1Less common etiologies of exocrine pancreatic insufficiency显示文摘Exocrine pancreatic insufficiency(EPI), an important cause of maldigestion and malabsorption, results from primary pancreatic diseases or secondarily impaired exocrine pancreatic function. Besides cystic fibrosis and chronic pancreatitis, the most common etiologies of EPI, other causes of EPI include unresectable pancreatic cancer, metabolic diseases(diabetes); impaired hormonal stimulation of exocrine pancreatic secretion by cholecystokinin(CCK); celiac or inflammatory bowel disease(IBD) due to loss of intestinal brush border proteins; and gastrointestinal surgery(asynchrony between motor and secretory functions, impaired enteropancreatic feedback, and inadequate mixing of pancreatic secretions with food). This paper reviews such conditions that have less straightforward associations with EPI and examines the role of pancreatic enzyme replacement therapy(PERT). Relevant literature was identified by database searches. Most patients with inoperable pancreatic cancer develop EPI(66%-92%). EPI occurs in patients with type 1(26%-57%) or type 2 diabetes(20%-36%) and is typically mild to moderate; by definition, all patients with type 3 c(pancreatogenic) diabetes have EPI. EPI occurs in untreated celiac disease(4%-80%), but typically resolves on a gluten-free diet. EPI manifests in patients with IBD(14%-74%) and up to 100% of gastrointestinal surgery patients(47%-100%; dependent on surgical site). With the paucity of published studies on PERT use for these conditions, recommendations for or against PERT use remain ambiguous. 2靶向uPAR的免疫毒素对人胶质母细胞瘤及其血管生成的影响(英文)显示文摘目的:观察靶向尿激酶型纤溶酶原激活物受体(uPAR)的免疫毒素DTAT和DTATEGF对人胶质母细胞瘤体外和祼小鼠脑内生长的影响。方法:MTT法检测靶向毒素DTAT,DTATEGF对体外培养的人胶质瘤U87-luc细胞系及人脐静脉细胞系(HUVEC)的增殖影响。建立祼小鼠脑内荧光素酶标记的人胶质母细胞瘤模型。加强对流给药(CED)方式瘤内泵入1μg的DTAT,DTATEGF或对照液,生物荧光显像(BLI)检测肿瘤生物荧光信号强度变化,免疫组织化学检测肿瘤微血管密度(MVD)。结果:体外实验显示DTAT和DTATEG高效抑制uPAR表达阳性的U87-luc和HUVEC细胞系增殖,DTATEGF和DTAT抑制U87-luc的IC50值分别<0.01 nmol/L和<1 nmol/L。裸小鼠肿瘤荧光信号检测显示两治疗组的信号强度较对照组增长缓慢,差异有统计学意义(P<0.05);DTATEGF和DTAT组肿瘤标本的MVD分别为(25.1±6.5)/mm2和(31.6±5.2)/mm2,明显少于对照组(51.3±7.4)/mm2(P<0.01)。结论:靶向uPAR的DTAT和DTATEGF能明显抑制胶质瘤U87-luc肿瘤细胞增殖,抑制祼小鼠脑内胶质瘤生长及其新生血管的形成。黄军 袁盾 刘定阳 李坚 李岩 Walter A Hall 李波 2014中南大学学报(医学版)2014,39,1:4
