| 1 | Prescribing physical activity to prevent and manage gestational diabetes显示文摘Gestational diabetes mellitus(GDM)is the most prevalent metabolic disorder during pregnancy.Women diagnosed with GDM have a substantially greater risk of developing type 2 diabetes within 5-10 years after delivery,and the risk is increased by excess body weight.Uncontrolled hyperglycemia during pregnancy is potentially harmful to both mother and fetus,resulting in a greater need for Caesarian-section deliveries,delivery of larger infants with more excess body fat,a greater risk of infant death and stillbirth,and an elevated risk of infant hypoglycemia immediately after birth.Fortunately,engaging in physical activity prior to and during pregnancy may lower the risk of developing GDM.Pregnant women should also be advised how to safely increase their physical activity during pregnancy and the postpartum period.An initial approach to becoming more physically active can simply be to encourage women to incorporate more unstructured physical activity into daily living,both before and during pregnancy.Giving women an appropriate exercise prescription can encourage them to participate in physical activity safely and effectively throughout pregnancy to prevent and/or manage GDM.Engaging in 30 min of moderate intensity physical activity on most,if not all,days of the week has been adopted as a recommendation for all pregnant women. | Sheri R Colberg Kristin Castorino Lois Jovanovi | 2013 | World Journal of Diabetes2013,4,6: | 12 |

| 5 | Practical strategies for increasing efficiency and effectiveness in critical care education显示文摘Technological advances and evolving demands inmedical care have led to challenges in ensuring adequate training for providers of critical care. Reliance on the traditional experience-based training model alone is insufficient for ensuring quality and safety in patient care. This article provides a brief overview of the existing educational practice within the critical care environment. Challenges to education within common daily activities of critical care practice are reviewed. Some practical evidence-based educational approaches are then described which can be incorporated into the daily practice of critical care without disrupting workflow or compromising the quality of patient care. | 6 | Enhancement in antinociceptive and anti-inflammatory effects of tramadol by transdermal proniosome gel显示文摘Oral therapy of tramadol,an opiate analgesic,undergoes extensive hepatic metabolism and requires frequent administration.Transdermal therapy by virtue can overcome these issues and can improve the efficacy and reduce abuse liability of tramadol.The aim of this research was to investigate the possibility of transdermal delivery of tramadol by formulating proniosome gel and evaluate its therapeutic potential in vivo.The effect of formulation composition as well as amount of drug on physicochemical characteristics of prepared proniosomes were examined.Best proniosome gel(F4)was selected and evaluated for drug release,stability and transdermal efficacy by ex vivo and in vivo experiments.The vesicles demonstrated optimal properties including spherical shape,nanosize with good entrapment efficiency,adequate zeta potential,higher stability and greater transdermal flux.The amorphization and dispersion of tramadol in the aqueous core of proniosome vesicles was confirmed by differential scanning calorimeter.Release profile of F4 was distinct(P<0.001)from control and displayed steady and prolonged tramadol release by Fickian diffusion.Transdermal therapy of F4 showed prominent reduction of induced twitches(P<0.005)in mice and edema(P<0.05)in rats,as compared to oral tramadol.The improvement in clinical efficacy of tramadol in transdermal therapy is correlated with the pharmacokinetic data observed.In conclusion,the observed improvement in antinociceptive and anti-inflammatory effects from proniosome carriers signifies its potential to be a suitable alternative to oral therapy of tramadol with greater efficacy. | Jigar Shah Anroop B.Nair Hiral Shah Shery Jacob Tamer M.Shehata Mohamed Aly Morsy | 2020 | Asian Journal of Pharmaceutical Sciences2020,15,6: | 3 |
| 7 | 酶消化法与机械法处理抽吸脂肪获得的基质血管细胞群组成成分对比显示文摘背景脂肪基质血管细胞群(stromal vascular fraction cells,SVFCs)是再生医学中脂肪来源间充质干细胞(adipose-derived mesenchymal stem cells,ADMSCs)的重要来源。从脂肪组织中获取SVFCs的方法有很多,包括机械法和酶消化法。获得的SVFCs的细胞组成可对其能力产生影响。因此,我们比较了采用不同方法从抽吸脂肪中获得的SVFCs的细胞组成和数量,包括离心、涡旋和胶原酶消化。方法采用真空负压抽吸法,从9例成年人皮下脂肪组织获取脂肪,年龄21~55岁,平均BMI为28。分别采用高速离心,离心后涡旋分离3min和酶消化法分离SVFCs。红细胞裂解后,对SVFCs进行细胞计数、增殖能力检测和流式细胞术检测。计量ADMSCs(CIM5-CD73+CD90+)、内皮细胞(CD45-CD31+)和单核细胞/巨噬细胞的相对百分比。结果酶消化法获得的SVFCs数量最高(2.3×10^5/ml抽吸脂肪),高速离心法获得的SVFCs数量为其1/10,涡旋分离法获得的SVFCs数量为其1/20。3种方法获得的细胞活性相似(80%~90%)。3种方法获得的SVFCs成分有很大差异。酶消化法获得的SVFCs中,造血源性细胞的比例(32%)少于机械分离法获得的SVFCs中造血源性细胞的比例(70%~85%);酶消化法获得的ADMSCs数量(60%)多于机械分离法获得的ADMSCs数量(6%~13%)。另外,与机械分离法相比,酶消化法获得的SVFCs中,炎性单核细胞/巨噬细胞的数量较少,而内皮细胞数量较多。结论虽然3种分离方法均能获得再生细胞,但机械分离法获得的造血细胞和炎性细胞的数量较高,而ADMSCs和内皮细胞的数量较低,这可能会降低其在再生医学中应用的潜能。 | Alexandra Conde-Green Ricardo L.
