| 2 | Clostridium difficile causing acute renal failure: Case presentation and review显示文摘AIM: Clostridium difficile infection is primarily a nosocomial infection but asymptomatic carriers of Clostridium difficile can be found in up to 5% of the general population.Ampicillin, cephalosporins and clindamycin are the antibiotics that are most frequently associated with Clostridium difficile-associated diarrhea or colitis. Little is known about acute renal failure as a consequence of Clostridium difficile-associated diarrhea.METHODS: In this case report, we describe the course of Clostridium difficile-associated diarrhea in an 82-yearold patient developing acute renal failure. Stopping the offending agent and symptomatic therapy brought a rapid improvement of diarrhea and acute renal failure, full recovery was gained 18 d after admission. In a systematic review we looked for links between the two conditions.RESULTS: The link between Clostridium difficilr-associated diarrhea and acute renal failure in our patient was most likely volume depletion. | 3 | Metastasized pancreatic carcinoma with neoadjuvant FOLFIRINOX therapy and R0 resection显示文摘Patients with metastasized carcinoma of the pancreas have a very poor prognosis, and long-term survival cannot be expected. This case report describes two patients with an initial diagnosis of metastatic pancreatic cancer, both with hepatic metastases and one with an additional peritoneal carcinomatosis. Initially, both patients were treated intravenously with the FOLFIRINOX chemotherapy regimen, consisting of 5-FU, folinic acid, irinotecan and oxaliplatin. Surprisingly, the FOLFIRINOX treatment resulted in complete resolution of the hepatic metastases in both patients, with no lesions detectable by computed tomography scan. Furthermore, treatment response included decreased diameter of the primary tumor in the tail of the pancreas and disappearance of the additional peritoneal carcinomatosis. Both patients were discussed by our multidisciplinary tumor board, which recommended surgical resections of the carcinoma. The R0 resection of the primary tumor was successful in both cases and, interestingly, the resected tissues showed no evidence of the hepatic metastases intraoperatively. In the first case, the patient received a postoperative 6-mo course of adjuvant chemotherapy with gemcitabine. In the second case, the patient continued to receive the FOLFIRINOX regimen for an additional 6 mo postoperatively. At 12 mo after the operation, a nonresectable retroperitoneal lymph node metastasis was detected in the first patient, whereas the second patient remained in complete remission at the time of this report(5 mo after the adjuvant therapy was discontinued). This case report is the first of its kind to describe two cases of hepatic metastatic pancreatic carcinoma that were resectable following treatment with FOLFIRINOX. | 5 | How to use magnetic resonance imaging following neoadjuvant chemotherapy in locally advanced breast cancer显示文摘Magnetic resonance imaging(MRI) is highly sensitive in identifying residual breast cancer following neoadjuvant chemotherapy(NAC), and consequently is a commonly used imaging modality in locally advanced breast cancer patients. In these patients, tumor response is an important prognostic indicator. However, discrepancies between MRI findings and surgical pathology are well documented. Overestimation of residual disease by MRI may result in greater surgery than is actually required while underestimation may result in insufficient surgery. Thus, it is important to understand when MRI findings are reliable and when they are less accurate. MRI most accurately predicts pathology in triple negative, Her2 positive and hormone receptor negative tumors, especially if they are of a solid imaging phenotype. In these cases, post-NAC MRI is highly reliable for surgical planning. Hormone receptor positive cancers and those demonstrating non mass enhancement show lower concordance with surgical pathology, making surgical guidance more nebulous in these cases. Radiologists and surgeons must assess MRI response to NAC in the context of tumor subtype. | 6 | Use of health-related quality of life tools in hepatobiliary surgery显示文摘BACKGROUND:The objective of this article is to review the literature and discuss the various tools used in hepatobiliary surgery for the measurement of health related quality of life (HR-QOL) and highlight various outcome variables that affect the HR-QOL among patients with common hepatobiliary disorders. DATA SOURCES: We reviewed HR-QOL articles published in the last 20 years on different hepatobiliary curative or palliative procedures in all languages. RESULTS: HR-QOL is a questionnaire tool which is utilized to assess the changes in the health status of patients after a hepatobiliary intervention. These surveys are of increasingly importance, as health care providers are challenged to justify treatment approaches and rationale for any surgical intervention. These HR-QOL tools are very helpful for the evaluation of subjective outcome of common hepatobiliary procedures like gastrointestinal quality of life index (GIQLI) for cholecystectomy, functional assessment in cancer therapy (FACT) for liver resection, short form 36 (SF-36) for liver transplantation, and quality of life questionnaire for patients with pancreatic cancer (QLQ-PAN). CONCLUSIONS: Use of validated and reliable health instruments in hepatobiliary surgery is directed at measuring the impact in a reproducible and valid fashion. Curative or palliative procedures should be offered to the patients of hepatobiliary disorders after the assessment