1Trend in gastric cancer:35 years of surgical experience in Japan显示文摘AIM:To investigate the trend in gastric cancer surgery in the context of rapid therapeutic advancement in Japan and East Asia.METHODS:A retrospective analysis was performed on 4163 patients who underwent gastric resection for gastric cancer with histological confirmation between 1971 and 2007 at the surgical unit in Kitasato University Hospital,to determine the trend in gastric cancer requiring surgery.RESULTS:Gastric cancer requiring surgical resection increased in our hospital,but the incidence adjusted for population was constant during the observed pe-riod.Interestingly,the ratio of diffuse type/intestinal type gastric cancer was unexpectedly unchanged,and that of advanced/early gastric cancer(EGC)was,however,markedly reduced,while the actual incidence of potentially curative advanced gastric cancer tended to decrease.The incidence of EGC requiring surgery tended to increase as a whole,which is consistent with increased prevalence of endoscopic surveillance.As a result,overall survival and mortality of gastric cancer requiring gastric resection has recently markedly improved.CONCLUSION:In Japan,planned interventions may improve surgical gastric cancer mortality,but an unexpected trend of persistent existence of intestinal type cancer suggests the need for more robust medical intervention.Keishi Yamashita Shinichi Sakuramoto Masayuki Nemoto Tomotaka Shibata Hiroaki Mieno Natsuya Katada Shiroh Kikuchi Masahiko Watanabe 2011World Journal of Gastroenterology2011,17,29:11
2DOG1 is useful for diagnosis of KIT-negative gastrointestinal stromal tumor of stomach显示文摘Approximately 80%-95%of gastrointestinal stromal tumors(GISTs)show positive staining for KIT,while the other 5%-20%show negative staining.If the tumor is negative for KIT,but is positive for CD34,a histological diagnosis is possible.However,if the tumor is negative for KIT,CD34,S-100,and SMA,a definitive diagnosis is often challenging.Recently,Discovered on GIST-1(DOG1)has received considerable attention as a useful molecule for the diagnosis of GIST.DOG1,a membrane channel protein,is known to be overexpressed in GIST.Because the sensitivity and specificity of DOG1 are higher than those of KIT,positive staining for DOG1has been reported,even in KIT-negative GISTs.KITnegative GISTs most commonly arise in the stomach and are mainly characterized by epithelioid features histologically.We describe our experience with a rare case of a KIT-negative GIST of the stomach that was diagnosed by positive immunohistochemical staining for DOG1 in a patient who presented with severe anemia.Our findings suggest that immunohistochemical staining for DOG1,in addition to gene analysis,is useful for the diagnosis of KIT-negative tumors that are suspected to be GISTs.Takuya Wada Satoshi Tanabe Kenji Ishido Katsuhiko Higuchi Tohru Sasaki Chikatoshi Katada Mizutomo Azuma Akira Naruke Myunguchul Kim Wasaburo Koizumi Tetsuo Mikami 2013World Journal of Gastroenterology2013,19,47:11
