| 1 | The PI3K/Akt/m TOR pathway in ovarian cancer: therapeutic opportunities and challenges显示文摘The phosphatidylinositol 3 kinase(PI3K) pathway is frequently altered in cancer, including ovarian cancer(OC). Unfortunately, despite a sound biological rationale and encouraging activity in preclinical models, trials of first-generation inhibitors of mammalian target of rapamycin(m TOR) in OC have demonstrated negative results. The lack of patient selection as well as resistance to selective m TOR complex-1(m TORC1) inhibitors could explain the disappointing results thus far. Nonetheless, a number of novel agents are being investigated, including dual m TORC1/m TORC2, Akt, and PI3 K inhibitors. Although it is likely that inhibition of the PI3K/Akt/m TOR pathway may have little effect in unselected OC patients, certain histological types, such as clear cell or endometrioid OC with frequent phosphatidylinositol-4,5-biphosphate 3-kinase, catalytic subunit alpha(PIK3CA) and/or phosphatase and tensin homolog(PTEN) alterations, may be particularly suited to this approach. Given the complexity and redundancy of the PI3 K signaling network, PI3 K pathway inhibition may be most useful in combination with either chemotherapy or other targeted therapies, such as MEK inhibitors, anti-angiogenic therapy, and hormonal therapy, in appropriately selected OC patients. Here, we discuss the relevance of the PI3 K pathway in OC and provide an up-to-date review of clinical trials of novel PI3 K inhibitors alone or in combination with cytotoxics and novel therapies in OC. | 2 | H pylori (CagA) and Epstein-Barr virus infection in gastric carcinomas:Correlation with p53 mutation and c-Myc,Bcl-2 and Bax expression显示文摘AIM: To investigate the interrelationship between H pylori and Epstein-Barr virus (EBV) infection in the gastric carcinogenesis having in focus the p53 mutation and the c-Myc, Bcl-2 and Bax expression. METHODS: seventy-one gastric carcinoma tissues were assessed by polymerase chain reaction (PCR) for H pylori and in situ hybridization for EBV. c-Myc, Bcl-2 and Bax expression were detected by immunohistochemistry and single-stranded conformational polymorphism (SSCP) for p53 mutation. RESULTS: The positivity rates for H pylori and EBV were 94.4% and 8.45%, respectively. The majority of the cases displayed only the H pylori presence. All EBV positive cases were also H pylori positive. None infectious agent was observed in 5.55% of the cases. The intestinal type tumor was more frequent in the co-infected and non-infected groups. The female predominated in the non-infected group showing statistical significance (70.4% vs 29.6%, P=0.039). The Bcl-2 was only detected in the group exclusively infected by H pylori. However, c-Myc and Bax were detected in the three groups but with a low frequency in the co-infected group. Mutation of p53 was present in all groups, with the highest frequencies in the H pylori positive groups. CONCLUSION: The frequency of H pylori infection in gastric carcinomas was high. The presented data indicated that gastric carcinogenesis has different pathways depending of the presence of the two investigated infectious agents, suggesting a possible involvement of H pylori with apoptotic process. The low expression of c-Myc and Bax in the EBV-positive groups suggests that EBV may inhibit the expression of these proteins. Here, we report the first use of MARS during a spontaneous pregnancy and its successful outcome in a patient with PFIC3 and intractable pruritus. | 4 | Educated consumers don't believe artificial meat is the solution to the problems with the meat industry显示文摘The production of in vitro meat by cell culture has been suggested by some scientists as one solution to address the major challenges facing our society. Firstly, consumers would like the meat industry to reduce potential discomfort of animals on modern farms, or even to avoid killing animals to eat them. Secondly, citizens would like meat producers to reduce potential environmental deterioration by livestock and finally, there is a need to reduce world hunger by increasing protein resources while the global population is predicted to grow rapidly. According to its promoters, artificial meat has a potential to make eating animals unnecessary, to reduce carbon footprint of meat production and to satisfy all the nutritional needs and desires of consumers and citizens. To check these assumptions, a total of 817 educated people(mainly scientists and students) were interviewed worldwide by internet in addition to 