| 1 | Design of a new dynamical core for global atmospheric models based on some efficient numerical methods显示文摘A careful study on the integral properties of the primitive hydrostatic balance equations for baroclinic atmosphere is carried out, and a new scheme todesign the global adiabatic model of atmospheric dynamics ispresented. This scheme includes a method of weighted equal-areamesh and a fully discrete finite difference method with quadraticand linear conservations for solving the primitive equationsystem. Using this scheme, we established a new dynamical corewith adjustable high resolution acceptable to the availablecomputer capability, which can be very stable without anyfiltering and smoothing. Especially, some important integralproperties are kept unchanged, such as the anti-symmetries of thehorizontal advection operators and the vertical convectionoperator, the mass conservation, the effective energy conservationunder the standard stratification approximation, and so on. | 7 | A complete sequence and comparative analysis of a SARS-associated virus(Isolate BJ01)显示文摘The genome sequence of the Severe Acute Respiratory Syndrome (SARS)-associated virus provides essential information for the identification of pathogen(s), exploration of etiology and evolution, interpretation of transmission and pathogenesis, development of diagnostics, prevention by future vaccination, and treatment by developing new drugs. We report the complete genome sequence and comparative analysis of an isolate (BJ01) of the coronavirus that has been recognized as a pathogen for SARS. The genome is 29725 nt in size and has 11 ORFs (Open Reading Frames). It is composed of a stable region encoding an RNA-dependent RNA polymerase (composed of 2 ORFs) and a variable region representing 4 CDSs (coding sequences) for viral structural genes (the S, E, M, N proteins) and 5 PUPs (putative uncharacterized proteins). Its gene order is identical to that of other known coronaviruses. The sequence alignment with all known RNA viruses places this virus as a member in the family of Coronaviridae. Thirty putative substitutions have been identified by comparative analysis of the 5 SARS- associated virus genome sequences in GenBank. Fifteen of them lead to possible amino acid changes (non-synonymous mutations) in the proteins. Three amino acid changes, with predicted alteration of physical and chemical features, have been detected in the S protein that is postulated to beinvolved in the immunoreactions between the virus and its host. Two amino acid changes have been detected in the Mprotein, which could be related to viral envelope formation. Phylogenetic analysis suggests the possibility of non-human origin of the SARS-associated viruses but provides noevidence that they are man-made. | 11 | Study of liver cirrhosis over ten consecutive years in Southern China显示文摘AIM: To investigate the etiology and complications of liver cirrhosis(LC) in Southern China. METHODS: In this retrospective, cross-sectional study, we identified cases of liver cirrhosis admitted between January 2001 to December 2010 and reviewed the medical records. Patient demographics, etiologies and complications were collected, and etiological changes were illustrated by consecutive years and within twotime periods(2001-2005 and 2006-2010). All results were expressed as the mean ± SD or as a percentage. The χ2 test or Student's t-test was used to analyze the differences in age, gender, and etiological distribution, and one-way analysis of variance was applied to estimate the trends in etiological changes. We analyzed the relationship between the etiologies and complications using unconditioned logistic regression, and the risk of upper gastrointestinal bleeding(UGIB) and hepatocellular carcinoma(HCC) in the major etiological groups was evaluated as ORs. A P value less than 0.05 was considered significant. Statistical computation was performed using SPSS 17.0 software.RESULTS: In this study, we identified 6719(83.16%) male patients and 1361(16.84%) female patients. The average age of all of the patients was 50.5 years at the time of diagnosis. The distribution of etiological agents was as follows: viral hepatitis, 80.62% [hepatitis B virus(HBV) 77.22%, hepatitis C virus(HCV) 2.80%,(HBV + HCV) 0.58%]; alcohol, 5.68%; mixed etiology, 4.95%; cryptogenic, 2.93%; and autoimmune hepatitis, 2.03%; whereas the other included etiologies accounted for less than 4% of the total. Infantile hepatitis syndrome LC patients were the youngest(2.5 years