1Metal Fabrication by Additive Manufacturing Using Laser and Electron Beam Melting Technologies显示文摘Selective laser melting (SLM) and electron beam melting (EBM) are relatively new rapid, additive manufac- turing technologies which can allow for the fabrication of complex, multi-functional metal or alloy monoliths by CAD-directed, selective melting of precursor powder beds. By altering the beam parameters and scan strategies, new and unusual, even non-equilibrium microstructures can be produced; including controlled microstructural architectures which ideally extend the contemporary materials science and engineering paradigm relating structure-properties-processing-performance. In this study, comparative examples for SLM and EBM fabricated components from pre-alloyed, atomized precursor powders are presented. These include Cu, Ti–6Al–4V, alloy 625 (a Ni-base superalloy), a Co-base superalloy, and 17-4 PH stainless steel. 2Management of duodenal stump fistula after gastrectomy for gastric cancer: Systematic review显示文摘AIM: To identify the most effective treatment of duodenalstump fistula(DSF) after gastrectomy for gastric cancer.METHODS: A systematic review of the literature was performed. Pub Med, EMBASE, Cochrane Library, CILEA Archive, BMJ Clinical Evidence and Up To Date databases were analyzed. Three hundred eighty-eight manuscripts were retrieved and analyzed and thirteen studies published between 1988 and 2014 were finally selected according to the inclusion criteria, for a total of 145 cases of DSF, which represented our group of study. Only patients with DSF after gastrectomy for malignancy were selected. Data about patients' characteristics, type of treatment, short and long-term outcomes were extracted and analyzed. RESULTS: In the 13 studies different types of treatment were proposed: conservative approach, surgical approach, percutaneous approach and endoscopic approach(3 cases). The overall mortality rate was 11.7% for the entire cohort. The more frequent complications were sepsis, abscesses, peritonitis, bleeding, pneumonia and multi-organ failure. Conservative approach was performed in 6 studies for a total of 79 patients, in patients with stable general condition, often associated with percutaneous approach. A complete resolution of the leakage was achieved in 92.3% of these patients, with a healing time ranging from 17 to 71 d. Surgical approach included duodenostomy, duodenojejunostomy, pancreatoduodenectomy and the use of rectus muscle flap. In-hospital stay of patients who underwent relaparotomy ranged from 1 to 1035 d. The percutaneous approach included drainage of abscesses or duodenostomy(32 cases) and percutaneous biliary diversion(13 cases). The median healing time in this group was 43 d. CONCLUSION: Conservative approach is the treatment of choice, eventually associated with percutaneus drainage. 3Multi-detector CT features of acute intestinal ischemia and their prognostic correlations显示文摘Acute intestinal ischemia is an abdominal emergency occurring in nearly 1% of patients presenting with acute abdomen. The causes can be occlusive or non occlusive. Early diagnosis is important to improve survival rates. In most cases of late or missed diagnosis, the mortality rate from intestinal infarction is very high, with a reported value ranging from 60% to 90%. Multidetector computed tomography(MDCT) is a fundamental imaging technique that must be promptly performed in all patients with suspected bowel ischemia. Thanks to the new dedicated reconstruction program, its diagnostic potential is much improved compared to the past and currently it is superior to that of any other noninvasive technique. The increased spatial and temporal resolution, high-quality multi-planar reconstructions, maximum intensity projections, vessel probe, surface-shaded volume rending and tissue transition projections make MDCT the gold standard for the diagnosis of intestinal ischemia, with reported sensitivity, specificity, positive and negative predictive values of 64%-93%, 92%-100%, 90%-100% and 94%-98%, respectively. MDCT contributes to appropriate treatment planning and provides important prognostic informationthanks to its ability to define the nature and extent of the disease. Comparing with the complete nucleotide sequences of the mitochondrial genome of other crocodiles, no variation on gene arrangement or quantity was found in the mtDNA of Crocodylus niloticus, however, the control region varied greatly. In addition to the analyses of complete sequences, partial sequences of 12S rRNA, 16S rRNA and Cyt b gene from other 13 crocodile species were combined and analyzed. 