3Lansoprazole ameliorates intestinal mucosal damage induced by ischemia-reperfusion in rats显示文摘AIM: To investigate the protective effect of lansoprazoleon ischemia and reperfusion (I/R)-induced rat intestinalmucosal injury in vivo.METHODS: Intestinal damage was induced by clampingboth the superior mesenteric artery and the celiac trunkfor 30 rain followed by reperfusion in male Sprague-Dawleyrats. lansoprazole was given to rats intraperitoneally 1 hbefore vascular clamping.RESULTS: Both the intraluminal hemoglobin and proteinlevels, as indices of mucosal damage, significantlyincreased in I/R-groups comparion with those of sham-operation groups. These increases in intraluminal hemoglobinand protein levels were significantly inhibited by the treatmentwith lansoprazole at a dose of 1 mg/kg. 4外源途径一氧化碳抑制小鼠严重烧伤后早期肺损伤和炎症反应的分子机制显示文摘目的探讨外源途径一氧化碳对严重烧伤后小鼠早期肺损伤和炎症反应的抑止作用及机制。方法建立小鼠15%Ⅲ度烧伤模型。实验动物共随机分成3组:假伤(sham)组(12只),烫伤组(12只)及烫伤加一氧化碳释放分子(CORM)-2组(12只)。烫伤组小鼠异氟烷吸入麻醉下,背部去毛,以92℃水,9 s,造成15%Ⅲ度烫伤,伤后腹腔内注射生理盐水1.5 ml 抗休克;烫伤加 CORM-2组小鼠烫伤后尾静脉注射CORM-2(8 mg/kg);Sham 组用37℃温水代替92℃水,其余操作相同。检测动物肺组织髓过氧化物酶(MPO)、核因子κB(NF-κB)活性、细胞问黏附分子-1(ICAM-1)的表达,肺泡灌洗液中 TNF-α和 IL-1β,肺组织损伤指标(湿干比、血管通透性)以及肺内皮细胞经烧伤血清刺激后对白细胞的黏附作用。结果烧伤组肺组织中 MPO(87±11)U/g 组织,肺泡灌洗液中 TNF-α(160±9)pg/ml 和 IL-1β(27.2±2.9)pg/ml,NF-κB的活性及 ICAM-1蛋白表达水平均明显高于Sham 组。与烫伤组比较,烫伤+CORM-2组上述情况明显得到改善(P<0.05);而肺湿干比、血管通透性未见明显改善。肺内皮细胞经烧伤血清刺激后对白细胞的黏附作用增强(46.5±8.5)%,经CORM-2处理后的烧伤血清刺激后肺内皮细胞对白细胞的黏附作用明显减弱(25.4±5.6)%。结论外源性一氧化碳释放分子能明显抑止肺组织 NF-κB活性,减轻严重烧伤后肺组织黏附分子的表达,从而减轻组织中白细胞扣留,有效减轻肺脏炎症反应。孙炳伟 陈曦 陈兆永 Kazuhiro Katada Gediminas Cepinskas 2007中华医学杂志2007,87,44:6
5外源性一氧化碳释放分子2对严重烧伤小鼠肝脏炎性反应的抑制作用显示文摘目的观察外源性一氧化碳释放分子2(CORM-2)对严重烧伤小鼠肝脏炎性反应的抑制作用,并探讨其机制。方法将C_(57)BL/6小鼠随机分成假伤组(模拟烧伤)、假伤+CORM-2组、烧伤组、烧伤+CORM-2组及烧伤+二甲亚砜(DMSO)组,每组9只。假伤+CORM-2组除伤后使用CORM-2以外,其他处理同假伤组。烧伤+CORM-2组及烧伤+DMSO组除伤后分别使用CORM-2、DMSO外,其余处理同烧伤组。于伤后24 h检测小鼠血清丙氨酸转氨酶(ALT)及天冬氨酸转氨酶(AST)的水平,肝组织髓过氧化物酶(MPO)及核因子κB(NF-κB)活性,胞间黏附分子1(ICAM-1)和血管细胞黏附分子1(VCAM-1)蛋白的表达;检测各组小鼠肝窦内皮细胞(HSEC)经各自血清刺激后对中性粒细胞(PMN)的黏附作用。结果与假伤组比较,烧伤组小鼠血清ALT、AST的水平[(398±34)、(122±22)U/L]及肝组织MPO活性、肝组织ICAM-1和VCAM-1蛋白表达水平均明显升高(P<0.05或P<0.01)。与烧伤组比较,烧伤+CORM-2组上述情况明显改善,且NF-κB活性下降。与假伤组比较,烧伤组小鼠HSEC对PMN的黏附作用增强;烧伤+CORM-2组该作用明显弱于烧伤组(P<0.05)。结论外源性CORM-2能明显抑制肝组织NF-κB活性,减少ICAM-1、VCAM-1蛋白的表达水平,减轻严重烧伤后组织中白细胞滞留,改善肝功能,可有效减轻肝脏炎性反应。孙炳伟 陈曦 陈兆永 Kazuhiro Katada Gediminas Cepinskas 2007中华烧伤杂志2007,23,3:5
6Delayed perforation after endoscopic submucosal dissection for early gastric cancer: Clinical features and treatment显示文摘Perforation is an important procedural complication of endoscopic submucosal dissection(ESD) for early gastric cancer. Although the incidence of delayed perforation after ESD is low, extreme caution is necessary because many cases require surgical intervention. Among 1984 lesions of early gastric cancer treated in our hospital by ESD in 1588 patients from September 2002 through March 2015, delayed perforation developed in 4 patients(4 lesions, 0.25%). 7Partially hydrolyzed guar gum attenuates non-alcoholic fatty liver disease in mice through the gut-liver axis显示文摘BACKGROUND The gut-liver axis has attracted much interest in the context of chronic liver disease pathogenesis.Prebiotics such as dietary fibers were shown to attenuate non-alcoholic fatty liver disease(NAFLD)by modulating gut microbiota.Partially hydrolyzed guar gum(PHGG),a water-soluble dietary fiber,has been reported to alleviate the symptoms of various intestinal diseases and metabolic syndromes.However,its effects on NAFLD remain to be fully elucidated.To determine whether treatment with PHGG attenuates NAFLD development in mice through the gut-liver axis.METHODS Seven-week-old male C57BL/6J mice with increased intestinal permeability were fed a control or atherogenic(Ath)diet(a mouse model of NAFLD)for 8 wk,with or without 5%PHGG.Increased