| 1 | PET imaging in ischemic cerebrovascular disease:current status and future directions显示文摘Cerebrovascular diseases are caused by interruption or significant impairment of the blood supply to the brain,which leads to a cascade of metabolic and molecular alterations resulting in functional disturbance and morphological damage.These pathophysiological changes can be assessed by positron emission tomography(PET),which permits the regional measurement of physiological parameters and imaging of the distribution of molecular markers.PET has broadened our understanding of the flow and metabolic thresholds critical for the maintenance of brain function and morphology:in this application,PET has been essential in the transfer of the concept of the penumbra(tissue with perfusion below the functional threshold but above the threshold for the preservation of morphology)to clinical stroke and thereby has had great impact on developing treatment strategies.Radioligands for receptors can be used as early markers of irreversible neuronal damage and thereby can predict the size of the final infarcts;this is also important for decisions concerning invasive therapy in large('malignant')infarctions.With PET investigations,the reserve capacity of blood supply to the brain can be tested in obstructive arteriosclerosis of the supplying arteries,and this again is essential for planning interventions.The effect of a stroke on the surrounding and contralateral primarily unaffected tissue can be investigated,and these results help to understand the symptoms caused by disturbances in functional networks.Chronic cerebrovascular disease causes vascular cognitive disorders,including vascular dementia.PET permits the detection of the metabolic disturbances responsible for cognitive impairment and dementia,and can differentiate vascular dementia from degenerative diseases.It may also help to understand the importance of neuroinflammation after stroke and its interaction with amyloid deposition in the development of dementia.Although the clinical application of PET investigations is limited,this technology had and still has a great impact on research into cerebrovascular diseases. | Wolf-Dieter Heiss | 2014 | Neuroscience Bulletin2014,30,5: | 10 |

| 4 | Systematic analysis of the safety and benefits of transvaginal hybrid-NOTES cholecystectomy显示文摘AIM: To evaluate transvaginal hybrid-NOTES cholecystectomy(TVC) during its clinical establishment and compare it with the traditional laparoscopic technique(LC).METHODS: The specific problems and benefits of TVC were reviewed using a registry analysis,a comparative cohort study and a randomized clinical trial. At first,feasibility,safety and specific complications of the TVC were analyzed based on the first 488 data sets of the German NOTES Registry(GNR). Hereafter,we compared the early postoperative results of our first 50 TVC-patients with those of 50 female LCpatients matched by age,BMI and ASA classification. The same cohort was contacted an average of two years later to evaluate long-term results concerning pain and satisfaction with the aesthetic results and the overall postoperative results as well as sexual intercourse by means of two domains of the German version of the Female Sexual Function Index(FSFI-d). Consequently,we performed a randomized clinical trial comparing 20 TVC-patients with 20 needlescopic/3-trocar cholecystectomies(NC) also concerning the early postoperative results as well as pain,satisfaction and quality of life by means of the Eypasch Gastrointestinal Quality of Life Index(GIQLI) in the later course. Finally,we discussed the results in accordance with other published studies.RESULTS: The complication(3.5%) and conversion rates(4.1%) for TVC were low in the GNR and comparable to those of the LC. Access related intraoperative complications included injuries to the bladder(n = 4; 0.8%) and bowel(n = 3; 0.6%). The study cohort revealed less postoperative pain after TVC comparing to the LC-patients on the day of surgery(NRS,1.5/10 vs 3.1/10,P = 0.003),in the morning(NRS,1.9/10 vs 2.8/10,P = 0.047) and in the evening(NRS,1.1/10 vs 1.8/10,P = 0.025) of postoperative day(POD) one. The randomized clinical trial consistently found less cumulative pain until POD 2(NRS,8/40 vs 14/40,P = 0.043),as well as until POD 10(NRS,22/190 vs 41/190,P = 0.010). Furthermore,the TVC-patients had a better quality of life on POD 10 than did the LC-patients(GIQLI,124/144 vs 107/144,P = 0.028). The complication rates were comparable and no specific problems were detected in the long-term follow-up for sexual intercourse for either group. | 6 | MR-arterioportography: A new technical approach for detection of liver lesions显示文摘AIM: To evaluate the benefit and effectiveness of MRarterioportography (MR-AP) to achieve the highest sensitivity for detection and evaluation of hepatocellular carcinoma (HCC). METHODS: Twenty liver cirrhosis patients with suspected HCC were included before transarterial chemoembolization. In all patients double-enhanced Magnetic resonance imaging (MRI) was performed. A bolus of 10 mL Magnevist was injected through a selectively placed catheter in the superior mesenteric artery and MRI of the liver was performed in arterioportographic phase. Two independent readers evaluated number, size and localization of detected lesions. Diagnostic quality was determined using a 4-point scale. Differences were analyzed for significance using a t -test. Interobserver variability was calculated. RESULTS: In all 20 patients (100%), MR-AP was feasible. Diagnostic quality was, in all cases, between 1 and2 for both modalities and readers. MR-AP detected significantly more lesions than double-enhanced MRI (102.5 vs 61, respectively, P < 0.0024). The inter-observer variability was 0.881 for MRI and 0.903 for MR-AP. | 8 | 肾功能和贫血是冠心病及其死亡率的危险因素:社区动脉粥样硬化风险(ARIC)研究显示文摘Background: Kidney failure causes anemia and is associated with a very high risk of coronary heart disease(CHD). Mildly to moderately decreased kidney function is far more common and also is associated with an elevated prevalence of anemia and CHD risk. Recent data suggest an even higher risk of CHD when both conditions are present. Methods: We investigated the association of kidney dysfunction and anemia with CHD events(fatal or nonfatal CHD or coronary revascularization procedures) and CHD and all- cause mortality over 12 years of follow- up in 14 971 adults aged 45 to 64 years in the ARIC Study. Glomerular filtration rate(GFR) was estimated from calibrated serum creatinine using the MDRD Study equation(< 30 mL/min per 1.73 m2 excluded, n=32). Anemia was defined as hemoglobin level<13.5 g/dL in men(648/6746, 9.6% ) and< 12 g/dL in women(1049/8225, 12.8% ). Results: The prevalence of anemia was progressively higher at lower estimated GFR< 75 mL/min per 1.73 m2(both P< .001) for both men and women. A total of 1635(10.9% ) participants had a CHD event, 360(2.4% ) died of CHD, and 1722(11.5% ) died of any cause during follow- up. After adjustment for known risk factors, including diabetes, lipid levels, blood pressure, and use of antihypertensive medication, decreased kidney function was associated with a higher risk of recurrent CHD events and mortality from CHD and all causes. These associations were significantly stronger among participants with anemia. The adjusted relative hazards of all- cause mortality associated with moderately decreased versus normal kidney function(GFR 30- 59 vs ≥ 90 mL/min per 1.73 m2) were 1.7(95% CI 1.3- 2.2) in the absence of anemia and 3.5(95% CI 2.4- 5.1) in the presence of anemia(P interaction=.001) . Conclusions: The combination of moderately decreased kidney function and anemia is associated with an increased risk of CHD events and mortality, emphasizing the need to identify individuals with these conditions and evaluate interventions to treat anemia and slow the progression of chronic kidney disease. | Astor B.C.
