1Risk factors for bleeding after endoscopic mucosal resection显示文摘AIM: To clarify the risk factors for bleeding after endoscopic mucosal resection (EMR).METHODS: A total of 297 consecutive patients who underwent EMR were enrolled. Some of the patients had multiple lesions. Bleeding requiring endoscopic treatment was defined as bleeding after EMR. Odds ratios (OR) with 95% confidence intervals (CI), calculated by logistic regression with multivariate adjustments for covariates,were the measures of association.RESULTS: Of the 297 patients, 57 (19.2%) patients with bleeding after EMR were confirmed. With multivariate adjustment, the cutting method of ENR, diameter, and endoscopic pattern of the tumor were associated with the risk of bleeding after ENR. The multivariate-adjusted OR for bleeding after EMR using endoscopic aspiration mucosectomy was 3.07 (95%CI, 1.59-5.92) compared with strip biopsy. The multiple-adjusted OR for bleeding after EMR for the highest quartile (16-50 mm) of tumor diameter was 5.63 (95%CI, 1.84-17.23) compared with that for the lowest (4-7 mm). 2Intraductal neoplasm of the intrahepatic bile duct: Clinicopathological study of 24 cases显示文摘AIM: To investigate the clinicopathological features of intraductal neoplasm of the intrahepatic bile duct (INihB). METHODS: Clinicopathological features of 24 cases of INihB, which were previously diagnosed as biliary papillomatosis or intraductal growth of intrahepatic biliary neoplasm, were reviewed. Mucin immunohistochemistry was performed for mucin (MUC)1, MUC2, MUC5AC and MUC6. Ki-67, P53 and β-catenin immunoreactivity were also examined. We categorized each tumor as adenoma (low grade), borderline (intermediate grade), and malignant (carcinoma in situ , high grade including tumors with microinvasion). RESULTS: Among 24 cases of INihB, we identified 24 tumors. Twenty of 24 tumors (83%) were composed of a papillary structure; the same feature observed in intraductal papillary neoplasm of the bile duct (IPNB). In contrast, the remaining four tumors (17%) showed both tubular and papillary structures. In three of the four tumors (75%), macroscopic mucin secretion was limited but microscopic intracellular mucin was evident. Histologically, 16 tumors (67%) were malignant, three (12%) were borderline, and five (21%) were adenoma. Microinvasion was found in four cases (17%). Immunohistochemical analysis revealed that MUC1 was not expressed in the borderline/adenoma group but was expressed only in malignant lesions (P = 0.0095). Ki-67 labeling index (LI) was significantly higher in the malignant group than in the borderline/adenoma group (22.2 ± 15.5 vs 7.5 ± 6.3, P < 0.01). In the 16 malignant cases, expression of MUC5AC showed borderline significant association with high Ki-67 LI (P = 0.0622). Nuclear expression of β-catenin was observed in two (8%) of the 24 tumors, and these two tumors also showed MUC1 expression. P53 was negative in all tumors. CONCLUSION: Some cases of INihB have a tubular structure, and are subcategorized as IPNB with tubular structure. 4Involvement of insulin receptor substrates in cognitive impairment and Alzheimer’s disease显示文摘Type 2 diabetes一associated with impaired insulin/insulin-like growth factor-1 (IGF1) signaling (IIS)一is a risk factor for cognitive impairment and dementia including Alzheimer's disease (AD). The insulin receptor substrate (IRS) proteins are major components of IIS, which transmit upstream signals via the insulin receptor and/or IGF1 receptor to multiple intracellular signaling pathways, including AKT/protein kinase B and extracellular-signal-regulated kinase cascades. Of the four IRS proteins in mammals, IRS1 and IRS2 play key roles in regulating growth and survival, metabolism, and aging. Meanwhile, the roles of IRS1 and IRS2 in the central nervous system with respect to cognitive abilities remain to be clarified. In contrast to IRS2 in peripheral tissues, inactivation of neural IRS2 exerts beneficial effects, resulting in the reduction of amyloid p accumulation and premature mortality in AD mouse models. On the other hand, the increased phosphorylation of IRS 1 at several serine sites is observed in the brains from patients with AD and animal models of AD or cognitive impairment induced by type 2 diabetes. However, these serine sites are also activated in a mouse model of type 2 diabetes, in which the diabetes drug metformin improves memory impairment. Because IRS1 and IRS2 signaling pathways are regulated through complex mechanisms including positive and negative feedback loops, whether the elevated phosphorylation of IRS1 at specific serine sites found in AD brains is a primary response to cognitive dysfunction remains unknown. 