1Cardioprotection by remote ischemic conditioning: Mechanisms and clinical evidences显示文摘In remote ischemic conditioning(RIC), several cycles of ischemia and reperfusion render distant organ and tissues more resistant to the ischemia-reperfusion injury. The intermittent ischemia can be applied before the ischemic insult in the target site(remote ischemic preconditioning), during the ischemic insult(remote ischemic perconditioning) or at the onset of reperfusion(remote ischemic postconditioning). The mechanisms of RIC have not been completely defined yet; however, these mechanisms must be represented by the release of humoral mediators and/or the activation of a neural reflex. RIC has been discovered in the heart, and has been arising great enthusiasm in the cardiovascular field. Its efficacy has been evaluated in many clinical trials, which provided controversial results. Our incomplete comprehension of the mechanisms underlying the RIC could be impairing the design of clinical trials and the interpretation of their results. 2Effects of major ozonated autohemotherapy in the treatment of dry age related macular degeneration: a randomized controlled clinical study显示文摘AIM: To evaluate the effect of systemic ozonated major autohaemotherapy (O3-AHT) in patients affected by dry age related macular degeneration (AMD). METHODS: This study was a randomized, controlled clinical study. One hundred and forty patients with the diagnosis of AMD in both eyes, with the study eye presenting dry AMD and soft drusen, were randomly assigned in a 1:1 ratio to either receive 27 major ozonated autohemotherapy treatments during 12-month period, or a standardized multi-vitamin therapy. Primary outcome was the change in best corrected visual acuity (mean logMar change) between the baseline and 6 and 12 months, end point of the study. In addition, to investigate the safety of prolonged ozonated autohaemotherapy, we measured the routine haemato- chemical parameters and biochemical oxidative stress values at baseline and after 12 months treatment time. RESULTS: The mean baseline best corrected visual acuity in study eyes was 0.36 in the treatment group and 0.38 in the control group (difference not statistically significant). At the primary endpoint, 6 months post-baseline, the mean logMAR change in the treated group improved by 0.1 and the values of the control group at the same time impaired by 0.2 respect to the baseline. Four percent and twenty-five percent of eyes in the group treated with O3-AHT gained 1 or more lines after 6 and 12 months respectively compared to 0% in the eyes which received no treatment (P <0.05 at 12 months). None of the treated patients experienced a loss in visual acuity in their study eye at 6 and 12 months, compared to 16% and 40 % of patients in the control group who lost 2 lines or more at 6 months and 12 months respectively (P <0.05 treated vs control group)). Major ozonated autohemotherapy was shown to be safe and well- tolerated by the patients. Moreover, the haematochemical parameters showed a decrease in the Reactive Oxygen Metabolites (300±10.1 UCARR at 12 months compared to a baseline value of 380±10.4 UCARR, P <0.05) and an increase in Biological Antioxidant Potential plasma values (2100±34.8 micromoles/ C vitamin after 12 months compared to the baseline value of 1610±36.2, P <0.05) in the treated patients when compared to the control group. This data suggests that major ozonated autohaemotherapy may exert a role in reducing oxidative stress by endogenously stimulating the production of antioxidant molecules. 3Role of CARD15,DLG5 and OCTN genes polymorphisms in children with inflammatory bowel diseases显示文摘AIM: To investigate the contribution of variants of CARD15, OCTN1/2 and DLG5 genes in disease predispo- sition and phenotypes in a large Italian cohort of pediatric patients with inflammatory bowel diseases (IBD). METHODS: Two hundred patients with Crohn’s disease (CD), 186 ulcerative colitis (UC) patients, 434 par- ents (217 trios), and 347 healthy controls (HC) were studied. Polymorphisms of the three major variants of CARD15, 1672C/T and -207G/C SNPs for OCTN genes, IGR2096a_1 and IGR2198a_1 SNPs for the IBD5 locus, and 113G/A variant of the DLG5 gene were evaluated. Potential correlations with clinical sub-phenotypes were investigated. RESULTS: Polymorphisms of CARD15 were significantly associated with CD, and at least one variant was found in 38% of patients (15% in HC, OR = 2.7, P < 0.001). Homozygosis for both OCTN1/2 variants was more com- mon in CD patients (1672TT 24%, -207CC 29%) than in HC (16% and 21%, respectively; P = 0.03), with an in- creased frequency of the TC haplotype (44.8% vs 38.3% in HC, P = 0.04). No association with the DLG5 variant was found. CD carriers of OCTN1/2 and DLG5 variants more frequently had penetrating disease (P = 0.04 and P = 0.01), while carriers of CARD15 more frequently had ileal localization (P = 0.03). No gene-gene interaction was found. In UC patients, the TC haplotype was morefrequent (45.4%, P = 0.03), but no genotype/phenotype correlation was observed. CONCLUSION: Polymorphisms of CARD15 and OCTN genes, but not DLG5 are associated with pediatric on- set of CD. 4Precision medicine in gastric cancer显示文摘Gastric cancer(GC)is a complex disease linked to a series of environmental factors and unhealthy lifestyle habits,and especially to genetic alterations.GC represents the second leading cause of cancer-related deaths worldwide.Its onset is subtle,and the majority of patients are diagnosed once the cancer is already advanced.In recent years,there have been innovations in the management of advanced GC including the introduction of new classifications based on its molecular characteristics.Thanks to