intestinal permeability was induced through chronic intermittent administration of low-dose dextran sulfate sodium.Body weight,liver weight,macroscopic findings in the liver,blood biochemistry[aspartate aminotransferase(AST)and alanine aminotransferase(ALT),total cholesterol,triglyceride,free fatty acids,and glucose levels],liver histology,myeloperoxidase activity in liver tissue,mRNA expression in the liver and intestine,serum endotoxin levels in the portal vein,intestinal permeability,and microbiota and short-chain fatty acid(SCFA)profiles in the cecal samples were investigated.RESULTS Mice with increased intestinal permeability subjected to the Ath diet showed significantly increased serum AST and ALT levels,liver fat accumulation,liver inflammatory(tumor necrosis factor-αand monocyte chemotactic protein-1)and fibrogenic(collagen 1a1 andαsmooth muscle actin)marker levels,and liver myeloperoxidase activity,which were significantly attenuated by PHGG treatment.Furthermore,the Ath diet combined with increased intestinal permeability resulted in elevated portal endotoxin levels and activated toll-like receptor(TLR)4 and TLR9 expression,confirming that intestinal permeability was significantly elevated,as observed by evaluating the lumen-to-blood clearance of fluorescein isothiocyanate-conjugated dextran.PHGG treatment did not affect fatty acid metabolism in the liver.However,it decreased lipopolysaccharide signaling through the gut-liver axis.In addition,it significantly increased the abundance of cecal Bacteroides and Clostridium subcluster XIVa.Treatment with PHGG markedly increased the levels of SCFAs,particularly,butyric acid,acetic acid,propionic acid,and formic acid,in the cecal samples.CONCLUSION PHGG partially prevented NAFLD development in mice through the gut-liver axis by modulating microbiota and downstream SCFA profiles.Shun Takayama Kazuhiro Katada Tomohisa Takagi Takaya Iida Tomohiro Ueda Katsura Mizushima Yasuki Higashimura Mayuko Morita Tetsuya Okayama Kazuhiro Kamada Kazuhiko Uchiyama Osamu Handa Takeshi Ishikawa Zenta Yasukawa Tsutomu Okubo Yoshito Itoh Yuji Naito 2021World Journal of Gastroenterology2021,27,18:4

9外源性一氧化碳释放分子抑制脓毒症炎症反应的实验研究(英文)显示文摘目的:探讨外源性一氧化碳释放分子对脓毒症炎症反应的抑制作用及可能的机制。方法:应用盲肠结扎及穿孔脓毒症小鼠模型,使用外源性一氧化碳释放分子(CORM-2,8mg/kg体质量,尾静脉注射)进行干预。检测肝、肺脏髓过氧化物酶(MPO)活性。应用内毒素(LPS,10g/ml)刺激的人脐静脉内皮细胞炎症模型,使用外源性一氧化碳释放分子(CORM-2,10~100mol/L)进行干预。检测核因子κB(NF—κB)活性,内皮细胞黏附分子的表达,氧化产物、NO产物以及多形核白细胞对内皮细胞的黏附作用。结果:盲肠结扎及穿孔脓毒症小鼠模型使用外源性一氧化碳释放分子干预后肝、肺组织MPO活性明显下降。CORM-2抑制了LPS刺激导致的NF-κB活性上调。同时,NO产物下降,内皮细胞ICAM-1的表达抑制,白细胞对内皮细胞的黏附作用明显抑制。结论:外源性一氧化碳释放分子通过抑制NF-κB活性,抑制ICAM-1蛋白和NO的表达,抑制白细胞对内皮细胞的黏附作用,进而有效抑制脓毒症炎症反应。孙炳伟 陈曦 Kazuhiro Katada Richard F Potter Gediminas Cepinskas 2008江苏大学学报(医学版)2008,18,5:4
10Argon plasma coagulation for superficial esophageal squamous-cell carcinoma in high-risk patients显示文摘AIM: To evaluate the usefulness and safety of argon plasma coagulation (APC) for superficial esophageal squamous-cell carcinoma (SESC) in high-risk patients. METHODS: We studied 17 patients (15 men and 2 women, 21 lesions) with SESC in whom endoscopic mucosal resection (EMR), endoscopic submucosal dissection (ESD), and open surgery were contraindicated from March 1999 through February 2009. None of the patients could tolerate prolonged EMR/ESD or open surgery because of severe concomitant disease (e.g., liver cirrhosis, cerebral infarction, or ischemic heart