865 French educated people. We also interviewed 208 persons(mainly scientists) after an oral presentation regarding artificial meat. Results of the three surveys were similar, but differed between males and females. More than half of the respondents believed that 'artificial meat' was feasible and realistic. However, there was no majority to think that artificial meat will be healthy and tasty, except respondents who were in favour of artificial meat. A large majority of the respondents believed that the meat industry is facing important problems related to the protection of the environment, animal welfare or inefficient meat production to feed humanity. However, respondents did not believe that artificial meat will be the solution to solve the mentioned problems with the meat industry, especially respondents who were against artificial meat. The vast majority of consumers wished to continue to eat meat even they would accept to consume less meat in a context of increasing food needs. Only a minority of respondents(from 5 to 11%) would recommend or accept to eat in vitro meat instead of meat produced from farm animals. Despite these limitations, 38 to 47% of the respondents would continue to support research on artificial meat, but a majority of them believed that artificial meat will not be accepted by consumers in the future, except for respondents who were in favour of artificial meat. | 6 | Helicobacter pylori and EBV in gastric carcinomas:Methylation status and microsatellite instability显示文摘AIM:To verify the methylation status of CDH1, DAPK, COX2, hMLH1 and CDKN2A genes and to evaluate their association with Helicobacter pylori (H. pylori)-cagA+ and Epstein Barr virus (EBV) infections in gastric adenocarcinomas.METHODS: Methylation-specific PCR (MSP) assay was performed in 89 primary gastric carcinomas (intestinal and diffuse types). Microsatellite instability (MSI) analysis was performed using the BAT26 primer set and PCR products were analyzed with the ABI PRISM 3100 Genetic Analyzer using Genescan 3.7 software (Applied Biosystems). Detection of H. pylori and genotyping were performed by PCR, using specifi c primers for ureaseC and cagA genes. The presence of EBV was assessed by in situ hybridization. Statistical analyses were performed using the χ2 or Fisher's exact test.RESULTS: The most frequent hypermethylated gene was COX-2 (63.5%) followed by DAPK (55.7%), CDH1 (51%), CDKN2A (36%) and hMLH1 (30.3%). Intestinal and diffuse adenocarcinomas showed different methylation profiles and there was an association between methylation of E-CDH1 and H. pylori-cagA+ in the intestinal adenocarcinoma type. MSI was correlated with hMLH1 methylation. | 10 | Chemical Availability of Cd, Pb and Zn in Anthropogenically Polluted Soil: Assessing the Geochemical Reactivity and Oral Bioaccessibility显示文摘The most recent in vitro tests used to determine metal bioaccessiblility are generally time-consuming and expensive. This study aimed at determining potential relationships between the concentrations of metals extracted using single-extraction methods and the concentrations of bioaccessible metals assessed by a harmonised in vitro test, the Unified BARGE Method(UBM). A total number of 27 soil samples were collected from kitchen gardens and lawns with various physicochemical parameters and contamination levels.Significant relationships were obtained between Cd, Pb and Zn extracted in gastric and gastrointestinal phases and using single extractions. The best relationhips were established using acetic and citric acids for Cd, whereas for Pb, citric acid and ethylenediaminetetraacetic acid(EDTA) were identified as the best extractants. These relationships were improved by means of a linear multiple regression with a downward stepwise procedure involving agronomic parameters(soil cation exchange capacity and assimilated P). This method highlighted the fact that the cation exchange capacity and P contents in soils were the two main parameters that controlled the human bioaccessibility of Cd, Pb and Zn in the gastric phase. | 11 | Characterization of two rat models of cystic fibrosis—KO and F508del CFTR—Generated by Crispr-Cas9显示文摘Background: Genetically engineered animals are essential for gaining a proper understanding of the disease mechanisms of cystic fibrosis(CF). The rat is a relevant laboratory model for CF because of its zootechnical capacity, size, and airway characteristics, including the presence of submucosal glands.Methods: We describe the generation of a CF rat model(F508 del) homozygous