| 1 | Magnetic resonance imaging in breast cancer:A literature review and future perspectives显示文摘Early detection and diagnosis of breast cancer are essential for successful treatment. Currently mammography and ultrasound are the basic imaging techniques for the detection and localization of breast tumors. The low sensitivity and specificity of these imaging tools resulted in a demand for new imaging modalities and breast magnetic resonance imaging(MRI) has become increasingly important in the detection and delineation of breast cancer in daily practice. However, the clinical benefits of the use of pre-operative MRI in women with newly diagnosed breast cancer is still a matter of debate. The main additional diagnostic value of MRI relies on specific situations such as detecting multifocal, multicentric or contralateral disease unrecognized on conventional assessment(particularly in patients diagnosed with invasive lobular carcinoma), assessing the response to neoadjuvant chemotherapy, detection of cancer in dense breast tissue, recognition of an occult primary breast cancer in patients presenting with cancer metastasis in axillary lymph nodes, among others. Nevertheless, the development of new MRI technolo-gies such as diffusion-weighted imaging, proton spectroscopy and higher field strength 7.0 T imaging offer a new perspective in providing additional information in breast abnormalities. | 2 | Role of matrix metalloproteinase,tissue inhibitor of metalloproteinase and tumor necrosis factor-α single nucleotide gene polymorphisms in inflammatory bowel disease显示文摘AIM:To study the (functional) relevance of single nucleotide polymorphisms (SNPs) in genes encoding matrix metalloproteinases (MMP)-1,-2,-3,-9,tissue inhibitors of metalloproteinases (TIMP)-1,-2 and tumor necrosis factor (TNF)-α in the etiopathogenesis of inflammatory bowel diseases (IBD),that may enhance susceptibility and/or disease severity. METHODS:Genomic DNA from 134 Crohn's disease (CD),111 ulcerative colitis (UC) patients and 248 control subjects was isolated from resected intestinal tissue or blood. Allelic composition at SNP loci was determined by PCR-RFLP or tetra primer ARMS PCR. RESULTS:The TIMP-1 genotype TT in women and T in men at SNP +372 T/C was found to increase CD susceptibility (39% vs 23.8%,P=0.018 and 67.9% vs 51.6%,P=0.055,respectively),while women with this genotype were less prone to development of fistulae during follow-up (41.4% vs 68.3%,P=0.025). Male IBD or CD patients carrying the TIMP-1 +372 T-allele expressed lower levels of TIMP-1 in surgically resected macroscopically inflamed tissue (0.065 < P < 0.01). The 5T5T genotype at MMP-3 SNP -1613 5T/6T increased the chance of stenotic complications in CD during follow-up (91.2% vs 71.8%,P = 0.022) but seemed to protect against colonic involvement of this disease at first endoscopic/radiologic examination (35.3% vs 59.5%,P=0.017). CONCLUSION:Allelic composition at the examinedSNPs in genes coding for TIMP-1 and MMP-3 affect CD susceptibility and/or phenotype,i.e.,fistulizing disease,stricture pathogenesis and first disease localisation. | 7 | 广泛经皮针刀筋膜切断术联合脂肪移植术:一种治疗掌腱膜挛缩的新疗法显示文摘背景手术切除治疗掌腱膜挛缩症复发率高且康复时间延长。本文将介绍一种新型微创手术替代治疗掌腱膜挛缩症的方法及其成果。方法过程包括采用广泛经皮针刀筋膜切断术将束带完全松解,并与真皮分离。随后,由此产生的松弛组织用以自体脂肪填充。术后伸直位制动1周,然后,建议术后3~6个月患者白天可以正常用手,夜间伸直位固定。本组患者共91例(99只手),手术在迈阿密和鹿特丹进行;对50例患者的手部伸展角度进行测量(平均随访44周)。结果近端指间关节挛缩的治疗效果显著,伸展角度由61°改善至27°,掌指关节挛缩的伸展角度由37°改善至-5°。94%患者在2~4周内手部恢复正常功能,95%患者对结果非常满意。无新的瘢痕生成,大部分恢复了柔软的手掌脂肪垫。发生了1例神经损伤的并发症,1例术后切口感染,4例复杂区域疼痛综合征。结论这一新的微创技术缩短了康复时间,增加了缺损的皮下脂肪组织,产生了柔软无痕的皮肤。并且有能力处理多个掌指纹,可解决整只手的异常情况。治疗过程安全有效,尤其对首次治疗者。目前,前瞻性随机对照研究过程充分确定了其治疗掌腱膜挛缩症的作用。(循证医学证据等级:Ⅳ级,疗法) | Steven E. | 10 | T-cell ageing in end-stage renal disease patients:Assessment and clinical relevance显示文摘End-stage renal disease(ESRD) patients have a defective T-cell-mediated immune system which is related to excessive premature ageing of the T-cell compartment.This is likely to be caused by the uremia-associated pro-inflammatory milieu,created by loss of renal function.Therefore,ESRD patients are highly susceptible for infections,have an increased risk for virus-associated cancers,respond poorly to vaccination and have an increased risk for atherosclerotic diseases.Three ageing parameters can be used to assess an immunological T-cell age.First,thymic output can be determined by assessing the T-cell receptor excision circlescontent together with CD31 expression within the naive T cells.Second,the telomere length of T cells and third the T-cell differentiation status are also indicators of T-cell ageing.Analyses based on these parameters in ESRD patients revealed that the immunological T-cell age is increased by on average 20 years compared to the chronological age.After kidney transplantation(KTx) the aged T-cell phenotype persists although the pro-inflammatory milieu is diminished.This might be explained by epigenetic modifications at hematopoietic stem cells level.Assessment of an immunological T-cell age could be an important tool to identify KTx recipients who are at risk for allograft rejection or to prevent over-immunosuppression. | Ruud WJ Meijers Michiel GH Betjes Carla C Baan Nicolle HR Litjens | 2014 | World Journal of Nephrology2014,3,4: | 2 |
