| 1 | Pediatric gastrointestinal bleeding: Perspectives from the Italian Society of Pediatric Gastroenterology显示文摘There are many causes of gastrointestinal bleeding(GIB) in children, and this condition is not rare, having a reported incidence of 6.4%. Causes vary with age, but show considerable overlap; moreover, while many of the causes in the pediatric population are similar to those in adults, some lesions are unique to children. The diagnostic approach for pediatric GIB includes definition of the etiology, localization of the bleeding site and determination of the severity of bleeding; timely and accurate diagnosis is necessary to reduce morbidity and mortality. To assist medical care providers in the evaluation and management of children with GIB, the 'Gastro-Ped Bleed Team' of the Italian Society of Pediatric Gastroenterology, Hepatology and Nutrition(SIGENP) carried out a systematic search on MEDLINE via Pub Med(http://gffzz16a62c593b5540cehk5q5kox6u56066pk.ffgz.tsg.suse.edu.cn/pubmed/) to identify all articles published in English from January 1990 to 2016; the following key words were used to conduct the electronic search: 'upper GIB' and 'pediatric' [all fields]; 'lower GIB' and 'pediatric' [all fields]; 'obscure GIB' and 'pediatric' [all fields]; 'GIB' and 'endoscopy' [all fields]; 'GIB' and 'therapy' [all fields]. The identified publications included articles describing randomized controlled trials, reviews, case reports, cohort studies, casecontrol studies and observational studies. References from the pertinent articles were also reviewed. This paper expresses a position statement of SIGENP that can have an immediate impact on clinical practice and for which sufficient evidence is not available in literature. | 2 | Human bocavirus: Current knowledge and future challenges显示文摘Human bocavirus(HBoV) is a parvovirus isolated about a decade ago and found worldwide in both respiratory samples, mainly from early life and children of 6-24 mo of age with acute respiratory infection, and in stool samples, from patients with gastroenteritis. Since then, other viruses related to the first HBoV isolate(HBoV 1), namely HBoV 2, HBoV 3 and HBoV 4, have been detected principally in human faeces. HBo Vs are small nonenveloped single-stranded DNA viruses of about 5300 nucleotides, consisting of three open reading frames encoding the first two the non-structural protein 1(NS1) and nuclear phosphoprotein(NP1) and the third the viral capsid proteins 1 and 2(VP1 and VP2). HBoV pathogenicity remains to be fully clarified mainly due to the lack of animal models for the difficulties in replicating the virus in in vitro cell cultures, and the fact that HBo V infection is frequently accompanied by at least another viral and/or bacterial respiratory and/or gastroenteric pathogen infection. Current diagnostic methods to support HBoV detection include polymerase chain reaction, real-time PCR, enzymelinked immunosorbent assay and enzyme immunoassay using recombinant VP2 or virus-like particle capsid proteins, although sequence-independent amplification techniques combined with next-generation sequencing platforms promise rapid and simultaneous detection of the pathogens in the future. This review presents the current knowledge on HBoV genotypes with emphasis on taxonomy, phylogenetic relationship and genomic analysis, biology, epidemiology, pathogenesis and diagnostic methods. | 5 | Clinical significance of NOD2/CARD15 and Toll-like receptor 4 gene single nucleotide polymorphisms in inflammatory bowel disease显示文摘AIM: To evaluate the role of genetic factors in the pathogenesis of Crohn's disease (CD) and ulcerative colitis (UC), we investigated the single nucleotide poly- morphisms (SNPs) of NOD2/CARD15 (R702W, G908R and L1007fi nsC), and Toll-like receptor 4 (TLR4) genes (D299G and T399I) in a selected inflammatory bowel disease (IBD) population coming from Southern Italy. METHODS: Allele and genotype frequencies of NOD2/ CARD15 (R702W, G908R and L1007finsC) and TLR4 (D299G and T399I) SNPs were examined in 133 CD pa-tients, in 45 UC patients, and in 103 healthy controls. A genotype-phenotype correlation was performed. RESULTS: NOD2/CARD15 R702W mutation was sig-nificantly more frequent in CD (9.8%) than in controls (2.4%, P = 0.001) and in UC (2.3%, P = 0.03). No sig-nificant difference was found between UC patients and control group (P > 0.05). In CD and UC patients, no signifi cant association with G908R variant was found. L1007f insC SNP showed an association with CD (9.8%) compared with controls (2.9%, P = 0.002) and UC patients (2.3%, P = 0.01). Moreover, in CD patients, G908R and L1007finsC mutations were significantly associated with different phenotypes compared to CD wild-type patients. No association of IBD with the TLR4 SNPs was found in either cohort (allele frequencies: D299G-controls 3.9%, CD 3.7%, UC 3.4%, P > 0.05; T399I-controls 2.9%, CD 3.0%, UC 3.4%, P > 0.05). | 6 | Inflammatory bowel diseases and human reproduction:A comprehensive evidence-based review显示文摘To evaluate the effects of inflammatory bowel diseases(IBDs)on human reproduction,we reviewed the current literature using a systematic search for published studies(articles and/or abstracts)without limits for English language.We searched on Medline(through PubMed),the Institute for Scientific Information,the Web of Science and the websites for the registration of controlled trials(http://gffzz872f90091fd340c7hk5q5kox6u56066pk.ffgz.tsg.suse.edu.cn/).Bibliographies of retrieved articles,books,expert opinion review articles and reviewed bibliographies from subject experts were manually searched.Titles