| 3 | Percutaneous endoscopic gastrostomy:Indications,technique,complications and management显示文摘Percutaneous endoscopic gastrostomy(PEG)is the preferred route of feeding and nutritional support in patients with a functional gastrointestinal system who require long-term enteral nutrition.Besides its wellknown advantages over parenteral nutrition,PEG offers superior access to the gastrointestinal system over surgical methods.Considering that nowadays PEG tube placement is one of the most common endoscopic procedures performed worldwide,knowing its indications and contraindications is of paramount importance in current medicine.PEG tubes are sometimes placed inappropriately in patients unable to tolerate adequate oral intake because of incorrect and unrealistic understanding of their indications and what they can accomplish.Broadly,the two main indications of PEG tube placement are enteral feeding and stomach decompression.On the other hand,distal enteral obstruction,severe uncorrectable coagulopathy and hemodynamic instability constitute the main absolute contraindications for PEG tube placement in hospitalized patients.Although generally considered to be a safe procedure,there is the potential for both minor and major complications.Awareness of these potential complications,as well as understanding routine aftercare of the catheter,can improve the quality of care for patients with a PEG tube.These complications can generally be classified into three major categories:endoscopic technical difficulties,PEG procedure-related complications and late complications associated with PEG tube use and wound care.In this review we describe a variety of minor and major tube-related complications as well as strategies for their management and avoidance.Different methods of percutaneous PEG tube placement into the stomach have been described in the literature with the'pull'technique being the most common method.In the last section of this review,the reader is presented with a brief discussion of these procedures,techniques and related issues.Despite the mentioned PEG tube placement complications,this procedure has gained worldwide popularity as a safe enteral access for nutrition in patients with a functional gastrointestinal system. | Ata A Rahnemai-Azar Amir A Rahnemaiazar Rozhin Naghshizadian Amparo Kurtz Daniel T Farkas | 2014 | World Journal of Gastroenterology2014,20,24: | 38 |
| 4 | Rubber band ligation for 750 cases of symptomatic hemorrhoids out of 2200 cases显示文摘AIM: To study the results for the treatment of symptomatic hemorrhoids using rubber band ligation (RBL) method. METHODS: A retrospective study for 750 patients who came to the colorectal unit from June, 1998 to September, 2006, data was retrieved from archived fi les. RBL was performed using the Mc Gown applicator on an outpatient basis. The patients were asked to return to out-patient clinic for follow up at 2 wk, 1 mo, 6 mo and through telephone call every 6 mo for 2 years). RESULTS: After RBL, 696 patients (92.8%) were cured with no difference in outcome for second or third degree hemorrhoids (P = 0.31). Symptomatic recurrence was detected in 11.04% after 2 years. A total of 52 patients (6.93%) had 77 complications from RBL which required no hospitalization. Complications were pain, rectal bleeding and vaso-vagal symptoms(4.13%, 4.13% and 1.33% of patients, respectively). At 1 mo there were a significant improvement in mean SF-36 scores over baseline in five items, while after 2 years there were improvement in all items over baseline, but not significant. No significant manometeric changes after band ligation. CONCLUSION: RBL is a simple, safe and effective method for treating symptomatic second and third degree hemorrhoids as an out patient procedure with signifi cant improvement in quality of life. | 5 | Machine learning in geosciences and remote sensing显示文摘Learning incorporates a broad range of complex procedures. Machine learning(ML) is a subdivision of artificial intelligence based on the biological learning process. The ML approach deals with the design of algorithms to learn from machine readable data. ML covers main domains such as data mining, difficultto-program applications, and software applications. It is a collection of a variety of algorithms(e.g. neural networks, support vector machines, self-organizing map, decision trees, random forests, case-based reasoning, genetic programming, etc.) that can provide multivariate, nonlinear, nonparametric regression or classification. The modeling capabilities