new technologies such as next-generation sequencing and microarray,the Cancer Genome Atlas and Asian Cancer Research Group classifications have also paved the way for precision medicine in GC,making it possible to integrate diagnostic and therapeutic methods.Among the objectives of the subdivision of GC into subtypes is to select patients in whom molecular targeted drugs can achieve the best results;many lines of research have been initiated to this end.After phase III clinical trials,trastuzumab,anti-Erb-B2 receptor tyrosine kinase 2(commonly known as ERBB2)and ramucirumab,anti-vascular endothelial growth factor receptor 2(commonly known as VEGFR2)monoclonal antibodies,were approved and introduced into first-and second-line therapies for patients with advanced/metastatic GC.However,the heterogeneity of this neoplasia makes the practical application of such approaches difficult.Unfortunately,scientific progress has not been matched by progress in clinical practice in terms of significant improvements in prognosis.Survival continues to be low in contrast to the reduction in deaths from many common cancers such as colorectal,lung,breast,and prostate cancers.Although several target molecules have been identified on which targeted drugs can act and novel products have been introduced into experimental therapeutic protocols,the overall approach to treating advanced stage GC has not substantially changed.Currently,surgical resection with adjuvant or neoadjuvant radiotherapy and chemotherapy are the most effective treatments for this disease.Future research should not underestimate the heterogeneity of GC when developing diagnostic and therapeutic strategies aimed toward improving patient survival.Patrizia Bonelli Antonella Borrelli Franca Maria Tuccillo Lucrezia Silvestro Raffaele Palaia Franco Maria Buonaguro 2019World Journal of Gastrointestinal Oncology2019,11,10:8
5Recruitment of endogenous progenitor cells by erythropoietin loaded particles for in situ cartilage regeneration显示文摘Cartilage injury affects millions of people throughout the world,and at this time there is no cure.While transplantation of stem cells has shown some success in the treatment of injured cartilage,such treatment is limited by limited cell sources and safety concerns.To overcome these drawbacks,a microscaffolds system was developed capable of targeting,reducing the inflammatory response and recruiting endogenous progenitor cells to cartilage-defect.Erythropoietin(EPO)-loaded-hyaluronic acid(HA)microscaffolds(HA+EPO)were fabricated and characterized.HA-microscaffolds showed good cell-compatibility and could target chondrocytes via CD44 receptors.HA+EPO was designed to slowly release EPO while recruiting progenitor cells.Finally,the ability of HA+EPO to repair cartilage-defects was assessed using a rabbit model of full-thickness cartilagedefect.Our results showed that the intra-articular administration of EPO,HA,and EPO+HA reduced the number of inflammatory cells inside the synovial-fluid,while EPO+HA had the greatest anti-inflammatory effects.Furthermore,among all groups,EPO+HA achieved the greatest progenitor cell recruitment and subsequent chondrogenesis.The results of this work support that,by targeting and localizing the release of growthfactors,HA+EPO can reduce inflammatory responses and promote progenitor cells responses.This new platform represents an alternative treatment to stem-cell transplantation for the treatment of cartilage injury.Amirhossein Hakamivala Shuxin li Kayti Robinson YiHui Huang Shuai Yu Baohong Yuan Joseph Borrelli Jr. Liping Tang 2020Bioactive Materials2020,5,1:7

7Short- and long-term efficacy of endoscopic balloon dilation in Crohn's disease strictures显示文摘AIM:To evaluate short- and long-term efficacy of endoscopic balloon dilation in a cohort of consecutive patients with symptomatic Crohn's disease (CD)-related strictures. METHODS:Twenty-six CD patients (11 men; median age 36.8 year, range 11-65 years) with 27 symptomatic strictures underwent endoscopic balloon dilation (EBD).Both naive and post-operative strictures, of any length and diameter, with or without associated fistula were included. After a clinical and radiological assessment, EBD was performed with a Microvasive Rigiflex through the scope balloon system. The procedure was considered successful if no symptom reoccurred in the following 6 mo. The long-term clinical outcome was to avoid surgery. RESULTS:The mean follow-up time was 40.7 ± 5.7 mo (range 10-94 mo). In this period, forty-six EBD were performed with a technical success of 100%. No procedure-related complication was reported. Surgery was avoided in 92.6% of the patients during the entire follow-up. Two patients, both presenting ileocecal strictures associated with fistula, failed to respond to the treatment and underwent surgical strictures resection. Of the 24 patients who did not undergo surgery, 11 patients received 1 EBD, and 13 required further dilations over time for the treatment of relapsing strictures (7 patients underwent 2 dilations, 5 patients 3 dilations, and 1 patient 4 dilations). Overall, the EBD success rate after the first dilation was 81.5%. No difference was observed between the EBD success rate for naive (n = 12) and post-operative (n = 15) CD related strictures (P > 0.05). 10采用间接复位、钢板固定、不植骨的方法治疗稳定的股骨干髓内假体周围骨折显示文摘背景:间接复位技术相对于较早期的开放复位技术具有能够提高骨愈合率及减少植骨的优势,我们对以间接复位治疗股骨干假体周围骨折的结果进行了研究。 方法:连续对50例稳定的股骨干髓内假体周围骨折(Vancourvr B1型)进行治疗,根据病例记录包括切开复位、间接复位、外侧单钢板固定且不行同种异体或其他植骨。4例术后早期死亡,5例随访时间短,对其余41例(平均年龄72岁)进行了平均24个月的随访,并进行临床和影像学评估。 结果:所有骨折平均12周(7~23周)愈合,对位满意。1例有1根钢丝断裂,2例有1枚螺钉断裂,骨折最终愈合且无明显的内置物松动和对位不良。1例发生围手术期深部感染。41例中30例行走功能恢复到骨折前的状态。 结论:结果证实间接复位骨膜外侧单钢板固定且不进行植骨的方法对于稳定的股骨干髓内假体周围骨折的疗效是应该肯定的。 可信水平:治疗性研究,Ⅳ级,进一步可信度参见作者介绍。WILLIAM M.
