1A rapid advice guideline for the diagnosis and treatment of 2019 novel coronavirus(2019-nCoV) infected pneumonia(standard version)显示文摘In December 2019, a new type viral pneumonia cases occurred in Wuhan, Hubei Province;and then named '2019 novel coronavirus(2019-nCoV)' by the World Health Organization(WHO) on 12 January 2020. For it is a never been experienced respiratory disease before and with infection ability widely and quickly, it attracted the world’s attention but without treatment and control manual. For the request from frontline clinicians and public health professionals of 2019-nCoV infected pneumonia management, an evidence-based guideline urgently needs to be developed. Therefore, we drafted this guideline according to the rapid advice guidelines methodology and general rules of WHO guideline development;we also added the first-hand management data of Zhongnan Hospital of Wuhan University. This guideline includes the guideline methodology, epidemiological characteristics, disease screening and population prevention, diagnosis, treatment and control(including traditional Chinese Medicine), nosocomial infection prevention and control, and disease nursing of the 2019-nCoV. Moreover, we also provide a whole process of a successful treatment case of the severe 2019-nCoV infected pneumonia and experience and lessons of hospital rescue for 2019-nCoV infections. 2A complete sequence and comparative analysis of a SARS-associated virus(Isolate BJ01)显示文摘The genome sequence of the Severe Acute Respiratory Syndrome (SARS)-associated virus provides essential information for the identification of pathogen(s), exploration of etiology and evolution, interpretation of transmission and pathogenesis, development of diagnostics, prevention by future vaccination, and treatment by developing new drugs. We report the complete genome sequence and comparative analysis of an isolate (BJ01) of the coronavirus that has been recognized as a pathogen for SARS. The genome is 29725 nt in size and has 11 ORFs (Open Reading Frames). It is composed of a stable region encoding an RNA-dependent RNA polymerase (composed of 2 ORFs) and a variable region representing 4 CDSs (coding sequences) for viral structural genes (the S, E, M, N proteins) and 5 PUPs (putative uncharacterized proteins). Its gene order is identical to that of other known coronaviruses. The sequence alignment with all known RNA viruses places this virus as a member in the family of Coronaviridae. Thirty putative substitutions have been identified by comparative analysis of the 5 SARS- associated virus genome sequences in GenBank. Fifteen of them lead to possible amino acid changes (non-synonymous mutations) in the proteins. Three amino acid changes, with predicted alteration of physical and chemical features, have been detected in the S protein that is postulated to beinvolved in the immunoreactions between the virus and its host. Two amino acid changes have been detected in the Mprotein, which could be related to viral envelope formation. Phylogenetic analysis suggests the possibility of non-human origin of the SARS-associated viruses but provides noevidence that they are man-made. 5EXPERIMENTAL STUDY OF FLOWFIELD STRUCTURE AROUND AN OGIVE-CYLINDER显示文摘The flowfield structure and their aerodynamic characteristics over an ogive cylinder were studied by means of flow visualization and surface pressure measurement in a water tunnel and a wind tunnel. The existence of multi asymmetric vortices over long slender bodies was experimentally confirmed at large angles of attack and in the subcritical Reynolds number range. The spatial 3 D characteristics of the multi vortices system were analyzed and a physical model was developed. The topological structure of different patterns in cross flow plane was studied and the mechanism governing the formation of asymmetric vortices and multi vortices was discussed from the viewpoint of stability of the topological structure. 6A newly discovered Gigantopithecus fauna from Sanhe Cave, Chongzuo, Guangxi, South China显示文摘Among the most important faunas in the Late Cenozoic, the Gigantopithecus faunas have received a good deal of attention. The Gigantopithecus fauna recently discovered in Sanhe Cave consists of more than 80 mammal species, including cf. Hominidae, Pongo sp., Hylobates sp., Sinomastodon yangzien-sis, Stegodon preorientalis, Cervavitus fenqii, Dicoryphochoerus ultimus and Sus xiaozhu. It is the southernmost Gigantopithecus fauna found so far in China. Its geological age is estimated to be Early Pleistocene based on the fauna and stratigraphic correlation. The significant increase in the estimated body sizes of Ailuropoda, Gigantopithecus and Tapirus shows that the Sanhe fauna is middle Early Pleistocene, later than those from Wushan and Liucheng (early Early Pleistocene). Paleomagnetic dating of the fossil-bearing strata in Sanhe Cave gives an age of approximately 1.2 Ma. The fauna is characterized by tropical-subtropical forest types, including Pongo sp., Tupaia sp., Ia sp., Typhlomys intermedius, etc., and it lacks Palaearctic types. It is a typical tropical forest fauna, suggesting an en-vironment with a lush forest and a warm and humid climate. 8Insight-HXMT observations of the first binary neutron star merger GW170817显示文摘Finding the electromagnetic(EM) counterpart of binary compact star merger, especially the binary neutron star(BNS) merger,is critically important for gravitational wave(GW) astronomy, cosmology and fundamental physics. On Aug. 17, 2017,Advanced LIGO and Fermi/GBM independently triggered the first BNS merger, GW170817, and its high energy EM counterpart,GRB 170817 A, respectively, resulting in a global observation campaign covering gamma-ray, X-ray, UV, optical, IR, radio as well as neutrinos. The High Energy X-ray telescope(HE) onboard Insight-HXMT(Hard X-ray Modulation Telescope) is the unique high-energy gamma-ray telescope that monitored the entire GW localization area and especially the optical counterpart(SSS17 a/AT2017 gfo) with very large collection area(~1000 cm^2) and microsecond time resolution in 0.2-5 MeV. In addition,Insight-HXMT quickly implemented a Target of Opportunity(ToO) observation to scan the GW localization area for potential X-ray emission from the GW source. Although Insight-HXMT did not detect any significant high energy(0.2-5 MeV) radiation from GW170817, its observation helped to confirm the unexpected weak and soft nature of GRB 170817 A. Meanwhile,Insight-HXMT/HE provides one of the most stringent constraints(~10^(-7) to 10^(-6) erg/cm^2/s) for both GRB170817 A and any other possible precursor or extended emissions in 0.2-5 MeV, which help us to better understand the properties of EM radiation from this BNS merger. 9Efficacy of noninvasive evaluations in monitoring inflammatory bowel disease activity: a prospective study in China显示文摘AIM To optimize the efficacy of noninvasive evaluations in monitoring the endoscopic activity of inflammatory bowel disease(IBD).METHODS Fecal calprotectin(FC), clinical activity index(CDAI or CAI), C-reactive protein(CRP), erythrocyte sedimentation rate(ESR), and procalcitonin(PCT) were measured for 136 IBD patients. Also, FC was measured in 25 irritable bowel syndrome(IBS) patients that served as controls. Then, endoscopic activity was determined by other two endoscopists for colonic or ileo-colonic Crohn's disease(CICD) with the 'simple endoscopic score for Crohn's disease'(SES-CD), CDrelated surgery patients with the Rutgeerts score, and ulcerative colitis(UC) with the Mayo score. The efficacies of these evaluations to predict the endoscopic disease activity were assessed by Mann-Whitney test, χ~2 test, Spearman's correlation, and multiple linear regression analysis.RESULTS The median FC levels in CD, UC, and IBS patients were 449.6(IQR, 137.9-1344.8), 497.9(IQR, 131.7-118.0), and 9.9(IQR, 0-49.7) μg/g, respectively(P < 0.001). For FC, CDAI or CAI, CRP, and ESR differed significantly between endoscopic active and remission in CICD and UC patients, but not in CD-related surgery patients. The SES-CD correlated closely with levels of FC(r = 0.802), followed by CDAI(r = 0.734), CRP(r = 0.658), and ESR(r = 0.557). The Mayo score also correlated significantly with FC(r = 0.837), CAI(r = 0.776), ESR(r = 0.644), and