1High rate of Helicobacter pylori reinfection in Lithuanianpeptic ulcer patients显示文摘AIM: To evaluate the frequency of Helicobacter pylori(H. pylori) reinfection in peptic ulcer patients during 9 years after H. pylori eradication.METHODS: We invited 117 peptic ulcer patients in whom eradication of H. pylori was confirmed 1 year after eradication treatment both by histology and by rapid urease test. In total, 57 patients were available for the study procedures: 34(59.6%) male, 23(40.4%) female; mean age 52.3 ± 13.0 years. There were 45(78.9%) patients with duodenal ulcer and 12(21.1%) with gastric ulcer. H. pylori was diagnosed by a rapid urease test and histology if endoscopy was performed. If endoscopy was refused, H. pylori was diagnosed by the C14-urea breath test and serology. H. pylori was established if at least one of the tests was positive.RESULTS: The mean follow-up was 8.9 ± 1.0 years(range, 6-12). H. pylori was established in 15 patients. In 2 H. pylori-negative patients, H. pylori was established during the follow-up period and eradicated. Therefore, we consider that reinfection occurred in 17 patients. In the per protocol analysis, reinfection was established in 17 of 57(29.8%; 95%CI: 19.2-42.2) patients during the follow-up period. The annual rate of infection was 3.36%. If all non-responders were considered H. pylori-negative, reinfection would be 14.5%(17/117), the annual ratebeing 1.63%. The mean age of patients with reinfection was 51.8 ± 14.0 years, and without reinfection was 52.5 ± 13.0 years, P > 0.05; the mean body mass index of patients with reinfection was 27.2 ± 4.1 kg/m2, and without reinfection was 25.7 ± 4.2 kg/m2, P > 0.05. There were no differences in the reinfection rates according the location of the peptic ulcer, the eradication regimen used, and smoking status.CONCLUSION: The reinfection rate of H. pylori is relatively high in Lithuania and probably related to the high prevalence of H. pylori, what may reflect differences in the socioeconomic status between Western and Eastern European countries.laimas jonaitis gediminas kiudelis paulius slepavicius limas kupcinskas 2016World Journal of Gastrointestinal Pathophysiology2016,7,1:7
2外源途径一氧化碳抑制小鼠严重烧伤后早期肺损伤和炎症反应的分子机制显示文摘目的探讨外源途径一氧化碳对严重烧伤后小鼠早期肺损伤和炎症反应的抑止作用及机制。方法建立小鼠15%Ⅲ度烧伤模型。实验动物共随机分成3组:假伤(sham)组(12只),烫伤组(12只)及烫伤加一氧化碳释放分子(CORM)-2组(12只)。烫伤组小鼠异氟烷吸入麻醉下,背部去毛,以92℃水,9 s,造成15%Ⅲ度烫伤,伤后腹腔内注射生理盐水1.5 ml 抗休克;烫伤加 CORM-2组小鼠烫伤后尾静脉注射CORM-2(8 mg/kg);Sham 组用37℃温水代替92℃水,其余操作相同。检测动物肺组织髓过氧化物酶(MPO)、核因子κB(NF-κB)活性、细胞问黏附分子-1(ICAM-1)的表达,肺泡灌洗液中 TNF-α和 IL-1β,肺组织损伤指标(湿干比、血管通透性)以及肺内皮细胞经烧伤血清刺激后对白细胞的黏附作用。结果烧伤组肺组织中 MPO(87±11)U/g 组织,肺泡灌洗液中 TNF-α(160±9)pg/ml 和 IL-1β(27.2±2.9)pg/ml,NF-κB的活性及 ICAM-1蛋白表达水平均明显高于Sham 组。与烫伤组比较,烫伤+CORM-2组上述情况明显得到改善(P<0.05);而肺湿干比、血管通透性未见明显改善。肺内皮细胞经烧伤血清刺激后对白细胞的黏附作用增强(46.5±8.5)%,经CORM-2处理后的烧伤血清刺激后肺内皮细胞对白细胞的黏附作用明显减弱(25.4±5.6)%。结论外源性一氧化碳释放分子能明显抑止肺组织 NF-κB活性,减轻严重烧伤后肺组织黏附分子的表达,从而减轻组织中白细胞扣留,有效减轻肺脏炎症反应。孙炳伟 陈曦 陈兆永 Kazuhiro Katada Gediminas Cepinskas 2007中华医学杂志2007,87,44:6

