1动脉瘤性蛛网膜下腔出血患者的重症监护处理:神经危重症监护学会多学科共识会议的推荐意见显示文摘蛛网膜下腔出血(subarachnoidhemorrhage,SAH)是一种可对中枢神经系统造成毁灭性打击并对其他多个器官产生显著影响的急性脑血管病。SAH患者被常规收入重症监护病房并由多学科团队进行治疗。由于高质量研究证据的缺乏,使得治疗方法各异,可供选择的指导意见不多。现有的指南主要强调危险因素、预防、自然史以及再出血的预防,很少涉及SAH患者的重症监护问题。美国神经危重症监护学会组织了一次国际性多学科共识会议来探讨SAH的重症监护处理。根据发表的文章和研究领域,此次会议召集了来自欧洲和北美地区的神经重症监护科、神经外科、神经内科、介入神经放射科和神经麻醉科方面的专家。根据临床经验和制定实践指南方面的经验,选择4名经验丰富的神经重症监护专家组成评判委员会。应用推荐分级的评估、制定与评价(GradesofRecommendationsAssessment,DevelopmentandEvaluation,GRADE)系统进行文献回顾,结合参会者的经验、评判委员会的审评以及文献讨论产生推荐意见。应用GRADE系统制定推荐意见,其制定原则不仅强调研究资料的质量,而且还重视利弊权衡与实践转化。对SAH患者日常处理过程中面临的所有问题均提供指导和推荐意见,即使缺乏高质量的证据。Michael N. 3Interleukin-6 mediates neutrophil mobilization from bone marrow in pulmonary hypertension显示文摘Myeloid cells,such as neutrophils,are produced in the bone marrow in high quantities and are important in the pathogenesis of vascular diseases such as pulmonary hypertension(PH).Although neutrophil recruitment into sites of inflammation has been well studied,the mechanisms of neutrophil egress from the bone marrow are not well understood.Using computational flow cytometry,we observed increased neutrophils in the lungs of patients and mice with PH.Moreover,we found elevated levels of IL-6 in the blood and lungs of patients and mice with PH.We observed that transgenic mice overexpressing Il-6 in the lungs displayed elevated neutrophil egress from the bone marrow and exaggerated neutrophil recruitment to the lungs,resulting in exacerbated pulmonary vascular remodeling,and dysfunctional hemodynamics.Mechanistically,we found that IL-6-induced neutrophil egress from the bone marrow was dependent on interferon regulatory factor 4(IRF-4)-mediated CX3CR1 expression in neutrophils.Consequently,Cx3cr1 genetic deficiency in hematopoietic cells in Il-6-transgenic mice significantly reduced neutrophil egress from bone marrow and decreased neutrophil counts in the lungs,thus ameliorating pulmonary remodeling and hemodynamics.In summary,these findings define a novel mechanism of IL-6-induced neutrophil egress from the bone marrow and reveal a new therapeutic target to curtail neutrophil-mediated inflammation in pulmonary vascular disease.Jonathan Florentin Jingsi Zhao Yi-Yin Tai Sathish Babu Vasamsetti Scott P.O’Neil Rahul Kumar Anagha Arunkumar Annie Watson John Sembrat Grant C.Bullock Linda Sanders Biruk Kassa Mauricio Rojas Brian B.Graham Stephen Y.Chan Partha Dutta 2021Cellular & Molecular Immunology2021,18,2:3

9Anti-cancer effects of aloe-emodin:a systematic review显示文摘Background:Anthraquinones are a possible treatment option for oncological patients due to their anti-cancer properties.Cancer patients often exhaust a plethora of resources that ultimately fail to provide fully curative measures.Alternative treatments are subsequently sought in the hope of finding a therapeutic remedy.Potential regimens include aloe-emodin and its related derivatives.This review therefore summarizes the effects of aloe-emodin and other aloe components in light of their anti-proliferative and anti-carcinogenic properties.Methods:A systematic search was performed in PubMed for aloe-emodin and cancer in humans.Sixty abstracts of in vitro studies were selected and reviewed with subsequent screening of the full text.Thirty-eight articles were summarized.Results:Aloe-emodin possesses multiple anti-proliferative and anti-carcinogenic properties in a host of human cancer cell lines,with often multiple vital pathways affected by the same molecule.The most notable effects include inhibition of cell proliferation,migration,and invasion;cycle arrest;induction of cell death;mitochondrial membrane and redox perturbations;and modulation of immune signaling.The effects of aloe-emodin are not ubiquitous across all cell lines but depend on cell type.Conclusions:On the basis of this systematic review,the multiple consistent effects of aloe-emodin in human-derived cancer cell lines suggest that aloe-emodin is a potential anti-cancer agent that acts on cancer cells in a pleiotropic manner.Relevance for patients:Cancer patients often utilize alternative therapies as a result of suboptimal efficacy of conventional treatments.Aloe-emodin might become a therapeutic option for cancer patients if the basic research is confirmed in clinical trials.Brian Sanders Anna M.Ray Sharon Goldberg Tyler Clark H.Reginald McDaniel Steven E.Atlas Ashar Farooqi Janet Konefal Lucas C.Lages Johanna Lopez Ammar Rasul Eduard Tiozzo Judi M.Woolger John E.Lewis 2017Journal of Clinical & Translational Research2017,3,3:0
