| 1 | 集合系统CT三维重建精准指导经皮肾镜手术目标肾盏选择和穿刺的临床研究显示文摘目的探讨集合系统CT三维重建帮助经皮肾镜手术(PCNL)精准进行目标肾盏选择和目标肾盏穿刺的可行性和安全性。方法肾结石PCNL患者196例,均行术前腹部泌尿系平片(KUB)、静脉肾盂造影(IVU)、CT平扫和增强扫描,分别获取肾结石、集合系统及肾脏周围器官的CT三维影像,在集合系统三维重建后面观影像上确定最佳PCNL目标肾盏,将确定的目标肾盏通过影像转化技术标识在IVU影像上,C臂机引导下穿刺该目标肾盏建立手术通道。结果所有PCNL手术均按术前的规划实现了精准穿刺目标肾盏建立手术通道,196例患者术中、术后无严重出血、邻近器官损伤等并发症,均未输血。鹿角形肾结石手术无石率为88%,肾盂肾盏结石94.1%。结论通过对影像的技术转化,将CT集合系统三维重建确定的目标肾盏准确标识在IVU影像上,指导PCNL术中对目标肾盏进行精准选择和精准穿刺,建立更加安全的手术通道,最大程度降低PCNL出血和器官损伤等并发症。 | 李炯明 王光 刘建和 姜永明 张劲松 方克伟 石鑫 Prashant Mishra | 2017 | 临床外科杂志2017,25,2: | 12 |
| 2 | Current Status of Conventional and Molecular Interventions for Blast Resistance in Rice显示文摘Pyricularia oryzae anamorph of Magnaporthe oryzae is one of the most notorious fungal pathogens causing severe economic loss in rice production worldwide. Various methods, viz. cultural, biological and molecular approaches, are utilized to counteract this pathogen. Moreover, some tolerant or resistant rice varieties have been developed with the help of breeding programmes. Isolation and molecular characterization of different blast resistance genes now open the gate for new possibilities to elucidate the actual allelic variants of these genes via various molecular breeding and transgenic approaches. However, the behavioral pattern of this fungus breakups the resistance barriers in the resistant or tolerant rice varieties. This host-pathogen barrier will be possibly countered in future research by comparative genomics data from available genome sequence data of rice and M. oryzae for durable resistance. Present review emphasized fascinating recent updates, new molecular breeding approaches, transgenic and genomics approaches(i.e. mi RNA and genome editing) for the management of blast disease in rice. | 5 | 554例部分无管化mPCNL临床分析显示文摘目的总结部分无管化mPCNL经验.方法回顾性分析2010年1月至2016年12月多中心部分无管化mPCNL手术经验.7 a间共完成1 320例mPCNL手术,其中部分无管化mPCNL554例,标准化mPCNL766例.除外手术时间大于2 h及各种原因被迫中止手术的85例,共681例标准化mPCNL与部分无管化mPCNL进行比较.结果随着部分无管化手术指征逐渐放宽,部分无管化手术率增加,并发症并无增加,2016年部分无管化mPCNL率达84%.与标准化mPCNL比较,二者手术净石率、平均血红蛋白下降、输血率、术后发热率无显著差异.术后止痛剂应用率、术后住院天数有显著差异,分别为5%:21%(P=0.001)和2.5:4.5 d(P=0.001).术后出血相关并发症发生率无管化与标准组分别为1.1%和2.5%,无显著性差异.部分无管化组发生1例尿外渗,1例胸膜损伤出血,分别予肾周引流、抗生素治疗及开放探查、胸腔引流后顺利出院.结论与标准mPCNL相比,部分无管化mPCNL明显增加患者术后的舒适性,缩短住院时间,术后相关并发症无增加,是安全可行的手术方式. | 姜永明 郭海翔 Mishra Prashant 陈戬 李炯明 王剑松 | 2018 | 昆明医科大学学报2018,39,5: | 3 |
