| 1 | Dual-sugar tests of small intestinal permeability are poor predictors of bacterial infections and mortality in cirrhosis: a prospective study显示文摘AIM: To prospectively analyze the impact of increased intestinal permeability(IP) on mortality and the occurrence of infections in patients with cirrhosis.METHODS: IP was quantified using the lactulose/mannitol(L/M) test in 46 hospitalized patients with cirrhosis(25 Child-Pugh A/B, 21 Child-Pugh C) and in 16 healthy controls. Markers of inflammation [LPSbinding protein, Interleukin-6(IL-6)] and enterocyte death [intestinal fatty-acid binding protein(I-FABP)] were determined in serum using enzyme-linked immunosorbent assays. Patients were followed for one year and assessed for survival, liver transplantation, the necessity of hospitalization and the occurrence of bacterial infections. The primary endpoint of the study was defined as differences in survival between patients with pathological and without pathological lactulose/mannitol test.RESULTS: Thirty-nine(85%) patients with cirrhosis had a pathologically increased IP index(L/M ratio > 0.07) compared to 4(25%) healthy controls(P < 0.0001). The IP index correlated with the ChildPugh score(r = 0.484, P = 0.001) and with serum IL-6(r = 0.342, P = 0.02). Within one year, nineteen(41%) patients developed a total of 33 episodes of hospitalization with bacterial or fungal infections. Although patients who developed spontaneous bacterial peritonitis(SBP)(n = 7) had a higher IP index than patients who did not(0.27 vs 0.14, P = 0.018), the baseline IP index did not predict time to infection, infection-free survival or overall survival, neither when assessed as linear variable, as tertiles, nor dichotomized using an established cut-off. | 2 | The efficacy and effectiveness of machine learning for weaning in mechanically ventilated patients at the intensive care unit: a systematic review显示文摘Weani ng from mechanical ventilation in the in tensive care unit (ICU) is a complex clinical problem and relevant for future organ engineering. Prolonged mechanical ventilation (MV) leads to a range of medical complications that increases length of stay and costs as well as contributes to morbidity and even mortality and long-term quality of life. The need to reduce MV is both clinical and economical. Artificial intelligence or machine learning (ML) methods are promising opportunities to positively influence patient outcomes. ML methods have been proposed to enhance clinical decisions processes by using the large amount of digital information generated in the ICU setting. There is a particular interest in empirical methods (such as ML) to improve management of 'difficult-to-wean' patients, due to the associated costs and adverse events associated with this population. A systematic literature search was performed using the OVID, IEEEXplore, PubMed, and Web of Science databases. All publications that included (1) the application of ML to weaning from MV in the ICU and (2) a clinical outcome measurement were reviewed. A checklist to assess the study quality of medical ML publications was modified to suit the critical assessment of ML in MV weaning literature. The systematic search identified nine studies that used ML for weaning management from MV in critical care. The weaning management application areas included (1) prediction of successful spontaneous breathing trials (SBTs),(2) prediction of successful extubation,(3) prediction of arterial blood gases, and (4) ventilator setting and oxygenation-adjustment advisory systems. Seven of the nine studies scored seven out of eight on the quality index. The remaining two of the nine studies scored one out of eight on the quality index. This scoring may, in part, be explained by the publications' focus on technical novelty, and therefore focusing on issues most important to a technical audience, instead of issues most important for a systematic medical review. This review showed that only a limited number of studies have started to assess the efficacy and effectiveness of ML for MV in the ICU. However, ML has the potential to be applied to the prediction of SBT failure, extubation failure, and blood gases, and also the adjustment of ventilator and oxygenation settings. The available databases for the development of ML in this clinical area may still be inadequate. None of the reviewed studies reported on the procedure, treatment, or sedation strategy undergone by patients. | 3 | Towards a versatile point-of-care systemcombining femtosecond laser generatedmicrofluidic channels and direct laserwritten microneedle arrays显示文摘Microneedle-based microfluidic systems have a great potential to become well-accepted medical devices for simple,accurate,and painless drug delivery and lab-on-a-chip diagnostics.In this work,we report on a novel hybrid approach combining femtosecond direct laser written microneedles with femtosecond laser generated microfluidic channels providing an important step towards versatile medical point-of-care systems.Hollow microneedle arrays are fabricated by a laser system designed for two-photon polymerization applications.Compression tests of two different types of truncated cone-shaped microneedle arrays prepared from OrmoComp^(■)give information about the microneedle mechanical strength,and the results are compared to skin insertion forces.Three-dimensional microchannels are directly created inside PMMA bulk material by