| 5 | Immune response after photodynamic therapy increases anti-cancer and anti-bacterial effects显示文摘Photodynamic therapy(PDT) is a clinically approved procedure for treatment of cancer and infections. PDT involves systemic or topical administration of a photosensitizer(PS), followed by irradiation of the diseased area with light of a wavelength corresponding to an absorbance band of the PS. In the presence of oxygen, a photochemical reaction is initiated, leading to the generation of reactive oxygen species and cell death. Besides causing direct cytotoxic effects on illuminated tumor cells, PDT is known to cause damage to the tumor vasculature and induce the release of pro-inflammatory molecules. Pre-clinical and clinical studies have demonstrated that PDT is capable of affecting both the innate and adaptive arms of the immune system. Immune stimulatory properties of PDT may increase its beneficial effects giving the therapy wider potential to become more extensively used in clinical practice. Be-sides stimulating tumor-specific cytotoxic T-cells capable to destroy distant untreated tumor cells, PDT leads to development of anti-tumor memory immunity that can potentially prevent the recurrence of cancer. | 6 | Expression of inducible nitric oxide synthase and cyclooxygenase-2 in pancreatic adenocarcinoma:Correlation with microvessel density显示文摘AIM: Cydooxygenases (COX) are key enzymes for conversion of arachidonic acid to prostaglandins. Nitric oxide synthase (NOS) is the enzyme responsible for formation of nitric oxide.Both have constitutive and inducible isoforms. The inducible isoforms (iNOS and COX-2) are of great interest as regulators of tumor angiogenesis, tumorigenesis and inflammatory processes. This study was to clarify their role in pancreatic adenocarcinomas.METHODS: We investigated the immunohistochemical iNOS and COX-2 expression in 40 pancreatic ductal adenocarcinomas of different grade and stage. The results were compared with microvessel density and clinicopathological data.RESULTS: Twenty-one (52.5%) of the cases showed iNOS expression, 15 (37.5%) of the cases were positive for COX-2.The immunoreaction was heterogeneously distributed within the tumors. Staining intensity was different between the tumors. No correlation between iNOS and COX-2 expression was seen. There was no relationship with microvessel density.However, iNOS positive tumors developed more often distant metastases and the more malignant tumors showed a higher COX-2 expression. There was no correlation with other clinicopathological data.CONCLUSION: Approximately half of the cases expressed iNOS and COX-2. These two enzymes do not seem to be the key step in angiogenesis or carcinogenesis of pancreatic adenocarcinomas. | 15 | Three-dimensional analysis of the physiological foramen geometry of maxillary and mandibular molars by means of micro-CT显示文摘The aim of this study was to investigate the physiological foramen diameter, shape and distance between physiological and anatomical apex of maxillary and mandibular first and second molars. Accurate knowledge of the physiological foramina morphology; thus, inherent mechanical shaping technical hindrances, is decisive when taking the corresponding root canal final preparation decision. The morphological dimensions of a total of 1 727 physiological foramina were investigated by means of micro-computed tomography. Mean narrow and wide(to a high number, oval) diameters of the physiological foramen were 0.24,0.22 and 0.33 mm and 0.33, 0.31 and 0.42 mm in mesiobuccal(MB), distobuccal(DB) and palatal(P) roots in maxillary first molars; 0.24, 0.22 and 0.33 mm and 0.41, 0.33 and 0.44 in MB, DB, and P roots in maxillary second molars. Mandibular first molars showed mean narrow and wide diameters of 0.24 and 0.30 mm and of 0.39 and 0.46 mm in mesial(M) and distal(D)roots; second mandibular molars showed 0.25 and 0.31 mm and 0.47 mm in M and D roots. The mean distance between the physiological foramina and anatomical apex was 0.82, 0.81 and 1.02 mm and 0.54, 0.43 and 0.63 mm in MB, DB and P roots of the maxillary first and second molars, respectively. A mean distance of 0.95 mm(M) and 1.05 mm(D) in the first and0.78 mm(M) and 0.81 mm(D) in the second mandibular molars was observed. Based on the results obtained, assumable recommendations for final preparation size of the physiological foramen were calculated. At present, however, it is unknown, which further(side) effects can be expected during orthodontic treatment at a nicotine exposure corresponding to that of an average European smoker. 