2Outline of the Report on Cardiovascular Disease in China,2010显示文摘Major and profound changes have taken place in China over the past 30 years.Rapid socioeconomic progress has exerted a great impact on lifestyle,ranging from food,clothing,working and living conditions,and means of transportation to leisure activities and entertainment.At the same time,new health problems have emerged,and health services are facing new challenges.Presently,cardiovascular diseases (CVD) are among the top health problems of the Chinese people,and pose a serious challenge to all engaged in the prevention and control of these diseases.An epidemic of CVD in China is emerging as a result of lifestyle changes,urbanization and longevity.Both national policy decision-making and medical practice urgently need an authoritative report which comprehensively reflects the trends in the epidemic of CVD and current preventive measures.Since 2005,guided by the Bureau of Disease Prevention of the Ministry of Health of the People's Republic of China and the National Center for Cardiovascular Diseases of China,nationwide experts in the fields of epidemiology,clinical medicine and health economics in the realms of CVD,cerebrovascular disease,diabetes and chronic kidney disease,completed the Report on Cardiovascular Diseases in China every year.The report aims to provide a timely review of the trend of the epidemic and to assess the progress of prevention and control of CVD.In addition,as the report is authoritative,representative and readable,it will become an information platform in the CVD field and an important reference book for government,academic institutes,medical organizations and clinical physicians.This publication is expected to play a positive role in the prevention and control of CVD in China.We present an abstract from the Report on Cardiovascular Diseases in China (2010),including trends in CVD,morbidity and mortality of major CVDs,up-to-date assessment of risk factors,as well as health resources for CVD,and a profile of medical expenditure,with the aim of providing evidence for decision-making in CVD prevention and control programs in China,and of delivering the most authoritative information on CVD prevention and control for all citizens.HUSheng Shou KONG Ling Zhi GAO Run Lin ZHU Man Lu WANG Wen WANG Yong Jun WU Zhao Su CHEN Wei Wei LIU Ming Bo For the editorial board 2012Biomedical and Environmental Sciences2012,25,3:93

6A complete sequence and comparative analysis of a SARS-associated virus(Isolate BJ01)显示文摘The genome sequence of the Severe Acute Respiratory Syndrome (SARS)-associated virus provides essential information for the identification of pathogen(s), exploration of etiology and evolution, interpretation of transmission and pathogenesis, development of diagnostics, prevention by future vaccination, and treatment by developing new drugs. We report the complete genome sequence and comparative analysis of an isolate (BJ01) of the coronavirus that has been recognized as a pathogen for SARS. The genome is 29725 nt in size and has 11 ORFs (Open Reading Frames). It is composed of a stable region encoding an RNA-dependent RNA polymerase (composed of 2 ORFs) and a variable region representing 4 CDSs (coding sequences) for viral structural genes (the S, E, M, N proteins) and 5 PUPs (putative uncharacterized proteins). Its gene order is identical to that of other known coronaviruses. The sequence alignment with all known RNA viruses places this virus as a member in the family of Coronaviridae. Thirty putative substitutions have been identified by comparative analysis of the 5 SARS- associated virus genome sequences in GenBank. Fifteen of them lead to possible amino acid changes (non-synonymous mutations) in the proteins. Three amino acid changes, with predicted alteration of physical and chemical features, have been detected in the S protein that is postulated to beinvolved in the immunoreactions between the virus and its host. Two amino acid changes have been detected in the Mprotein, which could be related to viral envelope formation. Phylogenetic analysis suggests the possibility of non-human origin of the SARS-associated viruses but provides noevidence that they are man-made. 7Effects of different resuscitation fluid on severe acute pancreatitis显示文摘AIM: To compare effects of different resuscitation fluid on