for the p.Phe508 del mutation in the transmembrane conductance regulator(Cftr) gene. This model was compared to new Cftr-/-rats(CFTR KO). Target organs in CF were examined by histological staining of tissue sections and tooth enamel was quantified by micro-computed tomography. The activity of CFTR was evaluated by nasal potential difference(NPD) and short-circuit current measurements. The effect of VX-809 and VX-770 was analyzed on nasal epithelial primary cell cultures from F508 del rats.Results: Both newborn F508 del and Knock out(KO) animals developed intestinal obstruction that could be partly compensated by special diet combined with an osmotic laxative. The two rat models exhibited CF phenotypic anomalies such as vas deferens agenesis and tooth enamel defects. Histology of the intestine, pancreas, liver, and lungs was normal. Absence of CFTR function in KO rats was confirmed ex vivo by short-circuit current measurements on colon mucosae and in vivo by NPD, whereas residual CFTR activity was observed in F508 del rats. | 13 | Quality of life, physical performance and nutritional status in older patients hospitalized in a cardiology department显示文摘Objectives Quality of life(QoL) is a priority outcome in older adults suffering from cardiovascular diseases. Frailty and poor nutritional status may affect the QoL through mobility disorders and exhaustion. The objective of this study was to determine if physical frailty and nutritional status were associated with QoL, in older cardiology patients. Methods Cross sectional, observational study conducted in a cardiology department from a university hospital. Participants(n = 100) were aged 70 and older. Collected data included age, sex, cardiac diseases, New York Heart Association(NYHA) classification, comorbidities(Charlson Index) and disability. A Short Physical Performance Battery(SPPB), including walking speed assessment was performed;handgrip strength were measured as well as Fried’s frailty phenotype. Nutritional status was assessed using the Mini Nutritional Assessment(MNA) and Body Mass Index(BMI), inflammation by C-reactive protein(CRP). QoL was assessed using the EORTC–QLQ questionnaire. Univariate and multivariate analyses were performed to study the associations between all recorded parameters and QoL. Results In participants(mean age: 79.3 ± 6.7 years;male: 59%), Charlson index, arrhythmia, heart failure, NYHA class III-IV, MNA, disability, walking speed, SPPB score, frailty and CRP were significantly associated with QoL in univariate analysis. Multivariate analysis showed that NYHA class III-IV(P < 0.001), lower MNA score(P = 0.03), frailty(P < 0.0001), and higher CRP(P < 0.001) were independently associated with decreased QoL. Conclusions Frailty, nutritional status and inflammation were independently associated with poor QoL. | 15 | Usefulness of the Hook knife in flexible endoscopic myotomy for Zenker's diverticulum显示文摘AIM To investigate the outcome of flexible endoscopic myotomy performed with the Hook knife in patients with symptomatic Zenker's diverticulum(ZD).METHODS All consecutive patients treated for ZD at our institution between 7/2012 and 12/2016 were included.The flexible endoscopic soft diverticuloscope-assisted technique with endoclips placement and Hook knife myotomy were performed in all patients.Here we report a retrospective review of prospectively collected data.Demographics,dysphagia score(Dakkak and Bennett),associated symptoms and adverse events were collected pre-procedure,at 2 and 6 mo post-procedure,and at the end of the follow-up period.Clinical success was defined as at least 1-point improvement in dysphagia score and a residual dysphagia score≤1,with no need for reintervention.Dysphagia scores were compared before treatment and at end-of-follow-up using the Wilcoxon test.RESULTS Twenty-four patients were included.Mean size of ZD was 3.0 cm(range 2-8 cm).Mean number of sessions was 1.17/patient(range 1-3 sessions).Overall clinical success was 91.7%.Two adverse events(8.3%) occurred,and both were managed conservatively.No bleeding or perforation was reported.Mild pain was reported by 9 patients(37.5%).Median hospital stay was 1 d(range 1-6).Median follow-up was 19.5 mo(range 6-53).Mean ± SD dysphagia score was 2.25±0.89 before treatment and decreased to 0.41 ± 0.92 at end-of-follow-up(P<0.001).Regurgitation and cough dropped from 91.7% and 50% to 12.5% and 0% at the end of follow-up,respectively.Recurrence was observed in 3 patients,and all 3 were symptom-free after one more session.CONCLUSION The Hook knife,used in the soft diverticuloscope-assisted technique setting,is efficient and safe for treatment of ZD. | Olivier Rouquette Armando Abergel Aurélien Mulliez Laurent Poincloux | 2017 | World Journal of Gastrointestinal Endoscopy2017,9,8: | 3 |