7Helicobacter pylori infection in bleeding peptic ulcer patients after non-steroidal antiinflammatory drug consumption显示文摘AIM:To establish the prevalence of Helicobacter pylori (H. pylori ) infection in patients with a bleeding peptic ulcer after consumption of non-steroidal antiinflamma- tory drugs (NSAIDs). METHODS:A very early upper endoscopy was performed to find the source of upper gastrointestinal bleeding and to take biopsy specimens for analysis of H. pylori infection by the rapid urease (CLO) test, histological examination, and bacterial culture. IgG anti-CagA were also sought. The gold standard for identifying H. pylori infection was positive culture of biopsy specimens or contemporary positivity of the CLO test and the presence of H. pylori on tissue sections. RESULTS:Eighty patients, 61 males (76.3%), mean age 61.2 ± 15.9 years, were consecutively enrolled. Forty-seven (58.8%) patients occasionally consumed NSAIDs, while 33 (41.3%) were on chronic treatment with low-dose aspirin (LD ASA). Forty-four (55.0%) patients were considered infected by H. pylori . The infection rate was not different between patients who occasionally or chronically consumed NSAIDs. The culture of biopsy specimens had a sensitivity of 86.4% and a specificity of 100%; corresponding figures for histological analysis were 65.9% and 77.8%, for the CLO test were 68.2% and 75%, for the combined use of histology and the CLO test were 56.8% and 100%, and for IgG anti-CagA were 90% and 98%. The highest accuracy (92.5%) was obtained with the culture of biopsy specimens.CONCLUSION:Patients with a bleeding peptic ulcer after NSAID/LD ASA consumption frequently have H. pylori infection. 8Pancreatic trauma: The role of computed tomography for guiding therapeutic approach显示文摘AIM: To evaluate the role of computed tomography(CT) for diagnosing traumatic injuries of the pancreas and guiding the therapeutic approach.METHODS: CT exams of 6740 patients admitted to our Emergency Department between May 2005 and January 2013 for abdominal trauma were retrospectively evaluated. Patients were identified through a search of our electronic archive system by using such terms as 'pancreatic injury', 'pancreatic contusion', 'pancreatic laceration', 'peri-pancreatic fluid', 'pancreatic active bleeding'. All CT examinations were performed before and after the intravenous injection of contrast material using a 16-slice multidetector row computed tomography scanner. The data sets were retrospectively analyzed by two radiologists in consensus searching for specific signs of pancreatic injury(parenchymal fracture and laceration, focal or diffuse pancreatic enlargement/edema, pancreatic hematoma, active bleeding, fluid between splenic vein and pancreas) and non-specific signs(inflammatory changes in peri-pancreatic fat and mesentery, fluid surrounding the superior mesentericartery, thickening of the left anterior renal fascia, pancreatic ductal dilatation, acute pseudocyst formation/peri-pancreatic fluid collection, fluid in the anterior and posterior pararenal spaces, fluid in transverse mesocolon and lesser sac, hemorrhage into peri-pancreatic fat, mesocolon and mesentery, extraperitoneal fluid, intraperitoneal fluid).RESULTS: One hundred and thirty-six/Six thousand seven hundred and forty(2%) patients showed CT signs of pancreatic trauma. Eight/one hundred and thirty-six(6%) patients underwent surgical treatment and the pancreatic injures were confirmed in all cases. Only in 6/8 patients treated with surgical approach, pancreatic duct damage was suggested in the radiological reports and surgically confirmed in all cases. In 128/136(94%) patients who underwent non-operative treatment CT images showed pancreatic edema in 97 patients, hematoma in 31 patients, fluid between splenic vein and pancreas in 113 patients. Non-specific CT signs of pancreatic injuries were represented by peri-pancreatic fat stranding and mesentery fluid in 89% of cases, thickening of the left anterior renal fascia in 65%, pancreatic ductal dilatation in 18%, acute pseudocyst/peri-pancreatic fluid collection in 57%, fluid in the pararenal spaces in 45%, fluid in transverse mesocolon and lesser sac in 29%, hemorrhage into peri-pancreatic fat, mesocolon and mesentery in 66%, extraperitoneal fluid in 66%, intra-peritoneal fluid in 41% cases.CONCLUSION: CT represents an accurate tool for diagnosing pancreatic trauma, provides useful information to plan therapeutic approach with a detection rate of 75% for recognizing ductal lesions.Marco Moschetta Michele Telegrafo Valeria Malagnino Laura Mappa Amato A Stabile Ianora Dario Dabbicco Antonio Margari Giuseppe Angelelli 2015World Journal of Radiology2015,7,11:4