4外源性一氧化碳释放分子2对严重烧伤小鼠肝脏炎性反应的抑制作用显示文摘目的观察外源性一氧化碳释放分子2(CORM-2)对严重烧伤小鼠肝脏炎性反应的抑制作用,并探讨其机制。方法将C_(57)BL/6小鼠随机分成假伤组(模拟烧伤)、假伤+CORM-2组、烧伤组、烧伤+CORM-2组及烧伤+二甲亚砜(DMSO)组,每组9只。假伤+CORM-2组除伤后使用CORM-2以外,其他处理同假伤组。烧伤+CORM-2组及烧伤+DMSO组除伤后分别使用CORM-2、DMSO外,其余处理同烧伤组。于伤后24 h检测小鼠血清丙氨酸转氨酶(ALT)及天冬氨酸转氨酶(AST)的水平,肝组织髓过氧化物酶(MPO)及核因子κB(NF-κB)活性,胞间黏附分子1(ICAM-1)和血管细胞黏附分子1(VCAM-1)蛋白的表达;检测各组小鼠肝窦内皮细胞(HSEC)经各自血清刺激后对中性粒细胞(PMN)的黏附作用。结果与假伤组比较,烧伤组小鼠血清ALT、AST的水平[(398±34)、(122±22)U/L]及肝组织MPO活性、肝组织ICAM-1和VCAM-1蛋白表达水平均明显升高(P<0.05或P<0.01)。与烧伤组比较,烧伤+CORM-2组上述情况明显改善,且NF-κB活性下降。与假伤组比较,烧伤组小鼠HSEC对PMN的黏附作用增强;烧伤+CORM-2组该作用明显弱于烧伤组(P<0.05)。结论外源性CORM-2能明显抑制肝组织NF-κB活性,减少ICAM-1、VCAM-1蛋白的表达水平,减轻严重烧伤后组织中白细胞滞留,改善肝功能,可有效减轻肝脏炎性反应。孙炳伟 陈曦 陈兆永 Kazuhiro Katada Gediminas Cepinskas 2007中华烧伤杂志2007,23,3:5
5外源性一氧化碳释放分子抑制脓毒症炎症反应的实验研究(英文)显示文摘目的:探讨外源性一氧化碳释放分子对脓毒症炎症反应的抑制作用及可能的机制。方法:应用盲肠结扎及穿孔脓毒症小鼠模型,使用外源性一氧化碳释放分子(CORM-2,8mg/kg体质量,尾静脉注射)进行干预。检测肝、肺脏髓过氧化物酶(MPO)活性。应用内毒素(LPS,10g/ml)刺激的人脐静脉内皮细胞炎症模型,使用外源性一氧化碳释放分子(CORM-2,10~100mol/L)进行干预。检测核因子κB(NF—κB)活性,内皮细胞黏附分子的表达,氧化产物、NO产物以及多形核白细胞对内皮细胞的黏附作用。结果:盲肠结扎及穿孔脓毒症小鼠模型使用外源性一氧化碳释放分子干预后肝、肺组织MPO活性明显下降。CORM-2抑制了LPS刺激导致的NF-κB活性上调。同时,NO产物下降,内皮细胞ICAM-1的表达抑制,白细胞对内皮细胞的黏附作用明显抑制。结论:外源性一氧化碳释放分子通过抑制NF-κB活性,抑制ICAM-1蛋白和NO的表达,抑制白细胞对内皮细胞的黏附作用,进而有效抑制脓毒症炎症反应。孙炳伟 陈曦 Kazuhiro Katada Richard F Potter Gediminas Cepinskas 2008江苏大学学报(医学版)2008,18,5:4

7NOD2,IL23R and ATG16L1 polymorphisms in Lithuanian patients with inflammatory bowel disease显示文摘AIM:To investigate the frequency of NOD2, IL23R and ATG16L1 genetic variants in a case-control panel for inflammatory bowel disease (IBD) from Lithuania.METHODS: One hundred and eighty unrelated IBD pa- tients [57 Crohn's disease (CD) and 123 ulcerative colitis (UC)] and 186 healthy controls were genotyped for the following known genetic susceptibility variants:NOD2-Arg702Trp (rs2066844), Gly908Arg (rs2066845) and Leu1007insC (rs2066847), as well as IL23R-Arg381Gln (rs11209026) and ATG16L1-Thr300Ala (rs2241880).RESULTS:The effect that carriership of at least one NOD2 risk allele predisposes to CD was replicated in the Lithuanian population (41.1% CD vs 16.9% controls, P=2×10-4, OR=3.48,95% CI:1.81-6.72). In the allelic single marker analysis, Leu1007insC was strongly associated with CD (21.4% CD vs 4.7% controls, P=3.687×10-8, OR=5.54, 95% CI:2.85-10.75). Neither the other two NOD2 variants, nor the known variants in IL23R and ATG16L1 were found to be risk factors for CD, UC or IBD. 8Polymorphisms of micro RNA target genes IL12B, INSR, CCND1 and IL10 in gastric cancer显示文摘AIM To evaluate associations between mi RNA target genes IL12B,INSR,CCND1 and IL10 polymorphisms and gastric cancer(GC)in European population.METHODS Gene polymorphisms were analyzed in 508 controls and474 GC patients from 3 tertiary centers in Germany,Lithuania and Latvia.Controls were patients from the out-patient departments,who were referred for upper endoscopy because of dyspeptic symptoms and had no history of previous malignancy.Gastric cancer(GC)patients had histopathological verification of gastric adenocarcinoma.Genomic DNA was extracted using salting out method from peripheral blood mononuclear cells.IL12B T>G(rs1368439),INSR T>C(rs1051690),CCND1 A>C(rs7177)and IL10 T>C(rs3024498)SNPs were genotyped by the real-time polymerase chain reaction.Associations between gene polymorphism and GC were evaluated using multiple logistic regression analysis with adjustment for sex,age and country of birth.RESULTS We