1Treatment of hepatocellular carcinoma accompanied by portal vein tumor thrombus显示文摘The prognosis of patients with hepatocellular carcinoma (HCC) accompanied by portal vein tumor thrombus (PVTT) is generally poor if left untreated: a median survival time of 2.7-4.0 mo has been reported. Furthermore, while transcatheter arterial chemoembolization (TACE) has been shown to be safe in selected patients, the median survival time with this treatment is still only 3.8-9.5 mo. Systemic single-agent chemotherapy for HCC with PVTT has failed to improve the prognosis, and the response rates have been less than 20%. While regional chemotherapy with low-dose cisplatin and 5-fluorouracil or interferon and 5-fluorouracil via hepatic arterial infusion has increased the response rate, the median survival time has not exceeded 12 (range 4.5-11.8) mo. Combined treatment consisting of radiation for PVTT and TACE for liver tumor has achieved a high response rate, but the median survival rates have still been only 3.8-10.7 mo. With hepatic resection as monotherapy, the 5-year survival rate and median survival time were reportedly 4%-28.5% and 6-14 mo. The most promising results were reported for combined treatments consisting of hepatectomy and TACE, chemotherapy, or internal radiation. The reported 5-year survival rates and median survival times were 42% and 31 mo for TACE followed by hepatectomy; 36.3% and 22.1 mo for hepatectomy followed by hepatic arterial infusion chemotherapy; and 56% for chemotherapy or internal radiation followed by hepatectomy.Masami Minagawa Masatoshi Makuuchi 2006World Journal of Gastroenterology2006,12,47:74
2Gut microbiota and liver diseases显示文摘Several studies revealed that gut microbiota are associated with various human diseases,e.g.,metabolic diseases,allergies,gastroenterological diseases,and liver diseases.The liver can be greatly affected by changes in gut microbiota due to the entry of gut bacteria or their metabolites into the liver through the portal vein,and the liver-gut axis is important to understand the pathophysiology of several liver diseases,especially non-alcoholic fatty liver disease and hepatic encephalopathy.Moreover,gut microbiota play a significant role in the development of alcoholic liver disease and hepatocarcinogenesis.Based on theseprevious findings,trials using probiotics have been performed for the prevention or treatment of liver diseases.In this review,we summarize the current understanding of the changes in gut microbiota associated with various liver diseases,and we describe the therapeutic trials of probiotics for those diseases.Masami Minemura Yukihiro Shimizu 2015World Journal of Gastroenterology2015,21,6:41
3Correlation of N-myc downstream-regulated gene 1 expression with clinical outcomes of colorectal cancer patients of different race/ethnicity显示文摘AIM: To evaluate the role of N-myc downstream-regulated gene 1 (NDRG1) expression in prognosis and survival of colorectal cancer patients with different ethnic backgrounds. METHODS: Because NDRG1 is a downstream target of p53 and hypoxia inducible factor-1α (HIF-1α),we examined NDRG1 expression together with p53 and HIF-1α by immunohistochemistry. A total of 157 colorectal cancer specimens including 80 from Japanese patients and 77 from US patients were examined. The correlation between protein expression with clinicopathological features and survival after surgery was analyzed.in colorectal tumor compared with normal epithelium in both Japanese and US patient groups. Expression of NDRG1 protein was significantly correlated with lymphatic invasion,venous invasion,depth of invasion,histopathological type,and Dukes' stage in Japanese colorectal cancer patients. NDRG1 expression was correlated to histopathological type,Dukes' stage and HIF-1α expression in US-Caucasian patients but not in