| 3 | Clinical characteristics of novel coronavirus cases in tertiary hospitals in Hubei Province显示文摘Background:The 2019 novel coronavirus(2019-nCoV)causing an outbreak of pneumonia in Wuhan,Hubei province of China was isolated in January 2020.This study aims to investigate its epidemiologic history,and analyze the clinical characteristics,treatment regimens,and prognosis of patients infected with 2019-nCoV during this outbreak.Methods:Clinical data from 1372019-nCoV-infected patients admitted to the respiratory departments of nine tertiary hospitals in Hubei province from December 30,2019 to January 24,2020 were retrospectively collected,including general status,clinical manifestations,laboratory test results,imaging characteristics,and treatment regimens.Results:None of the 137 patients(61 males,76 females,aged 20-83 years,median age 57 years)had a definite history of exposure to Huanan Seafood Wholesale Market.Major initial symptoms included fever(112/137,81.8%),coughing(66/137,48.2%),and muscle pain or fatigue(44/137,32.1%),with other,less typical initial symptoms observed at low frequency,including heart palpitations,diarrhea,and headache.Nearly 80%of the patients had normal or decreased white blood cell counts,and 72.3%(99/137)had lymphocytopenia.Lung involvement was present in all cases,with most chest computed tomography scans showing lesions in multiple lung lobes,some of which were dense;ground-glass opacity co-existed with consolidation shadows or cord-like shadows.Given the lack of effective drugs,treatment focused on symptomatic and respiratory support.Immunoglobulin G was delivered to some critically ill patients according to their conditions.Systemic corticosteroid treatment did not show significant benefits.Notably,early respiratory support facilitated disease recovery and improved prognosis.The risk of death was primarily associated with age,underlying chronic diseases,and median interval from the appearance of initial symptoms to dyspnea.Conclusions:The majority of patients with 2019-nCoV pneumonia present with fever as the first symptom,and most of them still showed typical manifestations of viral pneumonia on chest imaging.Middle-aged and elderly patients with underlying comorbidities are susceptible to respiratory failure and may have a poorer prognosis. | Kui Liu Yuan-Yuan Fang Yan Deng Wei Liu Mei-Fang Wang Jing-Ping Ma Wei Xiao Ying-Nan Wang Min-Hua Zhong Cheng-Hong Li Guang-Cai Li Hui-Guo Liu | 2020 | Chinese Medical Journal2020,,9: | 100 |
| 4 | A rapid advice guideline for the diagnosis and treatment of 2019 novel coronavirus(2019-nCoV) infected pneumonia(standard version)显示文摘In December 2019, a new type viral pneumonia cases occurred in Wuhan, Hubei Province;and then named '2019 novel coronavirus(2019-nCoV)' by the World Health Organization(WHO) on 12 January 2020. For it is a never been experienced respiratory disease before and with infection ability widely and quickly, it attracted the world’s attention but without treatment and control manual. For the request from frontline clinicians and public health professionals of 2019-nCoV infected pneumonia management, an evidence-based guideline urgently needs to be developed. Therefore, we drafted this guideline according to the rapid advice guidelines methodology and general rules of WHO guideline development;we also added the first-hand management data of Zhongnan Hospital of Wuhan University. This guideline includes the guideline methodology, epidemiological characteristics, disease screening and population prevention, diagnosis, treatment and control(including traditional Chinese Medicine), nosocomial infection prevention and control, and disease nursing of the 2019-nCoV. Moreover, we also provide a whole process of a successful treatment case of the severe 2019-nCoV infected pneumonia and experience and lessons of hospital rescue for 2019-nCoV infections. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. | 13 | Effect of subsoil tillage depth on nutrient accumulation, root distribution, and grain yield in spring maize显示文摘A four-year field experiment was conducted to investigate the effect of subsoiling depth on root morphology, nitrogen(N), phosphorus(P), and potassium(K) uptake, and grain yield of spring maize. The results indicated that subsoil tillage promoted root development,increased nutrient accumulation, and increased yield. Compared with conventional soil management(CK), root length, root surface area, and root dry weight at 0–80 cm soil depth under subsoil tillage to 30 cm(T1) and subsoil tillage to 50 cm(T2) were significantly increased, especially the proportions of roots in deeper soil. Root length, surface area, and dry weight differed significantly among three treatments in the order of T2 > T1 > CK at the12-leaf and early filling stages. The range of variation of root diameter in different soil layers in T2 treatment was the smallest, suggesting that roots were more likely to grow downwards with deeper subsoil tillage in soil. The accumulation of N, P, and K in subsoil tillage treatment was significantly increased, but the proportions of kernel and straw were different. In a comparison of T1 with T2, the grain accumulated more N and P, while K accumulation in kernel and straw varied in different years. Grain yield and biomass were increased by 12.8% and 14.6% on average in subsoil tillage treatments compared to conventional soil treatment. | 15 | Indicators of prognosis after liver transplantation in Chinese hepatocellular carcinoma patients显示文摘AIM: To identify prognostic factors of patients with hepatocellular carcinoma (HCC), who were treated by orthotopic liver transplantation (OLT). METHODS: From January 2000 to October 2006, 165 patients with HCC underwent OLT. Various clinicopathological risk factors for actuarial and recurrence-free survival were identified using the Kaplan-Meier method with the log-rank test. The Cox proportional hazards model was used to identify independently predictive factors for actuarial and recurrence-free survival, which were used to propose new selection