| 1 | Age at diagnosis of type 2 diabetes and cardiovascular risk factor profile:A pooled analysis显示文摘BACKGROUND The diagnosis of type 2 diabetes(T2D)in younger adults,an increasingly common public health issue,is associated with a higher risk of cardiovascular complications and mortality,which may be due to a more adverse cardiovascular risk profile in individuals diagnosed at a younger age.AIM To investigate the association between age at diagnosis and the cardiovascular risk profile in adults with T2D.METHODS A pooled dataset was used,comprised of data from five previous studies of adults with T2D,including 1409 participants of whom 196 were diagnosed with T2D under the age of 40 years.Anthropometric and blood biomarker measurements included body weight,body mass index(BMI),waist circumference,body fat percentage,glycaemic control(HbA1c),lipid profile and blood pressure.Univariable and multivariable linear regression models,adjusted for diabetes duration,sex,ethnicity and smoking status,were used to investigate the association between age at diagnosis and each cardiovascular risk factor.RESULTS A higher proportion of participants diagnosed with T2D under the age of 40 were female,current smokers and treated with glucose-lowering medications,compared to participants diagnosed later in life.Participants diagnosed with T2D under the age of 40 also had higher body weight,BMI,waist circumference and body fat percentage,in addition to a more adverse lipid profile,compared to participants diagnosed at an older age.Modelling results showed that each one year reduction in age at diagnosis was significantly associated with 0.67 kg higher body weight[95%confidence interval(CI):0.52-0.82 kg],0.18 kg/m^(2) higher BMI(95%CI:0.10-0.25)and 0.32 cm higher waist circumference(95%CI:0.14-0.49),after adjustment for duration of diabetes and other confounders.Younger age at diagnosis was also significantly associated with higher HbA1c,total cholesterol,low-density lipoprotein cholesterol and triglycerides.CONCLUSION The diagnosis of T2D earlier in life is associated with a worse cardiovascular risk factor profile,compared to those diagnosed later in life. | Mary M Barker Francesco Zaccardi Emer M Brady Gaurav S Gulsin Andrew P Hall Joseph Henson Zin ZinHtike Kamlesh Khunti Gerald P McCann Emma L Redman David R Webb Emma G Wilmot Tom Yates Jian Yeo Melanie J Davies Jack A Sargeant | 2022 | World Journal of Diabetes2022，13，3： | 2 |
| 2 | 地氟烷与七氟烷在门诊手术全身麻醉维持中的对比研究:对苏醒早晚期及围手术期咳嗽的影响显示文摘背景目前地氧垸已经用于门诊全身麻醉手术的维持阶段,其相对于七氖烷在围手术期的优略性尚存争议。尽管各研究一致指出,地氟烷比七氟烷苏醒更快,但这种差异对苏醒晚期的影响却未有明确报道。并且,地氟烷与七氟烷相比,对咳嗽发生率的影响也存在争议。方法本研究中纳入了130例在全身麻醉下行浅表门诊手术的患者,随机分为2组。所有患者都静脉注射丙泊酚2mg/kg诱导,置入喉罩,采用1%-3%的七氟烷或3%-8%的地氟炕维持麻醉,吸入混合空气或氧气。调节吸入麻醉药的浓度,维持血流动力学稳定,并维持脑电双频谱指数(BIS)于50—60。并用局部浸润和静脉注射酮洛酸30mg镇痛。在手术结束时给予预防性止吐药物,包括昂丹司琼4mg、地塞米松4mg及甲氧氨普胺10mg。评估标准包括睁眼恢复时间,对指令做出反应的时间,定向力恢复的时间,14分快通道评分,首次进食时间,能坐立,可站立,独立行走及出院时间。手术后早期及离院后24小时内,记录患者对于麻醉的满意度,手术后第1天恢复正常活动的能力,不良反应的发生(如咳嗽、意向性运动、氧饱和度〈90%、咽痛及手术后恶心、呕吐),手术后镇痛药及止吐药的使用情况。结果2组患者的流行病学资料相近。尽管围手术期地氯烷组咳嗽的总体发生率高于七氟烷组(分别为60%和32%,P〈0.05),然而在真正应用吸入麻醉药期间(即麻醉维持中),咳嗽发生率的组间差异无显著性。地氧烷组患者苏醒较快,但两组所有患者在离开手术室之前都达到了快通道恢复标准(快通道评分≥12)。此外,离院时间(七氟炕组90±31分钟,地氟炕组98±5分钟)及手术后第1天能恢复正常活动的患者比率(七氟烷48%,地氟炕60%),在两组间的差异无显著性。结论用地氧烷进行维持全身麻醉具有苏醒迅速、咳嗽发生率高的特点。尽管与七氟烷相比,地氟炕在麻醉早期恢复较快,但两组晚期恢复的差异无显著性。两种吸入麻醉药都适用于门诊手术的麻醉。 | Paul F. | 4 | 东莨菪碱透皮贴剂:可替代昂丹司琼和氟哌利多用于预防手术后呕吐症状的新制剂显示文摘背景鉴于对使用氟哌利多的争议和5-HT3受体拮抗剂的高昂价格,人们期望出现一种效价合理、可常规用于预防呕吐的药物来替代它们。我们设计了两个平行、随机、双盲、空白和安慰剂对照研究来比较东莨菪碱透皮贴剂(TDS)1.5mg和静脉注射昂丹司琼4mg及氟哌利多1.25mg作为多模式给药方案的一部分,对手术后呕吐“高危”人群早期和晚期的止吐效果及其副作用。方法150例患者参与本研究,其中包括腹腔镜手术(n=80)和整形外科手术(n=70)。患者入手术室前60分钟接受一枚有效的TDS贴剂(含东莨菪碱1.5mg)或一个外表相同的空白贴片。所有患者均接受标准全身麻醉。第2次给药是在手术接近结束时给予患者静脉注射2ml生理盐水(TDS贴剂的两组)、氟哌利多1.25mg或昂丹司琼4mg(空白贴片组)。记录患者手术后恶心、呕吐/干呕的发生率、是否需要止吐药物治疗、用药后的完全有效率(即用药后既无迁延性恶心亦无反复发生呕吐,无需使用止吐药物进行治疗)。此外,对手术后72小时之内的视觉紊乱、口干、昏睡和烦躁不安等不良反应进行记录。结果手术后72小时内,TDS贴剂组、氟哌利多组和昂丹司琼组患者呕吐和需要止吐药物治疗的患者差异无显著性。各治疗组中完全有效率为41%-51%,各组间差异无显著性。TDS组口干的发生率显著高于氟哌利多组和昂丹司琼组(21%vs3%)。结论TDS贴剂作为手术前给药用于预防手术后早期和晚期恶心、呕吐与氟哌利多1.25mg或昂丹司琼4mg效果相同,但是TDS贴剂更易引起口干。 | Patti F.