3全球药物滥用的疾病负担:来自2010年全球疾病负担研究显示文摘背景:直到目前还没有研究者系统地估计过全球及区域性的苯丙胺类、大麻、可卡因和阿片类药物依赖的患病率及其造成的疾病负担。我们旨在用伤残损失寿命年(YLDs)、过早死亡损失寿命年(YLLs)和伤残调整寿命年(DALYs)来估计药物依赖的患病率和其造成的疾病负担。方法:采用贝叶斯Meta回归的方法(Dis Mod-MR)对药物依赖的流行病学研究及2010年全球疾病、伤害及危险因素负担研究(GBD 2010)进行了系统综述,估计人群水平的药物依赖患病率。GBD 2010通过代表性社区调查和基于互联网的调查计算出新的失能权重。我们把估计的药物依赖患病率和新的失能权重结合起来估算药物依赖导致的YLDs、YLLs、DALYs和其作为相关疾病危险因素时导致的YLDs、YLLs和DALYs。结果:2010年,药物依赖直接造成了2千万的DALYs(95%UI(不确定区间)1.3-2.54千万),占全球全因DALYs的0.8%(0.6%-1.0%)。世界范围内,相比于其他药物,越来越多的人依赖于阿片类和苯丙胺类药物。阿片类药物依赖直接造成的疾病负担最大(DALYs为920万,95%UI为710-1140万)。在某些地区,药物依赖导致的DALYs所占比例比其他地区高出20倍,收入越高的国家药物依赖造成的负担所占比例也越高。注射吸毒所致HIV感染造成210万的DALYs(95%UI 110-360万),所致HCV感染导致50.2万的DALYs(95%UI 28.6-89.1万)。苯丙胺类药物依赖所致自杀造成85.4万的DALYs(95%UI 29.1-179.1万),阿片类药物依赖所致自杀造成67.1万的DALYs(95%UI 32.9-173.0万),可卡因依赖所致自杀造成32.4万的DALYs(95%UI 10.9-68.2万)。药物依赖导致的疾病负担最高的国家(>650 DALYs/10万人)包括美国,英国,俄罗斯和澳大利亚。结论:药物滥用是造成全球疾病负担的一个重要因素。为减少人群水平的疾病负担,我们需采取有效的措施(比如给予阿片类药物依赖者替代治疗及针具交换等)降低阿片类药物依赖和注射吸毒导致的疾病负担。王同瑜 Degenhardt L Whiteford HA Ferrari AJ Baxter AJ Charlson FJ Hall WD Freedman G Burstein R Johns Engell RE Flaxman A Murray CJ Vos T 2015中国药物依赖性杂志2015,24,6:3
4渗透泵强化对流释放给药治疗恶性脑胶质瘤的裸小鼠模型显示文摘目的探寻渗透泵强化对流释放给药治疗脑肿瘤的祼小鼠模型,为新药的临床前期应用提供良好的实验动物模型。方法 14只祼小鼠基底节区种植4×105个人胶质瘤U87细胞,并安置渗透泵强化对流释放给药系统。裸小鼠按体重和活体荧光信号强度分为二组:治疗组(n=7)的泵内注入100μl(1μg)的DTATEGF,对照组(n=7)泵内注入100μl(1μg)的无关毒素DT。观察动物的生长特征,Kaplan-Meier生存曲线分析和脑内肿瘤的病理学特征。结果实验动物没有与渗透泵及外科手术相关的病残和病死发生。动物对渗透泵给药系统耐受好,无抓搔及渗透泵脱位,伤口无感染及裂开等情况。Kaplan-Meier生存曲线显示治疗组动物的中位生存期较对照组明显延长,差异有显著性。(治疗组126天vs.对照组68天,P=0.0002)。治疗组中有两只小鼠生存期超过180天。病理学检测祼小鼠脑内种植生长的肿瘤与人类胶质母细胞瘤的病理特征极为相似。结论该祼小鼠实验动物模型简便、实用、可靠,适用于新的药物包括单克隆抗体、靶向小分子、肿瘤疫苗等通过强化对流投递治疗脑内恶性肿瘤的临床前期应用。黄军 龙小艳 李波 Yan Michael Li Walter A Hall 刘定阳 刘庆 王君宇 袁贤瑞 2012国际神经病学神经外科学杂志2012,39,4:3
5Chromodomain-helicase-DNA binding protein 5, 7 and pronecrotic mixed lineage kinase domain-like protein serve as potential prognostic biomarkers in patients with resected pancreatic adenocarcinomas显示文摘Pancreatic cancer is one of the deadliest cancers with a very poor prognosis. Recently, there has been a significant increase in research directed towards identifying potential biomarkers that can be used to diagnose and provide prognostic information for pancreatic cancer. These markers can be used clinically to optimize and personalize therapy for individual patients. In this review, we focused on 3 biomarkers involved in the DNA damage response pathway and the necroptosis pathway: Chromodomainhelicase-DNA binding protein 5, chromodomain-helicaseDNA binding protein 7, and mixed lineage kinase domain-like protein. The aim of this article is to review present literature provided for these biomarkers and current studies in which their effectiveness as prognostic biomarkers are analyzed in order to determine their future use as biomarkers in clinical medicine. 6脑肿瘤靶向毒素治疗的现状和展望显示文摘靶向毒素,亦称为免疫毒素或细胞毒素,是一种肿瘤细胞表面特异性抗原或受体相结合的细胞分子。如与EG-FR、IL-13受体等相结合的细胞分子,其毒素成份可杀死肿瘤细胞。目前研究的毒素多为以白喉毒素、假单胞杆菌外毒素等为母基合成的毒素。双重特异性靶向毒素能同时靶向肿瘤细胞和/或其新生血管上的两个过表达受体的毒素,如DTAT13靶向uPAR和IL-13受体,DTATEGF靶向EGFR和uPAR,其较单一靶向毒素具有更高的特异性和更强的毒性,同时免疫源性亦大为降低。寻找特异性的肿瘤细胞表面标志物、降低毒素的免疫源性及研究实体肿瘤毒素的投递方式是靶向毒素治疗脑肿瘤的研究方向。黄军 Yan Michael Li Walter A Hall 2011国际神经病学神经外科学杂志2011,38,5:2