US-African American patients. Interestingly,Kaplan-Meier survival analysis demonstrated that NDRG1 expression correlated significantly with poorer survival in US-African American patients but not in other patient groups. However,in p53-positive US cases,NDRG1 positivity correlated significantly with better survival. In addition,NDRG1 expression also correlated significantly with improved survival in US patients with stages Ⅲ and Ⅳ tumors without chemotherapy. In Japanese patients with stages Ⅱ and Ⅲ tumors,strong NDRG1 staining in p53-positive tumors correlated significantly with improved survival but negatively in patients without chemotherapy. CONCLUSION: NDRG1 expression was correlated with various clinicopathological features and clinical outcomes in colorectal cancer depending on the race/ethnicity of the patients. 4Randomized controlled trial of pancreatic stenting to prevent pancreatitis after endoscopic retrograde cholangiopancreatography显示文摘AIM:To determine the effectiveness of pancreatic duct(PD) stent placement for the prevention of pancreatitis after endoscopic retrograde cholangiopancreatography(ERCP) in high risk patients.METHODS:Authors conducted a single-blind,randomized controlled trial to evaluate the effectiveness of a pancreatic spontaneous dislodgement stent against post-ERCP pancreatitis,including rates of spontaneous dislodgement and complications.Authors defined high risk patients as having any of the following:sphincter of Oddi dysfunction,difficult cannulation,prior history of post-ERCP pancreatitis,pre-cut sphincterotomy,pancreatic ductal biopsy,pancreatic sphincterotomy,intraductal ultrasonography,or a procedure time of more than 30 min.Patients were randomized to a stent group(n = 60) or to a non-stent group(n = 60).An abdominal radiograph was obtained daily to assessspontaneous stent dislodgement.Post-ERCP pancreatitis was diagnosed according to consensus criteria.RESULTS:The mean age(± standard deviation) was 67.4 ± 13.8 years and the male:female ratio was 68:52.In the stent group,the mean age was 66 ± 13 years and the male:female ratio was 33:27,and in the non-stent group,the mean age was 68 ± 14 years and the male:female ratio was 35:25.There were no significant differences between groups with respect to age,gender,final diagnosis,or type of endoscopic intervention.The frequency of post-ERCP pancreatitis in PD stent and non-stent groups was 1.7%(1/60) and 13.3%(8/60),respectively.The severity of pancreatitis was mild in all cases.The frequency of post-ERCP pancreatitis in the stent group was significantly lower than in the non-stent group(P = 0.032,Fisher's exact test).The rate of hyperamylasemia were 30%(18/60) and 38.3%(23 of 60) in the stent and non-stent groups,respectively(P = 0.05,χ2 test).The placement of a PD stent was successful in all 60 patients.The rate of spontaneous dislodgement by the third day was 96.7%(58/60),and the median(range) time to dislodgement was 2.1(2-3) d.The rates of stent migration,hemorrhage,perforation,infection(cholangitis or cholecystitis) or other complicationss were 0%(0/60),0%(0/60),0%(0/60),0%(0/60),0%(0/60),respectively,in the stent group.Univariate analysis revealed no significant differences in high risk factors between the two groups.The pancreatic spontaneous dislodgement stent safely prevented post-ERCP pancreatitis in high risk patients.CONCLUSION:Pancreatic stent placement is a safe and effective technique to prevent post-ERCP pancreatitis.Therefore authors recommend pancreatic stent placement after ERCP in high risk patients.Yoshiaki Kawaguchi Masami Ogawa Fumio Omata Hiroyuki Ito Tooru Shimosegawa Tetsuya Mine 2012World Journal of Gastroenterology2012,18,14:25