5Autoimmune pancreatitis with IgG4-positive plasma cell infiltration in salivary glands and biliary tract显示文摘A 62-year-old male was referred to our hospital because of liver dysfunction, diffuse pancreatic swelling, and trachelophyma. At admission, the patient was free of pain.Physical examination showed enlarged and palpable bilateral submandibular masses, but no palpable mass or organomegaly in the abdomen. Laboratory findings were as follows: total protein 90 g/L with γ-globulin of 37.3% (33 g/L), total bilirubin 4 mg/L, aspartate aminotransferase 39 IU/L, alanine aminotransferase 67 IU/L, γ-glutamyl transpeptidase 1 647 IU/L, and amylase 135 IU/L. Autoantibodies were negative, and tumor markers were within the normal range. Serum IgG4 level was markedly elevated (18 900 mg/L). Computed tomography (CT) showed diffuse swelling of the pancreas and dilatation of both common and intra-hepatic bile ducts. Endoscopic retrograde pancreatography (ERP) revealed diffuse irregular and narrow main pancreatic duct and stenosis of the lower common bile duct. Biopsy specimens from the pancreas, salivary gland and liver showed marked periductal IgG4-positive plasma cell infiltration with fibrosis. We considered this patient to be autoimmune pancreatitis (AIP) with fibrosclerosis of the salivary gland and biliary tract, prescribed prednisolone at an initial dose of 40 mg/d. Three months later, the laboratory data improved almost to normal. Abdominal CT reflected prominent improvement in the pancreatic lesion. Swelling of the salivary gland also improved. At present, the patient is on 10 mg/d of prednisolone without recurrence of the pancreatitis. 7Mechanical properties of nerve roots and rami radiculares isolated from fresh pig spinal cords显示文摘No reports have described experiments designed to determine the strength characteristics of spinal nerve roots and rami radiculares for the purpose of explaining the complexity of symptoms of medullary cone lesions and cauda equina syndrome. In this study, to explain the pathogenesis of cauda equina syndrome, monoaxial tensile tests were performed to determine the strength characteristics of spinal nerve roots and rami radiculares, and analysis was conducted to evaluate the stress-strain relationship and strength characteristics. Using the same tensile test device, the nerve root and ramus radiculares isolated from the spinal cords of pigs were subjected to the tensile test and stress relaxation test at load strain rates of 0.1, 1, 10, and 100 s–1 under identical settings. The tensile strength of the nerve root was not rate dependent, while the ramus radiculares tensile strength tended to decrease as the strain rate increased. 8Comparative Gene Expression Analysis of Mouse and Human Cardiac Maturation显示文摘Understanding how human cardiomyocytes mature is crucial to realizing stem cell-based heart regeneration, modeling adult heart diseases, and facilitating drug discovery. However, it is not feasible to analyze human samples for maturation due to inaccessibility to samples while cardiomyocytes mature during fetal development and childhood, as well as dif?culty in avoiding variations among individuals. Using model animals such as mice can be a useful strategy; nonetheless, it is not well-understood whether and to what degree gene expression pro?les during maturation are shared between humans and mice. Therefore, we performed a comparative gene expression analysis of mice and human samples. First, we examined two distinct mice microarray platforms for shared gene expression pro?les, aiming to increase reliability of the analysis. We identi?ed a set of genes displaying progressive changes during maturation based on principal component analysis. Second, we demonstrated that the genes identi?ed had a differential expression pattern between adult and earlier stages(e.g., fetus) common in mice and humans. 11Micromolar sodium fluoride mediates anti-osteoclastogenesis in Porphyromonas gingivalis-induced alveolar bone loss显示文摘Osteoclasts are bone-specific multinucleated cells generated by the differentiation of monocyte/macrophage lineage precursors.Regulation of osteoclast differentiation is considered an effective therapeutic approach to the treatment of bone-lytic diseases.Periodontitis is an inflammatory disease characterized by extensive bone resorption. In this study, we investigated the effects of sodium fluoride(Na F) on osteoclastogenesis induced by Porphyromonas gingivalis, an important colonizer of the oral cavity that has been implicated in periodontitis. Na F strongly inhibited the P. gingivalis-induced alveolar bone loss. That effect was accompanied by decreased levels of cathepsin K, interleukin(IL)-1β, matrix metalloproteinase 9(MMP9), and tartrate-resistant acid phosphatase,which were up-regulated during P. gingivalis-induced osteoclastogenesis. Consistent with the in vivo anti-osteoclastogenic effect,Na F inhibited osteoclast formation caused by the differentiation factor RANKL(receptor activator of nuclear factor kB ligand)and macrophage colony-stimulating factor(M-CSF). The RANKL-stimulated induction of the transcription factor nuclear factor of activated T cells(NFAT) c1 was also abrogated by Na F. 12Sputtered nano-cobalt on H-USY zeolite for selectively converting syngas to gasoline显示文摘Selectively converting CO and H2 to gasoline product(isoparaffin and olefin) in one step still remains a great challenge. We demonstrate effective H-USY zeolite supported nano-cobalt bifunctional catalysts for this catalytic reaction, which are prepared by the novel physical sputtering process. Particles of the sputtered cobalt exist in nano-level and are well-dispersed on acid USY zeolite. Easy activation of the loaded nano-cobalt is also achieved in a low-temperature hydrogen reduction atmosphere. In the tandem catalytic reaction, the sputtered bifunctional Co/USY catalyst exhibits a much higher CO conversion and higher isoparaffin selectivity than the conventional impregnated one. Compared with H-Mor, H-Beta and other zeolites supported catalysts, H-USY zeolite supported cobalt catalyst shows the clearest promotional effect on the activity of FischerTropsch synthesis. 