| 6 | 压力性尿失禁大鼠中环氧合酶2表达的变化及意义显示文摘目的压力性尿失禁(SUI)的发病机制研究较少。文中旨在检测环氧合酶2(COX?2)在SUI大鼠模型中的尿道、阴道、尿道平滑肌中的表达变化。方法健康雌性未育SD大鼠60只,雄性SD大鼠15只,合拢自然妊娠,取分娩后雌性大鼠分别行扩张阴道(单纯扩张组)、扩张阴道+切除卵巢(扩张+切除组)建立SUI大鼠模型,另使用正常妊娠分娩后大鼠为对照组,每组6只。通过喷嚏实验及尿动力学检查最大膀胱容量(MBC)和腹压漏尿点压(ALPP),荧光定量PCR、Western blot检测COX?2 mRNA及蛋白水平的表达,免疫组化检测COX?2在尿道、阴道、尿道平滑肌的表达。结果扩张+切除组、单纯扩张组ALPP较对照组明显下降,扩张+切除组较单纯扩张组明显下降(P<0.05)。单纯扩张组、扩张+切除组COX?2mRNA表达量[(1.40±0.07)、(1.75±0.10)]、蛋白的相对表达量[(1.64±0.05)、(1.95±0.08)]较对照组[(1.00±0.16)、(0.77±0.26)]均明显升高(P<0.001);扩张+切除组较单纯扩张组明显升高(P<0.05)。免疫组化结果显示,对照组、单纯扩张组、扩张+切除组阴道组织中COX2的染色强度表达积分依次为0.50±0.54、5.55±0.54、9.33±0.81,组间两两比较差异有统计学意义(P<0.05);尿道平滑肌组织中染色强度表达依次为0.66±0.51、5.33±0.51、8.50±0.54,组间两两比较差异有统计学差异(P<0.05)。结论切除卵巢后的雌激素下降可致COX?2表达的升高,COX?2与SUI的发病机制相关。 | 李江 杨智明 柯建 Prashant Mishra 罗伟 刘上 刘建和 袁志伟 孙浩 杨昭庆 方克伟 | 2019 | 医学研究生学报2019,32,2: | 3 |

Ayurveda is a popular traditional medicine in South Asia, emanating worldwide for the treatment of metabolic disorders and chronic illness. Techniques of in silico biology are not much explored for the investigation of a variety of bioactive phytochemicals of Ayurvedic herbs. Drug repurposing, reverse pharmacology, and polypharmacology in Ayurveda are areas in silico explorations that are needed to understand the rich repertoire of herbs, minerals, herbo-minerals, and assorted Ayurvedic formulations. This review emphasizes exploring the concept of Ayurveda with in silico approaches and the need for Ayurinformatics studies. It also provides an overview of in silico studies done on phytoconstituents of some important Ayurvedic plants, the utility of in silico studies in Ayurvedic phytoconstituents/formulations, limitations/challenges, and prospects of in silico studies in Ayurveda. This article discusses the convergence of in silico work, especially in the least explored field of Ayurveda. The focused coalesce of these two domains could present a predictive combinatorial platform to enhance translational research magnitude. | 15 | 腹腔镜和机器人辅助回肠代输尿管术在临床应用中的可行性和治疗效果(英文)显示文摘回肠代输尿管术是治疗患者长段输尿管狭窄的恰当方式.微创手术技术已被证明和开放手术具有相同的安全性,且在减少手术创伤、出血和术后康复周期方面明显优于开放手术.为了研究腹腔镜下回肠代输尿管术的可行性和临床疗效,对相关文献中该手术方案的手术指征、禁忌症、手术方法和术后结果进行了综述,并结合昆明医科大学第二附属医院对该手术的实践经验进行综合分析.共有20例腹腔镜下回肠代输尿管术报道纳入综述,其手术时间在195~480 min之间,出血量在50~375 mL之间;共有7例机器人辅助的回肠代输尿管术报道纳入综述,其手术时长在320~720 min之间,出血量少于10~100 mL.该手术的并发症包括腹腔内尿瘘、小肠梗阻、吻合口瘘、暂时性氮质血症和泌尿系感染等.术后所有患者的肾功能都得到不同程度恢复.综上,腹腔镜或机器人辅助回肠代输尿管术是治疗长段输尿管狭窄的一种可行且有效的手术方案,尽管其疗效和传统开放手术相当,但腹腔镜和机器人辅助方法因其微创特点仍具有相对优势,它们均可有效降低手术创伤、术中出血量、术后并发症、手术瘢痕形成和术后康复时间.由于现有报道的病例数量有限,故还需对回肠代输尿管的微创手术方案进行更多和长期的研究,以便建立和完善该术式的标准手术步骤和手术技巧. | Mishra Prashant 李炯明 | 2018 | 昆明医科大学学报2018,39,3: | 0 |
