| 1 | 颅内出血患者抗栓药逆转指南美国神经重症监护学会和美国重症医学会对医疗卫生专业人员的声明显示文摘背景抗栓药的使用,包括抗凝药、抗血小板药和溶栓药,在过去10年中日益增多并且预计将继续上升。虽然抗栓药相关性颅内出血可能是灾难性的,但快速逆转凝血功能障碍可能有助于限制血肿增大和改善转归。方法美国神经重症监护学会联合美国重症医学会组建了一个国际性多机构委员会,涉及的专业领域包括神经重症监护、神经内科、神经外科、卒中、血液学、血液病理学、急诊医学、药学、护理和指南制定,目的是进行文献评价并制定一份循证实践指南。该委员会进行了正式的文献检索,并对符合标准的研究进行了评价。结果利用GRADE方法学,委员会提出了在颅内出血情况下逆转维生素K拮抗药、直接Xa因子拮抗药、直接凝血酶抑制药、普通肝素、低分子肝素、类肝素、戊多糖、溶栓药和抗血小板药的推荐意见。结论该指南提供了及时和循证的抗栓药逆转策略以协助临床医生治疗抗栓药相关性颅内出血。 | Jennifer A. | 9 | Controversial role of arthroscopic meniscectomy of the knee: A review显示文摘The role of arthroscopic partial meniscectomy(APM) in reducing pain and improving function in patients with meniscal tears remains controversial. Five recent highquality randomized controlled trials(RCTs) compared non-operative management of meniscal tears to APM, with four showing no difference and one demonstrating superiority of APM. In this review, we examined the strengths and weaknesses of each of these RCTs, with particular attention to the occurrence of inadvertent biases. We also completed a quantitative analysis that compares treatment successes in each treatment arm, considering crossovers as treatment failures. Our analysis revealed that each study was an excellent attempt to compare APM with non-surgical treatment but suffered from selection, performance, detection, and/or transfer biases that reduce confidence in its conclusions. While the RCT remains the methodological gold standard for establishing treatment efficacy, the use of an RCT design does not in itself ensure internal or external validity. Furthermore, under our alternative analysis of treatment successes, two studies had significantly more treatment successes in the APM arm than the non-operative arm although original intention-totreat analyses showed no difference between these two groups. Crossovers remain an important problem in surgical trials with no perfect analytical solution. With the studies available at present, no conclusion can be drawn concerning the optimal treatment modality for meniscal tears. | 16 | Population-based frequency of surfactant dysfunction mutations in a native Chinese cohort显示文摘Background: Rare mutations in surfactant-associatedgenes contribute to neonatal respiratory distress syndrome.The frequency of mutations in these genes in the Chinesepopulation is unknown.Methods: We obtained blood spots from the GuangxiNeonatal Screening Center in Nanning, China thatincluded Han (n=443) and Zhuang (n=313) ethnic groups.We resequenced all exons of the surfactant proteins-B(SFTPB), -C (SFTPC), and the ATP-binding cassettemember A3 (ABCA3) genes and compared the frequenciesof 5 common and all rare variants.Results: We found minor differences in thefrequencies of the common variants in the Han andZhuang cohorts. We did not find any rare mutations inSFTPB or SFTPC, but we found three ABCA3 mutationsin the Han [minor allele frequency (MAF)=0.003] and 7 inthe Zhuang (MAF=0.011) cohorts (P=0.10). The ABCA3mutations were unique to each cohort;five were novel.The collapsed carrier rate of rare ABCA3 mutations inthe Han and Zhuang populations combined was 1.3%,which is signifi cantly lower than that in the United States(P<0.001).Conclusions: The population-based frequency ofmutations in ABCA3 in south China newborns is signifi cantlylower than that in United States. | 19 | Assessing gully erosion and rehabilitation using multi temporal LiDAR DEMs:Case study from the Great Barrier Reef catchments,Australia显示文摘Millions of dollars are being spent on gully rehabilitation to help reduce excess fine sediment delivery to the Great Barrier Reef(GBR).There is an urgent need for(i)prioritisation of active gullies for rehabilitation and(ii)the development of methodologies to inform the effectiveness of remediation.In this study we analyse DEMs of Difference derived from 0.5 m resolution 2-3 year interval multi-temporal LiDAR data collected pre and post rehabilitation at three variable gully morphologies in the Burdekin catchment.Our analysis indicates that the highest annual average fine sediment erosion rates for the untreated control gullies occur at the linear gully(53.38 t ha^(-1)y^(-1))followed by linear-alluvial gully(34.24 t ha^(-1)y^(-1))and least at the alluvial gully(14.41 t ha^(-1)y^(-1)).The proportional loss or export of fine sediment from the gullies in their un-treated condition ranges from∼68 to 90%of what is eroded,and when the gullies are treated the proportion of fine sediment that is retained in the gully proportional to what is eroded increases to∼60%at all sites.Without pre-treatment baseline erosion rates,and additional post treatment LiDAR captures,it is difficult to quantify the treatment effectiveness.Our results offer insights in the erosion mechanisms within different geomorphic gully morphologies and rehabilitation effects in these erosional landforms.This study provides crucial knowledge of gully dynamics that can be coupled with other lines of evidence for better prioritisation of rehabilitation in the GBR catchments. | Sana Khan Rebecca Bartley Anne Kinsey-Henderson Aaron Hawdon | 2024 | International Soil and Water Conservation Research2024,12,1: | 0 |