of the ML-based methods have resulted in their extensive applications in science and engineering. Herein, the role of ML as an effective approach for solving problems in geosciences and remote sensing will be highlighted. The unique features of some of the ML techniques will be outlined with a specific attention to genetic programming paradigm. | 6 | Clinical outcomes of isolated renal failure compared to other forms of organ failure in patients with severe acute pancreatitis显示文摘AIM To assess differences in clinical outcomes of isolated renal failure(RF) compared to other forms of organ failure(OF) in patients with severe acute pancreatitis(SAP).METHODS Using a prospectively maintained database of patients with acute pancreatitis admitted to a tertiary medical center between 2003 and 2016, those with evidence of persistent OF were classified to renal, respiratory, cardiovascular, or multi-organ(2 or more organs). Data regarding demographics, comorbidities, etiology of acute pancreatitis, and clinical outcomes were prospectively recorded. Differences in clinical outcomes after development of isolated RF in comparison to other forms of OF were determined using independent t and Mann-Whitney U tests for continues variables, and χ~2 test for discrete variables.RESULTS Among 500 patients with acute pancreatitis, 111 patients developed persistent OF: mean age was 54 years, and 75(67.6%) were male. Forty-three patients had isolated OF: 17(15.3%) renal, 25(21.6%) respiratory, and 1(0.9%) patient with cardiovascular failure. No differences in demographics, etiology of acute pancreatitis, systemic inflammatory response syndrome scores, or development of pancreatic necrosis were seen between patients with isolated RF vs isolated respiratory failure. Patients with isolated RF were less likely to require nutritional support(76.5% vs 96%, P = 0.001), ICU admission(58.8% vs 100%, P = 0.001), and had shorter mean ICU stay(2.4 d vs 15.7 d, P < 0.001), compared to isolated respiratory failure. None of the patients with isolated RF or isolated respiratory failure died.CONCLUSION Among patients with SAP per the Revised Atlanta Classification, approximately 15% develop isolated RF. This subgroup seems to have a less protracted clinical course compared to other forms of OF. | 8 | Current status of adoption of no-till farming in the world and some of its main benefits显示文摘In 1999 no-tillage farming,synonymous of zero tillage farming or conservation agriculture,was adopted on about 45 million ha world wide,growing to 72 million ha in 2003 and to 111 million ha in 2009,corresponding to an growth rate of 6 million ha per annum.Fastest adoption rates have been experienced in South America where some countries are using no-tillage farming on about 70%of the total cultivated area.Opposite to countries like the USA where often fields under no-tillage farming are tilled every now and then,more than two thirds of the area under no-tillage systems in South America is permanently not tilled;in other words once adopted,the soil is never tilled again.The spread of no-tillage systems on more than 110 million ha world-wide shows the great adaptability of the systems to all kinds of climates,soils and cropping conditions.No-tillage is now being practiced from the artic circle over the tropics to about 50ºlatitude south,from sea level to 3,000 m altitude,from extremely rainy areas with 2,500 mm a year to extremely dry conditions with 250 mm a year.No-till farming offers a way of optimizing productivity and ecosystem services,offering a wide range of economic,environmental and social benefits to the producer and to the society.At the same time,no-till farming is enabling agriculture to respond to some of the global challenges associated with climate change,land and environmental degradation,and increasing cost of food,energy and production inputs.The wide recognition of no-till farming as a truly sustainable system should ensure the spread of the no-till technology and the associated practices of organic soil cover and crop rotation,as soon as the barriers to its adoption have been overcome,to areas where adoption is currently still low.The widespread adoption globally also shows that no-tillage farming cannot any more be considered a temporary fashion or a craze;instead largely through farmers’own effort,the system has established itself as a farming practice and a different way of thinking about sustainable agro-ecosystem management that can no longer be ignored by scientists,academics,extension workers,farmers at large as well as equipment and machine manufacturers and politicians. | Rolf Derpsch Theodor Friedrich Amir Kassam Li Hongwen | 2010 | International Journal of Agricultural and Biological Engineering2010,3,1: | 18 |