63 male Fischer344 rats were randomized in three consecutive experiments of 21 animals each(A/B/C) to 3 experimental groups(7 rats, 1/2/3):(A) cone-beam-computed tomography(CBCT);(B) histology/serology;(C) reversetranscription quantitative real-time polymerase chain reaction(RT-q PCR)/cotinine serology—(1) control;(2) orthodontic tooth movement(OTM) of the first and second upper left molar(Ni Ti closed coil spring, 0.25 N);(3) OTM with 1.89 mg·kg^(-1) per day s.c. of L(-)-nicotine. After 14 days of OTM, serum cotinine and IL-6 concentration as well as orthodontically induced inflammatory root resorption(OIIRR), osteoclast activity(histology), orthodontic tooth movement velocity(CBCT, within 14 and 28 days of OTM)and relative gene expression of known inflammatory and osteoclast markers were quantified in the dental-periodontal tissue(RT–q PCR). Animals exposed to nicotine showed significantly heightened serum cotinine and IL-6 levels corresponding to those of regular European smokers. Both the extent of root resorption, osteoclast activity, orthodontic tooth movement and gene expression of inflammatory and osteoclast markers were significantly increased compared to controls with and without OTM under the influence of nicotine. We conclude that apart from increased periodontal bone loss, a progression of dental root resorption and accelerated orthodontic tooth movement are to be anticipated during orthodontic therapy, if nicotine consumption is present. | 18 | Comparative effectiveness of i-SCAN^(TM) and high-definition white light characterizing small colonic polyps显示文摘AIM:To evaluate accuracy of in vivo diagnosis of adenomatous vs non-adenomatous polyps using i-SCAN digital chromoendoscopy compared with high-definition white light.METHODS:This is a single-center comparative effectiveness pilot study.Polyps(n = 103) from 75 averagerisk adult outpatients undergoing screening or surveillance colonoscopy between December 1,2010 and April 1,2011 were evaluated by two participating endoscopists in an academic outpatient endoscopy center.Polyps were evaluated both with high-definition white light and with i-SCAN to make an in vivo prediction of adenomatous vs non-adenomatous pathology.We determined diagnostic characteristics of i-SCAN and highdefinition white light,including sensitivity,specificity,and accuracy,with regards to identifying adenomatous vs non-adenomatous polyps.Histopathologic diagnosis was the gold standard comparison.RESULTS:One hundred and three small polyps,detected from forty-three patients,were included in the analysis.The average size of the polyps evaluated in the analysis was 3.7 mm(SD 1.3 mm,range 2 mm to 8 mm).Formal histopathology revealed that 54/103(52.4%) were adenomas,26/103(25.2%) were hyperplastic,and 23/103(22.3%) were other diagnoses include 'lymphoid aggregates','non-specific colitis,' and 'no pathologic diagnosis.' Overall,the combined accuracy of endoscopists for predicting adenomas was identical between i-SCAN(71.8%,95%CI:62.1%-80.3%) and high-definition white light(71.8%,95%CI:62.1%-80.3%).However,the accuracy of each endoscopist differed substantially,where endoscopist A demonstrated 63.0% overall accuracy(95%CI:50.9%-74.0%) as compared with endoscopist B demonstrating 93.3% overall accuracy(95%CI:77.9%-99.2%),irrespective of imaging modality.Neither endoscopist demonstrated a significant learning effect with i-SCAN during the study.Though endoscopist A increased accuracy using i-SCAN from 59%(95%CI:42.1%-74.4%) in the first half to 67.6%(95%CI:49.5%-82.6%) in the second half,and endoscopist B decreased accuracy using i-SCAN from 100%(95%CI:80.5%-100.0%) in the first half to 84.6%(95%CI:54.6%-98.1%) in the second half,neither of these differences were statistically significant.CONCLUSION:i-SCAN and high-definition white light had similar efficacy predicting polyp histology.Endoscopist training likely plays a critical role in diagnostic test characteristics and deserves further study. | Johanna L Chan Li Lin Michael Feiler Andrew I Wolf Diana M Cardona Ziad F Gellad | 2012 | World Journal of Gastroenterology2012,18,41: | 3 |
