| 1 | Systematic review of the outcomes of surgical resection for intermediate and advanced Barcelona Clinic Liver Cancer stage hepatocellular carcinoma:A critical appraisal of the evidence显示文摘AIM To perform a systematic review to determine the survival outcomes after curative resection of intermediate and advanced hepatocellular carcinomas(HCC).METHODS A systematic review of the published literature was performed using the PubM ed database from 1 st January 1999 to 31 st Dec 2014 to identify studies that reported outcomes of liver resection as the primary curative treatment for Barcelona Clinic Liver Cancer(BCLC) stage B or C HCC. The primary end point was to determine the overall survival(OS) and disease free survival(DFS) of liver resection of HCC in BCLC stage B or C in patients with adequate liver reserve(i.e., Child's A or B status). The secondary end points were to assess the morbidity and mortality of liver resection in large HCC(defined as lesions larger than 10 cm in diameter) and to compare the OS and DFS after surgical resection of solitary vs multifocal HCC.RESULTS We identified 74 articles which met the inclusion criteria and were analyzed in this systematic review. Analysis of the resection outcomes of the included studies were grouped according to(1) BCLC stage B or C HCC,(2) Size of HCC and(3) multifocal tumors. The median 5-year OS of BCLC stage B was 38.7%(range 10.0-57.0); while the median 5-year OS of BCLC stage C was 20.0%(range 0.0-42.0). The collective median 5-year OS of both stages was 27.9%(0.0-57.0). In examining the morbidity and mortality following liver resection in large HCC, the pooled RR for morbidity [RR(95%CI) = 1.00(0.76-1.31)] and mortality [RR(95%CI) = 1.15(0.73-1.80)] were not significant. Within the spectrum of BCLC B and C lesions, tumors greater than 10 cm were reported to have median 5-year OS of 33.0% and multifocal lesions 54.0%.CONCLUSION Indication for surgical resection should be extended to BCLC stage B lesions in selected patients. | 2 | Gastric peritoneal carcinomatosis-a retrospective review显示文摘AIM To characterize patients with gastric peritoneal carcinomatosis(PC) and their typical clinical and treatment course with palliative systemic chemotherapy as the current standard of care.METHODS We performed a retrospective electronic chart review of all patients with gastric adenocarcinoma with PC diagnosed at initial metastatic presentation between January 2010 and December 2014 in a single tertiary referral centre.RESULTS We studied a total of 271 patients with a median age of 63.8 years and median follow-up duration of 5.1 mo. The majority(n = 217, 80.1%) had the peritoneum as the only site of metastasis at initial presentation. Palliative systemic chemotherapy was eventually planned for 175(64.6%) of our patients at initial presentation, of which 171 were initiated on it. Choice of first-line regime was in accordance with the National Comprehensive Cancer Network Guidelines for Gastric Cancer Treatment. Thesepatients underwent a median of one line of chemotherapy, completing a median of six cycles in total. Chemotherapy disruption due to unplanned hospitalizations occurred in 114(66.7%), while cessation of chemotherapy occurred in 157(91.8%), with 42 cessations primarily attributable to PC-related complications. Patients who had initiation of systemic chemotherapy had a significantly better median overall survival than those who did not(10.9 mo vs 1.6 mo, P < 0.001). Of patients who had initiation of systemic chemotherapy, those who experienced any disruptions to chemotherapy due to unplanned hospitalizations had a significantly worse median overall survival compared to those who did not(8.7 mo vs 14.6 mo, P < 0.001).CONCLUSION Gastric PC carries a grim prognosis with a clinical course fraught with disease-related complications which may attenuate any survival benefit which palliative systemic chemotherapy may have to offer. | 3 | Influencing Factors on Rice Sheath Blight Epidemics in Integrated Rice-Duck System显示文摘Sheath blight, a disease caused by the fungus Rhizoctonia solani Kuhn (anamorph), has been the most economically significant disease of rice. It was frequently reported that the disease was well-controlled in integrated rice-duck system without the employment of fungicides. However, the effecting factors behind this phenomenon were rarely reported. In this research, experiment was carried out between two treatments, rice combined with ducks (RD) and conventional rice field without ducks rearing (CK) in early season rice paddy, to investigate the variations of sclerotia in floodwater and on rice plant, microclimate 10 cm above the waterline in rice paddy and activity of protective enzymes in rice plants. The results showed that the floating sclerotia in floodwater in RD was 86-91% lower than that in CK, and adhering sclerotia in rice plant in RD was 67-78% lower than that in CK. The relative humidity tested significantly lower and light intensity tested significantly higher in RD. The temperature in the early rice growth stages in RD was slightly lower than that in CK, but it was significantly higher (32.3-36.5°C) in the middle stage rice growth stages. | 4 | Capecitabine with radiation is an effective adjuvant therapy in gastric cancers显示文摘AIM:To analyze the outcome of patients who received concurrent capecitabine(Xeloda) and radiation(XRT) compared to the established concurrent 5-fluorouracil(5-FU) with radiation(5FU-RT) and fluoropyrimidine-based chemotherapy alone as adjuvant treatment in gastric cancers.METHODS:All patients with gastric cancers who received adjuvant treatment at the National Cancer Centre Singapore between 1996 and 2006 were reviewed.Treatment outcomes of patients who received XRT were compared with those who had 5FU-RT or chemotherapy alone as adjuvant therapy for gastric cancers.RESULTS:A total of 108 patients were reviewed.Median age at diagnosis was 60.The majority of the patients(64.8%) had advanced stage Ⅲ and Ⅳ disease(with no distant metastasis).All except 4 patients had D2 gastrectomy.Twenty one patients(19.4%) had positive surgical resection margins.Thirty three patients received XRT compared with 52 who had 5FU-RT and 23 who received chemotherapy alone.For the patients in the chemotherapy-only group,all had fluoropyrimidine-based therapy,with added cisplatin in 7 patients and epirubicin in 2 patients.Median recurrence-free survival was longer for the XRT group(52 mo) compared to the 5FU-RT(35 mo) and chemotherapy-only groups(25 mo)(P=0.48).The patients in the XRT group achieved similar median overall survival(53 mo) as the 5FU-RT(54 mo) and the chemotherapy-only groups(44 mo)(P=0.5).CONCLUSION:Capecitabine with concurrent radiation was as effective as concurrent 5FU with radiation or fluoropyrimidine-based chemotherapy alone when used as adjuvant treatment in patients with gastric cancers. | Chee Kian Tham Su Pin Choo Donald Yew Hee Poon Han Chong Toh Simon Yew Kuang Ong Sze Huey Tan Michael Lian Chek Wang Kian Fong Foo | 2010 | World Journal of Gastroenterology2010,16,29: | 6 |
| 5 | Cerebrospinal fluid phosphorylated tau,visinin-like protein-1,and chitinase-3-like protein 1 in mild cognitive impairment and Alzheimer’s disease显示文摘Background:Visinin-like protein-1(VILIP-1)and chitinase-3-like protein 1(CHI3L1 or YKL-40)in cerebrospinal fluid(CSF)are newly discovered markers indicating neuronal damage and microglial activation,respectively.Phosphorylated tau(p-tau)reflects the neuropathology of Alzheimer’s disease(AD)and is useful as diagnostic markers for AD.However,it is unknown whether these biomarkers have similar or complementary information in AD.Methods:We stratified 121 participants from the Alzheimer’s Disease Neuroimaging Initiative(ADNI)database into cognitively normal(CN),stable mild cognitive impairment(sMCI),progressive MCI(pMCI),and dementia due to AD.Analysis of covariance(ANOVA)and chi-square analyses,Spearman correlation,and logistic regression models were performed to test the demographic,associations between biomarkers,and diagnostic accuracies,respectively.Linear mixed-effects models were used to evaluate the effects of CSF amyloid-β(Aβ)on above biomarkers within diagnostic groups,the combination of diagnostic group and Aβstatus as predictor,and CSF biomarkers as predictors of AD features,including cognition measured by Mini–Mental State Examination(MMSE)and brain structure and white matter hyperintensity(WMH)measured by magnetic resonance imaging(MRI).Results:P-tau,VILIP-1,and YKL-40 were all predictors of AD diagnosis,but combinations of biomarkers did not improve the diagnostic accuracy(AUC 0.924 for p-tau,VILIP-1,and YKL-40)compared to p-tau(AUC 0.922).P-tau and VILIP-1 were highly correlated(r=0.639,p<0.001)and strongly associated with Aβpathology across clinical stages of AD,while YKL-40 was correlated with Aβpathology in CN and AD groups.VILIP-1 was associated with acceleration of cognitive decline,hippocampal atrophy,and expansion of ventricles in longitudinal analyses.YKL-40 was associated with hippocampal atrophy at baseline and follow-up,while p-tau was only associated with worsening WMH at baseline.Conclusions:CSF levels of p-tau,VILIP-1,and YKL-40 may have utility for discriminating between cognitively normal subjects and patients with AD.Increased levels of both VILIP-1 and YKL-40 may be associated with disease degeneration.These CSF biomarkers should be considered for future assessment in the characterization of the natural history of AD. | Hua Zhang Kok Pin Ng Joseph Therriault Min Su Kang Tharick APascoal Pedro Rosa-Neto Serge Gauthier the Alzheimer’s Disease Neuroimaging Initiative | 2018 | Translational Neurodegeneration2018,7,1: | 5 |

The paper presents a process where Cu-Cr-Sn-Zn lead frame alloy is produced by rapid solidification and aging. The microcrystalline structure of rapidly solidified Cu-Cr-Sn-Zn alloy is smaller grain structure examined by optical metallography. The effects of aging processes on the microstructure and properties of the lead frame alloy were investigated. | 7 | Cenozoic Calcareous Nannofossil Biostratigraphy of Sediments from Lingfeng Well in East China Sea显示文摘A biostratigraphic study of calcareous nannofossils of sediments from Lingfeng No.1 well in the East China Sea was carried out to provide more detailed nannofossil data and a stratigraphic correlation among offshore wells located in the East China Sea.14 Paleocene to Pleistocene calcareous nannofossil zones were recognized based on a detailed examination. According to these nannofossil data, the Paleocene/Eocene and Pliocene/Pleistocene boundaries, as well as the Miocene base were determined.Oligocene NP23 and Miocene NN1 zones are firstly recognized by this study. The existence of Miocene NN1 at this well suggests that the Miocene transgression began in the Early Miocene in this area. | 11 | The application of proton spectrometers at the SG-III facility for ICF implosion areal density diagnostics显示文摘Charged particle diagnostics is one of the required techniques for implosion areal density diagnostics at the SG-III facility.Several proton spectrometers are under development, and some preliminary areal density diagnostics have been carried out. The response of the key detector, CR39, to charged particles was investigated in detail. A new track profile simulation code based on a semi-empirical model was developed. The energy response of the CR39 detector was calibrated with the accelerator protons and alphas from a241 Am source. A proton spectrometer based on the filtered CR39 detector was developed, and D–D primary proton measurements were implemented. A step range filter spectrometer was developed,and preliminary areal density diagnostics was carried out. A wedged range filter spectrometer array made of Si with a higher resolution was designed and developed at the SG-III facility. A particle response simulation code by the Monte Carlo method and a spectra unfolding code were developed.
