2Diffusion weighted imaging: Technique and applications显示文摘Diffusion weighted imaging(DWI) is a method of signal contrast generation based on the differences in Brownian motion. DWI is a method to evaluate the molecular function and micro-architecture of the human body. DWI signal contrast can be quantified by apparent diffusion coefficient maps and it acts as a tool for treatment respon-se evaluation and assessment of disease progression. Ability to detect and quantify the anisotropy of diffusion leads to a new paradigm called diffusion tensor imaging(DTI). DTI is a tool for assessment of the organs with highly organised fibre structure. DWI forms an integral part of modern state-of-art magnetic resonance imaging and is indispensable in neuroimaging and oncology. DWI is a field that has been undergoing rapid technical evolution and its applications are increasing every day. 3Potential antibacterial activity of berberine against multi drug resistant enterovirulent Escherichia coli isolated from yaks(Poephagus grunniens) with haemorrhagic diarrhoea显示文摘Objective:To evaluate the antimicrobial efficacy of berberine,a plant alkaloid.Methods:Five multi-drug resistant(MDR) STEC/EPEC and five MDR ETEC isolates from yaks with haemorrhagic diarrhoea were selected for the study.Antibacterial activity of berberine was evaluated by broth dilution and disc diffusion methods.The binding kinetics of berberine to DNA and protein was also enumerated.Results:For both categories of enterovirulent Escherichia coli(E.roli) isolates, berberine displayed the antibaclerial effect in a dose dependent manner.The MIC50 of berberine chloride for STEC/EPEC isolates varied from 2.07μM to 3.6μM with a mean of(2.95±0.33)μM where as for ETEC strains it varied from 1.75 to 1.96μM with a mean of(1.87±0.03)μM. 4Multi-objective optimization of oil well drilling using elitist non-dominated sorting genetic algorithm显示文摘A multi-objective optimization of oil well drilling has been carried out using a binary coded elitist non-dominated sorting genetic algorithm. A Louisiana offshore field with abnormal formation pressure is considered for optimization. Several multi-objective optimization problems involving two-and three-objective functions were formulated and solved to fix optimal drilling variables. The important objectives are:(i) maximizing drilling depth,(ii) minimizing drilling time and(iii) minimizing drilling cost with fractional drill bit tooth wear as a constraint. Important time dependent decision variables are:(i) equivalent circulation mud density,(ii) drill bit rotation,(iii) weight on bit and(iv) Reynolds number function of circulating mud through drill bit nozzles. A set of non-dominated optimal Pareto frontier is obtained for the two-objective optimization problem whereas a non-dominated optimal Pareto surface is obtained for the three-objective optimization problem. Depending on the trade-offs involved, decision makers may select any point from the optimal Pareto frontier or optimal Pareto surface and hence corresponding values of the decision variables that may be selected for optimal drilling operation. 5Perfusion computed tomography in renal cell carcinoma显示文摘Various imaging modalities are available for the diagnosis, staging and response evaluation of patients with renal cell carcinoma(RCC). While contrast enhanced computed tomography(CT) is used as the standard of imaging for size, morphological evaluation and response assessment in RCC, a new functional imaging technique like perfusion CT(p CT), goes down to the molecular level and provides new perspectives in imaging of RCC. p CT depicts regional tumor perfusion and vascular permeability which are indirect parameters of tumor angiogenesis and thereby provides vital information regarding tumor microenvironment. Also response evaluation using p CT may predate the size criteria used in Response Evaluation Criteria in Solid Tumors, as changes in the perfusion occurs earlier following tissue kinase inhibitors before any actual change in size. This may potentially help in predicting prognosis, better selection of therapy and more accurate and better response evaluation in patients with RCC. 6Peripheral primitive neuroectodermal tumor of the kidney presenting with pulmonary tumor embolism: A case report显示文摘Peripheral primitive neuroectodermal tumor(PNET) of the kidney is a rare, aggressive tumor known for its recurrence and metastatic potential. Despite the frequency of venous extension to the renal veins and inferior vena cava, pulmonary tumor embolism at the initial presentation is not common. We report a case of 22-year-old female with PNET of the kidney who presented with tumor embolism in the inferior vena cava(IVC) and bilateral pulmonary artery. The patient underwent surgical resection and histopathological analysis confirmed the presence of tumor within the IVC and pulmonary arteries. 