criteria. We compared the outcome of the subgroup patients meeting different criteria. Survival analysis was performed using the Kaplan-Meier method with the log-rank test. RESULTS: The median follow-up was 13.0 mo (2.8-69.5 mo). Overall, 1-, 2-, 3-and 5-year actuarial survival was 73.3%, 45.6%, 35.4% and 32.1%, respectively. One-, 2-, 3-and 5-year overall recurrencefree survival was 67.0%, 44.3%, 34.5% and 34.5%, respectively. In univariate analysis, number of tumors, total tumor size, lobar distribution, differentiation, macrovascular invasion, microvascular invasion, capsulationof the tumor, and lymph node metastasis were found to be associated significantly with actuarial and tumor-free survival. By means of using the multivariate Cox proportional hazards model, total tumor size and macrovas-cular invasion were found to be independent predictors of actuarial and tumor-free survival. When the selection criteria were expanded into the proposed criteria, there was no significant difference in 1-, 2-, 3-and 5-year actuarial and tumor-free survival of the 49 patients who met the proposed criteria (97.6%, 82.8%, 82.8% and 82.8%, and 90.7%, 82.8%, 68.8% and 68.8%, respectively) compared with that of patients who met the Milan or University of California, San Francisco (UCSF) criteria. CONCLUSION: Macrovascular invasion and total tumor diameter are the strongest prognostic factors. | 16 | Resveratrol Induces Apoptosis and Autophagy in T-cell Acute Lymphoblastic Leukemia Cells by Inhibiting Akt/mTOR and Activating p38-MAPK显示文摘Objective To explore the effects of resveratrol-induced apoptosis and autophagy in T-cell acute lymphoblastic leukemia(T-ALL) cells and potential molecular mechanisms. Methods The anti-proliferation effect of resveratrol-induced, apoptosis and autophagy on T-ALL cells were detected by using MTT test, immunofluorescence, electronic microscope, and flow cytometry, respectively. Western blotting was performed for detecting changes of apoptosis-associated proteins, cell cycle regulatory proteins and state of activation of Akt, mTOR, p70S6K, 4E-BP1, and p38-MAPK. Results Resveratrol inhibited the proliferation and induced apoptosis and autophagy in T-ALL cells in a dose and time-dependent manner. It also induced cell cycle arrest at G0/G1 phase via up regulating cyclin-dependent kinase(CDK) inhibitors p21 and p27 and down regulating cyclin A and cyclin D1. Western blotting revealed that resveratrol significantly decreased the expression of antiapoptotic proteins(Mcl-1 and Bcl-2) and increased the expression of proapoptotic proteins(Bax, Bim, and Bad), and induced cleaved-caspase-3 in a time-dependent manner. Significant increase in ratio of LC3-II/LC3-I and Beclin 1 was also detected. Furthermore, resveratrol induced significant dephosphorylation of Akt, mTOR, p70S6K, and 4E-BP1, but enhanced specific phosphorylation of p38-MAPK which could be blocked by SB203580. | 18 | The status and distribution characteristics of residual mulching film in Xinjiang, China显示文摘Pollution of residual plastic film in arable lands is a severe problem in China. In this study, the status of residual film and influential factors were investigated using the methods of farm survey in combination with questionnaires and quadrat sampling at a large number of field sites in Xinjiang Uygur Autonomous Region, China. The results showed that the amount of film utilization increased largely and reached to 1.8×10~5 t in 2013. Similarly, the mulching area also substantially increased in recent decades, and reached to 2.7×10~5 ha in the same year. According to the current survey, 60.7% of the sites presented a greater mulch residue than the national film residue standard(75 kg ha^(–1)), and the maximum residual amount reached 502.2 kg ha^(–1) in Turpan, Xinjiang. The film thickness, the mulching time and the crop type all influenced mulch residue. The thickness of the film had significantly negative correlation with the amount of residual film(P<0.05), while the mulching years had significantly positive correlation with it(P<0.05). The total amount of residual film in Xinjiang was 3.43×105 t in 2011, which accounted for 15.3% of the cumulative dosage of mulching. | 20 | Inflammation-related factors predicting prognosis of gastric cancer显示文摘Gastric cancer(GC),which is mainly induced by Helicobacter pylori(H.pylori)infection,is one of the leading causes of cancer-related death in the developing world.Active inflammation initiated by H.pylori infection and maintained by inherent immune disorders promotes carcinogenesis and postoperative recurrence.However,the presence with H.pylori in tumors has been linked to a better prognosis,possibly due to the induction of antitumor immunity.Tumor infiltrations of tumorassociated macrophages,myeloid-derived suppressor cells,neutrophils,Foxp3+regulatory T cells are correlated with poor prognosis.Tumor infiltrating CD8+cytotoxic T lymphocytes,dendritic cells,and CD45RO T cells are generally associated with good prognosis of GC,although some subsets of these immune cells have inverse prognosis prediction values.High ratios of Foxp3+/CD4+and Foxp3+/CD8+in tumors are associated with a poor prognosis;whereas high Th1/Th2ratio in tumors predicts a good prognosis.High levels of interleukin(IL)-6,IL-10,IL-32,and chemokine C-C motif ligands(CCL)7 and CCL21 in circulation,high expression of CXC chemokine receptor 4,chemokine C-C motif receptor(CCR)3,CCR4,CCR5,CCR7,hypoxiainducible factor-1α,signal transducer activator of transcription-3,cyclooxygenase-2,and orphan nuclear receptor 4A2 in tumors are associated with an unfavorable prognosis.Increased serum levels of matrix metalloproteinases(MMP)-3,MMP-7,and MMP-11 and increased levels of MMP-9,MMP-12,and MMP-21 in tumors are consistently associated with poor survival of GC.Further emphasis should be put on the integration of these biomarkers and validation in large cohorts for personalized prediction of GC postoperative prognosis. | Wen-Jun Chang Yan Du Xin Zhao Li-Ye Ma Guang-Wen Cao | 2014 | World Journal of Gastroenterology2014,20,16: | 39 |
