目的:确定ST段抬高型心肌梗死(ST-segment elevation myocardial infarction,STEMI)和NSTE ACS患者的院内治疗改变是否伴随临床结果的改善。
设计、地点及患者:全球急性冠脉事件注册研究(Global Registry of Acute Coronary Events,GRACE)是一项多国家队列研究。自1999年7月1日至2006年12月31日,该研究共入选并随访了来自14个国家、113家医院的44372例ACS患者。
主要观测指标:当前循证药物和介入治疗应用的趋势及患者预后(死亡、充血性心力衰竭、肺水肿、心源性休克、卒中和心肌梗死)。
结果:随着研究时间的进展,相关药物的应用增加(β-受体阻滞剂、他汀类药物、血管紧张素转换酶抑制剂、行或未行经皮冠状动脉介入治疗[percutaneous coronary intervention,PCI]患者应用噻氯毗啶、糖蛋白Ⅱb/Ⅲa抑制剂和低分子肝素;P〈0.001)。STEMI患者药物再灌注治疗下降了22个百分点(95%可信区间[confidence interval,CI],-27~-17),而直接PCI增加了37个百分点(95%CI,33—41)。在NSTEMI患者中,PCI的比例显著升高了18个百分点(95%CI,15~20)。充血性心力衰竭和肺水肿在STEMI患者中降低了9个百分点(95%CI,-12~-6),在NSTEACS患者中降低了6.9个百分点(95%CI,-8.4~-4.7)。STEMI患者的院内死亡率降低了18个百分点(95%CI,-5.3~-1.9),心源性休克降低了24个百分点(95%CI,-4.3~-0.5)。校正了危险评分后,NSTEACS患者的院内死亡率降低了0.7个百分点(95%CI,-1.7~0.3);STEMI患者6个月随访时卒中发生率降低了0.8个百分点(95%CI,-1.7~0.1),心肌梗死发生率降低了2.8个百分点(95%CI,-6.4~0.9)。NSTEACS患者6个月死亡率降低了1.6个百分点(95%CI,-3.0~-0.1),卒中发生率降低了0.7个百分点(95%CI,-1.4~0.1)。

| 3 | Assessment of potential impacts to surface and subsurface water bodies due to longwall mining显示文摘Ground movements due to longwall mining operations have the potential to damage the hydrological balance within as well as outside the mine permit area in the form of increased surface ponding and changes to hydrogeological properties. Recently, the Office of Surface Mining, Reclamation and Enforcement(OSMRE) in the USA, has completed a public comment period on a newly proposed rule for the protection of streams and groundwater from adverse impacts of surface and underground mining operations(80 FR 44435). With increased community and regulatory focus on mining operations and their potential to adversely affect streams and groundwater, now there is a greater need for better prediction of the possible effects mining has on both surface and subsurface bodies of water. With mining induced stress and strain within the overburden correlated to changes in the hydrogeological properties of rock and soil, this paper investigates the evaluation of the hydrogeological system within the vicinity of an underground mining operation based on strain values calculated through a surface deformation prediction model. | 5 | Evaluation of a point of care ultrasound curriculum for Indonesian physicians taught by fi rst-year medical students显示文摘BACKGROUND: The purpose of this study was to assess the short-term efficacy of a 4-week ultrasound curriculum taught by American first-year medical students to general practitioners working in public health care clinics, or puskesmas, in Bandung, Indonesia.METHODS: We performed a prospective, observational study of Indonesian health care practitioners from public clinics in Bandung, Indonesia. These practitioners were enrolled in a 4-week ultrasound training course taught by first-year American medical students. A total of six sessions were held comprising of 38 ultrasound milestones. A pre-course and post-course written exam and practical exam was taken by each participant.RESULTS: We enrolled 41 clinicians in the course. The average pre-course exam score was 35.2% with a 2.4% pass rate, whereas the average post-course exam score was 82.0% with a 92.7% pass rate. The average practical score at the completion of the course was 83.2%(SD=0.145) with 82.9% of the class passing(score above 75.0%).CONCLUSION: Our data suggests that first-year medical students can effectively teach ultrasound to physicians in Indonesia using a 4-week intensive ultrasound training course. | 10 | Comparison of ultrasound-measured properties of the common carotid artery to tobacco smoke exposure in a cohort of Indonesian patients显示文摘BACKGROUND: The purpose of this study was to use point-of-care ultrasound(POCUS) to investigate the relationship between tobacco smoke exposure and the characteristics of the common carotid artery(CCA). The effect of both primary and secondary smoking on CCA properties was evaluated.METHODS: We performed a prospective cross-sectional study across 20 primary care clinics in Bandung, West Java, Indonesia in July 2016. Point of care ultrasound was performed on a convenience sample of Indonesian patients presenting to clinic. The CCA wall stiffness and carotid intima-media thickness(CIMT) were measured during diastole and systole. These measurements were correlated with smoke exposure and cardiovascular disease.RESULTS: We enrolled 663 patients in the study, with 426 patients enrolled in the smoking category and 237 patients enrolled in the second-hand smoke category. There was an overall positive correlation with the measured lifestyle factors and the ultrasound-measured variables in the group of individuals who smoked. For all variables, age seemed to contribute the most out of all of the lifestyle factors for the positive changes in CIMT and CCA wall stiffness.CONCLUSION: Our data yielded correlations between CCA properties and cardiovascular risk, as well as between CIMT and arterial stiffness. We were also able to demonstrate an increase in thickness of the CIMT in patients who have been exposed by tobacco through the use of ultrasound.
