| 1 | Usefulness of vonoprazan,a potassium ion-competitive acid blocker,for primary eradication of Helicobacter pylori显示文摘AIM To investigate usefulness of triple therapy with vonoprazan,a potassium ion-competitive acid blocker and antibiotics,for Helicobacter pylori(H.pylori) eradication.METHODS The H.pylori eradication rate was examined in 2507 patients(2055 undergoing primary eradication and 452 undergoing secondary eradication,excluding patients with subtotal gastrectomy) at the Japanese Red Cross Kyoto Daiichi Hospital from March 2013 to September 2015.For patients treated from March 2013 to February 2015,a proton pump inhibitor(PPI) was used to reduce acid secretion,while vonoprazan was used after March 2015.The success rates of the 2 regimens(PPI + amoxicillin + clarithromycin/metronidazole,or vonoprazan + amoxicillin + clarithromycin/metronidazole) were compared.RESULTS The success rate of primary H.pylori eradication was significantly higher in the vonoprazan group.When stratified by the underlying disease,a significant increase of the H.pylori eradication rate was observed in patients with chronic gastritis.A significantly lower H.pylori eradication rate was observed in younger patients compared to older patients in the PPI group,but there was no difference according to age in the vonoprazan group.On the otherhand,the success rate of secondary eradication was similar at approximately 90% in both groups.CONCLUSION Vonoprazan is very useful for primary eradication of H.pylori,and may become a first-line acid secretion inhibitor instead of PPIs. | Shinya Yamada Takumi Kawakami Yoshikazu Nakatsugawa Takahiro Suzuki Hideki Fujii Naoya Tomatsuri Hideki Nakamura Hideki Sato Yusuke Okuyama Hiroyuki Kimura Norimasa Yoshida | 2016 | World Journal of Gastrointestinal Pharmacology and Therapeutics2016，7，4： | 8 |
| 2 | Attenuation and Distortion of Compression Waves Propagating in Very Long Tube显示文摘A lot of phenomena related to propagating various waves are seen when the high-speed train goes through the tunnel,the gas pipeline is broken due to an accident or the air brake of the wagon operates.For instance,a compression wave generated ahead of a high-speed train entering a tunnel propagates to the tunnel exit and spouts as a micro pressure wave,which causes an exploding sound.In order to estimate the magnitude correctly,the mechanism of the attenuation and distortion of a compression wave propagating along a very long tunnel must be understood and the experimental information on these phenomena is required.An experimental investigation is carried out to clarify the attenuation and distortion of the propagating compression wave in a very long tube.Experimental results show that the strength of a compression wave decreases with distance.The attenuation and distortion of compression waves are affected by the initial waveform of the compression wave and by the unsteady boundary layer induced by the propagating wave.The shape of a compression wave becomes different with the propagating distance;that is,a shock wave appears just head of a wavefront and an overshoot on pressure distribution is observed behind a shock wave due to the transition of the unsteady boundary layer. | Shinya NAKAMURA Daisuke SASA Toshiyuki AOKI | 2011 | Journal of Thermal Science2011，20，1： | 2 |

| 19 | Carcinoembryonic antigen-producing adrenal adenoma resected using combined lateral and anterior transperitoneal laparoscopic surgery显示文摘A 74-year-old woman presented with symptoms consistent with hyperadrenocorticism and hyperca- techolaminism. She had a cushingoid appearance and her cortisol level was elevated. Her serum dopamine and noradrenalin levels were also elevated. Computed tomography detected a left adrenal mass measuring 3.5 cm × 3.0 cm in diameter. Metaiodobenzylguanidine scintigraphy was negative. Unexpectedly, the serum Serum carcinoembryonic antigen (CEA) level waselevated. Fluorodeoxyglucose positron emission tomography showed increased uptake in the adrenal tumor only, with a maximum standardized uptake value of 2.8. Selective venography and blood sampling revealed that the concentrations of cortisol, catecholamines and CEA were significantly elevated in the vein draining the tumor. A diagnosis of CEA-producing benign adenoma was made. After preoperative management, we performed a combined lateral and anterior transperitoneal laparoscopic adrenectomy. Her vital signs remained stable during surgery. Histopathological examination revealed a benign adenoma. Her cortisol, catecholamine and CEA levels normalized immediately after surgery.
