| 1 | Recent advances in endovascular techniques for management of acute nonvariceal upper gastrointestinal bleeding显示文摘Over the past two decades,transcatheter arterial embolization has become the first-line therapy for the management of upper gastrointestinal bleeding that is refractory to endoscopic hemostasis.Advances in catheter-based techniques and newer embolic agents, as well as recognition of the effectiveness of minimally invasive treatment options,have expanded the role of interventional radiology in the management of hemorrhage for a variety of indications,such as peptic ulcerbleeding,malignant disease,hemorrhagic Dieulafoy lesions and iatrogenic or trauma bleeding.Transcatheter interventions include the following:selective embolization of the feeding artery,sandwich coil occlusion of the gastroduodenal artery,blind or empiric embolization of the supposed bleeding vessel based on endoscopic findings and coil pseudoaneurysm or aneurysm embolization by three-dimensional sac packing with preservation of the parent artery.Transcatheter embolization is a fast,safe and effective,minimally invasive alternative to surgery when endoscopic treatment fails to control bleeding from the upper gastrointestinal tract.This article reviews the various transcatheter endovascular techniques and devices that are used in a variety of clinical scenarios for the management of hemorrhagic gastrointestinal emergencies. | Romaric F Loffroy Basem A Abualsaud Ming D Lin Pramod P Rao | 2011 | World Journal of Gastrointestinal Surgery2011,3,7: | 28 |
| 2 | Hypoalbuminemia in colorectal cancer prognosis:Nutritional marker or inflammatory surrogate?显示文摘Albumin is the single most abundant protein in the human serum. Its roles in physiology and pathology are diverse. Serum albumin levels have been classically thought to reflect the nutritional status of patients. This concept has been challenged in the last two decades as multiple factors, such as inflammation, appeared to affect albumin levels independent of nutrition. In general, cancer patients have a high prevalence of hypoalbuminemia. As such, the role of hypoalbuminemia in patients with colorectal cancer has received significant interest. We reviewed the English literature on the prognostic value of pretreatment albumin levels in colorectal cancer. | 3 | Endoscopic closure of a gastrocolic fistula using the over-the-scope-clip-system显示文摘Gastrointestinal (GI) defects such as fistulas and leaks can be potentially closed endoscopically using hemo-clips and loops. However, hemoclips may not allow for closure of large defects and they do not exert enough tensile force to keep fibrotic defects larger than 5 mm approximated. Herein we present a case of successful endoscopic closure of a gastrocolic fistula in a severely malnourished patient with complex post-surgical upper GI anatomy. We strongly believe that this device is a major breakthrough for the management of various types of discontinuity defects or fistulas. In addition, we show the usefulness of placing a direct jejunostomy using the double balloon enteroscopy (DBE) technique during the same procedure. The concept of providing direct jejunal feedings while allowing for upper gas-trointestinal bowel rest to promote the healing of the minimally invasive endoscopic operation is novel. | 4 | Efficacy and safety of botulinum toxin in treatment of anismus: A systematic review显示文摘AIM: To evaluate the efficacy and safety of botulinum toxin type A(BTX-A) in the management of patients with anismus. METHODS: An organized search of published literature was conducted using electronic databases including: Pub Med/MEDLINE, and Cochrane Central Register of Control ed Trials, also an internet-based search using 'Google Scholar' service was conducted. Both comparative and observational studies were included. We excluded irrelevant articles, editorials, case reports, reviews, and meta-analyses. The studies that followed the patients less than 6 mo were excluded. Variables collected were demographic data of the patients, technique of BTX-A injection and number of sessions, short-term and longterm clinical improvement, post-injection changes in electromyography(EMG), defecography, manometry, and balloon expulsion test, and complications recorded after BTX-A injection.RESULTS: Seven studies comprising 189 patients were included in the review. The median age of the patients was 41.2 years and female-to-male ratio was 1.3:1. The median dose of BTX-A injected per procedure was 100 IU(range, 20-100 IU). Lateral injection