CRP(r = 0.634). For FC, a cut-off value of 250 μg/g indicated endoscopic active inflammation with accuracies of 87.5%, 60%, and 91.1%, respectively, for CICD, CD-related surgery, and UC patients. Moreover, clinical FC activity(CFA) calculated as 0.8 × FC + 4.6 × CDAI showed higher area under the curve(AUC) of 0.962 for CICD and CFA calculated as 0.2 × FC + 50 × CAI showed higher AUC(0.980) for UC patients than the FC. Also, the diagnostic accuracy of FC in identifying patients with mucosal inflammation in clinical remission was reflected by an AUC of 0.91 for CICD and 0.96 for UC patients. CONCLUSION FC is the most promising noninvasive evaluation for monitoring the endoscopic activity of CICD and UC. 12Efficacy of a Chinese herbal formula on hepatitis B e antigenpositive chronic hepatitis B patients显示文摘BACKGROUND No guideline recommends antiviral therapy for hepatitis B e antigen(HBeAg)-positive chronic hepatitis B patients with persistently normal alanine aminotransferase levels and a high hepatitis B virus(HBV)DNA viral load.AIM To evaluate the feasibility and safety of a Chinese herbal formula as a therapeutic option for chronic HBV infection.METHODS In total,395 patients(30–65 years old)with confirmed HBeAg-positive chronic hepatitis B infection and persistently normal alanine aminotransferase were randomized to receive either Chinese herbal formula or placebo for 96 wk.Endpoints to evaluate therapeutic efficacy included:(1)HBV DNA levels decreased to less than 4 log10 IU/mL at weeks 48 and 96;and(2)HBeAg clearance and seroconversion rates at weeks 48 and 96.RESULTS HBV DNA levels≤4 log10 IU/mL were 10.05%at week 48 and 18.59%at week 96 in the treatment group.The HBeAg clearance and conversion rates were 8.54%and 8.04%at week 48 and 16.08%and 14.57%at week 96,respectively.However,HBV DNA levels≤4 log10 IU/mL were 2.55%and 2.55%at weeks 48 and 96,respectively,and the HBeAg clearance rates were 3.06%and 5.61%at weeks 48 and 96,respectively,in the control group.The quantitative hepatitis B surface antigen and HBeAg levels at baseline and changes during the treatment period as well as the alanine aminotransferase elevation at weeks 12 and 24 were strong predictors of HBeAg clearance.CONCLUSION High rates of HBV DNA reduction,HBeAg clearance and seroconversion could be achieved with Chinese herbal formula treatments,and the treatments were relatively safe for HBeAg-positive chronic hepatitis B-infected patients with persistently normal alanine aminotransferase.The ability of the compound to modulate host immune function probably contributed to this effect.Yu-Feng Xing Chun-Shan Wei Tian-Ran Zhou Dan-Ping Huang Wei-Chao Zhong Bin Chen Hua Jin Xiao-Yu Hu Zhi-Yun Yang Qing He Kai-Ping Jiang Jun-Min Jiang Zhen-Bin Hu Xin Deng Fan Yang Feng-Yi Li Gang Zhao Li-Chun Wang Yu-Qiang Mi Zuo-Jiong Gong Peng Guo Jian-Hua Wu Wei-Qun Shi Hong-Zhi Yang Da-Qiao Zhou Guang-Dong Tong 2020World Journal of Gastroenterology2020,26,30:7
13Prediction of the Residual Welding Stress in 2.25Cr-1Mo Steel by Taking into Account the Effect of the Solid-State Phase Transformations显示文摘A computational approach based on the thermal elastic plastic finite element method was developed for predicting welding residual stress in low carbon alloyed steel welds by taking into account the effect of the solid-state phase transformations. The kinetics of phase transformations was described by Johnson Mehl Avrami Kolmogrov (JMAK) equation for bainitic transition and by Koistinen-Marburger (K-M) relationship for martensitic transition. Moreover, an additive rule depending on volumetric phase fraction was adopted to represent the material property changes during heating and cooling. Consequently, the residual welding stresses in a 2.25Cr1Mo steel TIG welded plate were computed. 