7Efficacy of pancreatoscopy for pancreatic duct stones:A systematic review and meta-analysis显示文摘BACKGROUND Pancreatic duct stones can lead to significant abdominal pain for patients.Per oral pancreatoscopy(POP)-guided intracorporal lithotripsy is being increasingly used for the management of main pancreatic duct calculi(PDC)in chronic pancreatitis.POP uses two techniques:Electrohydraulic lithotripsy(EHL)and laser lithotripsy(LL).Data on the safety and efficacy are limited for this procedure.We performed a systematic review and meta-analysis with a primary aim to calculate the pooled technical and clinical success rates of POP.The secondary aim was to assess pooled rates of technical success,clinical success for the two individual techniques,and adverse event rates.AIM To perform a systematic review and meta-analysis of POP,EHL and LL for management of PDC in chronic pancreatitis.METHODS We conducted a comprehensive search of multiple electronic databases and conference proceedings including PubMed,EMBASE,Cochrane,Google Scholar and Web of Science databases(from 1999 to October 2019)to identify studies with patient age greater than 17 and any gender that reported on outcomes of POP,EHL and LL.The primary outcome assessed involved the pooled technical success and clinical success rate of POP.The secondary outcome included the pooled technical success and clinical success rate for EHL and LL.We also assessed the pooled rate of adverse events for POP,EHL and LL including a subgroup analysis for the rate of adverse event subtypes for POP:Hemorrhage,post-endoscopic retrograde cholangiopancreatography pancreatitis(PEP),perforation,abdominal pain,fever and infections.Technical success was defined as the rate of clearing pancreatic duct stones and clinical success as the improvement in pain.Randomeffects model was used for analysis.Heterogeneity between study-specific estimates was calculated using the Cochran Q statistical test and I2 statistics.Publication bias was ascertained,qualitatively by visual inspection of funnel plot and quantitatively by the Egger test.RESULTS A total of 16 studies including 383 patients met the inclusion criteria.The technical success rate of POP was 76.4%(95%CI:65.9-84.5;I2=64%)and clinical success rate was 76.8%(95%CI:65.2-85.4;I2=66%).The technical success rate of EHL was 70.3%(95%CI:57.8-80.3;I2=36%)and clinical success rate of EHL was 66.5%(95%CI:55.2-76.2;I2=19%).The technical success rate of LL was 89.3%(95%CI:70.5-96.7;I2=70%)and clinical success rate of LL was 88.2%(95%CI:66.4-96.6;I2=77%).The incidence of pooled adverse events for POP was 14.9%(95%CI:9.2-23.2;I2=49%),for EHL was 11.2%(95%CI:5.9-20.3;I2=15%)and for LL was 13.1%(95%CI:6.3-25.4;I2=31%).Subgroup analysis of adverse events showed rates of PEP at 7%(95%CI:3.5-13.6;I2=38%),fever at 3.7%(95%CI:2-6.9;I2=0),abdominal pain at 4.7%(95%CI:2.7-7.8;I2=0),perforation at 4.3%(95%CI:2.1-8.4;I2=0),hemorrhage at 3.4%(95%CI:1.7-6.6;I2=0)and no mortality.There was evidence of publication bias based on funnel plot analysis and Egger’s test.CONCLUSION Our study highlights the high technical and clinical success rates for POP,EHL and LL.POP-guided lithotripsy could be a viable option for management of chronic pancreatitis with PDC.Syed M Saghir Harmeet S Mashiana Babu P Mohan Banreet S Dhindsa Amaninder Dhaliwal Saurabh Chandan Neil Bhogal Ishfaq Bhat Shailender Singh Douglas G Adler 2020World Journal of Gastroenterology2020,26,34:5

9Diffusion weighted imaging in gynecological malignancies-present and future显示文摘The management of gynaecological malignancies has undergone a significant change in recent years with our improved understanding of cancer biogenetics, development of new treatment regimens and enhanced screening. Due to the rapid blooming of newer methods and techniques in gynaecology, surgery and oncology the scope and the role of imaging has also widened. Functional imaging in the form of diffusion weighted imaging(DWI) has been recently found to be very useful in assessing various tumours. Its ability to identify changes in the molecular level has dramatically changed the diagnostic approach of radiologists which was solely based on morphological criteria. It can improve the diagnostic accuracy of conventional magnetic resonance imaging, lend a hand in assessing tumour response to treatment regimens and detect tumour recurrence with better spatial resolution, negative radiation and diagnostic accuracy compared to positron emission tomography scan. The ability to quantify the diffusion has also lead to potential prediction of tumour aggressiveness and grade which directly correlate with the patient prognosis and management. Hence, it has become imperative for a radiologist to understand the concepts of DWI and its present and evolving role. 10Endoscopic ultrasound-guided vascular interventions: Current insights and emerging techniques显示文摘Endoscopic ultrasound(EUS)is one of the significant breakthroughs in the field of advanced endoscopy.In the last two decades,EUS has evolved from a diagnostic tool to a real-time therapeutic modality.The luminal gastrointestinal(GI)tract provides a unique opportunity to access multiple vascular structures,especially in the mediastinum and abdomen,thus permitting a variety of EUS-guided vascular interventions.The addition of the doppler and contrast-enhanced capability to EUS has further helped provide real-time visualization of blood flow in vessels through the GI tract.EUS-guided vascular interventions rely on standard endoscopic accessories and interventional tools such as fine-needle aspiration needles and fine-needle biopsy.EUS allows the visualization of various structures in real-time by differentiating tissue densities and vascularity,thus,avoiding radiation exposure.EUS-guided techniques also allow real-time microscopic examination after target biopsy.Furthermore,many necessary interventions can be done during the same procedure after diagnosis.This article provides an overview of EUS-guided vascular interventions such as variceal,non-variceal bleeding interventions,EUSguided portal vein(PV)access with the formation of an intrahepatic portosystemic shunt,and techniques related to diagnosis of GI malignancies.Furthermore,we discuss current insights and future outlook of therapeutic modalities like PV embolization,PV sampling,angiography,drug administration,and portal pressure measurement.Rupinder Mann Hemant Goyal Abhilash Perisetti Saurabh Chandan Sumant Inamdar Benjamin Tharian 2021World Journal of Gastroenterology2021,27,40:4

