| 1 | Role of 18F-fluorodeoxyglucose positron emission tomography imaging in surgery for pancreatic cancer显示文摘AIM: To evaluate the role of positron emission tomo-graphy using 18F-fluorodeoxyglucose (FDG-PET) in the surgical management of patients with pancreatic cancer, including the diagnosis, staging, and selection of patients for the subsequent surgical treatment.METHODS: This study involved 53 patients with proven primary pancreatic cancer. The sensitivity of diagnosing the primary cancer was examined for FDG-PET, CT, cytological examination of the bile or pancreatic juice, and the serum levels of carcinoembrionic antigens (CEA) and carbohydrate antigen 19-9 (CA19-9). Next, the accuracy of staging was compared between FDG-PET and CT. Finally, FDG-PET was analyzed semiquantitatively using the standard uptake value (SUV). The impact of the SUV on patient management was evaluated by examining the correlations between the SUV and the histological findings of cancer.RESULTS: The sensitivity of FDG-PET, CT, cytological examination of the bile or pancreatic juice, and the serum levels of CEA and CA19-9 were 92.5%, 88.7%, 46.4%, 37.7% and 69.8%, respectively. In staging, FDG-PET was superior to CT only in diagnosing distant disease (bone metastasis). For local staging, the sensitivity of CT was better than that of FDG-PET. The SUV did not correlate with the pTNM stage, grades, invasions to the vessels and nerve, or with the size of the tumor. However, there was a statistically significant difference (4.6 ± 2.9 vs 7.8 ± 4.5, P = 0.024) in the SUV between patients with respectable and unresectable disease.CONCLUSION: FDG-PET is thus considered to be useful in the diagnosis of pancreatic cancer. However, regarding the staging of the disease, FDG-PET is not considered to be a sufficiently accurate diagnostic modality. | 2 | Inflammation-based factors and prognosis in patients with colorectal cancer显示文摘Several parameters for predicting survival in patients with colorectal cancer have been identified, including the performance status, age, gender and tumor-nodemetastasis(TNM) stage. Although the TNM stage is important and useful for predicting the prognosis and determining the appropriate treatment, it is well known that the survival time varies widely, even in patients with the same stage of disease. Therefore, the identification of new parameters capable of more precisely predicting patient survival is needed to help select the optimal treatment, especially in patients in the advanced stage of disease. Although the TNM stage reflects the tumor characteristics, cancer progression and survival are not determined solely based on the local characteristics of the tumor, but also the host systemic immune/inflammatory response. | 5 | Image analysis of soil failure on defective underground pipe due to cyclic water supply and drainage using X-ray CT显示文摘The ground subsidence on the underground pipe often is caused with the reduction of the effective stress and the loss of suction in the base course and then,soil drainage into the pipe.The final formation of the cavity growth in the ground was observed as the ground subsidence.Authors focused this problem and hence performed model tests with water-inflow and drainage cycle in the model ground.The mechanism of cavity generation in the model ground was observed using an X-ray Computed Tomography(CT)scanner.In those studies,water was supplied into the model grounds from the defected underground pipe model in case of the change of relative density and grain size distribution.As results,it was observed that the loosening area was generated from the defected part with water-inflow and some of the soil particles in the ground were drained into the underground pipe through the defected part.And afterward,the cavity was generated just above the defected part of the model pipe in the ground.Based on this observation,it might be said that the bulk density of soil around the defected pipe played one of key factor to generate the cavity in the ground.Moreover,the dimension of the defected part should be related to the magnification of the ground subsidence,in particular,crack width on a sewerage pipe and particle size would be the quantitative factor to evaluate the magnification of the ground subsidence.In this paper,it was concluded that the low relative density of soil would become the critical factor to cause the fatal failure of model ground if the maximum grain size was close to the dimension of crack width of defective part.The fatal collapse of the ground with high relative density more than 80%would be avoided in a few cycles of water inflow and soil drainage. | Toshifumi MUKUNOKI Naoko KUMANO Jun OTANI | 2012 | Frontiers of Structural and Civil Engineering2012,6,2: | 5 |

She presented with obstructive jaundice and eosinophilia. Endoscopic retrograde cholangiopancreatography showed diffuse narrowing from the distal common bile duct to the bifurcation of the hepatic ducts. An endoscopic plastic biliary stent was inserted; the specimen obtained from the common bile duct wall revealed dense infiltration by eosinophils. Treatment was started with prednisolone 60 mg daily. The patient's biliary stenosis and eosinophilia gradually improved. Eosinophilic infiltration in the lungs or stomach is relatively common, but it is rare in the common bile duct. Most of the reported cases of eosinophilic cholangitis presented with eosinophilia; our patient's eosinophil count was over 1000/mm3. | 11 | Dopaminergic modulation of synaptic plasticity in rat prefrontal neurons显示文摘The prefrontal cortex(PFC) is thought to store the traces for a type of long-term memory – the abstract memory that determines the temporal structure of behavior often termed a 'rule' or 'strategy'. Long-term synaptic plasticity might serve as an underlying cellular mechanism for this type of memory. We therefore studied the induction of synaptic plasticity in rat PFC neurons, maintained in vitro, with special emphasis on the functionally important neuromodulator dopamine. First, the induction of long-term potentiation(LTP) was facilitated in the presence of tonic/background dopamine in the bath, and the dose-dependency of this background dopamine followed an 'inverted-U' function, where too high or too low dopamine levels could not facilitate LTP. Second, the induction of long-term depression(LTD) by low-frequency stimuli appeared to be independent of background dopamine, but required endogenous, phasically-released dopamine during the stimuli. Blockade of dopamine receptors during the stimuli and exaggeration of the effect of this endogenouslyreleased dopamine by inhibition of dopamine transporter activity both blocked LTD. Thus, LTD induction also followed an inverted-U function in its dopamine-dependency.