and abstracts were screened initially,and potential relevant articles were identified and reviewed.Whenever possible,data were analyzed by comparing IBD patients vs healthy controls,and patients with active IBDs vs those with disease in remission.The effects of IBDs on female fertility,fertility in infertile couples,pregnancy and male infertility were examined separately.Patients with IBDs in remission have normal fertility.At the moment,there is no established guideline for the preservation of fertility in women with IBD undergoing surgery.Further data are needed regarding guidelines for the management of these patients.Data regarding IBDs and infertility are currently completely lacking.Considering the prevalence of intestinal pathology in young adults of childbearing age,this field is of great scientific and clinical interest,opening up important future perspectives.Another important and as yet unexplored point is the response to treatments for infertility in patients with IBDs.In particular,the question is whether the reproductive outcomes(clinical and biological)can be influenced by the IBD of one of the partners.The goals for successful reproductive outcomes in IBD population are correct counseling and disease remission.IBDs significantly affect several reproductive aspects of human(female,male,couple)reproduction.Further data are needed to develop guidelines for the clinical management of subjects of reproductive age with IBDs. | Stefano Palomba Giuliana Sereni Angela Falbo Marina Beltrami Silvia Lombardini Maria Chiara Boni Giovanni Fornaciari Romano Sassatelli Giovanni Battista La Sala | 2014 | World Journal of Gastroenterology2014,20,23: | 6 |
| 7 | An eighteen-month follow-up study on the effects of Intravitreal Dexamethasone Implant in diabetic macular edema refractory to anti-VEGF therapy显示文摘AIM: To evaluate the long-term efficacy and safety of dexamethasone implants in subjects affected by diabetic macular edema(DME) resistant to anti-vascular endothelial growth factor(VEGF) therapy.METHODS: Thirty-two DME patients were enrolled.A700 microgram slow release Intravitreal Dexamethasone Implant(Ozurdex~) was placed in the vitreous cavity.All patients were followed for 18 mo.Best-corrected visual acuity(BCVA) measured with Early Treatment Diabetic Retinopathy Study(ETDRS) and central macular thickness(CMT) exams were carried out at baseline(T0)and after 1(T1),3(T3),4(T4),6(T6),9(T9),12(T12),15(T15),and 18mo(T18) post injection. RESULTS: Repeated measures ANOVA showed an effect of treatment on ETDRS(P<0.0001).Post hoc analyses revealed that ETDRS values were significantly increased at T1,T3,T4,T9,and T15(P <0.001) as compared to baseline value(T0).At T6,T12,and T18,ETDRS values were still statistically higher than baseline(P<0.001 vs T0).However,at these time points,we observed a trend to return to baseline conditions.ANOVA also showed an effect of treatment(P <0.0001).CMT decreased significantly at T1,T3,T4,T9,and T15(P<0.001).At T6(P<0.01),T12 and T18(P<0.001) CMT was also significantly lower than T0 although a trend to return to the baseline conditions was also observed.CONCLUSION: Our findings demonstrate that Intravitreal Dexamethasone Implant is a good option to improveBCVA and CMT in DME patients resistant to anti-VEGF therapy.Our data also show that the use of drugs administered directly into the vitreous allows achieving appropriate and long-lasting concentration at the site of disease without systemic side effects. | Fernanda Pacella Maria Rosaria Romano Paolo Turchetti Giovanna Tarquini Anna Carnovale Antonella Mollicone Alessandra Mastromatteo Elena Pacella | 2016 | International Journal of Ophthalmology(English edition)2016,9,10: | 5 |

| 16 | Porous titanium granules in critical size defects of rabbit tibia with or without membranes显示文摘Recently,porous titanium granules(PTGs)have been indicated for the preservation of the dimensions of post-extraction sockets,as a filler in sinus lift procedures and for the treatment of peri-implant and periodontal defects,based on the osteoconductivity and dimensional stability of the titanium granules.However,there is a lack of information regarding the use of this material in larger defects and in conjunction with membranes.The objective of this study is to test the behavior of PTGs used to fill critical size defects in rabbit tibiae,with and without membranes.Critical defects were created in both tibiae of rabbits,divided randomly into three groups:Group A(defect filled with PTG),Group B(defect filled with PTG1collagen membrane)and a control group(empty defect).After six weeks,histomorphometric analysis was performed.The results showed more defect closures at the cortical area(87.37%62.2%)and more bone formation at the marrow area(57.6%61.3%)in Group B,in comparison with the other groups(P,0.05);the use of membranes improved the material stability expressed as more percentages of the original material when membranes were used(P,0.05).Finally,inflammatory reactions were observed when the granules were not protected by membranes.In spite of the limitations of this animal study,it may be concluded that PTG particles are osteoconductive and allow bone growth.The PTG particles must be covered by a membrane,especially when grafting larger defects,in order to control particle migration,promote clot stabilization and separate the PTG graft from undesired soft tissue cells. | Rafael Arcesio Delgado-Ruiz Jose Luis Calvo-Guirado Marcus Abboud Maria Piedad Ramirez-Ferna'ndez Jose Eduardo Maté-Snchez Bruno Negri Alex Won Georgios Romanos | 2014 | International Journal of Oral Science2014,6,2: | 1 |
