| 1 | A Modified Plate Assay for Rapid Screening of Potassium-Solubilizing Bacteria显示文摘The utility of microorganisms for solubilizing the unavailable forms of potassium(K) from soil has led to renewed interest in fabrication of rapid and sensitive plate assays for their isolation and screening. The present study developed a modified plate assay and compared it with previously reported methods for the isolation and screening of K-solubilizing bacteria. The newly developed plate assay is based on improved visualization of halo zone formation around the colonies on agar plates, through inclusion of an acid-base indicator dye, bromothymol blue(BTB), to modify the previously reported Aleksandrov medium. The halo zone exhibited a significant correlation(R = 0.939) with K released in liquid medium. The visualization of potential K solubilizers was improved using this method, which would help in detection of weak/non-acid producers based on secretion of organic acids in the medium. Organic acids in plate diffuse radially and form halo zones in response to reaction with the acid-base indicator dye BTB. Furthermore, K solubilization on plates with this method can be observed within 48–72 h, against the incubation time of 4–5 d needed in the earlier method. | 2 | Helicobacter pylori eradication in patients with chronic immune thrombocytopenic purpura显示文摘AIM:To assess the effect of Helicobacter pylori(H.pylori)eradication on platelet counts in patients with chronic immune thrombocytopenic purpura(cITP).METHODS:A total of 36 cITP patients were included in the study.The diagnosis of H.pylori was done by rapid urease test and Giemsa staining of the gastric biopsy specimen.All H.pylori positive patients received standard triple therapy for 14 d and were subjected for repeat endoscopy at 6 wk.Patients who continued to be positive for H.pylori on second endoscopyreceived second line salvage therapy.All the patients were assessed for platelet response at 6 wk,3rd and 6th months.RESULTS:Of the 36 patients,17 were positive for H.pylori infection and eradication was achieved in16 patients.The mean baseline platelet count in the eradicated patients was 88615.38±30117.93/mm3and platelet count after eradication at 6 wk,3 mo and6 mo was 143230.77±52437.51/mm3(P=0.003),152562.50±52892.3/mm3(P=0.0001),150187.50±41796.68/mm3(P=0.0001)respectively and in the negative patients,the mean baseline count was71000.00±33216.46/mm3 and at 6 wk,3rd and 6th month follow up was 137631.58±74364.13/mm3(P=0.001),125578.95±71472.1/mm3(P=0.005),77210.53±56892.28/mm3(P=0.684)respectively.CONCLUSION:Eradication of H.pylori leads to increase in platelet counts in patients with cITP and can be recommended as a complementary treatment with conventional therapy. | Ravinder Naik Noonavath Chandrasekharan Padma Lakshmi Tarun Kumar Dutta Vikram Kate | 2014 | World Journal of Gastroenterology2014,20,22: | 6 |
| 3 | Predictors of fifty days in-hospital mortality in decompensated cirrhosis patients with spontaneous bacterial peritonitis显示文摘AIM: To determine the predictors of 50 d in-hospital mortality in decompensated cirrhosis patients with spontaneous bacterial peritonitis(SBP).METHODS: Two hundred and eighteen patients admitted to an intensive care unit in a tertiary care hospital between June 2013 and June 2014 with the diagnosis of SBP(during hospitalization) and cirrhosis were retrospectively analysed. SBP was diagnosed by abdominal paracentesis in the presence of polymorphonuclear cell count ≥ 250 cells/mm3 in the peritoneal fluid. Student's t test, multivariate logistic regression, cox proportional hazard ratio(HR), receiver operating characteristics(ROC) curves and Kaplan-Meier survival analysis were utilized for statistical analysis. Predictive abilities of several variables identified by multivariate analysis were compared using the area under ROC curve. P < 0.05 were considered statistical significant. RESULTS: The 50 d in-hospital mortality rate attributable to SBP is 43.11%(n = 94). Median survival duration for those who died was 9 d. In univariate analysis acute kidney injury(AKI), hepatic encephalopathy, septic shock, serum bilirubin, international normalized ratio, aspartate transaminase, and model for end-stage liver disease- sodium(MELD-Na) were significantly associated with in- hospital mortality in patients with SBP(P ≤ 0.001). Multivariate coxproportional regression analysis showed AKI(HR = 2.16, 95%CI: 1.36-3.42, P = 0.001) septic shock(HR = 1.73, 95%CI: 1.05-2.83, P = 0.029) MELD-Na(HR = 1.06, 95%CI: 1.02-1.09, P ≤ 0.001) was significantly associated with 50 d in-hospital mortality. | 4 | Risk factors for the development of colorectal carcinoma:A case control study from South India显示文摘AIM:To study the association of colorectal carcinoma(CRC) with diet,smoking,alcohol,physical activity,body mass index,family history and diabetes.METHODS: All consecutive patients with CRC confirmed by histopathology diagnosis were included. Age(±5 years) and gender matched controls were selected among the patients admitted in surgery ward for various conditions without any co-existing malignancy. Food frequency questionnaire(FFQ) was developed and validated after pretesting by investigator trained in data col ection techniques. Cases and controls were interviewed ensuring privacy,in similar interview setting,with same duration of time for both cases and controls without any leading question. Biological variables like family history of CRC in first degree relatives,history of diabetes mellitus; behavioral factors like tobacco use both smoking and smokeless form,alcohol consumption and physical activity were recorded. Dietary details were recorded using a FFQ consisting 29 food items with