12Variable retention harvesting in Great Lakes mixed-pine forests: emulating a natural model in managed ecosystems显示文摘Variable retention harvesting(VRH)systems have gained wide use in many different forest types across the globe,but largely have been implemented in forests characterized by severe,infrequent disturbance regimes.There has been less attention given to developing VRH approaches in forests that are characterized as having a mixed-severity disturbance regime that often results in only partial mortality of canopy trees in spatially heterogeneous patterns.One example of such a forest type is red pine(Pinus resinosa Ait.)-dominated ecosystem of the western Great Lakes region of North America.The purpose of this review is to provide a conceptual foundation for developing VRH approaches in red pine ecosystems that are based on a mixed-severity disturbance regime.Our contention is that red pine forests managed following a natural model are more resilient to disturbances and external threats such as climate change.For the red pine ecosystem,VRH application should reflect the often severe,but partial canopy removal from natural disturbance that is characteristic of this ecosystem and that results in more than trivial numbers of surviving overstory trees across a range of spatial configurations in regenerating stands.Retained live trees should span a range of diameters,but favor the larger end of the diameter distribution,as this reflects the likely pattern of survival after natural disturbance and is often a key structural element lacking from managed areas.VRH should be applied in ways that vary the spatial pattern of legacy trees in and among stands,but largely in ways that reflect the pattern of spatially patchy canopy structure,with large openings surrounded by a less disturbed matrix,as occurs with a natural disturbance regime.Legacy trees and deadwood structures should reflect the composition of the pre-disturbance forest,including species in addition to dominant red pine.Finally,retained structures should be viewed as dynamic entities that grow,die,and decay and that need to be documented and accounted for over time.While more organizations are incorporating some form of VRH into policy and practice for red pine-dominated ecosystems,this application is not always based on a comprehensive understanding of the actual natural model of development,which reflects a mixed-severity disturbance regime.Our goal is to review the ecological evidence for this disturbance regime and interpret the structural and compositional outcomes of the disturbance model,so as to advance VRH approaches that better emulate the actual disturbance and development model for this regionally important ecosystem.Brian J.Palik Anthony W.D’Amato 2019Ecological Processes2019,8,1:3

14Spectrum of Cytogenomic Abnormalities Revealed by Array Comparative Genomic Hybridization on Products of Conception Culture Failure and Normal Karyotype Samples显示文摘Approximately 30% of pregnancies after implantation end up in spontaneous abortions, and 50% of them are caused by chromosomal abnormalities. However, the spectrum of genomic copy number variants(CNVs) in products of conception(POC) and the underlying genedosage-sensitive mechanisms causing spontaneous abortions remain largely unknown. In this study, array comparative genomic hybridization(a CGH) analysis was performed as a salvage procedure for 128 POC culture failure(POC-CF) samples and as a supplemental procedure for106 POC normal karyotype(POC-NK) samples. Chromosomal abnormalities were detected in 10% of POC-CF and pathogenic CNVs were detected in 3.9% of POC-CF and 5.7% of POC-NK samples. Compiled results from this study and relevant case series through a literature review demonstrated an abnormality detection rate(ADR) of 35% for chromosomal abnormalities in POC-CF samples, 3.7% for pathogenic CNVs in POC-CF samples, and 4.6% for pathogenic CNVs in POC-NK samples. Ingenuity Pathway Analysis(IPA) was performed on the genes from pathogenic CNVs found in POC samples. The denoted primary gene networks suggested that apoptosis and cell proliferation pathways are involved in miscarriage. In summary, a similar spectrum of cytogenomic abnormalities was observed in POC culture success and POC-CF samples. A threshold effect correlating the number of dosage-sensitive genes in a chromosome with the observed frequency of autosomal trisomy is proposed.