microcirculation, inflammation, intestinal barrier and clinical results in severe acute pancreatitis (SAP). METHODS: One hundred and twenty patients with SAP were enrolled at the Pancreatic Disease Institute between January 2007 and March 2010. The patients were randomly treated with normal saline (NS group), combination of normal saline and hydroxyethyl starch (HES) (SH group), combination of normal saline, hydroxyethyl starch and glutamine (SHG group) in resuscitation. The ratio of normal saline to HES in the SH and SHG groups was 3:1. The glutamine (20% glutamine dipeptide, 100 mL/d) was supplemented into the resuscitation liquid in the SHG group. Complications and outcomes including respiratory and abdominal infection, sepsis, abdominal hemorrhage, intra-abdominal hypertension, abdominal compartment syndrome (ACS), renal failure, acute respiratory distress syndrome (ARDS), multiple organ dysfunction syndrome (MODS), operation intervention, length of intensive care unit stay, length of hospital stay, and mortality at 60 d were compared. Moreover, blood oxygen saturation (SpO 2 ), gastric intramucosal pH value (pHi), intra-abdominal pressure (IAP), inflammation cytokines, urine lactulose/mannitol (L/M) ratio, and serum endotoxin were investigated to evaluate the inflammatory reaction and gut barrier. RESULTS: Compared to the NS group, patients in the SH and SHG groups accessed the endpoint more quickly (3.9 ± 0.23 d and 4.1 ± 0.21 d vs 5.8 ± 0.25 d, P < 0.05) with less fluid volume (67.26 ± 28.53 mL/kg/d, 61.79 ± 27.61 mL/kg per day vs 85.23 ± 21.27 mL/kg per day, P < 0.05). Compared to the NS group, incidence of renal dysfunction, ARDS, MODS and ACS in the SH and SHG groups was obviously lower. Furthermore, incidence of respiratory and abdominal infection was significantly decreased in the SH and SHG groups, while no significant difference in sepsis was seen. Moreover, less operation time was needed in the SH and SHG group than the NS group, but the difference was not significant. The mortality did not differ significantly among these groups. Blood SpO 2 and gastric mucosal pHi in the SH and SHG groups increased more quickly than in the NS group, while IAP was significantly decreased in the SH and SHG group. Moreover, the serum tumor necrosis factor-α, interleukin-8 and C-reactive protein levels in the SH and SHG groups were obviously lower than in the NS group at each time point. 8Mapping wetland changes in China between 1978 and 2008显示文摘Four wetland maps for all China have been produced,based on Landsat and CBERS-02B remote sensing data between 1978 and 2008 (1978,1990,2000 and 2008).These maps were mainly developed by manual interpretation and validated by substantial field investigation in 2009.Based on these maps,we analyzed the 2008 wetland distribution in China and discussed wetland changes and their drivers over the past 30 years.(i) There were about 324097 km 2 of wetlands in 2008,for which inland marshes or swamps were the most common wetland type (35%),with lakes (26%) second.Most of the wetlands were in Heilongjiang,Inner Mongolia,Qinghai and Tibet,occupying about 55% of the national wetland area.(ii) From 1978 to 2008,China's wetland area continually and significantly decreased,by about 33% based on changes in the wetland map.This was in sharp contrast to the increase in artificial wetlands,which increased by about 122%.Inland marshes accounted for the main loss of total wetlands from 1978 to 2000.From 2000 through 2008,riverine and lacustrine wetlands constituted the main wetland loss.Fortunately however,the rate of wetland loss decreased from 5523 to 831 km 2 /a.(iii) The change ratio of lost natural wetlands (including inland and coastal wetlands) to non-wetlands has decreased slightly over the past 30 years.From 1978 to 1990,nearly all natural wetlands (98%) lost were transformed into non-wetlands.However,the ratio declined to 86% from 1990 to 2000,and to 77% from 2000 to 2008.(iv) All Chinese provinces were divided into three groups according to patterns of wetland changes,which could relate to the driving forces of such changes.Tibet was completely different from other provinces,as it was one representative example in which there was a net wetland increase,because of global warming and decreased human activity since 1990.Increased economic development caused considerable wetland loss in most eastern provinces,and artificial wetlands increased.NIU ZhenGuo ZHANG HaiYing WANG XianWei YAO WenBo ZHOU DeMin ZHAO KuiYi ZHAO Hui LI NaNa HUANG HuaBing LI CongCong YANG Jun LIU CaiXia LIU Shuang WANG Lin LI Zhan YANG ZhenZhong QIAO Fei ZHENG YaoMin CHEN YanLei SHENG YongWei GAO XiaoHong ZHU WeiHong WANG WenQing WANG Hong WENG YongLing ZHUANG DaFang LIU JiYuan LUO ZhiCai CHENG Xiao GUO ZiQi GONG Peng 2012Chinese Science Bulletin2012,57,22:51