was done in five trails and combined lateral and posterior injections in two trials. Three studies used endorectal ultrasonographyguided technique, one study used EMG-guided technique,whereas the remaining three studies used manual palpation with the index finger. The median percentage of patients who reported initial improvement of symptoms was 77.4%(range 37.5%-86.7%), this percentage declined to a median of 46%(range 25%-100%) at 4 mo after injection of BTX-A. Rates of improvement evaluated by balloon expulsion test, EMG, and defecography ranged between(37.5%-80%),(54%-86.7%), and(25%-86.6%), respectively. Fourteen(7.4%) patients developed complications after injection of BTX-A. Complication rates across the studies ranged from 0% to 22.6%. CONCLUSION: Initial satisfactory improvement of symptoms after BTX-A injection remarkably deteriorated after 3 mo of the procedure. However, repeated injection may provide better sustained results with no additional morbidities. | 5 | Survival and outcomes for co-infection of chronic hepatitis C with and without cirrhosis and COVID-19: A multicenter retrospective study显示文摘BACKGROUND Chronic liver disease,particularly cirrhosis,is associated with worse outcomes in patients infected with coronavirus disease 2019(COVID-19).AIM To assess outcomes of COVID-19 infection among patients with pre-existing hepatitis C with or without liver cirrhosis.METHODS This multicenter,retrospective cohort study included all cases of confirmed coinfection of severe acute respiratory syndrome coronavirus 2 and chronic hepatitis C with or without liver cirrhosis who were admitted to six hospitals(Al-Sahel Hospital,Al-Matareya Hospital,Al-Ahrar Hospital,Ahmed Maher Teaching Hospital,Al-Gomhoreya Hospital,and the National Hepatology and Tropical Medicine Research Institute)affiliated with the General Organization for Teaching Hospitals and Institutes in Egypt.Patients were recruited from May 1,2020,to July 31,2020.Demographic,laboratory,imaging features,and outcomes were collected.Multivariate regression analysis was performed to detect factors affecting mortality.RESULTS This retrospective cohort study included 125 patients with chronic hepatitis C and COVID-19 co-infection,of which 64(51.20%)had liver cirrhosis and 40(32.00%)died.Fever,cough,dyspnea,and fatigue were the most frequent symptoms in patients with liver cirrhosis.Cough,sore throat,fatigue,myalgia,and diarrhea were significantly more common in patients with liver cirrhosis than in noncirrhotic patients.There was no difference between patients with and without cirrhosis regarding comorbidities.Fifteen patients(23.40%)with liver cirrhosis presented with hepatic encephalopathy.Patients with liver cirrhosis were more likely than non-cirrhotic patients to have combined ground-glass opacities and consolidations in CT chest scans:28(43.75%)vs 4(6.55%),respectively(P value<0.001).These patients also were more likely to have severe COVID-19 infection,compared to patients without liver cirrhosis:29(45.31%)vs 11(18.04%),respectively(P value<0.003).Mortality was higher in patients with liver cirrhosis,compared to those with no cirrhosis:33(51.56%)vs 9(14.75%),respectively(P value<0.001).All patients in Child-Pugh class A recovered and were discharged.Cirrhotic mortality occurred among decompensated patients only.A multivariate regression analysis revealed the following independent factors affecting mortality:Male gender(OR 7.17,95%CI:2.19–23.51;P value=0.001),diabetes mellitus(OR 4.03,95%CI:1.49–10.91;P value=0.006),and liver cirrhosis(OR 1.103,95%CI:1.037–1.282;P value<0.0001).We found no differences in liver function,COVID-19 disease severity,or outcomes between patients who previously received direct-acting antiviral therapy(and achieved sustained virological response)and patients who did not receive this therapy.CONCLUSION Patients with liver cirrhosis are susceptible to higher severity and mortality if infected with COVID-19.Male gender,diabetes mellitus,and liver cirrhosis are independent factors associated with increased mortality risk. | Shimaa Afify Basem Eysa Fatma Abdel Hamid Omnia M Abo-Elazm Mohamed A Edris Rabab Maher Ahmed Abdelhalim Muhammad Mostafa Abdel Ghaffar Dalia A Omran Hend Ibrahim Shousha | 2021 | World Journal of Gastroenterology2021,27,42: | 2 |