5Development of osteomalacia in a post-liver transplant patient receiving adefovir dipivoxil显示文摘We report the case of a patient treated with living donor-related liver transplantation who suffered from osteomalacia during adefovir dipivoxil (ADV)-containing antiviral therapy for lamivudine-resistant hepatitis B virus infection. The patient had generalized bone pain,with severe hypophosphatemia after 20 mo of ADV therapy. Radiographic studies demonstrated the presence of osteomalacia. The peak plasma ADV level was 38 ng/mL after administration of ADV at 10mg/day. It was also found that ADV affected the metabolism of tacrolimus,a calcineurin-inhibitor,and caused an increase in the plasma levels of tacrolimus. The disability was reversed with the withdrawal of ADV and with mineral supplementation. ADV can cause an elevation of plasma tacrolimus levels,which may be associated with renal dysfunction. High levels of ADV and tacrolimus can cause nephrotoxicity and osteomalacia. 6Cytokines in adipose-derived mesenchymal stem cells promote the healing of liver disease显示文摘Adipose-derived mesenchymal stem cells(ADSCs) are a treatment cell source for patients with chronic liver injury. ADSCs are characterized by being harvested from the patient's own subcutaneous adipose tissue, a high cell yield(i.e., reduced immune rejection response), accumulation at a disease nidus, suppression of excessive immune response, production of various growth factors and cytokines, angiogenic effects, antiapoptotic effects, and control of immune cells via cellcell interaction. We previously showed that conditioned medium of ADSCs promoted hepatocyte proliferation and improved the liver function in a mouse model of acute liver failure. Furthermore, as found by many other groups, the administration of ADSCs improved liver tissue fibrosis in a mouse model of liver cirrhosis. A comprehensive protein expression analysis by liquid chromatography with tandem mass spectrometry showed that the various cytokines and chemokines produced by ADSCs promote the healing of liver disease. In this review, we examine the ability of expressed protein components of ADSCs to promote healing in cell therapy for liver disease. Previous studies demonstrated that ADSCs are a treatment cell source for patients with chronic liver injury. 7Clinical significance of serum levels of vascular endothelial growth factor and its receptor in biliary disease and carcinoma显示文摘AIM: To investigate the clinical significance of serum vascular endothelial growth factor (VEGF) and soluble VEGF receptor-1 (VEGFR1/Flt-1) (sVEGFR1) levels in biliary diseases.METHODS: We analyzed the serum levels of these proteins in patients with acute cholangitis (group 1), biliary malignancies (group 2), and primary biliary cirrhosis or primary sclerosing cholangitis (group 3), and in healthy donors (group 4). The influence of inflammation was also analyzed. Serum VEGF levels were expressed as VEGF per platelet (VEGF/PLT, pg/106) in order to exclude the influence of platelet counts.RESULTS: sVEGFR1 levels were significantly higher in groups 1 and 2 than in the control group, but did not correlate with inflammatory markers. VEGF/PLT levels were generally higher in patients with active inflammation than in those with carcinoma. C-reactive protein strongly correlated with the levels of serum VEGF independently of platelet and leukocyte counts, even in cancer patients. 