13Yield of OH radicals in water under heavy ion irradiation.Dependence on mass,specific energy,and elapsed time显示文摘Dependence of yields of OH (hydroxyl) radicals on the mass and specific energy of heavy ions and elapsed time after irradiation was investigated,to understand chemical reactions of aqueous solutions.The yields of irradiation products of phenol,super-linearly increased with the incident energy of He,C,and Ne ions ranging from 2 to 18 MeV/u.The yields of the OH radicals were estimated by analyzing the yields of the irradiation products of phenol. 15A large impact of tropical biomass burning on CO and CO_2 in the upper troposphere显示文摘A large interannual variation of biomass burning emissions from Southeast Asia is associated with the ENSO events. During 1997/98 and 1994 El Nino years, uncontrolled wildfires of tropical rainforests and peat lands in Indonesia were enlarged due to a long drought. Enhanced CO injection into the upper troposphere from the intense Indonesian fires was clearly observed in the 8-year measurements from a regular flask sampling over the western Pacific using a JAL airliner between Australia and Japan. This airliner observation also revealed that upper tropospheric CO2 cycle largely changed during the 1997 El Nino year due partly to the biomass burning emissions. Widespread pollution from the biomass burnings in Southeast Asia was simulated using a CO tracer driven by a 3D global chemical transport model. This simulation indicates that tropical deep convections connected to rapid advection by the subtropical jet play a significant role in dispersing biomass-burning emissions from Southeast Asia on a global scale.Hidekazu Matsueda Shoichi Taguchi Hisayuki Y. 16Microstructure and Electrochemical Corrosion Behavior of Fe-Cr System Alloys as Substitutes for Ni-Based Brazing Filler Metal显示文摘Two new Fe-Cr system alloys,Fe-20Cr-43Ni-10P(mass%)and Fe-20Cr-20Ni-8P-5Si-2Mo(mass%),have been developed as substitutes for the expensive Ni-based brazing filler metal used in brazing exhaust gas recirculation coolers.The microstructures and melting properties of the alloys were analyzed by electron probe X-ray microanalyzer and differential scanning calorimetry.The electrochemical characteristics of the alloys were investigated by potentiodynamic polarization testing in an electrolyte solution made in accordance with the standards of the Automobile Manufacturers Association of Germany.Furthermore,the corrosion behaviors of the alloys were investigated by constant-potential polarization testing and surface characterization.It is found that both alloys are composed of solid-solution phases and phosphide phases.The solid-solution phases serve as the anode,and the phosphide phases serve as the cathode in the corrosion reaction for both alloys.Fe-20Cr-43Ni-10 P exhibits galvanic corrosion on the entire surface.In contrast,Fe-20Cr-20Ni-8P-5Si-2Mo is attacked at a few localized areas so that the cavities form on the surface.The corrosion potential(E_(corr)) is lower than that of Ni-29Cr-6P-4Si(mass%)for both alloys.This means that the Fe-Cr system alloys are more easily corroded than Ni-29Cr-6P-4Si.The corrosion rate and corrosion resistance cannot be investigated by the corrosion current density(i_(corr)) and polarization resistance(R_p),respectively,because of localized corrosion of the two alloys.kangdao shi takahiro tsunoda ikuo shohji kotaro matsu yasuhiro taguchi 2016Acta Metallurgica Sinica(English Letters)2016,29,8:2

20Synchronous rectal and esophageal cancer treated with chemotherapy followed by two-stage resection显示文摘We report a case of 61-year-old male who had synchronous advanced rectal cancer involving the urinary bladder massively associated with multiple liver metastases, and esophageal cancer successfully treated by neoadjuvant chemotherapy followed by two-stage resection. Although complete resection of each of the lesions was considered possible by performing anterior pelvic exenteration, liver resection, and esophagectomy, it might be impossible for the patient to endure the stress of all of these operative procedures at once. Therefore, we planned to perform staged treatment with prioritizing consideration. First, we instituted chemotherapy with the FOLFOX(oxaliplatin + fluorouracil + leucovorin) plus cetuximab regimen, which could adequately control both rectal and esophageal cancer. After 6 cycles of chemotherapy, high anterior resection combined with cystoprostatectomy and lateral segmentectomy plus partial hepatectomy was performed followed by staged esophagectomy with three-field lymph node dissection.