seven categories. Analysis was done using appropriate statistical methods.RESULTS: Ninety-four histopathologically confirmed cases of CRC and equal number of age and gender matched controls treated over a period of two years were studied. Age distribution,mean age,male to female ratio,education level and socioeconomic status were similar in cases and controls. Intake of food items was categorized into tertile due to skewed distribution of subjects as per recommended cut off for consumption of food item. On univariate analysis red meat [OR=7.4(2.935-18.732)],egg [OR=5.1(2.26-11.36)],fish,fried food and oil consumption were found to be risk factors for CRC. On multivariate analysis red meat consumption of more than 2-3 times a month(OR=5.4; 95%CI:1.55-19.05) and egg consumption of more than 2-3 times a week(OR = 3.67; 95%CI:1.23-9.35) were found to be independent risk factors for the development of CRC.CONCLUSION: Egg and red meat consumption found to be independent risk factors for CRC. | 6 | Evaluation of potential landslide damming: Case study of Urni landslide,Kinnaur, Satluj valley, India显示文摘This work aims to understand the process of potential landslide damming using slope failure mechanism,dam dimension and dam stability evaluation. The Urni landslide, situated on the right bank of the Satluj River, Himachal Pradesh(India) is taken as the case study. The Urni landslide has evolved into a complex landslide in the last two decade(2000-2016) and has dammed the Satluj River partially since year 2013,damaging ~200 m stretch of the National Highway(NH-05). The crown of the landslide exists at an altitude of ~2180-2190 m above msl, close to the Urni village that has a human population of about 500.The high resolution imagery shows ~50 m long landslide scarp and ~100 m long transverse cracks in the detached mass that implies potential for further slope failure movement. Further analysis shows that the landslide has attained an areal increase of 103,900 ± 1142 m^2 during year 2004-2016. About 86% of this areal increase occurred since year 2013. Abrupt increase in the annual mean rainfall is also observed since the year 2013. The extreme rainfall in the June, 2013; 11 June(~100 mm) and 16 June(~115 mm),are considered to be responsible for the slope failure in the Urni landslide that has partially dammed the river. The finite element modelling(FEM) based slope stability analysis revealed the shear strain in the order of 0.0-0.16 with 0.0-0.6 m total displacement in the detachment zone. Further, kinematic analysis indicated planar and wedge failure condition in the jointed rockmass. The debris flow runout simulation of the detached mass in the landslide showed a velocity of ~25 m/s with a flow height of ~15 m while it(debris flow) reaches the valley floor. | 7 | Risk factors of thrombosis in abdominal veins显示文摘AIM: To estimate the prevalence of inherited and acquired thrombophilic risk factors in patients with abdominal venous thrombosis and to compare the risk factor profiles between Budd-Chiari syndromes (BCS) and splanchnic vein thrombosis (SVT). METHODS: In this retrospective study, 36 patients with abdominal venous thrombosis were studied. The patients were divided into Budd-Chiari group (hepatic vein, IVC thrombosis) and splanchnic venous thrombosis group (portal, splenic, superior mesenteric veins) based on the veins involved. Hereditary and acquired thrombophilic risk factors were evaluated in all patients. RESULTS: Twenty patients had SVT, 14 had BCS, and 2 had mixed venous thrombosis. Ten patients (28%) had hereditary and 10 patients (28%) acquired thrombophilic risk factors. The acquired risk factors were significantly more common in the SVT group (SVT vs BCS: 45% vs 7%, χ2 = 5.7, P = 0.02) while hereditary risk factors did not show significant differences between the two groups (SVT vs BCS: 25% vs 36%, χ2 = 0.46, P = 0.7). Multiple risk factors were present in one (7%) patient with BCS and in 3 patients (15%) with SVT. No risk factors were identified in 57% of patients with BCS and in 45% of patients with SVT. CONCLUSION: Hereditary and acquired risk factors play an important role in the etiopathogenesis of abdominal venous thrombosis. | 8 | Evaluation of potential surface instability using finite element method in Kharsali Village,Yamuna Valley,Northwest Himalaya显示文摘Kharsali village, located in the Northwest Himalaya near the confluence of the Yamuna River and Unta Gad, is situated on a thick(>150 m) paleolandslide deposit. The village is continuously being eroded at its base by the two rivers. Cracks are noted in most houses while the ancient Shani Temple lying to the south of the village has tilted ~5° towards the northeast. Three slope sections(S-1, S-2, S-3) were modelled and analysed to determine the displacement and shear strain patterns of the slopes. Based on surface failure conditions, potential slope instability of the Kharsali village was evaluated from 2D Finite Element Method(FEM) using Shear Strain Reduction(SSR) analysis in the Phase2 software. Results indicate a critical Stress Reduction Factor(SRF) of 1.5 for the southern edge of the village(S-1) housing the Shani Temple. The development of failure surfaces at its lower portion signifies the propagating, progressive nature of the slope. The S-2 slope section is most vulnerable to slope failure, with a critical SRF of 1.08. This has been inferred by the formation of failure surfaces with displacements of 0.05-0.08 m. The S-3 section in the northern part of the Kharsali shows highest critical SRF of 2.76. The un-metalled road section in the north of the village near S-3 hasdeveloped a failure surface with displacement of 0.003-0.004 m, and a zone of subsidence.
