRC)is the third most common malignancy and the third leading cause of cancer related deaths in the United States.Almost 90%of the patients diagnosed with CRC die due to metastases.Micro RNAs(mi RNAs)are evolutionarily conserved molecules that modulate the expression of their target genes post-transcriptionally,and they may participate in various physiological and pathological processes including CRC metastasis by influencing various factors in the human body.Recently,the role mi RNAs play throughout the CRC metastatic cascade has gain attention.Many studies have been published to link them with CRC metastasis.In this review,we will briefly discuss metastatic steps in the light of mi RNAs,along with their target genes.We will discuss how the aberration in the expression of mi RNAs leads to the formation of CRC by effecting the regulation of their target genes.As mi RNAs are being exploited for diagnosis,prognosis,and monitoring of cancer and other diseases,their high tissue specificity and critical role in oncogenesis make them new biomarkers for the diagnosis and classification of cancer as well as for predicting patients’outcome.Mi RNA signatures have been identified for many human tumors including CRC,and mi RNA-based therapies to treat cancer have been emphasized lately.These will also be discussed in this review. | Shan Muhammad Kavanjit Kaur Rui Huang Qian Zhang Paviter Kaur Hamza Obaid Yazdani Muhammad Umar Bilal Jiang Zheng Liu Zheng Xi-Shan Wang | 2014 | World Journal of Gastroenterology2014，20，45： | 13 |

| 3 | Novel therapy for non-cirrhotic hyperammonemia due to a spontaneous splenorenal shunt显示文摘Spontaneous splenorenal shunts in the absence of cirrhosis have rarely been reported as a cause hyperammonemia with encephalopathy. Several closure techniques of such lesions have been described. Here we report a case of a patient with no history of liver disease who developed significant confusion. After an extensive workup, he was found to have hyperammonemia and encephalopathy due to formation of a spontaneous splenorenal shunt. There was no evidence of cirrhosis on biopsy or imaging and no portal hypertension when directly measured. The shunt was 18 mm and too large for embolization so the segment of the splenic vein between the portal vein and the shunt was occluded using an Amplatzer plug. Thus, the superior mesenteric flow was directed entirely to the liver. | 4 | Celecoxib-induced cholestatic liver failure requiring orthotopic liver transplantation显示文摘Selective cyclooxygenase-2 (COX-2) inhibitors are widely used due to their efficacy and good safety profile. However, recent case reports have described varying degrees of liver injuries associated with the use of COX-2 inhibitors. We report the case of a patient who developed acute cholestatic hepatitis progressing to hepatic failure requiring liver transplantation, following a 3-d course of celecoxib for treatment of generalized muscle aches and pains. | 5 | Low-radiation and high image quality coronary computed tomography angiography in “real-world” unselected patients显示文摘AIM To determine the radiation dose and image quality in coronary computed tomography angiography(CCTA)using state-of-the-art dose reduction methods in unselected'real world'patients.METHODS In this single-centre study,consecutive patients in sinus rhythm underwent CCTA for suspected coronary artery disease(CAD)using a 320-row detector CT scanner.All patients underwent the standard CT acquisition protocol at our institute(Morriston Hospital)a combination of dose saving advances including prospective electrocardiogram-gating,automated tube current modulation,tube voltage reduction,heart rate reduction,and the most recent novel adaptive iterative dose reconstruction 3D(AIDR3D)algorithm.The cohort comprised real-world patients for routine CCTA who were not selected on age,body mass index,or heart rate.Subjective image quality was graded on a 4-point scale(4=excellent,1=non-diagnostic).RESULTS A total of 543 patients were included in the study with a mean body weight of 81±18 kg and a pre-scan mean heart rate of 70±11 beats per minute(bpm).When indicated,patients received rate-limiting medication with an oral beta-blocker followed by additional intravenous beta-blocker to achieve a heart rate below 65 bpm.The median effective radiation dose was 0.88 mSv(IQR,0.6-1.4 mSv)derived from a Dose Length Product of61.45 mGy.cm(IQR,42.86-100.00 mGy.cm).This also includes what we believe to be the lowest ever-reported radiation dose for a routine clinical CCTA(0.18 mSv).The mean image quality(SD)was 3.65±0.61,with a subjective image quality score of 3('good')or above for 93%of patient CCTAs.CONCLUSION Combining a low-dose scan protocol and AIDR3D with a 320-detector row CT scanner can provide high quality images at exceptionally low radiation dose in unselected patients being investigated for CAD. | Caryl Elizabeth Richards Stephen Dorman Patricia John Anthony Davies Sharon Evans Tishi Ninan David Martin Sriranj Kannoly Gail Roberts-Davies Mark Ramsey Daniel Rhys Obaid | 2018 | World Journal of Radiology2018，10，10： | 3 |

| 7 | Development of bis-thiobarbiturates as successful urease inhibitors and their molecular modeling studies显示文摘Bis-thiobarbiturate derivatives 1–15 have been synthesized, characterized by1 HNMR and EI-MS and screened for urease inhibition. All compounds showed various degree of urease inhibitory activity with IC_(50) values ranging 7.45 0.12 74.24 0.81 mmol/L while the standard thiourea behaved normally(IC_(50) = 21.10 0.12). Compounds 1(IC_(50) = 7.45 0.12 μmol/L), 9(IC_(50) = 18.17 1.03 μmol/L) and 13(IC_(50) = 8.61 0.45 μmol/L) showed excellent urease inhibitory activity in the series. Molecular modeling studies were performed to understand the binding site with the bimetallic nickel center of the enzyme.Structure-activity relationship has also been established for these compounds. | 9 | Pancreatic pseudocyst or a cystic tumor of the pancreas?显示文摘Pancreatic pseudocysts are the most common cystic lesions of the pancreas and may complicate acute pancreatitis, chronic pancreatitis, or pancreatic trauma. While the majority of acute pseudocysts resolve spontaneously, few may require drainage. On the other hand, pancreatic cystic tumors, which usually require extirpation, may disguise as pseudocysts. Hence, the distinction between the two entities is crucial for a successful outcome. We conducted this study to highlight the fundamental differences between pancreatic pseudocysts and cystic tumors so that relevant management plans can be devised. We reviewed the data of patients with pancreatic cystic lesions that underwent intervention between June 2007 and December 2010 in our hospital. We identified 9 patients(5 males and 4 females) with a median age of 40 years(range, 30–70 years). Five patients had pseudocysts, 2 had cystic tumors, and 2 had diseases of undetermined pathology. Pancreatic pseudocysts were treated by pseudocystogastrostomy in 2 cases and percutaneous drainage in 3 cases. One case recurred after percutaneous drainage and required pseudocystogastrostomy. The true pancreatic cysts were serous cystadenoma, which was treated by distal pancreatectomy, and mucinous cystadenocarcinoma, which was initially treated by drainage, like a pseudocyst, and then by distal pancreatectomy when its true nature was revealed. We conclude that every effort should be exerted to distinguish between pancreatic pseudocysts and cystic tumors of the pancreas to avoid the serious misjudgement of draining rather than extirpating a pancreatic cystic tumor.
