1Minimizing treatment complexity of combat-related soft tissue injuries using a dedicated tension relief system and negative pressure therapy augmented by high-dose in situ antibiotic therapy and oxygen delivery: a retrospective study显示文摘Background:Following combat-related,extensive soft tissue injury from gunshot wounds or blasts,prolonged duration from injury to full wound closure is associated with infection,increased morbidity and mortality,failure to mobilize,poor functional outcome and increased cost.The purpose of this study was to evaluate a novel treatment enabling early primary closure of combat wounds.Methods:This was a retrospective study of 10 soldiers and civilians with extensive combat-related soft tissue limb injuries(5 gunshot wounds,5 blasts)treated using the TopClosure^(■)Tension Relief System(TRS)with simultaneous administration of regulated oxygen-enriched and irrigation negative pressure-assisted wound therapy(ROINPT)via the Vcareα^(■)device.Results:Nine patients were treated during the acute phase of injury and one was treated following removal of a flap due to deep infection 20 years after injury and flap reconstruction.Two patients had upper limb injury and the rest lower limb injury.With the aid of the TRS and/or ROINPT,immediate primary closure during reconstruction was achieved in 6 patients and delayed primary closure in three.Only one patient required a skin graft to close a small area of the wound after most of the wound had been closed by delayed primary closure.Wound closure was achieved within 0–37 days(median:12.5 days,interquartile range:2.75–19.75)from injury.Conclusions:The TRS is a novel device for effective,early skin stretching and secure wound closure through the application of stress relaxation and mechanical creep,achieving primary closure of large defects using a simplified surgical technique and reducing the need for closure using skin grafts and flaps and the use of tissue expanders.Delivering supplemental oxygen to the wound by ROINPT reverses the reduced oxygen levels inherent in conventional negative pressure-assisted wound therapy,mitigating anaerobic contamination and reducing infection.Irrigation may accelerate the evacuation of infectious material from the wound and provide a novel method for antibiotic administration.The combination of TRS and ROINPT devices allow for early primary closure with improved functionality of combat-related limb injuries.Moris Topaz Itamar Ashkenazi Oren Barzel Seema Biswas Dan Atar Nurit Shadmi Itzhak Siev-Ner 2021Burns & Trauma2021,9,1:7

3Urban Park Soil and Vegetation: Effects of Natural and Anthropogenic Factors显示文摘Various soil surface components, such as trees, shrubs and biological crusts, and human recreational activities, e.g., barbecues and trampling by visitors, may divide the area of the urban park into smaller fragments/microenvironments, differentiated by their microenvironmental conditions, which may differ in soil and vegetation characteristics. The spatial changes in the soil and vegetation characteristics and their causes were investigated in an urban park located south of Tel Aviv-Jaffa, Israel. The area of the park is 0.5km2, including groves, a lake, lawns, and rest areas. Soil was sampled in nine microenvironments, of which seven were within the park:under Ceratonia siliqua trees(Cs U), under Ficus sycomorus trees(Fi U), rest area between tables under F. sycomorus(Fi B), rest area under tables under F. sycomorus(Fi T), open area with bare soil(Oa S), open area with biological crust cover(Oa C), and open area covered by herbaceous vegetation(Oa V). Two more microenvironments, planar and sloping open areas(Co P and Co S, respectively) in the vicinity of the park, were used as the controls. Electrical conductivity, concentrations of soluble ions(Mg2+, Ca2+, Na+, K+and HCO-3), p H, contents of organic carbon, calcium carbonate and moisture, and grain size distribution were determined. In addition,herbaceous vegetation cover, number and diversity of herbaceous vegetation species were measured. It was found that soil properties and herbaceous vegetation characteristics varied within the park. Soil organic carbon, electrical conductivity, soluble salts, penetration depth, and vegetation characteristics were affected by human activities, mainly in the rest area between the tables. In contrast, the above characteristics were affected by natural factors mainly in the rest of the microenvironments, which were subjected to low levels of anthropogenic intervention. 6Transnasal endoscopy: Technical considerations, advantages and limitations显示文摘Transnasal endoscopy(TNE) is an upper endoscopy method which is performed by the nasal route using a thin endoscope less than 6 mm in diameter. The primary goal of this method is to improve patient tolerance and convenience of the procedure. TNE can be performed without sedation and thus eliminates the risks associated with general anesthesia. In this way,TNE decreases the cost and total duration of endoscopic procedures, while maintaining the image quality of standard caliber endoscopes, providing good results for diagnostic purposes. However, the small working channel of the ultra-thin endoscope used for TNE makes it difficult to use for therapeutic procedures except in certain conditions which require a thinner endoscope.Biopsy is possible with special forceps less than 2 mm in diameter. Recently, TNE has been used for screening endoscopy in Far East Asia, including Japan. In most controlled studies, TNE was found to have better patient tolerance when compared to unsedated endoscopy. Nasal pain is the most significant symptom associated with endoscopic procedures but can be reduced with nasal pretreatment. Despite the potential advantage of TNE, it is not common in Western countries, usually due to a lack of training in the technique and a lack of awareness of its potential advantages.