7Age at diagnosis of type 2 diabetes and cardiovascular risk factor profile:A pooled analysis显示文摘BACKGROUND The diagnosis of type 2 diabetes(T2D)in younger adults,an increasingly common public health issue,is associated with a higher risk of cardiovascular complications and mortality,which may be due to a more adverse cardiovascular risk profile in individuals diagnosed at a younger age.AIM To investigate the association between age at diagnosis and the cardiovascular risk profile in adults with T2D.METHODS A pooled dataset was used,comprised of data from five previous studies of adults with T2D,including 1409 participants of whom 196 were diagnosed with T2D under the age of 40 years.Anthropometric and blood biomarker measurements included body weight,body mass index(BMI),waist circumference,body fat percentage,glycaemic control(HbA1c),lipid profile and blood pressure.Univariable and multivariable linear regression models,adjusted for diabetes duration,sex,ethnicity and smoking status,were used to investigate the association between age at diagnosis and each cardiovascular risk factor.RESULTS A higher proportion of participants diagnosed with T2D under the age of 40 were female,current smokers and treated with glucose-lowering medications,compared to participants diagnosed later in life.Participants diagnosed with T2D under the age of 40 also had higher body weight,BMI,waist circumference and body fat percentage,in addition to a more adverse lipid profile,compared to participants diagnosed at an older age.Modelling results showed that each one year reduction in age at diagnosis was significantly associated with 0.67 kg higher body weight[95%confidence interval(CI):0.52-0.82 kg],0.18 kg/m^(2) higher BMI(95%CI:0.10-0.25)and 0.32 cm higher waist circumference(95%CI:0.14-0.49),after adjustment for duration of diabetes and other confounders.Younger age at diagnosis was also significantly associated with higher HbA1c,total cholesterol,low-density lipoprotein cholesterol and triglycerides.CONCLUSION The diagnosis of T2D earlier in life is associated with a worse cardiovascular risk factor profile,compared to those diagnosed later in life.Mary M Barker Francesco Zaccardi Emer M Brady Gaurav S Gulsin Andrew P Hall Joseph Henson Zin ZinHtike Kamlesh Khunti Gerald P McCann Emma L Redman David R Webb Emma G Wilmot Tom Yates Jian Yeo Melanie J Davies Jack A Sargeant 2022World Journal of Diabetes2022,13,3:2