15Improvement cues of lesion absorption using the adjuvant therapy of traditional Chinese medicine Qinbudan tablet for retreatment pulmonary tuberculosis with standard anti-tuberculosis regimen显示文摘Background:China is the second highest pulmonary tuberculosis(PTB)burden country worldwide.However,retreatment of PTB has often developed resistance to at least one of the four first-line anti-TB drugs.The cure rate(approximately 50.0–73.3%)and management of retreatment of PTB in China needs to be improved.Qinbudan decoction has been widely used to treat PTB in China since the 1960s.Previously clinical studies have shown that the Qinbudan tablet(QBDT)promoted sputum-culture negative conversion and lesion absorption.However,powerful evidence from a randomized controlled clinical trial is lacking.Therefore,the aim of this study was to compare the efficacy and safety of QBDT as an adjunct therapy for retreatment of PTB.Methods:We conducted a multicenter,randomized,double-blind,placebo-controlled clinical trial in China.People diagnosed with PTB were enrolled who received previous anti-TB treatment from April 2011 to March 2013.The treatment group received an anti-TB regimen and QBDT,and the control group was administered an anti-TB regimen plus placebo.Anti-TB treatment options included isoniazid,rifampicin,pyrazinamide,ethambutol,streptomycin for 2 months(2HRZES),followed by isoniazid,rifampicin,ethambutol for 6 months(6HRE),daily for 8 months.Primary outcome was sputum-culture conversion using the MGIT 960 liquid medium method.Secondary outcomes included lung lesion absorption and cavity closure.Adverse events and reactions were observed after treatment.A structured questionnaire was used to record demographic information and clinical symptoms of all subjects.Data analysis was performed by SPSS 25.0 software in the full analysis set(FAS)population.Results:One hundred eighty-one cases of retreatment PTB were randomly divided into two groups:the placebo group(88 cases)and the QBDT group(93 cases).A total of 166 patients completed the trial and 15 patients lost to follow-up.The culture conversion rate of the QBDT group and placebo group did not show a noticeable improvement by using the covariate sites to correct the rate differences(79.6%vs 69.3%;rate difference=0.10,95%confidence interval(CI):-0.02–0.23;F=2.48,P=0.12)after treatment.A significant 16.6%increase in lesion absorption was observed in the QBDT group when compared with the placebo group(67.7%vs 51.1%;rate difference=0.17,95%CI:0.02–0.31;χ2=5.56,P=0.02).The intervention and placebo group did not differ in terms of cavity closure(25.5%vs 21.1%;rate difference=0.04,95%CI:-0.21–0.12;χ2=0.27,P=0.60).Two patients who received chemotherapy and combined QBDT reported pruritus/nausea and vomiting.Conclusions:No significant improvement in culture conversion was observed for retreatment PTB with traditional Chinese medicine plus standard anti-TB regimen.However,QBDT as an adjunct therapy significantly promoted lesion absorption,thereby reducing lung injury due to Mycobacterium tuberculosis infection.Trial registration:This trial is registered at ClinicalTrials.gov,NCT02313610.Shao-Yan Zhang Ji-You Fu Xiao-Yan Guo Ding-Zhong Wu Tong Zhang Cui Li Lei Qiu Chang-Rong Shao He-Ping Xiao Nai-Hui Chu Qun-Yi Deng Xia Zhang Xiao-Feng Yan Zhao-Long Wang Zhi-Jie Zhang Xin Jiang Yue-Juan Zheng Pei-Yong Zheng Hui-Yong Zhang Zhen-Hui Lu 2020Infectious Diseases of Poverty2020,9,3:7
16The Association of Socioeconomic Status with the Burden of Cataract-related Blindness and the Effect of Ultraviolet Radiation Exposure: An Ecological Study显示文摘Objective To assess the association of socioeconomic status with the burden of cataract blindness in terms of year lived with disability(YLD) rates and to determine whether ultraviolet radiation(UVR) levels modify the effect of socioeconomic status on this health burden.Methods National and subnational age-standardized YLD rates associated with cataract-related blindness were derived from the Global Burden of Disease(GBD) study 2017. The human development index(HDI) from the Human Development Report was used as a measure of socioeconomic status.Estimated ground-level UVR exposure was obtained from the Ozone Monitoring Instrument(OMI)dataset of the National Aeronautics and Space Administration(NASA).Results Across 185 countries, socioeconomic status was inversely associated with the burden of cataract blindness. Countries with a very high HDI had an 84% lower age-standardized YLD rate [95%confidence interval(CI): 60%–93%, P < 0.001] than countries with a low HDI;for high-HDI countries, the proportion was 76%(95% CI: 53%–88%, P < 0.001), and for medium-HDI countries, the proportion was48%(95% CI: 15%–68%, P = 0.010;P for trend < 0.001). The interaction analysis showed that UVR exposure played an interactive role in the association between socioeconomic status and cataract blindness burden(P value for interaction = 0.047).Conclusion Long-term high-UVR exposure amplifies the association of poor socioeconomic status with the burden of cataract-related blindness. 