an ultrashort pulse laser system with vertical channels having adjustable cross-sectional areas,which allow attaching of microneedles to the microfluidic system.A comprehensive parameter study varying pulse duration and repetition rate is performed on two-photon polymerization to identify an optimal laser power range for fabricating microneedles using the same pulse duration and repetition rate as for microchannels.This addresses the advantage of a single laser system process that overcomes complex fabrication methods.A proof of concept flow test with a rhodamine B dye solution in distilled water demonstrates that the combination of microneedles and microchannels qualifies for microfluidic injection and extraction applications. | Anika Trautmann Gian-Luca Roth Benedikt Nujiqi Thomas Walther Ralf Hellmann | 2019 | Microsystems & Nanoengineering2019,5,1: | 3 |
| 4 | Chemometric analysis of femtosecond transient absorption spectroscopy data:Study of the photochromism of anils显示文摘Chemometric methods are applied for the purpose of extracting relevant information from transient absorption spectroscopy data probing the photochromism of molecules from the family of salicylidene aniline. The process consists of an ultrafast excited state intramolecular proton transfer occurring from an enol form which is then followed by a cis-trans isomerization to finally reach a trans-keto photo-product. This work focuses on the potential of combining multivariate curve resolution for modeling pure profiles and two dimensional correlation spectroscopy data analysis for providing information on the dynamics of spectral features. The results obtained for one derivative of salicylidene aniline provide information regarding the number of species created after the proton transfer and characterization of their absorption spectra and their kinetics in the picosecond time scale. The spectral resolution of two cis-keto* forms is proposed for the first time. It is also found that both cis-keto* species are involved in the formation of the trans-keto photo-product. | 5 | Microplastic fiber and drought effects on plants and soil are only slightly modified by arbuscular mycorrhizal fungi显示文摘Microplastics are increasingly recognized as a factor of global change. By altering soil inherentproperties and processes, ripple-on effects on plants and their symbionts can be expected.Additionally, interactions with other factors of global change, such as drought, can influence theeffect of microplastics. We designed a greenhouse study to examine effects of polyester microfibers,arbuscular mycorrhizal (AM) fungi and drought on plant, microbial and soil responses. We found thatpolyester microfibers increased the aboveground biomass of Allium cepa under well-watered anddrought conditions, but under drought conditions the AM fungal-only treatment reached the highestbiomass. Colonization with AM fungi increased under microfiber contamination, however, plantbiomass did not increase when both AM fungi and fibers were present. The mean weight diameter ofsoil aggregates increased with AM fungal inoculation overall but decreased when the system wascontaminated with microfibers or drought stressed. Our study adds additional support to themounting evidence that microplastic fibers in soil can affect the plant–soil system by promoting plantgrowth, and favoring key root symbionts, AM fungi. Although soil aggregation is usually positivelyinfluenced by plant roots and AM fungi, and microplastic promotes both, our results show that plasticstill had a negative effect on soil aggregates. | 7 | Dentin matrix protein 1 and phosphate homeostasis are critical for postnatal pulp, dentin and enamel formation显示文摘Deletion or mutation of dentin matrix protein 1 (DMP1) leads to hypophosphatemic rickets and defects within the dentin. However, it is largely unknown if this pathological change is a direct role of DMP1 or an indirect role of phosphate (Pi) or both. It has also been previously shown that Klotho-deficient mice, which displayed a high Pi level due to a failure of Pi excretion, causes mild defects in the dentinal structure. This study was to address the distinct roles of DMP1 and Pi homeostasis in cell differentiation, apoptosis and mineralization of dentin and enamel. Our working hypothesis was that a stable Pi homeostasis is critical for postnatal tooth formation, and that DMP1 has an antiapoptotic role in both amelogenesis and dentinogenesis. To test this hypothesis, Dmp1-null (Dmp1 2/2 ), Klotho-deficient (kl/kl), Dmp1/Klotho-double-deficient (Dmp1 2/2 /kl/kl) and wild-type (WT) mice were killed at the age of 6 weeks. Combinations of X-ray, microcomputed tomography (mCT), scanning electron microscopy (SEM), histology, apoptosis and immunohistochemical methods were used for characterization of dentin, enamel and pulp structures in these mutant mice. Our results showed that Dmp1 2/2 (a low Pi level) or kl/kl (a high Pi level) mice displayed mild dentin defects such as thin dentin and a reduction of dentin tubules. Neither deficient mouse line exhibited any apparent changes in enamel or pulp structure. However, the double-deficient mice (a high Pi level) displayed severe defects in dentin and enamel structures, including loss of dentinal tubules and enamel prisms, as well as unexpected ectopic ossification within the pulp root canal. TUNEL assay showed a sharp increase in apoptotic cells in ameloblasts and odontoblasts.