| 10 | Adenoviral gene therapy in gastric cancer: A review显示文摘Gastric cancer is one of the most common malignancies worldwide. With current therapeutic approaches the prognosis of gastric cancer is very poor, as gastric cancer accounts for the second most common cause of death in cancer related deaths. Gastric cancer like almost all other cancers has a molecular genetic basis which relies on disruption in normal cellular regulatory mechanisms regarding cell growth, apoptosis and cell division. Thus novel therapeutic approaches such as gene therapy promise to become the alternative choice of treatment in gastric cancer. In gene therapy, suicide genes, tumor suppressor genes and anti-angiogenesis genes among many others are introduced to cancer cells via vectors. Some of the vectors widely used in gene therapy are Adenoviral vectors. | 11 | Fluid overload as a major target in management of cardiorenal syndrome:Implications for the practice of peritoneal dialysis显示文摘Congestion is an integral component of cardiorenal syndrome and portends an adverse impact on the outcomes.Recent studies suggest that congestion has the ability of modulating the interactions between the kidney and the heart in this setting.Peritoneal dialysis(PD) is a homebased therapeutic modality that is not only offered to patients with end-stage renal disease to provide solute clearance and ultrafiltration,but it has also been used in patients with refractory heart failure and fluid overload to help optimize volume status.Several uncontrolled studies and case series have so far evaluated the role of PD in management of hypervolemia for patients with heart failure.They have generally reported favorable results in this setting.However,the data on the outcomes of patients with end-stage renal disease and concomitant heart failure is mixed,and the proposed theoretical advantages of PD might not translate into improved clinical endpoints.Congestion is prevalent in this patient population and has a significant effect on their survival.As studies suggest that a significant subset of patients with end-stage renal disease who receive PD therapy are hypervolemic,suboptimal management of congestion could at least in part explain these conflicting results.PD is a highly flexible therapeutic modality and the choice of techniques,regimens,and solutions can affect its ability for optimization of fluid status.This article provides an overview of the currently available data on the role and clinical relevance of congestion in patients with cardiorenal syndrome and reviews potential options to enhance decongestion in these patients. | Amir Kazory | 2017 | World Journal of Nephrology2017,6,4: | 16 |
| 12 | Factors affecting healing after arthroscopic rotator cuff repair显示文摘Rotator cuff repair has been shown to have good longterm results. Unfortunately, a significant proportion of repairs still fail to heal. Many factors, both patient and surgeon related, can influence healing after repair. Older age, larger tear size, worse muscle quality, greater muscle-tendon unit retraction, smoking, osteoporosis, diabetes and hypercholesterolemia have all shown to negatively influence tendon healing. Surgeon related factors that can influence healing include repair construct-single vs double row, rehabilitation, and biologics including platelet rich plasma and mesenchymalstem cells. Double-row repairs are biomechanically stronger and have better healing rates compared with single-row repairs although clinical outcomes are equivalent between both constructs. Slower, less aggressive rehabilitation programs have demonstrated improved healing with no negative effect on final range of motion and are therefore recommended after repair of most full thickness tears. Additionally no definitive evidence supports the use of platelet rich plasma or mesenchymal stem cells regarding improvement of healing rates and clinical outcomes. | 16 | Systematic review and meta-analysis on the prophylactic role of non-steroidal anti-inflammatory drugs to prevent post-endoscopic retrograde cholangiopancreatography pancreatitis显示文摘AIM: To critically appraise the published randomized, controlled trials on the prophylactic effectiveness of the non-steroidal anti-inflammatory drugs(NSAIDs), in reducing the risk of post-endoscopic retrograde