Approximately one-quarter of newly diagnosed patients for CRC have metastases, and a further 40%-50% experience disease recurrence or develop metastases after all standard therapies. Therefore, understanding the molecular mechanisms involved in the progression of CRC and subsequently developing novel therapeutic targets is crucial to improve management of CRC and patients' long-term survival. Several tyrosine kinase receptors have been implicated in CRC development, progression and metastasis, including epidermal growth factor receptor(EGFR) and vascular EGFR. Recently, tropomyosin-related kinase B(Trk B), a tyrosine kinase receptor, has been reported in CRC and found to clearly exert several biological and clinical features, such as tumor cell growth and survival in vitro and in vivo, metastasis formation and poor prognosis. Here we review the significance of Trk B and its ligand brain derived-neurotrophic factor in CRC. We focus on their expression in CRC tumor samples, and their functional roles in CRC cell lines and in in vivo models. | 18 | Transjugular intrahepatic portosystemic shunt as bridge-to-surgery in refractory gastric antral vascular ectasia显示文摘Gastric antral vascular ectasia(GAVE) may cause gastrointestinal bleeding(GIB). The treatment of GAVE relies on endoscopic approaches such as electrocoagulation(argon plasma coagulation, laser therapy, heater probe therapy, radiofrequency ablation), cryotherapy, and band ligation. In refractory cases, antrectomy may be considered. In the event of an associated cirrhosis and portal hypertension, it has been suggested that antrectomy could be an option, provided the mortality risk isn't considered too great. We report the case of a 67-year-old cirrhotic patient who presented with GAVE related GIB, unresponsive to multiple endoscopic treatments. The patient had a good liver function(model for end-stage disease 10). After a multidisciplinary meeting, a transjugular intrahepatic portosystemic shunt(TIPS) procedure was performed, in order to treat the cirrhosis associated ascites. The outcome was successful. | 20 | Perfusion characterization of liver metastases from endocrine tumors:Computed tomography perfusion显示文摘AIM:To assess prospectively parameters of computed tomography perfusion(CT p) for evaluation of vascularity of liver metastases from neuroendocrine tumors.METHODS:This study was approved by the hospital's institutional review board.All 18 patients provided informed consent.There were 30 liver metastases from neuroendocrine tumors.Patients were divided into three groups depending on the appearance of the liver metastases at the arterial phase of morphological CT(hyperdense,hypodense and necrotic).Sequential acquisition of the liver was performed before and for 2 min after intravenous injection of 0.5 mg/kg contrast medium,at 4 mL/s.Data were analyzed using deconvolution analysis to calculate blood flow(BF),blood volume(BV),mean transit time(MTT),hepatic arterial perfusion index(HAPI) and a bi-compartmental analysis was performed to obtain vascular permeability-surface area product(PS).Post-treatment analysis was performed by a radiologist and regions of interest were plotted on the metastases,normal liver,aorta and portal vein.RESULTS:At the arterial phase of the morphological CT scan,the aspects of liver metastases were hyperdense(n=21),hypodense(n=7),and necrotic(n=2).In cases of necrotic metastases,none of the CT p parameters were changed.Compared to normal liver,a significant difference in all CT p parameters was found in cases of hyperdense metastases,and only for HAPI and MTT in cases of hypodense metastases.No significant difference was found for MTT and HAPI between hypo-and hyperdense metastases.A significant decrease of PS,BV and BF was demonstrated in cases of patients with hypodense lesions PS(23±11.6 mL/100 g per minute) compared to patients with hyperdense lesions;PS(13.5±10.4 mL/100 g per minute),BF(93.7±75.4 vs 196.0±115.6mL/100 g per minute) and BV(9.7±5.9 vs 24.5 10.9 mL/100 g).CONCLUSION:CT p provides additional information compared to the morphological appearance of liver metastases. | Aurélie Guyennon Marius Mihaila John Palma Catherine Lombard-Bohas Jean-Alain Chayvialle Frank Pilleul | 2010 | World Journal of Radiology2010,2,11: | 2 |
