1Hepatic steatosis is associated with an increased risk of carotid atherosclerosis显示文摘AIM: Although an association between helatic steatosis and vascular risk factors has been described, direct relationships between fatty liver and atherosclerosis have not yet been investigated. The aim of the present study has been to investigate those relationships.METHODS: The Study of Health in Pomerania examined a random population sample aged between 20 and 79 years.A study population of 4 222 subjects without hepatitis B and C infections and without liver cirrhosis was available for the present analysis. Hepatic steatosis was defined sonographically and intima-media thickness (IMT) as well as plaque prevalence were estimated by carotid ultrasound.RESULTS: The prevalence rate of hepatic steatosis was 29.9%. Among subjects aged ≥45 years, an association between hepatic steatosis and IMT of the carotid arteries was found in bivariate analysis, but not after adjustment for atherosclerotic risk factors. Individuals with fatty liver had more often carotid plaques than persons without fatty liver (plaque prevalence rate 76.8% vs 66.6%; P<0.001).This association persisted after adjustment for confounding factors and was predominantly present in subjects with no to mild alcohol consumption.CONCLUSION: There is an independent association between hepatic steatosis and carotid atherosclerotic plaques. 2Exploring new avenues in high repetition rate table-top coherent extreme ultraviolet sources显示文摘The process of high harmonic generation(HHG)enables the development of table-top sources of coherent extreme ultraviolet(XUV)light.Although these are now matured sources,they still mostly rely on bulk laser technology that limits the attainable repetition rate to the low kilohertz regime.Moreover,many of the emerging applications of such light sources(e.g.,photoelectron spectroscopy and microscopy,coherent diffractive imaging,or frequency metrology in the XUV spectral region)require an increase in the repetition rate.Ideally,these sources are operated with a multi-MHz repetition rate and deliver a high photon flux simultaneously.So far,this regime has been solely addressed using passive enhancement cavities together with low energy and high repetition rate lasers.Here,a novel route with significantly reduced complexity(omitting the requirement of an external actively stabilized resonator)is demonstrated that achieves the previously mentioned demanding parameters.A krypton-filled Kagome photonic crystal fiber is used for efficient nonlinear compression of 9 μJ,250 fs pulses leading to,7 μJ,31 fs pulses at 10.7 MHz repetition rate.The compressed pulses are used for HHG in a gas jet.Particular attention is devoted to achieving phase-matched(transiently)generation yielding.10^(13) photons s^(-1)(.50 μW)at 27.7 eV.This new spatially coherent XUV source improved the photon flux by four orders of magnitude for direct multi-MHZ experiments,thus demonstrating the considerable potential of this source.Steffen Hadrich Manuel Krebs Armin Hoffmann Arno Klenke Jan Rothhardt Jens Limpert Andreas Tunnermann 2015Light(Science & Applications)2015,4,1:4
3Performance of tacrolimus in hospitalized patients with steroid-refractory acute severe ulcerative colitis显示文摘BACKGROUND Acute severe ulcerative colitis unresponsive to systemic steroid treatment is a lifethreatening medical condition requiring hospitalization and often colectomy.Despite the increasing choice of medical therapy options for ulcerative colitis, the condition remains a great challenge in the field of inflammatory bowel diseases(IBD). The performance of the calcineurin inhibitor tacrolimus in this clinical setting is insufficiently elucidated.AIM To evaluate the short and long-term outcomes of tacrolimus therapy in adult inpatients with steroid-refractory acute severe ulcerative colitis.METHODS We conducted a retrospective monocentric study enrolling 22 patients at a tertiary care center for the treatment of IBD. All patients who were admitted to one of the wards of the Department of Gastroenterology and Hepatology of the Heidelberg University Hospital with acute severe ulcerative colitis between 2007 and 2018, and who received oral or intravenous tacrolimus for steroid-refractory disease were included. Baseline characteristics and data on the disease courses were retrieved from entirely computerized patient charts. The primary study endpoint was clinical response to tacrolimus therapy, resulting in discharge from the hospital. Secondary study endpoints were colectomy rate and time to colectomy, achievement of clinical remission under tacrolimus therapy, and the occurrence of side effects.RESULTSIn the majority of the 22 included patients(68.2%), tacrolimus therapy was initiated intravenously and subsequently converted to oral administration. The treatment duration was 128 ± 28.5 d(mean ± SEM), and the patients were followed up for 705 ± 110 d after treatment initiation. Among all patients, 86.4%were discharged from the hospital under continued oral tacrolimus therapy. In36.4% of the patients, the administration of tacrolimus resulted in clinical remission at some point during the treatment. Thirty-two percent of the patients underwent colectomy between 5 and 194 d after the initiation of tacrolimus treatment(mean: 97.4 ± 20.8 d). Colectomy-free survival rates at 1, 3, 6 and 12 mo after the initiation of tacrolimus therapy were 90.9%, 86.4%, 77.3% and 68.2%,respectively. The safety profile of tacrolimus was overall favorable. Only two patients discontinued the treatment due to side effects.CONCLUSION The short-term outcome of tacrolimus in steroid-refractory acute severe ulcerative colitis was beneficial, and side effects were rare. 17Reducing SARS-CoV-2 pathological protein activity with small molecules显示文摘Coronaviruses are dangerous human and animal pathogens.The newly identified coronavirus SARS-CoV-2 is the causative agent of COVID-19 outbreak,which is a real threat to human health and life.The world has been struggling with this epidemic for about a year,yet there are still no targeted drugs and effective treatments are very limited.Due to the long process of developing new drugs,reposition of existing ones is one of the best ways to deal with an epidemic of emergency infectious diseases.Among the existing drugs,there are candidates potentially able to inhibit the SARS-CoV-2 replication,and thus inhibit the infection of the virus.Some therapeutics target several proteins,and many diseases share molecular paths.In such cases,the use of existing pharmaceuticals for more than one purpose can reduce the time needed to design new drugs.The aim of this review was to analyze the key targets of viral infection and potential drugs acting on them,as well as to discuss various strategies and therapeutic approaches,including the possible use of natural products.We highlighted the approach based on increasing the involvement of human deaminases,particularly APOBEC deaminases in editing of SARS-CoV-2 RNA.This can reduce the cytosine content in the viral genome,leading to the loss of its integrity.We also indicated the nucleic acid technologies as potential approaches for COVID-19 treatment.Among numerous promising natural products,we pointed out curcumin and cannabidiol as good candidates for being antiSARS-CoV-2 agents.Donata Pluskota-Karwatka Marcin Hoffmann Jan Barciszewski 2021Journal of Pharmaceutical Analysis2021,11,4:0
