| 1 | Mechanisms of drug resistance in colon cancer and its therapeutic strategies显示文摘Drug resistance develops in nearly all patients with colon cancer, leading to a decrease in the therapeutic efficacies of anticancer agents. This review provides an up-to-date summary on over-expression of ATPbinding cassette(ABC) transporters and evasion of apoptosis, two representatives of transport-based and non-transport-based mechanisms of drug resistance, as well as their therapeutic strategies. Different ABC transporters were found to be up-regulated in colon cancer, which can facilitate the efflux of anticancer drugs out of cancer cells and decrease their therapeutic effects. Inhibition of ABC transporters by suppressing their protein expressions or co-administration of modulators has been proven as an effective approach to sensitize drug-resistant cancer cells to anticancer drugs in vitro. On the other hand, evasion of apoptosis observed in drug-resistant cancers also results in drug resistance to anticancer agents, especially to apoptosis inducers. Restoration of apoptotic signals by BH3 mimetics or epidermal growth factor receptor inhibitors and inhibition of cancer cell growth by alternative cell death pathways, such as autophagy, are effective means to treat such resistant cancer types. | 2 | Repurposing vitamin D for treatment of human malignancies via targeting tumor microenvironment显示文摘Tumor cells along with a small proportion of cancer stem cells exist in a stromal microenvironment consisting of vasculature, cancer-associated fibroblasts, immune cells and extracellular components.Recent epidemiological and clinical studies strongly support that vitamin D supplementation is associated with reduced cancer risk and favorable prognosis. Experimental results suggest that vitamin D not only suppresses cancer cells, but also regulates tumor microenvironment to facilitate tumor repression. In this review, we have outlined the current knowledge on epidemiological studies and clinical trials of vitamin D. Notably, wesummarized and discussed the anticancer action of vitamin D in cancer cells, cancer stem cells and stroma cells in tumor microenvironment, providing a better understanding of the role of vitamin D in cancer. | 5 | Transfusion-transmitted hepatitis E:What we know so far?显示文摘Hepatitis E virus(HEV)is a major cause of viral hepatitis globally.There is growing concern about transfusion-transmitted HEV(TT-HEV)as an emerging global health problem.HEV can potentially result in chronic infection in immunocompromised patients,leading to a higher risk of liver cirrhosis and even death.Between 0.0013%and 0.281%of asymptomatic blood donors around the world have HEV viremia,and 0.27%to 60.5%have anti-HEV immunoglobulin G.HEV is infectious even at very low blood concentrations of the virus.Immunosuppressed patients who develop persistent hepatitis E infection should have their immunosuppressant regimen reduced;ribavirin may be considered as treatment.Pegylated interferon can be considered in those who are refractory or intolerant to ribavirin.Sofosbuvir,a nucleotide analog,showed modest antiviral activity in some clinical studies but sustained viral response was not achieved.Therefore,rescue treatment remains an unmet need.The need for HEV screening of all blood donations remains controversial.Universal screening has been adopted in some countries after consideration of risk and resource availability.Various pathogen reduction methods have also been proposed to reduce the risk of TT-HEV.Future studies are needed to define the incidence of transmission through transfusion,their clinical features,outcomes and prognosis. | Carmen Ka Man Cheung Sunny Hei Wong Alvin Wing Hin Law Man Fai Law | 2022 | World Journal of Gastroenterology2022,28,1: | 5 |
| 6 | Hepatoid carcinoma of the pancreas:A case report and review of the literature显示文摘BACKGROUND Hepatoid carcinoma(HC)is an extremely rare neoplasm that is morphologicallysimilar to hepatocellular carcinoma.HC has been described in various organs;however,HC of the pancreas is extremely rare.To our knowledge,only 38 caseshave been reported.We present a case of HC of the pancreas in a 36-year-oldmale patient.CASE SUMMARY A 36-year-old cachexic man with no significant past medical history wastransferred to our hospital with a history of painless jaundice,elevated bloodglucose and significant weight loss.Lab tests showed elevated serumtransaminases,bilirubin and alpha-fetoprotein levels.Magnetic resonanceimaging of the upper abdomen showed a diffusely enlarged pancreas,appearing“sausage-shaped”.Magnetic resonance cholangiopancreatography showedupstream ductal dilation secondary to stricture of the main pancreatic duct andthe common bile duct,which were not visible.Immunohistochemistry ofbiopsied tissue from a percutaneous pancreatic biopsy showed tumor cellpositivity for HepPar1,polyclonal carcinoembryonic antigen and CK19,suggestive of HC of the pancreas.The characteristics of 39 patients with HC ofthe pancreas were reviewed.CONCLUSION HC of the pancreas is more prevalent in males,and patients have a median age of57 years.It is most commonly asymptomatic or presents as abdominal back pain,and the pancreatic tail is the most common location.At the time of diagnosis,liver metastasis is often present. | Shao-Xiong Zeng Si-Wei Tan Christ-Jonathan Tsia Hin Fong Qiong Liang Bin-Liang Zhao Ke Liu Jia-Xiang Guo Jin Tao | 2020 | World Journal of Clinical Cases2020,8,6: | 4 |

| 8 | Mapping bone marrow niches of disseminated tumor cells显示文摘Breast cancer cells may disseminate early, before tumor diagnosis. Disseminated tumor cells, or DTCs, reside in the bone marrow, and may persist for years or even decades. Some of these cells may be re-activated to resume aggressive growth, and eventually become overt bone metastases. Recent studies have begun to shed light on this complicated process and revealed multiple steps and intermediate states of colonizing DTCs. However, how cancer-host interactions evolve during this process needs to be further understood. Most of our current knowledge of the bone microenvironment is obtained through studies looking for the hematopoietic stem cell(HSC) niche. Although this long-standing question has not yet been resolved, our search for the HSC niche has resulted in a detailed map of various cell types in the bone marrow. Furthermore, various techniques used to find the HSC niche may also be adapted for finding the cancer cell niche. In this article, we will review the recent progress in both the DTC and HSC areas with a focus on their potential microenvironment niches. We will also discuss how to apply what we have learned from HSC studies to map DTCs in the bone context.
