| 1 | Colorectal cancer in patients under 50 years of age:A retrospective analysis of two institutions' experience显示文摘AIM:To investigate the epidemiological characteristics of colorectal cancer(CRC)in patients under 50 years of age across two institutions.METHODS:Records of patients under age 50 years of age who had CRC surgery over a 16 year period were assessed at two institutions.The following documents where reviewed:admission notes,operative notes,and discharge summaries.The main study variables included:age,presenting symptoms,family history,tumor location,operation,stage/differentiation of disease,and post operative complications.Stage of disease was classified according to the American Joint Committee on Cancer TNM staging system:tumor depth;node status;and metastases.RESULTS:CRC was found in 180 patients under age50 years(87 females,93 males;mean age 41.4±6.2years).Young patients accounted for 11.2%of cases during a 6 year period for which the full data set wasavailable.Eight percent had a 1stdegree and 12%a 2nd degree family CRC history.Almost all patients(94%)were symptomatic at diagnosis;common symptoms included:bleeding(59%),obstruction(9%),and abdominal/rectal pain(35%).Evaluation was often delayed and bleeding frequently attributed to hemorrhoids.Advanced stage CRC(Stage 3 or 4)was noted in 53%of patients.Most tumors were distal to the splenic flexure(77%)and 39%involved the rectum.Most patients(95%)had segmental resections;6 patients had subtotal/total colectomy.Poorly differentiated tumors were noted in 12%and mucinous lesions in 19%of patients of which most had Stage 3 or 4 disease.Twenty-two patients(13%)developed recurrence and/or progression of disease to date.Three patients(ages 42,42and 49 years)went on to develop metachronous primary colon cancers within 3 to 4 years of their initial resection.CONCLUSION:CRC was common in young patients with no family history.Young patients with symptoms merit a timely evaluation to avoid presentation with late stage CRC. | Elizabeth A Myers Daniel L Feingold Kenneth A Forde Tracey Arnell Joon Ho Jang Richard L Whelan | 2013 | World Journal of Gastroenterology2013，19，34： | 9 |

| 3 | Effects of mindfulness-based intervention programs on sleep among people with common mental disorders:A systematic review and meta-analysis显示文摘BACKGROUND Sleep problems are particularly prevalent in people with depression or anxiety disorder.Although mindfulness has been suggested as an important component in alleviating insomnia,no comprehensive review and meta-analysis has been conducted to evaluate the effects of different mindfulness-based intervention(MBI)programs on sleep among people with depression or anxiety disorder.AIM To compare the effects of different MBI programs on sleep among people with depression or anxiety disorder.METHODS Related publications in Embase,Medline,PubMed and PsycINFO databases were systematically searched from January 2010 to June 2020 for randomised controlled trials.Data were synthesized using a random-effects or a fixed-effects model to analyse the effects of various MBI programs on sleep problems among people with depression or anxiety disorder.The fixed-effects model was used when heterogeneity was negligible,and the random-effects model was used when heterogeneity was significant to calculate the standardised mean differences(SMDs)and 95%confidence intervals(CIs).RESULTS We identified 397 articles,of which 10 randomised controlled trials,involving a total of 541 participants,were included in the meta-analysis.Studies of internet mindfulness meditation intervention(IMMI),mindfulness meditation(MM),mindfulness-based cognitive therapy(MBCT),mindfulness-based stress reduction(MBSR)and mindfulness-based touch therapy(MBTT)met the inclusion criteria.The greatest effect sizes are reported in favour of MBTT,with SMDs of-1.138(95%CI:-1.937 to-0.340;P=0.005),followed by-1.003(95%CI:-1.645 to-0.360;P=0.002)for MBCT.SMDs of-0.618(95%CI:-0.980 to-0.257;P=0.001)and-0.551(95%CI:-0.842 to-0.260;P<0.0001)were reported for IMMI and MBSR in the pooling trials,respectively.Significant effects on sleep problem improvement are shown in all reviewed MBI programs,except MM,for which the effect size was shown to be nonsignificant.CONCLUSION All MBI programs(MBTT,MBCT,IMMI and MBSR),except MM,are effective options to improve sleep problems among people with depression or anxiety disorder. | Sunny Ho-Wan Chan Danielle Lui Hazel Chan Kelly Sum Ava Cheung Hayley Yip Chong Ho Yu | 2022 | World Journal of Psychiatry2022，12，4： | 5 |