92018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). 11Persistence and clearance of viral RNA in 2019 novel coronavirus disease rehabilitation patients显示文摘Background:A patient’s infectivity is determined by the presence of the virus in different body fluids,secretions,and excreta.The persistence and clearance of viral RNA from different specimens of patients with 2019 novel coronavirus disease(COVID-19)remain unclear.This study analyzed the clearance time and factors influencing 2019 novel coronavirus(2019-nCoV)RNA in different samples from patients with COVID-19,providing further evidence to improve the management of patients during convalescence.Methods:The clinical data and laboratory test results of convalescent patients with COVID-19 who were admitted to from January 20,2020 to February 10,2020 were collected retrospectively.The reverse transcription polymerase chain reaction(RT-PCR)results for patients’oropharyngeal swab,stool,urine,and serum samples were collected and analyzed.Convalescent patients refer to recovered non-febrile patients without respiratory symptoms who had two successive(minimum 24 h sampling interval)negative RT-PCR results for viral RNA from oropharyngeal swabs.The effects of cluster of differentiation 4(CD4)+T lymphocytes,inflammatory indicators,and glucocorticoid treatment on viral nucleic acid clearance were analyzed.Results:In the 292 confirmed cases,66 patients recovered after treatment and were included in our study.In total,28(42.4%)women and 38 men(57.6%)with a median age of 44.0(34.0-62.0)years were analyzed.After in-hospital treatment,patients’inflammatory indicators decreased with improved clinical condition.The median time from the onset of symptoms to first negative RT-PCR results for oropharyngeal swabs in convalescent patients was 9.5(6.0-11.0)days.By February 10,2020,11 convalescent patients(16.7%)still tested positive for viral RNA from stool specimens and the other 55 patients’stool specimens were negative for 2019-nCoV following a median duration of 11.0(9.0-16.0)days after symptom onset.Among these 55 patients,43 had a longer duration until stool specimens were negative for viral RNA than for throat swabs,with a median delay of 2.0(1.0-4.0)days.Results for only four(6.9%)urine samples were positive for viral nucleic acid out of 58 cases;viral RNA was still present in three patients’urine specimens after throat swabs were negative.Using a multiple linear regression model(F=2.669,P=0.044,and adjusted R2=0.122),the analysis showed that the CD4+T lymphocyte count may help predict the duration of viral RNA detection in patients’stools(t=-2.699,P=0.010).The duration of viral RNA detection from oropharyngeal swabs and fecal samples in the glucocorticoid treatment group was longer than that in the non-glucocorticoid treatment group(15 days vs.8.0 days,respectively;t=2.550,P=0.013)and the duration of viral RNA detection in fecal samples in the glucocorticoid treatment group was longer than that in the non-glucocorticoid treatment group(20 days vs.11 days,respectively;t=4.631,P<0.001).There was no statistically significant difference in inflammatory indicators between patients with positive fecal viral RNA test results and those with negative results(P>0.05).Conclusions:In brief,as the clearance of viral RNA in patients’stools was delayed compared to that in oropharyngeal swabs,it is important to identify viral RNA in feces during convalescence.Because of the delayed clearance of viral RNA in the glucocorticoid treatment group,glucocorticoids are not recommended in the treatment of COVID-19,especially for mild disease.The duration of RNA detection may relate to host cell immunity.Yun Ling Shui-Bao Xu Yi-Xiao Lin Di Tian Zhao-Qin Zhu Fa-Hui Dai Fan Wu Zhi-Gang Song Wei Huang Jun Chen Bi-Jie Hu Sheng Wang En-Qiang Mao Lei Zhu Wen-Hong Zhang Hong-Zhou Lu 2020Chinese Medical Journal2020,,9:58