cholangiopancreatography(ERCP) pancreatitis. METHODS: A systematic literature search(MEDLINE, Embase and the Cochrane Library, from inception of the databases until May 2015) was conducted to identify randomized, clinical trials investigating the role of NSAIDs in reducing the risk of post-ERCP pancreatitis. Random effects model of the meta-analysis was carried out, and results were presented as odds ratios(OR) with corresponding 95%CI.RESULTS: Thirteen randomized controlled trials on 3378 patients were included in the final meta-analysis. There were 1718 patients in the NSAIDs group and 1660 patients in non-NSAIDs group undergoing ERCP. The use of NSAIDs(through rectal route or intramuscular route) was associated with the reduced risk of post-ERCP pancreatitis [OR, 0.52(0.38-0.72), P = 0.0001]. The use of pre-procedure NSAIDs was effective in reducing approximately 48% incidence of post-ERCP pancreatitis, number needed to treat were 16 with absolute risk reduction of 0.05. But the risk of post-ERCP pancreattis was reduced by 55% if NSAIDs were administered after procedure. | 18 | Seismic isolation effect of lined circular tunnels with damping treatments显示文摘The Havriliak-Negami model for dynamic viscoelastic material behavior and Biot’s theory of poroelasticity are employed to develop an exact solution for three-dimensional scattering effect of harmonic plane P–SV waves from a circular cavity lined with a multilayered fluid-filled shell of infinite length containing viscoelastic damping materials and embedded within a fluid-saturated permeable surrounding soil medium. The analytical results are illustrated with numerical examples where the effects of liner/coating structural arrangement, viscoelastic material properties, liner-soil interface bonding condition, seismic excitation frequency, and angle of incidence on the induced dynamic stress concentrations are evaluated and discussed to obtain representative values of the parameters that characterize the system. It is demonstrated that incorporating viscoelastic damping materials with a low shear modulus in the constrained layer configuration is an efficient means of enhancing the overall seismic isolation performance, especially for near-normally incident seismic shear waves where the amplitudes of induced dynamic stresses may be reduced by up to one-third of those without isolation in a relatively wide frequency range. | 19 | TREM-1 multimerization is essential for its activation on monocytes and neutrophils显示文摘The triggering receptor expressed on myeloid cells-1(TREM-1)is a receptor expressed on innate immune cells.By promoting the amplification of inflammatory signals that are initially triggered by Toll-like receptors(TLRs),TREM-1 has been characterized as a major player in the pathophysiology of acute and chronic inflammatory diseases,such as septic shock,myocardial infarction,atherosclerosis,and inflammatory bowel diseases.However,the molecular events leading to the activation of TREM-1 in innate immune cells remain unknown.Here,we show that TREM-1 is activated by multimerization and that the levels of intracellular Ca 2+release,reactive oxygen species,and cytokine production correlate with the degree of TREM-1 aggregation.TREM-1 activation on primary human monocytes by LPS required a two-step process consisting of upregulation followed by clustering of TREM-1 at the cell surface,in contrast to primary human neutrophils,where LPS induced a rapid cell membrane reorganization of TREM-1,which confirmed that TREM-1 is regulated differently in primary human neutrophils and monocytes.In addition,we show that the ectodomain of TREM-1 is able to homooligomerize in a concentration-dependent manner,which suggests that the clustering of TREM-1 on the membrane promotes its oligomerization.We further show that the adapter protein DAP12 stabilizes TREM-1 surface expression and multimerization.TREM-1 multimerization at the cell surface is also mediated by its endogenous ligand,a conclusion supported by the ability of the TREM-1 inhibitor LR12 to limit TREM-1 multimerization.These results provide evidence for ligand-induced,receptor-mediated dimerization of TREM-1.Collectively,our findings uncover the mechanisms necessary for TREM-1 activation in monocytes and neutrophils. | Kevin Carrasco Amir Boufenzer Lucie Jolly Helene Le Cordier Guanbo Wang Albert JR Heck Adelheid Cerwenka Emilie Vinolo Alexis Nazabal Alexandre Kriznik Pierre Launay Sebastien Gibot Marc Derive | 2019 | Cellular & Molecular Immunology2019,16,5: | 13 |