17A draft sequence of the rice (Oryza sativa ssp. indica) genome显示文摘The sequence of the rice genome holds fundamental information for its biology, including physiology, genetics, development, and evolution, as well as information on many beneficial phenotypes of economic significance. Using a 'whole genome shotgun' approach, we have pro-duced a draft rice genome sequence of Oryza sativa ssp. in-dica, the major crop rice subspecies in China and many other regions of Asia. The draft genome sequence is constructed from over 4.3 million successful sequencing traces with an accumulative total length of 2214.9 Mb. The initial assembly of the non-redundant sequences reached 409.76 Mb in length, based on 3.30 million successful sequencing traces with a total length of 1797.4 Mb from an indica variant cultivar 93-11, giving an estimated coverage of 95.29% of the rice genome with an average base accuracy of higher than 99%. 18Epidural electrical stimulation effectively restores locomotion function in rats with complete spinal cord injury显示文摘Epidural electrical stimulation can restore limb motor function after spinal cord injury by reactivating the surviving neural circuits.In previous epidural electrical stimulation studies,single electrode sites and continuous tetanic stimulation have often been used.With this stimulation,the body is prone to declines in tolerance and locomotion coordination.In the present study,rat models of complete spinal cord injury were established by vertically cutting the spinal cord at the T8 level to eliminate disturbance from residual nerve fibers,and were then subjected to epidural electrical stimulation.The flexible extradural electrode had good anatomical topology and matched the shape of the spinal canal of the implanted segment.Simultaneously,the electrode stimulation site was able to be accurately applied to the L2–3 and S1 segments of the spinal cord.To evaluate the biocompatibility of the implanted epidural electrical stimulation electrodes,GFAP/Iba-1 doublelabeled immunofluorescence staining was performed on the spinal cord below the electrodes at 7 days after the electrode implantation.Immunofluorescence results revealed no significant differences in the numbers or morphologies of microglia and astrocytes in the spinal cord after electrode implantation,and there was no activated Iba-1~+cell aggregation,indicating that the implant did not cause an inflammatory response in the spinal cord.Rat gait analysis showed that,at 3 days after surgery,gait became coordinated in rats with spinal cord injury under burst stimulation.The regained locomotion could clearly distinguish the support phase and the swing phase and dynamically adjust with the frequency of stimulus distribution.To evaluate the matching degree between the flexible epidural electrode(including three stimulation contacts),vertebral morphology,and the level of the epidural site of the stimulation electrode,micro-CT was used to scan the thoracolumbar vertebrae of rats before and after electrode implantation.Based on the experimental results of gait recovery using three-site stimulation electrodes at L2–3 and S1 combined with burst stimulation in a rat model of spinal cord injury,epidural electrical stimulation is a promising protocol that needs to be further explored.This study was approved by the Animal Ethics Committee of Chinese PLA General Hospital(approval No.2019-X15-39)on April 19,2019.Song Wang Li-Cheng Zhang Hai-Tao Fu Jun-Hao Deng Gao-Xiang Xu Tong Li Xin-Ran Ji Pei-Fu Tang 2021Neural Regeneration Research2021,16,3:6