by HR-QOL tools. | 8 | Blood glucose response after oral intake of lactulose in healthy volunteers:A randomized,controlled,cross-over study显示文摘AIM To investigate possible changes of blood glucose levels after oral intake of lactulose in healthy subjects.METHODS The study was performed as prospective, randomized, two-part study with 4-way cross-over design with n = 12 in each study arm. Capillary blood glucose levels were determined over a time period of 180 min after intake of a single dose of 10 g or 20 g lactulose provided as crystal or liquid formulation. During the manufacturing process of lactulose, impurities with sugars(e.g., lactose, fructose, galactose) occur. Water and 20 g glucose were used as control and reference. Because lactulose is used as a functional food ingredient, it may also be consumed by people with impaired glucose tolerance, including diabetics. Therefore, it is of interest to determine whether the described carbohydrate impurities may increase blood glucose levels after ingestion. RESULTS The blood glucose concentration-time curves after intake of 10 g lactulose, 20 g lactulose, and water were almost identical. None of the three applications showed any changes in blood glucose levels. After intake of 20 g glucose, blood glucose concentration increased by approximately 3 mmol/L(mean Cmax = 8.3 mmol/L), reaching maximum levels after approximately 30 min and returning to baseline within approximately 90 min, which was significantly different to the corresponding 20 g lactulose formulations(P < 0.0001). Comparing the two lactulose formulations, crystals and liquid, in the dosage of 10 g and 20 g, there was no difference in the blood glucose profile and calculated pharmacokinetic parameters despite the different amounts of carbohydrate impurities(1.5% for crystals and 26.45% for liquid). Anyhow, the absolute amount of single sugars was low with 0.3 g in crystals and 5.29 g in liquid formulation in the 20 g dosages. Lactulose was well tolerated by most volunteers, and only some reported mild to moderate mainly gastrointestinal side effects. | 9 | Formation Mechanism and Numerical Model of Quartz in Fine-Grained Organic-Rich Shales: A Case Study of Wufeng and Longmaxi Formations in Western Hubei Province, South China显示文摘The difference in quartz types in shales not only affects the porosity and permeability of the rocks,but also reflects the difference in the sedimentary environments.We established the formation mechanism and numerical model of quartz in shales of Wufeng and Longmaxi formations in the Wangjiawan Section,South China,based on thin-section studies using SEM(scanning electron microscope),SEM-CL(cathodoluminescence),XRD(X-ray diffraction)and geochemical analyses.There are two types of quartz in the shales:detrital quartz and authigenic quartz.Detrital quartz is mostly silt-size,typically ranging from 10 to 60μm in size and subangular to angular monocrystal in shape,and brighter than authigenic quartz by CL intensity;authigenic quartz is present in two phases in shape:grain overgrowths and crystallite grains.Overgrowth surfaces are subhedral.Crystallite grains are typically less than 10μm in size,euhedral or subhedral monocrystal in shape.Authigenic quartz can be subdivided into biogenic quartz and clay mineral transformed quartz according to the source of silicon.In the numerical model,the content of detrital quartz is relatively consistent(20%);the content of biogenic quartz ranges from 40%to 70%,with a sharp fall(0–30%)in the Guanyinqiao mudstone.During the Katian,a lower anoxic and dense water column make the dissolution of biogenic silica well preserved.Biogenic quartz is the major contributor to the sediment.During the early Hirnantian interval,due to the drop of sea level and the oxygenation of seafloor,the sediment is mainly composed of clay transformed quartz and detrital quartz.During the latest Hirnatian and Rhuddanian,rapid sea level rise and anoxic ocean enhance the preservation of the biogenic silica,thereby biogenic quartz re-emerges as the major contributors to the sediment.Authigenic crystallite grains and grain overgrowths have filled in primary pore space and have decreased the interparticle porosity,however,as a rigid framework,they can suppress compaction and maintain the internal pore structure.The formation of authigenic quartz results in the increase of total quartz,which fortifies the brittleness of rocks and is beneficial to the development of shale gas. | Bao Zhang Detian Yan Hassan Jasmine Drawarh Xiangrong Yang Jin He Liwei Zhang | 2020 | Journal of Earth Science2020,31,2: | 4 |

Management of CMV infection and disease has evolved over the years,and clinical guidelines have been recently updated.Universal antiviral prophylaxis and a pre-emptive treatment strategy are options for prevention. A currentlyrecruiting randomized clinical trial is comparing the efficacy and safety of the two prevention strategies in the highest risk D+R- liver recipients. Drug-resistant CMV infection remains uncommon but is now increasing in incidence. | 18 | Influence of experimental soil disturbances on the diversity of plants in agricultural grasslands显示文摘Aims and Methods Disturbance is supposed to play an important role for biodiversity and ecosystem stability as described by the intermediate disturbance hypoth-esis(IDH),which predicts highest species richness at intermediate levels of disturbances.In this study,we tested the effects of artificial soil distur-bances on diversity of annual and perennial vascular plants and bryo-phytes in a field experiment in 86 agricultural grasslands differing in land use in two regions of