of age), followed by the metabolic LC group(27.2 years of age). Viral hepatitis, alcohol, and mixed etiology were more prevalent in the male group, whereas autoimmune diseases, cryptogenic cirrhosis, and metabolic diseases were more prevalent in the female group. When comparing the etiological distribution in 2001-2005 with that in 2006-2010, the proportion of viral hepatitis decreased from 84.7% to 78.3%(P < 0.001), and the proportion of HBV-induced LC also decreased from 81.9% to 74.6%(P < 0.001). The incidence of mixed etiology, cryptogenic cirrhosis, and autoimmune diseases increased by 3.1%(P < 0.001), 0.5%(P = 0.158), and 1.3%(P < 0.001), respectively. Alcohol-induced LC remained relatively steady over the 10-year period. The ORs of the development of UGIB between HBV and other major etiologies were as follows: HCV, 1.07; alcohol, 1.89; autoimmune, 0.90; mixed etiology, 0.83; and cryptogenic, 1.76. The ORs of the occurrence of HCC between HBV and other major etiologies were as follows: HCV, 0.54; alcohol, 0.16; autoimmune, 0.05; mixed etiology, 0.58; and cryptogenic, 0.60.CONCLUSION: The major etiology of liver cirrhosis in Southern China is viral hepatitis. However, the proportions of viral hepatitis and HBV are gradually decreasing. | 13 | Risk factors for intrahepatic cholangiocarcinoma:A case-control study in China显示文摘AIM: To carry out a hospital-based case-control study to investigate risk factors for intrahepatic cholangiocarcinoma (ICC) in China. METHODS: A total of 312 ICC cases and 438 matched controls were included in the study. The presence of diabetes mellitus,hypertention,hepatolithiasis,primary sclerosing cholangitis,liver fluke infection (Clonorchis sinensis ),was investigated through clinical records. Blood from all participants was tested for hepatitis B surface antigen (HBsAg) and anti-HCV antibodies. Odds ratios (OR) and 95% confi dence intervals (95% CI) were estimated using conditional logistic regression. RESULTS: Compared with controls,ICC patients had a higher prevalence of HBsAg seropositivity (48.4% vs 9.6%,P < 0.000),and hepatolithiasis (5.4% vs 1.1%,P = 0.001). By multivariate analysis,the signif icant risk factors for development of ICC were HBsAg seropositivity (adjusted OR,8.876,95% CI,5.973-13.192),and hepatolithiasis (adjusted OR,5.765,95% CI,1.972-16.851). The prevalence of anti-HCV seropositivity,diabetes mellitus,hypertention,cigarette smoking,and alcohol consumption were not significantly different between cases and controls. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). | 20 | Trend of declining stroke mortality in China:reasons and analysis显示文摘Introduction There is a downward trend of stroke-related mortality in the USA.By reviewing all published articles on stroke mortality in China,we analysed its trend and possible factors that have influenced the trend.Methods Both English and Chinese literatures were searched on the mortality of stroke or cerebrovascular diseases in China.Potential papers related to this topic were identified from PubMed,Medline,Embase,Cochrane Library,Wanfang Database,SINOMED and China National Knowledge Infrastructure databases.Results Comparing the results from the most recent population-based epidemiological survey and databank from the national Center for Disease Control and Prevention,the age-adjusted stroke mortality rate has shown a downward trend among both urban and rural population in the past 30 years in China.Comparing with 30 years ago,the rate of stroke mortality has decreased by more than 31%in urban/suburban population and 11%in rural population.In men,the age-adjusted stroke mortality rate decreased by 18.9%and in women by 24.9%between 1994 and 2013.Factors that may have contributed to the trend of decreased stroke mortality rate include(1)improved healthcare coverage and healthcare environment;(2)improved treatment options and medical technology;(3)support by government to educate the public on stroke and stroke prevention;and(4)improved public knowledge on stroke.Conclusions The age-adjusted stroke mortality rate in China has shown a downward trend among both urban and rural population in the past 30 years.The major influencing factors that helped in reducing stroke mortality in China included improved healthcare coverage,healthcare environment,the updated treatment options and modern medical technology. | Wenzhi Wang David Wang Hongmei Liu Haixin Sun Bin Jiang Xiaojuan Ru Dongling Sun Zhenghong Chen Yongjun Wang | 2017 | Stroke & Vascular Neurology2017,2,3: | 52 |
