| 1 | Portal vein thrombosis, mortality and hepatic decompensation in patients with cirrhosis: A meta-analysis显示文摘AIM: To determine the clinical impact of portal vein thrombosis in terms of both mortality and hepatic decompensations(variceal hemorrhage, ascites, portosystemic encephalopathy) in adult patients with cirrhosis.METHODS: We identified original articles reported through February 2015 in MEDLINE, Scopus, Science Citation Index, AMED, the Cochrane Library, and relevant examples available in the grey literature. Two independent reviewers screened all citations for inclusion criteria and extracted summary data. Random effects odds ratios were calculated to obtain aggregate estimates of effect size across included studies, with 95%CI.RESULTS: A total of 226 citations were identified and reviewed, and 3 studies with 2436 participants were included in the meta-analysis of summary effect. Patients with portal vein thrombosis had an increased risk of mortality(OR = 1.62, 95%CI: 1.11-2.36, P = 0.01). Portal vein thrombosis was associated with an increased risk of ascites(OR = 2.52, 95%CI: 1.63-3.89, P < 0.001). There was insufficient data available to determine the pooled effect on other markers of decompensation including gastroesophageal variceal bleeding or hepatic encephalopathy. CONCLUSION: Portal vein thrombosis appears to increase mortality and ascites, however, the relatively small number of included studies limits more generalizable conclusions. | 2 | The Plant Vascular System: Evolution, Development and Functions显示文摘The emergence of the tracheophyte-based vascular system of land plantshad major impacts on the evolution of terrestrial biology, in general,through its role in facilitating the development of plants with increasedstature, photosynthetic output, and ability to colonize a greatly expandedrange of environmental habitats. Recently, considerable progress hasbeen made in terms of our understanding of the developmental andphysiological programs involved in the formation and function of theplant vascular system. In this review, we first examine the evolutionaryevents that gave rise to the tracheophytes, followed by analysis of thegenetic and hormonal networks that cooperate to orchestrate vasculardevelopment in the gymnosperms and angiosperms. The two essentialfunctions performed by the vascular system, namely the delivery of resources (water, essential mineralnutrients, sugars and amino acids) to the various plant organs and provision of mechanical support arenext discussed. Here, we focus on critical questions relating to structural and physiological propertiescontrolling the delivery of material through the xylem and phloem. Recent discoveries into the role ofthe vascular system as an effective long-distance communication system are next assessed in termsof the coordination of developmental, physiological and defense-related processes, at the whole-plantlevel. A concerted effort has been made to integrate all these new findings into a comprehensive pictureof the state-of-the-art in the area of plant vascular biology. Finally, areas important for future researchare highlighted in terms of their likely contribution both to basic knowledge and applications to primaryindustry. | William J. | 8 | Spontaneous bacterial peritonitis prevalence in pretransplant patients and its effect on survival and graft loss post-transplant显示文摘AIM To investigate the incidence of spontaneous bacterial peritonitis(SBP) in pre-transplant patients and its effect on post transplant mortality and graft failure. METHODS We conducted a retrospective cohort study of patient records from the organ procurement and transplant network data set. Patients were identified by the presence of SBP pre-transplant. Univariate post-transplant survival models were constructed using the Kaplan-Meier technique and multivariate models were constructed using the Cox proportional hazards model. Variables that affected post-transplant graft survival were identified in the SBP population. RESULTS Forty-seven thousand eight hundred and eighty patient records were included in the analysis for both groups, and 1966(4.11%) patients were identified in the data set as having pre-transplant SBP. Patients that had pre-transplant SBP had higher rates of graft loss from recurrent hepatitis C virus(HCV)(3.6% vs 2.0%, P < 0.0001), infections leading to graft loss(1.9% vs 1.3%, P = 0.02), primary non-function(4.3% vs 3.0%, P < 0.0001) and chronic rejection(1.1% vs 0.7%, P = 0.04). Kaplan-Meier survival analysis showed a statistically significant difference in all-cause survival in patients with a history of SBP vs those without(P < 0.0001). Pretransplant history of SBP was independently predictiveof mortality due to recurrent HCV(HR = 1.11, 95%CI: 1.02-1.21, P < 0.017) after liver transplantation.CONCLUSION HCV patients prior to the advent of directing acting anti-viral agents had a higher incidence of pre-transplant SBP than other patients on the liver transplant wait list. | 11 | Client-level predictors of treatment engagement,outcome and dropout:moving beyond demographics显示文摘Background Despite the availability of evidence-based treatments for posttraumatic stress disorder(PTSD),significant heterogeneity in the effectiveness of PTSD treatment persists,especially in community settings.Client demographics used to understand this variability in treatment outcome and dropout have yielded mixed results.Despite increasing evidence for the importance of attending to treatment engagement in community settings,few studies have explored client-level predictors.Aim The purpose of this study is to explore client-level predictors of treatment outcome and dropout beyond client demographics,and to identify client-level predictors of treatment engagement in community settings.Method Secondary data analysis was conducted with data collected as part of an implementation-effectiveness hybrid study of cognitive processing therapy(CPT)for PTSD in a diverse community healthcentre.Providers(n=19)treated(n=52)clients as part of their routine clinical care.Non-demographic client-level predictors included barriers to treatment,quality of life,session-level language and employment history assessed at baseline.Treatment engagement included number of weeks in the study,number of sessions with repeated CPT content,number of unique CPT sessions attended,frequency of session attendance and consistency of session attendance.Results Results showed language as a significant predictor of treatment engagement.There were significant differences between Spanish and English-speaking clients,with the former having a tendency to repeat more session content than the latter(β=1.4 sessions,p=0.003),and also less likely to attend treatment frequently(r=0.62,p=0.009)and consistently(r=0.57,p=0.027)if high logistical and financial barriers were endorsed.Irrespective of language,clients who reported high quality of life at baseline were less likely to repeat CPT session content(β=-0.3,p=0.04),and those with increased baseline barriers to treatment had deceleration in PTSD symptom improvement over time(β=-0.62,p<0.05).In terms of treatment engagement moderators impacting treatment outcome,clients who repeated more session content were more likely to complete treatment(0R=1.84,p=0.037).Conclusion Identification of client-level predictors of treatment engagement,outcome and dropout is essential to optimise treatment,particularly in community settings. | Soo-jeong Youn Margaret-Anne Mackintosh Shannon Wiltsey Stirman Kay lie A Patrick Yesenia Aguilar Silvan Anna D Bartuska Derri L Shtasel Luana Marques | 2019 | General Psychiatry2019，32，6： | 2 |