observed similar distribution of genotypes and allelic frequencies of all polymorphisms between GC patients and controls except of INSR rs1051690.The frequency of the T allele of INSR gene was significantly higher in GC patients than in controls(23.26%and 19.19%respectively,P=0.028).CT genotype was also more prevalent in patients compared to control group(38.48%and 30.12%respectively,P<0.021).Logistic regression analysis revealed that only one polymorphism(rs1051690 in INSR gene)was associated with increased risk of GC.Carriers of CT genotype had higher odds of GC when compared to CC genotype(OR=1.45,95%PI:1.08-1.95,P=0.01).Similar association was observed in a dominant model for INSR gene,where comparison of TT+CT vs CC genotypes showed an increased risk of GC(OR=1.44,95%PI:1.08-1.90,P=0.01).Other analyzed SNPs were not associated with the presence of GC.CONCLUSION INSR rs1051690 SNP is associated with increased risk of GC,while polymorphisms in IL12B,CCND1 and IL10genes are not linked with the presence of GC.Vytenis Petkevicius Violeta Salteniene Simonas Juzenas Thomas Wex Alexander Link Marcis Leja Ruta Steponaitiene Jurgita Skieceviciene Limas Kupcinskas Laimas Jonaitis Gediminas Kiudelis Peter Malfertheiner Juozas Kupcinskas 2017World Journal of Gastroenterology2017,23,19:2

17Inhalation treatment of lung cancer: the influence of composition, size and shape of nanocarriers on their lung accumulation and retention显示文摘Objective: Various nanoparticles have been designed and tested in order to select optimal carriers for the inhalation delivery of anticancer drugs to the lungs. Methods: The following nanocarriers were studied: micelles, liposomes, mesoporous silica nanoparticles(MSNs), poly propyleneimine(PPI) dendrimer-siRNA complexes nanoparticles, quantum dots(QDs), and poly(ethylene glycol) polymers. All particles were characterized using the following methods: dynamic light scattering, zeta potential, atomic force microscopy, in vitro cyto- and genotoxicity. In vivo organ distribution of all nanoparticles, retention in the lungs, and anticancer effects of liposomes loaded with doxorubicin were examined in nude mice after the pulmonary or intravenous delivery. Results: Significant differences in lung uptake were found after the inhalation delivery of lipid-based and non-lipid-based nanoparticles. The accumulation of liposomes and micelles in lungs remained relatively high even 24 h after inhalation when compared with MSNs, QDs, and PPI dendrimers. There were notable differences between nanoparticle accumulation in the lungs and other organs 1 and 3 h after inhalation or intravenous administrations, but 24 h after intravenous injection all nanoparticles were mainly accumulated in the liver, kidneys, and spleen. Inhalation delivery of doxorubicin by liposomes significantly enhanced its anticancer effect and prevented severe adverse side effects of the treatment in mice bearing the orthotopic model of lung cancer.Conclusion: The results of the study demonstrate that lipid-based nanocarriers had considerably higher accumulation and longer retention time in the lungs when compared with non-lipid-based carriers after the inhalation delivery.