| 13 | Geochemical and geochronological characteristics of the Um Rus granite intrusion and associated gold deposit,Eastern Desert,Egypt显示文摘The Um Rus tonalite-granodiorite intrusion(~6 km2)occurs at the eastern end of the Neoproterozoic,ENE-trending Wadi Muba rak shear belt in the Central Eastern Desert of Egypt.Gold-bearing quartz veins hosted by the Um Rus intrusion were mined intermittently,and initially by the ancient Egyptians and until the early 1900 s.The relationship between the gold mineralization,host intrusion,and regional structures has always been unclear.We present new geochemical and geochronological data that help to define the tectonic environment and age of the Um Rus intrusion.In addition,field studies are integrated with EPMA and LA-ICP-MS data for gold-associated sulfides to better understand the formation and distribution of gold mineralization.The bulk-rock geochemical data of fresh host rocks indicate a calc-alkaline,metaluminous to mildly peraluminous,I-type granite signature.Their trace element composition reflects a tectonic setting intermediate between subduction-related and within-plate environments,presumably transitional between syn-and post-collisional stages.The crystallization age of the Um Rus intrusion was determined by in situ SHRIMP 206 Pb/238 U and 207Pb/235U measurements on accessory monazite grains.The resultant monazite U-Pb weighted mean age(643±9 Ma;MSWD 1.8)roughly overlaps existing geochronological data for similar granitic intrusions that are confined to major shear systems and are locally associated with gold mineralization in the Central Eastrn Desert(e.g.,Fawakhir and Hangaliya).This age is also consistent with magmatism recognized as concomitant to transpressional tectonics(D2:~650 Ma)during the evolution of the Wadi Mubark belt.Formation of the gold-bearing quartz veins in NNE-SSW and N-S striking fault segments was likely linked to the change from transpressional to transtensional tectonics and terrane exhumation(D3:620-580 Ma).The development of N-S throughgoing fault arrays and dike swarms(~595 Ma)led to heterogeneous deformation and recrystallization of the mineralized quartz veins.Ore minerals in the auriferous quartz veins include ubiquitous pyrite and arsenopyrite,with less abundant pyrrhotite,chalcopyrite,sphalerite,and galena.Uncommon pentlandite,gersdorffite,and cobaltite inclusions hosted in quartz veins with meladiorite slivers are interpreted as pre-ore sulfide phases.The gold-sulfide paragenesis encompasses an early pyrite-arsenopyrite±loellingite assemblage,a transitional pyrite-arsenopyrite assemblage,and a late pyrrhotite-chalcopyrite-sphalerite±galena assemblage.Free-milling gold/electrum grains(10 sμm-long)are scattered in extensively deformed vein quartz and in and adjacent to sulfide grains.Marcasite,malachite,and nodular goethite are authigenic alteration phases after pyrrhotite,chalcopyrite,and pyrite and arsenopyrite,respectively.A combined ore petrography,EPMA,and LA-ICP-MS study distinguishes morphological and compositional differences in the early and transitional pyrites(PyⅠ,PyⅡ)and arsenopyrite(ApyⅠ,ApyⅡ).Py I forms uncommon small euhedral inclusions in later PyⅡand Apy II.PyⅡforms large subhedral crystals with porous inner zones and massive outer zones,separated by narrow As-rich irregular mantles.The Fe and As contents in PyⅡare variable,and the LA-ICP-MS analysis shows erratic concentrations of Au(<1 to 177 ppm)and other trace elements(e.g.,Ag,Te,and Sb)in the porous inner zones,most likely related to discrete sub-microscopic sulfide inclusions.The outer massive zones have a rather homogenous composition,with consistently lower abundances of base metals and Au(mean 1.28 ppm).The early arsenopyrite(Apy I)forms fine-grained euhedral crystals enriched in Au(mean 17.7 ppm)and many other trace elements(i.e.,Ni,Co,Se,Ag,Sb,Te,Hg,and Bi).On the other hand,ApyⅡoccurs as coarsegrained subhedral crystals with lower and less variable concentrations of Au(mean 4 ppm).Elevated concentrations of Au(max.327 ppm)and other trace elements are measured in fragmented and aggregated pyrite and arsenopyrite grains,whereas the undeformed intact zones of the same grains are poor in all trace elements.The occurrence of gold/electrum as secondary inclusions in deformed pyrite and arsenopyrite crystals indicates that gold introduction was relatively late in the paragenesis.The LAICP-MS results are consistent with gold redistribution by the N-S though-going faults/dikes overprinted the earlier NNW-SSE quartz veins in the southeastern part of the intrusion,where the underground mining is concentrated.Formation of the Um Rus intrusion and gold-bearing quartz veins can be related to the evolution of the Wadi Mubarak shear belt,where the granitic intrusion formed during or just