12Panax notogiseng saponin inhibits ischemia-induced apoptosis by activating PI3K/Akt signal pathway in cardiomyocytes显示文摘The panax notoginseng saponin(PNS) had been clinically used for the treatment of cardiovascular diseases and stroke in China.It had been demonstrated that PNS could protect cardiomyocytes from injury induced by ischemi- a,but the underlying molecular mechanisms of this protective effect were still unclear.This study was aimed to investigate the protective effect and molecular mechanisms of PNS on apoptosis in H9c2 cells in vitro and rat myocardial ischemia injury model in vivo.Annexin-V/PI assay shew that PNS could protect H9c2 cells from apoptosis induced by serum, glucose and oxygen deprivation(SGOD) in a dose-dependent manner.However,the anti-apoptotic effect of PNS was reversed by LY294002,a specific PI3K inhibitor.This antiapoptotic effect of PNS was confirmed by JC-1,a specific probe of mitochondrial membrane potential staining.PNS could significantly increase phos-Akt in H9c2 cells by Western blot assays and its effect could be inhibited by LY294002.Furthermore,PNS could improve ischemic-induced left ventricular function as reflected by EF,LVDd and LVDs.PNS could also inhibited cellular apoptosis in myocardial tissues in ischemic rats by TUNEL assay.PNS administration also increased the expression of phos-Akt in rat ischemic myocardial tissues.These results suggested that PNS could protect myocardial cells from apoptosis induced by ischemia in vitro model and in vivo model through activating-PI3K/Akt signal pathway which may be meaningful for further understanding the molecular mechanisms of cardiac protection of PNS.And the results might be useful in treatment of myocardial ischemia in future.YANG Min,CHEN Shao-Xian,LIU Ju-Li,LIU Xiao-Ying,FU Yong-Heng,ZHANGMeng-zhen,LIN Qiu-Xiong,ZHU Jie-Ning, SHAN Zhi-Xin,YU Xi-yong (Medical Research Center,Guangdong General Hospital, Guangdong Academy of Medical Sciences,Guangzhou 510100,China) 2011岭南心血管病杂志2011,17,S1:47
13Spatiotemporal changes in vegetation coverage and its driving factors in the Three-River Headwaters Region during 2000-2011显示文摘The Three-River Headwaters Region(TRHR), which is the source area of the Yangtze River, Yellow River, and Lancang River, is of key importance to the ecological security of China. Because of climate changes and human activities, ecological degradation occurred in this region. Therefore, 'The nature reserve of Three-River Source Regions' was established, and 'The project of ecological protection and construction for the Three-River Headwaters Nature Reserve' was implemented by the Chinese government. This study, based on MODIS-NDVI and climate data, aims to analyze the spatiotemporal changes in vegetation coverage and its driving factors in the TRHR between 2000 and 2011, from three dimensions. Linear regression, Hurst index analysis, and partial correlation analysis were employed. The results showed the following:(1) In the past 12 years(2000–2011), the NDVI of the study area increased, with a linear tendency being 1.2%/10a, of which the Yangtze and Yellow River source regions presented an increasing trend, while the Lancang River source region showed a decreasing trend.(2) Vegetation coverage presented an obvious spatial difference in the TRHR, and the NDVI frequency was featured by a bimodal structure.(3) The area with improved vegetation coverage was larger than the degraded area, being 64.06% and 35.94%, respectively during the study period, and presented an increasing trend in the north and a decreasing trend in the south.(4) The reverse characteristics of vegetation coverage change are significant. 17Chinese deserts and sand fields in Last Glacial Maximum and Holocene Optimum显示文摘The Last Glacial Maximum (LGM, c. 26-16 ka) and the Holocene Optimum (HO, c. 9-5 ka) were characterized by cold-dry and warm-wet climates respectively in the recently geological Earth. How Chinese deserts and sand fields responded to these distinctive climatic changes is still not clear, however. To reconstruct environments of the deserts and sand fields during the LGM and HO is helpful to understand the forcing mechanisms of environment change in this arid region, and to test paleoclimatic modeling results. Through our long-term field and laboratory investigations, 400 optically stimulated luminescence (OSL) ages and more than 100 depositional records