| 8 | Intravenous iron supplementation may be superior to observation in acute isovolemic anemia after gastrectomy for cancer显示文摘AIM:To determine whether the application of postoperative intravenous(IV)-iron for acute isovolemic anemia after gastrectomy for cancer may be effective.METHODS:Among 2078 gastric cancer patients who underwent surgery between February 2007 and August2009 at the National Cancer Center Korea,368 patients developed post-operative anemia[hemoglobin-(Hb)-level<9 g/dL]within the first postoperative week.Patients requiring transfusions were excluded.IV-iron was administered to 63 patients(iron group).Sixty patients were observed without treatment(observation group).The clinical outcomes of the groups were compared concerning clinicopathologic data,morbidity,and changes in Hb levels using Fisher’s exact test,Student’s t-test and the Z-test.RESULTS:The initial Hb level was higher in the iron group than in the observation group(7.3±1.0 g/dL vs8.4±0.5 g/dL,P<0.001).The slope of the changes in the Hb level was significantly higher in the iron group than in the observation group(0.648±0.054 vs 0.349±0.038,P<0.001).The Hb level 1 and 3 mo postoperatively increased from 10.7±1.3 to 11.9±1.3g/dL in the iron group(P=0.033)and from 10.1±1.0to 10.8±1.4 g/dL in the observation group(P<0.001).The postoperative hospital stay was significantly longer in the iron group than in the observation group(10.5±6.8 d vs 7.6±5.5 d,P=0.011).There were no significant differences in the major and surgical complications between the groups(6.3%vs 13.3%,P=0.192;9.5%vs 3.3%,P=0.164).CONCLUSION:IV-iron supplementation may be an effective treatment for post-operative isovolemic postgastrectomy anemia and may be a better alternative than observation. | Hong Man Yoon Young-Woo Kim Byung Ho Nam Daniel Reim Bang Wool Eom Ji Yeon Park Keun Won Ryu | 2014 | World Journal of Gastroenterology2014，20，7： | 2 |

| 13 | Drug-induced autoimmune hepatitis:A minireview显示文摘Drug-induced autoimmune hepatitis(DIAIH)is a specific phenotype of druginduced liver injury that may lead to the devastating outcome of acute liver failure requiring liver transplantation.Drugs implicated in DIAIH include antimicrobials such as nitrofurantoin and minocycline,non-steroidal anti-inflammatory drugs,statins as well as anti-tumor necrosis agents.The clinical features of druginduced liver injury are indistinguishable from idiopathic autoimmune hepatitis(AIH)as both may have positive AIH-related autoantibodies,elevated immunoglobulin G,as well as similar histopathological findings.In patients who show no clinical improvement,or there is progressive liver injury despite cessation of the suspected drug,a liver biopsy should be considered,whereby the presence of advance fibrosis on histology favors the diagnosis of idiopathic AIH.Empirical treatment with corticosteroids may be required in patients with non-resolving liver injury.A typical clinical scenario supportive of DIAIH includes a history of drug exposure with spontaneous resolution of liver injury after drug withdrawal and the absence of relapse after rapid steroid taper.In this article we report two cases of DIAIH secondary to Sorafenib and Atorvastatin along with a review of currently available literature.Early identification and treatment often lead to a favorable outcome in DIAIH. | Chin Kimg Tan Danielle Ho Lai Mun Wang Rahul Kumar | 2022 | World Journal of Gastroenterology2022，28，24： | 2 |