13Genetic variation in relation to adaptability of three mangrove species from the Indian Sundarbans assessed with RAPD and ISSR markers显示文摘Rich genetic polymorphism is important for plants to adapt to changes because it enables the plant to make anatomical,physiological and biochemical changes in response to abiotic stress.Geomorphologic characteristics,demographic interference and a cumulative decrease in freshwater influx in the Indian Sundarbans region have proved detrimental to some economically important plants.In this study,genetic polymorphism of three mangrove species,Xylocarpus granatum,Excoecaria agallocha,and Phoenix paludosa,was assessed using RAPD and ISSR molecular markers.X.granatum,already in distress in the Sundarbans,had the least genetic polymorphism,14.56%in the RAPD analysis and 12.92% in the ISSR.Relatively higher genetic polymorphism was recorded for the profusely growing E.agallocha and P.paludosa: 24.66 and26.4% in RAPD; 24.87 and 20.32% in ISSR analysis respectively.A UPGMA dendrogram constructed using the similarity matrix from RAPD,ISSR and combined datashowed that for X.granatum,the least and highest salinity zones clustered together,whereas for E.agallocha and P.paludosa,higher and lower salinity areas clustered in different clades.Nei's genetic diversity,calculated from RAPD and ISSR data,was also in accordance with 0.0637 and 0.0583 for X.granatum,respectively,much lower than0.0794 and 0.0818 for E.agallocha and 0.0799 and 0.0688 for P.paludosa.This opposing degree of polymorphism might be attributed to the profusely growing E.agallocha and P.paludosa and precarious status of X.granatum throughout the Indian Sundarbans.Nirjhar Dasgupta Paramita Nandy Chandan Sengupta Sauren Das 2018Journal of Forestry Research2018,29,2:3
14Sarcopenia in hepatocellular carcinoma:Current knowledge and future directions显示文摘Liver cancer is the second most occurring cancer worldwide and is one of the leading causes of cancer-related deaths.Hepatocellular carcinoma(HCC)is the most common(80%-90%)type among malignant liver cancers.Sarcopenia occurs very early in HCC and can predict and provide an opportunity to improve muscle health before engaging in the treatment options such as loco-regional,systemic,and transplant management.Multiple prognostic stating systems have been developed in HCC,such as Barcelona Clinic Liver Cancer,Child-Pugh score and Albumin-Bilirubin grade.However,the evaluation of patients’performance status is a major limitation of these scoring systems.In this review,we aim to summarize the current knowledge and recent advances about the role of sarcopenia in cirrhosis in general,while focusing specifically on HCC.Additionally,the role of sarcopenia in predicting clinical outcomes and prognostication in HCC patients undergoing loco-regional therapies,liver resection,liver transplantation and systematic therapy has been discussed.A literature review was performed using databases PubMed/MEDLINE,EMBASE,Cochrane,Web of Science,and CINAHL on April 1,2021,to identify published reports on sarcopenia in HCC.Sarcopenia can independently predict HCC-related mortality especially in patients undergoing treatments such as loco-regional,surgical liver transplantation and systemic therapies.Basic research is focused on evaluating a balance of anabolic and catabolic pathways responsible for muscle health.Early clinical studies have shown promising results in methods to improve sarcopenia in HCC which can potentially increase prognosis in these patients.As sarcopenia occurs very early in HCC,it can predict and provide an opportunity to improve muscle health before engaging in the treatment options such as loco-regional,systemic,and transplant management.Further,sarcopenia measurement can obviate the confounding caused by the abdominal ascites in these patients.The use of sarcopenia can add to the existing scoring systems to better prognosticate the HCC.Abhilash Perisetti Hemant Goyal Rachana Yendala Saurabh Chandan Benjamin Tharian Ragesh BabuThandassery 2022World Journal of Gastroenterology2022,28,4:3

19Effects of solubility parameter differences among PEG,PVP and CA on the preparation of ultrafiltration membranes:Impacts of solvents and additives on morphology,permeability and fouling performances显示文摘The effects of two different hydrophilic additives and two solvents on the membrane morphological structure,permeability property and anti-fouling performances of cellulose acetate(CA) ultrafiltration membranes were investigated. During the phase-inversion process, cellulose acetate was selected as a membrane forming polymer; polyethylene glycol(PEG) and polyvinyl pyrrolidone(PVP) were used as additives; acetone(Ac): N,N-Dimethylacetamide(DMAc) and N, N-Dimethylformamide(DMF) were used as solvents; and deionized(DI)water was used in the coagulation bath. All the prepared membranes were characterized in terms of hydraulic permeability(Pm), membrane resistance, average pore radius, and hydrophilicity. The top surface and crosssectional view of the prepared membranes were also observed by using field emission scanning electron microscopy.
