| 6 | Nutritional and health benefits of semi-elemental diets: A comprehensive summary of the literature显示文摘AIM:To critically review and summarize the literature on nutritional and health outcomes of semi-elemental formulations on various nutritionally vulnerable patient populations who are unable to achieve adequate nutrition from standard oral diets.METHODS:We conducted a comprehensive literature search of Pubmed and Embase databases.We manually screened articles that examined nutritional and health outcomes(e.g.,growth,disease activity,gastrointestinal impairment,mortality,and economic impact)among various patient groups receiving semi-elemental diets.This review focused on full-text articles of randomized controlled clinical trials and other intervention studies,but pertinent abstracts and case studies were also included.Results pertaining primarily to tolerance,digestion,and absorption were summarized for each patient population in this systematic review.RESULTS:Results pertaining primarily to tolerance,digestion,and absorption were summarized for each patient population.The efficacy of semi-elemental whey hydrolyzed protein(WHP)diet have been reported in various nutritionally high risk patient populations including-Crohn’s disease,short bowel syndrome,acute and chronic pancreatitis,cerebral palsy,cystic fibrosis,cerebrovascular accidents,human immunodeficiency virus,critically ill,and geriatrics.Collectively,the evidence from the medical literature indicates that feeding with a semi-elemental diet performs as well or better than parenteral or amino acid based diets in terms of toler-ance,digestion,and nutrient assimilation measures across various disease conditions.CONCLUSION:Based on this comprehensive review of the literature,patient populations who have difficulty digesting or absorbing standard diets may be able to achieve improved health and nutritional outcomes through the use of semi-elemental WHP diets. | Dominik D Alexander Lauren C Bylsma Laura Elkayam Douglas L Nguyen | 2016 | World Journal of Gastrointestinal Pharmacology and Therapeutics2016，7，2： | 3 |

| 9 | Measures of patient radiation exposure during endoscopic retrograde cholangiography:Beyond fluoroscopy time显示文摘AIM:to determine whether fluoroscope time is a good predictor of patient radiation exposure during endoscopic retrograde cholangiopancreatography.METHODS:this is a prospective observational study of consecutive patients undergoing endoscopic retrograde cholangiopancreatography in a tertiary care setting.Data related to radiation exposure were collected.The following measures were obtained:Fluoroscopy time(FT),dose area product(DAP)and dose at reference point(DOSERP).Coefficients of determination were calculated to analyze the correlation between FT,DAP and DOSRP.Agreement between FT and DAP/DOSRP was assessed using Bland Altman plots.RESULTS:Four hundred sixty-three data sets were obtained.Fluoroscopy time average was 7.3 min.Fluoroscopy related radiation accounted for 86%of the total DAP while acquisition films related radiation accounted for 14%of the DAP.For any given FT there are wide ranges of DAP and DOSERP and the variability in both increases as fluoroscopy time increases.The coefficient of determination(R2)on the non transformed data for DAP and DOSERP versus FT were respectively 0.416 and0.554.While fluoroscopy use was the largest contributor to patient radiation exposure during endoscopic retrograde cholangiography(ERCP),there is a wide variability in DAP and DOSERP that is not accounted for by FT.DAP and DOSERP increase in variability as FT increases.This translates into poor accuracy of FT in predicting DAP and DOSERP at higher radiation doses.CONCLUSION:DAP and DOSERP in addition to FT should be adopted as new ERCP quality measures to estimate patient radiation exposure. | Toufic Kachaamy Edwyn Harrison Rahul Pannala William Pavlicek Michael D Crowell Douglas O Faigel | 2015 | World Journal of Gastroenterology2015，21，6： | 2 |

| 12 | Outcomes of submucosal(T1b) esophageal adenocarcinomas removed by endoscopic mucosal resection显示文摘AIM To investigate the outcomes and recurrences of p T1 b esophageal adenocarcinoma(EAC) following endoscopic mucosal resection(EMR) and associated treatments.METHODS Patients undergoing EMR with pathologically confirmed T1 b EAC at two academic referral centers were retrospectively identified.Patients were divided into 4 groups based on treatment following EMR:Endoscopic therapy alone(group A),endoscopic therapy with either chemotherapy,radiation or both(group B),surgicalresection(group C) or no further treatment/lost to follow-up(<12 mo)(group D).Pathology specimens were reviewed by a central pathologist.Follow-up data was obtained from the academic centers,primary care physicians and/or referring physicians.Univariate analysis was performed to identify factors predicting recurrence of EAC.RESULTS Fifty-three patients with T1 b EAC underwent EMR,of which 32(60%) had adequate follow-up ≥ 12 mo(median 34 mo,range 12-103).There were 16 patients in group A,9 in group B,7 in group C and 21 in group D.Median follow-up in groups A to C was 34 mo(range 12-103).Recurrent EAC developed overall in 9 patients(28%) including 6(38%) in group A(median:21 mo,range:6-73),1(11%) in group B(median:30 mo,range:30-30) and 2(29%) in group C(median 21 mo,range:7-35.Six of 9 recurrences were local;of the 6 recurrences,5 were treated with endoscopy alone.No predictors of recurrence of EAC were identified.CONCLUSION Endoscopic therapy of T1 b EAC may be a reasonable strategy for a subset of patients including those either refusing or medically unfit for esophagectomy. | Darren D Ballard Neel Choksi Jingmei Lin Eun-Young Choi B Joseph Elmunzer Henry Appelman Douglas K Rex Hala Fatima William Kessler John M DeWitt | 2016 | World Journal of Gastrointestinal Endoscopy2016，8，20： | 2 |