Germany.On each grassland,we implemented four treatments:three treatments differing in application time of soil dis-turbances and one control.One year after experimental disturbance,we recorded vegetation and measured biomass productivity and bare ground.We analysed the disturbance response taking effects of region and land-use-accompanied disturbance regimes into account.Important Findings Region and land-use type strongly determined plant species rich-ness.Experimental disturbances had small positive effects on the species richness of annuals,but none on perennials or bryophytes.Bare ground was positively related to species richness of bryophytes.However,exceeding the creation of 12%bare ground further distur-bance had a detrimental effect on bryophyte species richness,which corresponds to the IDH.As biomass productivity was unaffected by disturbance our results indicate that the disturbance effect on spe-cies richness of annuals was not due to decreased overall produc-tivity,but rather due to short-term lowered inter-and intraspecific competition at the newly created microsites.Generally,our results highlight the importance of soil disturbances for species richness of annual plants and bryophytes in agricultural grasslands.However,most grasslands were disturbed naturally or by land-use practices and our additional experimental soil dis-turbances only had a small short-term effect.Overall,total plant diversity in grasslands seemed to be more limited by the availabil-ity of propagules rather than by suitable microsites for germination.Thus,nature conservation efforts to increase grassland diversity should focus on overcoming propagule limitation,for instance by additional sowing of seeds,while the creation of additional open patches by disturbance might only be appropriate where natural disturbances are scarce. | Jörg Müller Johannes Heinze Jasmin Joshi Steffen Boch Valentin H.Klaus Markus Fischer Daniel Prati | 2014 | Journal of Plant Ecology2014,7,6: | 3 |

| 20 | Long-term follow-up of liver alveolar echinococcosis using echinococcosis multilocularis ultrasound classification显示文摘BACKGROUND When Echinococcus multilocularis infects humans as a false intermediate host,alveolar echinococcosis(AE)usually manifests primarily intrahepatically and is initially asymptomatic.If the disease remains undiagnosed and untreated,progressive growth occurs,reminiscent of malignant tumours.The only curative therapy is complete resection,which is limited to localised stages,and palliative drug therapy is used otherwise.Consequently,early diagnosis and reliable detection of AE lesions are important.For this reason,abdominal ultrasonography,as the most common primary imaging for AE,relies on classification systems.AIM To investigate how hepatic AE lesion sonomorphology changes over time in the Echinococcosis Multilocularis Ulm Classification(EMUC)-ultrasound(US)classification.METHODS Based on data from Germany’s national echinococcosis database,we evaluated clinical and US imaging data for 59 patients according to the AE case definition in our preliminary retrospective longitudinal study.There had to be at least two liver sonographies≥6 mo apart,≥1 hepatic AE lesion,and complete documentation in all US examinations.The minimum interval between two separately evaluated US examinations was 4 wk.The AE reference lesion was the largest hepatic AE lesion at the time of the first US examination.To classify the sonomorphologic pattern,we used EMUC-US.In addition to classifying the findings of the original US examiner,all reference lesions at each examination time point were assigned EMUC-US patterns in a blinded fashion by two investigators experienced in US diagnosis.Statistical analysis was performed using SAS version 9.4(SAS Institute Inc.,Cary,NC,United Stated).P values<0.05 were considered statistically significant.RESULTS The preliminary study included 59 patients,38(64.5%)women and 21(35.6%)men.The mean age at initial diagnosis was 59.9±16.9 years.At the time of initial ultrasonography,a hailstorm pattern was present in 42.4%(25/59)of cases,a hemangioma-like pattern in 16.9%(10/59),a pseudocystic pattern in 15.3%(9/59),and a metastasis-like pattern in 25.4%(15/59).For the hailstorm pattern,the average lesion size was 67.4±26.3 mm.The average lesion size was 113.7±40.8 mm with the pseudocystic pattern and 83.5±27.3 mm with the hemangioma-like pattern.An average lesion size of 21.7±11.0 mm was determined for the metastasis-like pattern.Although the sonomorphologic pattern remained unchanged in 84.7%(50/59)of AE reference lesions,15.3%(9/59)showed a change over time.A change in pattern was seen exclusively for AE lesions initially classified as hemangioma-like or pseudocystic.A total of 70%(7/10)of AE lesions initially classified as hemangioma-like showed a relevant change in pattern over time,and 85.7%(6/7)of these were secondarily classified as having a hailstorm pattern,with the remainder(1/7;14.3%)classified as having a pseudocystic pattern.A total of 22.2%(2/9)of AE lesions initially classified as pseudocystic showed a relevant change in pattern over time and were classified as having a hailstorm pattern.For AE lesions initially classified as having a hailstorm or metastatic pattern,no pattern change was evident.All patients with pattern change were on continuous drug therapy with albendazole.CONCLUSION The sonomorphology of hepatic AE lesions may change over time.The hemangioma-like and pseudocystic patterns are affected. | Jasmin Schuhbaur Melissa Schweizer Jana Philipp Julian Schmidberger Patrycja Schlingeloff Wolfgang Kratzer | 2021 | World Journal of Gastroenterology2021,27,40: | 3 |