disease) or scar formation after EMR/ESD and chemoradiotherapy. After conventional endoscopy, an iodine stain was sprayed on the esophageal mucosa to determine the lesion margins. The lesion was then ablated by APC. We retrospectively studied the treatment time, number of APC sessions per site, complications, presence or absence of recurrence, and time to recurrence.RESULTS: The median duration of follow-up was 36 mo (range: 6-120 mo). All of the tumors were macroscopically classified as superficial and slightly depressed type (0-Ⅱc). The preoperative depth of invasion was clinical T1a (mucosal cancer) for 19 lesions and clinical T1b (submucosal cancer) for 2. The median treatment time was 15 min (range: 10-36 min). The median number of treatment sessions per site was 2 (range: 1-4). The median hospital stay was 14 d (range: 5-68 d). Among the 17 patients (21 lesions), 2 (9.5%) had recurrence and underwent additional APC with no subsequent evidence of recurrence. There were no treatment-related complications, such as bleeding or perforation. 13Macroscopic appearance of TypeⅣand giant Type Ⅲ is a high risk for a poor prognosis in pathological stage Ⅱ/Ⅲ advanced gastric cancer with postoperative adjuvant chemotherapy显示文摘AIM To evaluate whether a high risk macroscopic appearance(Type Ⅳ and giant Type Ⅲ) is associated with a dismal prognosis after curative surgery, because its prognostic relevance remains elusive in pathological stage Ⅱ/Ⅲ(p Stage Ⅱ/Ⅲ) gastric cancer.METHODS One hundred and seventy-two advanced gastric cancer(defined as pT2 or beyond) patients with p Stage Ⅱ/Ⅲ who underwent curative surgery plus adjuvant S1 chemotherapy were evaluated, and the prognostic relevance of a high-risk macroscopic appearance was examined. RESULTS Advanced gastric cancers with a high-risk macroscopic appearance were retrospectively identified by preoperative recorded images. A high-risk macroscopic appearance showed a significantly worse relapse free survival(RFS)(35.7%) and overall survival(OS)(34%) than an average risk appearance(P = 0.0003 and P < 0.0001, respectively). A high-risk macroscopic appearance was significantly associated with the 13^(th) Japanese Gastric Cancer Association(JGCA) pT(P = 0.01), but not with the 13^(th) JGCA pN. On univariate analysis for RFS and OS, prognostic factors included 13^(th) JGCA p Stage(P < 0.0001)and other clinicopathological factors including macroscopic appearance. A multivariate Cox proportional hazards model for univariate prognostic factors identified highrisk macroscopic appearance(P = 0.036, HR = 2.29 for RFS and P = 0.021, HR = 2.74 for OS) as an independent prognostic indicator. 16多层螺旋CT计算心室容积的准确性及最佳层厚选择显示文摘目的(1)确定Simpson方法计算心室容积的最佳层厚;(2)评价多层螺旋CT计算心室容积的准确性。方法采用多层螺旋CT(东芝Aquilion)扫描系统对14个人的左室铸型和15个右室铸型进行扫描。分别以2.0、3.5、5.0、7.0及10.0mm层厚重建左、右心室短轴多平面重组(multiplanarreformattedimage,MPR)图像。采用标准Simpson法计算左、右心室铸型的容积。心室铸型的实际容积通过铸型排除水的容量来计算。结果各层厚计算的心室容积均高估相应心室的实际容积[左心室高估值介于(3.21±5.95)ml至(12.58±8.56)ml之间,右心室高估值介于(10.22±8.45)ml至(23.91±12.24)ml之间]。各层厚计算的心室容积均与相应的实际心室容积高度相关(左心室r值均>0.97,右心室r值均>0.95);心室容积的高估程度与选择的层厚呈显著正相关(左心室r=0.998,右心室r=0.996)。当层厚降至5mm时,计算的心室容积对实际容积的高估值与2mm层厚相比不再差异有统计学意义。结论采用多排Simpson法计算左、右心室容积时,5mm层厚是最有效率的层厚。MSCT可以准确计算左、右心室容积。崔炜 Takeshi Kondo Hirofumi Anno 郭玉印 Takahisa Sato Masayoshi Sarai Hitoshi Shinozaki Satoshi Kakizawa Atsushi Sugiura Keita Oshima Kazuhiro Katada Hitoshi Hishida 2005中华放射学杂志2005,39,12:2
