| 1 | Immunopathogenesis of inflammatory bowel disease显示文摘Crohn’s disease and ulcerative colitis are chronic relapsing immune mediated disorders that results from an aberrant response to gut luminal antigen in genetically susceptible host. The adaptive immune response that is then triggered was widely considered to be a T-helper-1 mediated condition in Crohn’s disease and T-helper-2 mediated condition in ulcerative colitis. | 2 | Long-term follow-up after complete ablation of Barrett's esophagus with argon plasma coagulation显示文摘AIM: To report the long-term outcome of patients after complete ablation of non-neoplastic Barrett's esophagus (BE) with respect to BE relapse and development of intraepithelial neoplasia or esophageal adenocarcinoma.METHODS: In 70 patients with histologically proven nonneoplastic BE, complete BE ablation was achieved by argon plasma coagulation (APC) and high-dose proton pump inhibitor therapy (120 mg omeprazole daily). Sixty-six patients (94.4%) underwent further surveillance endoscopy. At each surveillance endoscopy four-quadrant biopsies were taken from the neo-squamous epithelium at 2 cm intervals depending on the pre-treatment length of BE mucosa beginning at the neo-Z-line, and from any endoscopically suspicious lesion.RESULTS: The median follow-up of 66 patients was 51 mo (range 9-85 mo) giving a total of 280.5 patient years.A mean of 6 biopsies were taken during surveillance endoscopies. In 13 patients (19.7%) tongues or islands suspicious for BE were found during endoscopy. In 8 of these patients (12.1%) non-neoplastic BE relapse was confirmed histologically giving a histological relapse rate of 3% per year. | 3 | Metastatic type 1 gastric carcinoid:A real threat or just a myth?显示文摘AIM:To describe disease characteristics and treatment modalities in a group of rare patients with metastatic gastric carcinoid type 1(GCA1).METHODS:Information on clinical,biochemical,radiological,histopathological findings,the extent of the disease,as well as the use of different therapeutic modalities and the long-term outcome were recorded.Patients’data were assessed at presentation,and thereafter at 6 to 12 monthly intervals both clinically and biochemically,but also endoscopically and histopathologically.Patients were evaluated for the presence of specific symptoms;the presence of autoimmune disorders and the presence of other gastrointestinal malignancies in other family members were also recorded.The evaluation of response to treatment was defined using established WHO criteria.RESULTS:We studied twenty consecutive patients with a mean age of 55.1 years.The mean follow-up period was 83 mo.Twelve patients had regional lymph node metastases and 8 patients had liver metastases.The primary tumor mean diameter was 20.13±10.83mm(mean±SD).The mean Ki-67 index was 6.8%±11.2%.All but one patient underwent endoscopic or surgical excision of the tumor.The disease was stable in all but 3 patients who had progressive liver disease.All patients remained alive during the follow-up period.CONCLUSION:Metastatic GCA1 carries a good overall prognosis,being related to a tumor size of≥1 cm,an elevated Ki-67 index and high serum gastrin levels. | Simona Grozinsky-Glasberg Dimitrios Thomas Jonathan R Strosberg Ulrich-Frank Pape Stephan Felder Apostolos V Tsolakis Krystallenia I Alexandraki Merav Fraenkel Leonard Saiegh Petachia Reissman Gregory Kaltsas David J Gross | 2013 | World Journal of Gastroenterology2013，19，46： | 11 |

| 5 | Could the number of CT angiograms be reduced in emergency department patients suspected of pulmonary embolism?显示文摘BACKGROUND:This study was undertaken to identify the prevalence of pulmonary embolism(PE) in the emergency department(ED) of an urban teaching hospital and also to test a Bayesian model in estimating the number of CT pulmonary angiography(CTA) expected to be performed in an emergency department.METHODS:The data for this study was obtained through a retrospective review of electronic medical records for all ED patients suspected of PE who underwent chest CTA or ventilation perfusion scanning(V/Q) between 2009 and 2010.The data is presented as means and standard deviation for continuous variables and percentages with 95%confidence intervals(95%CI) for proportions.The prevalence of PE was used as pre-test probability in the Bayesian model.Post-test probability was obtained using a Fagan nomogram and likelihood ratios for CTA.RESULTS:A total of 778 patients(560 females) with mean age of 50 years(range 18-98 years)were enrolled(98.3%underwent chest CTA and 1.7%underwent V/Q scan).A total of 69 patients had PE,rendering an overall prevalence of 8.9%(95%CI,7.1%to 11.1%) for PE.We calculated that132 CTA's per year could be avoided in our institution,without compromising safe exclusions of PE(keeping post-test probability of PE below 2%).CONCLUSIONS:Despite differences in our patient populations and /or study designs,the prevalence of PE in our institution is about average compared to other institutions.Our proposed model for calculating redundant chest CTA is simple and can be used by institutions to identify overuse of CTA. | Zehtabchi Shahriar Rinnert Stephan Malhotra Shweta Subramanian Arun Timberger Mathew Patel Brijal Toro David Hassan Khaled Sinert Richard | 2012 | World Journal of Emergency Medicine2012，3，3： | 7 |
| 6 | Review of the impact of heat stress on reproductive performance of sheep显示文摘Heat stress significantly impairs reproduction of sheep,and under current climatic conditions is a significant risk to the efficiency of the meat and wool production,with the impact increasing as global temperatures rise.Evidence from field studies and studies conducted using environmental chambers demonstrate the effects of hot temperatures(≥32℃)on components of ewe fertility(oestrus,fertilisation,embryo survival and lambing)are most destructive when experienced from 5 d before until 5 d after oestrus.Temperature controlled studies also demonstrate that ram fertility,as measured by rates of fertilisation and embryo survival,is reduced when mating occurs during the period 14 to 50 d post-heating.However,the contribution of the ram to heat induced reductions in flock fertility is difficult to determine accurately.Based primarily on temperature controlled studies,it is clear that sustained exposure to high temperatures(≥32℃)during pregnancy reduces lamb birthweight and will,therefore,decrease lamb survival under field conditions.It is concluded that both ewe and ram reproduction is affected by relatively modest levels of heat stress(≥32℃)and this is a concern given that a significant proportion of the global sheep population experiences heat stress of this magnitude around mating and during pregnancy.Despite this,strategies to limit the impacts of the climate on the homeothermy,behaviour,resource use and reproduction of extensively grazed sheep are limited,and there is an urgency to improve knowledge and to develop husbandry practices to limit these impacts. | William H.E.J.van Wettere Karen L.Kind Kathryn L.Gatford Alyce M.Swinbourne Stephan T.Leu Peter T.Hayman Jennifer M.Kelly Alice C.Weaver David O.Kleemann Simon K.Walker | 2021 | Journal of Animal Science and Biotechnology2021，12，3： | 6 |