in the Chinese deserts and sand fields were obtained; on the basis of these data, we reconstruct spatial distributions of the deserts and sand fields during the LGM and HO. Our results show that the sand fields of Mu Us, Hunshandake, Horqin and Hulun Buir in northern and northeastern China had expanded 25%, 37%, 38% and 270%, respectively, during the LGM; the sand fields of Gonghe in the northeastern Qinghai-Tibetan Plateau had expanded 20%, and the deserts of Badain Jaran, Tengger in central northern China had expanded 39% and 29% separately during the LGM; the deserts of Taklimakan, Gurbantünggüt and Kumtag in northwestern China had expanded 10%-20% respectively, compared to their modern areas. On the other hand, all of the sand fields were nearly completely covered by vegetation during the HO; the deserts in northwestern and central northern China were reduced by around 5%-20% in area during this time. Lakes in this arid region were probably expanded during the HO but this conclusion needs more investigation. Compared with the geological distributions of deserts and sand fields, human activity has clearly changed (expanded) the area of active sand dunes at the present time. 18Transplantation of collagen scaffold with autologous bone marrow mononuclear cells promotes functional endometrium reconstruction via downregulating ΔNp63 expression in Asherman's syndrome显示文摘Asherman's syndrome(AS) is a common disease that presents endometrial regeneration disorder. However, little is known about its molecular features of this aregenerative endometrium in AS and how to reconstruct the functioning endometrium for the patients with AS. Here, we report that ΔNp63 is significantly upregulated in residual epithelial cells of the impaired endometrium in AS; the upregulated-ΔNp63 induces endometrial quiescence and alteration of stemness. Importantly, we demonstrate that engrafting high density of autologous bone marrow mononuclear cells(BMNCs) loaded in collagen scaffold onto the uterine lining of patients with AS downregulates ΔNp63 expression, reverses ΔNp63-induced pathological changes, normalizes the stemness alterations and restores endometrial regeneration. Finally, five patients achieved successful pregnancies and live births. Therefore, we conclude that ΔNp63 is a crucial therapeutic target for AS. 20Molecular subtyping and genomic profiling expand precision medicine in refractory metastatic triple-negative breast cancer:the FUTURE trial显示文摘Triple-negative breast cancer(TNBC)is a highly heterogeneous disease,and molecular subtyping may result in improved diagnostic precision and targeted therapies.Our previous study classified TNBCs into four subtypes with putative therapeutic targets.Here,we conducted the FUTURE trial(ClinicalTrials.gov identifer:NCT03805399),a phaseⅠb/Ⅱsubtyping-based and genomic biomarker-guided umbrella trial,to evaluate the efficacy of these targets.Patients with refractory metastatic TNBC were enrolled and stratifed by TNBC subtypes and genomic biomarkers,and assigned to one of these seven arms:(A)pyrotinib with capecitabine,(B)androgen receptor inhibitor with CDK4/6 inhibitor,(C)anti PD-1 with nab-paclitaxel,(D)PARP inhibitor included,(E)and(F)anti-VEGFR included,or(G)mTOR inhibitor with nab paclitaxel.The primary end point was the objective response rate(ORR).We enrolled 69 refractory metastatic TNBC patients with a median of three previous lines of therapy(range,1-8).Objective response was achieved in 20(29.0%,95%confidence interval(CI):18.7%-41.2%)of the 69 intention-to-treat(ITT)patients.Our results showed that immunotherapy(arm C),in particular,achieved the highest ORR(52.6%,95%CI:28.9%-75.6%)in the ITT population.Arm E demonstrated favorable ORR(26.1%,95%C:10.2%-48.4%in the ITT population)but with more high grade(23)adverse events.Somatic mutations of TOP2A and CD8 immunohistochemical score may have the potential to predict immunotherapy response in the immunomodulatory subtype of TNBC.In conclusion,the phaseⅠb/ⅡFUTURE trial suggested a new concept for TNBC treatment,demonstrating the clinical beneft of subtyping-based targeted therapy for refractory metastatic TNBC.Yi-Zhou Jiang Yin Liu Yi Xiao Xin Hu Lin Jiang Wen-ia Zuo Ding Ma Jiahan Ding Xiaoyu Zhu Jianjun Zou Claire Verschraegen Daniel G.Stover Virginia Kaklamani Zhong-Hua Wang Zhi-Ming Shao 2021Cell Research2021,31,2:37