8Chemical characteristics of size-resolved aerosols in winter in Beijing显示文摘Size-resolved aerosols were continuously collected by a Nano Sampler for 13 days at an urban site in Beijing during winter 2012 to measure the chemical composition of ambient aerosol particles. Data collected by the Nano Sampler and an ACSM(Aerodyne Aerosol Chemical Speciation Monitor) were compared. Between the data sets,similar trends and strong correlations were observed,demonstrating the validity of the Nano Sampler. PM10 and PM2.5concentrations during the measurement were 150.5 ± 96.0 μg/m3(mean ± standard variation)and 106.9 ± 71.6 μg/m3,respectively. The PM2.5/PM10 ratio was 0.70 ± 0.10,indicating that PM2.5dominated PM10. The aerosol size distributions showed that three size bins of 0.5–1,1–2.5 and 2.5–10 μm contributed 21.8%,23.3% and 26.0% to the total mass concentration(TMC),respectively. OM(organic matter) and SIA(secondary ionic aerosol,mainly SO42-,NO3-and NH4+) were major components of PM2.5. Secondary compounds(SIA and secondary organic carbon) accounted for half of TMC(about 49.8%) in PM2.5,and suggested that secondary aerosols significantly contributed to the serious particulate matter pollution observed in winter. Coal burning,biomass combustion,vehicle emissions and SIA were found to be the main sources of PM2.5. 11Successful laparoscopic splenectomy after living-donor liver transplantation for thrombocytopenia caused by antiviral therapy显示文摘Although interferon (IFN) based therapy for recurrent hepatitis C virus (HCV) infection after liver transplantation has been widely accepted, it induces various adverse effects such as thrombocytopenia, resulting in its interruption. Recently, concomitant splenectomy at the time of living donor liver transplantation (LDLT) has been tried to overcome this problem, but this procedure leads to several complications such as excessive intraoperative bleeding and serious infection. A 60-year-old female received LDLT using a left lobe graft from her second son for liver failure caused by hepatitis C-related cirrhosis. Six months after LDLT, she was diagnosed as recurrent HCV infection by liver biopsy. IFN monotherapy was started from 7 mo after LDLT and her platelet count decreased to less than 50 000/μL, which thus made it necessary to discontinue the treatment. We decided to attempt laparoscopic splenectomy (LS) under general anesthesia. Since intra-abdominal findings did not show any adhesion formations around the spleen, LS could be successfully performed. After LS, since her platelet count immediately increased to 225 000/μL 14 d after operation, IFN therapy was restarted and we could convert the combination therapy of IFN and ribavirin, resulting in no detectable viral marker. 14癌症的时间生物学:治疗癌症的时间重要吗?(英文)显示文摘我们周围的世界是一个已知有生命进化的世界,一个在不停变化的世界.这些变化多数是周期性的.这些周期性动态变化起源于地球、太阳和月亮之间有规律的时空关系,这种关系正是节律产生的本质.日夜的交替、日照时间的季节性变化以及由于过强或过弱的寻常节律导致的突发性气候变化,所有这些使得地球生命节律得以产生(火星生命尚未深入研究).这些近乎完美的节律变化引起了每日两次(昼夜)和每月一次(月经)的生命节律及相呼应的复杂生理变化.这些信息均已被深深地编入在我们的基因密码中.还有更低频率的节律,如:反映太阳黑子活动的规律(周期为10.5年)及世纪性周期或更长周期的节律,如气候的变更.所有这些节律都具有生物学意义.过去30年中,这些节律对健康、对疾病和宿主间平衡的影响已日显明朗.这些概念早在三世纪前的西方医学中就已经发现,在几千年前的中国传统医学中就有描述和应用.中医的因时施治和时令用药等正合此理.该文通过深入的理论探讨和详实的实验数据阐述了在认识、预防、诊断和治疗癌症时要仔细地考虑时间的科学基础,也为这方面的中医理论提供了现代科学的依据.时间生物学(chronobiology)是一门研究生物现象节律性变化的科学.这种动态生物学研究揭示的是在生物物理和生物化学过程中,以时间为基础的变化规律.最常见的生物节律按其周期的长短有3种:以(24±4)h为周期的昼夜节律,以(30±7)d为周期的月节律,和以(12±2)月为周期的年节律.目前,对昼夜生物节律现象研究的最多,对其生物学意义了解也最深.生物节律最基本的特性有:①内源性和遗传性;②外界刺激可调节生物节律;③在没有外界时间相关信息提示时仍可保持节律性.机体内的生物钟是维持这些节律的基本结构,下丘脑的视交叉上核是这个时间中枢的所在.外界环境,如光线或日照每天都对这个时间中枢进行调整,并进而影响机体的许多生理功能,如睡眠、体温、激素分泌及细胞增殖周期等,使得健康机体中这些生理过程都维持着显著的昼夜周期性节律.不适当外界环境刺激,尤其是长期在夜间暴露于强光之下(如从事护士和娱乐业的人员),可导致机体生物钟的持续紊乱.这种持续的紊乱可导致睡眠紊乱、激素分泌失衡、精神状态失调及冠心病等.近来流行病学调查还发现这种紊乱和乳腺癌、结肠癌的发病有关.生物体内存在维持时间节律的分子机制,这就是生物钟基因.它存在于生物钟中枢及每一个外周组织中.机体的时间节律正是由这些生物钟基因及受其调控的相关基因(钟控基因,约占人基因的10%~15%)的协同表达所致.目前已知正常增生活跃的组织中,DNA合成和细胞分裂都具有明显的昼夜周期性节律.其中以对骨髓、肠道粘膜研究得最多,因为这两个组织对许多细胞毒化疗药物都很敏感,造成它们损伤的程度是许多化疗药物及放疗应用受限的原因.癌组织也有昼夜时间节律,这表现在:①它在宿主体内的生长有时间节律;②癌细胞的增生和凋亡受生物钟基因调控;③同一种抗癌药物,在某些时间给药可能是有效的,而在另外一些时间给药则不仅无效反而造成对正常组织损伤.临床的随机抽样调查表明只要用药时间适当,细胞毒化疗药物的毒性可以减弱,药物的剂量可以加大,治疗效果可以改善,癌症病人的生存可以延长.对人体癌细胞在一天中某一时刻、处于某一细胞周期阶段的细胞比例的研究,目前仍需要系列地采集一天内不同时段的组织标本来完成.我们曾对一例恶性表皮癌及周边正常皮肤进行了系列的、跨24小时的研究.结果表明病人在日常光照环境中,其癌症及正常表皮细胞的分裂都是有规律的、时多时少的过程.这种动态的昼夜变化节律毫无疑问在癌症的化疗和放疗中有很大应用价值. 动物和临床实验已证明调整用药时间的重要性和可行性.有一个典型的动物实验可证明药物毒性和用药时间的关系.如果连续6 d在白天或其睡眠期给小鼠注射一定剂量的阿拉伯糖苷(arcC),只有15%的小鼠会因药物毒性而死亡;而如果在晚上或其活动期注射同样剂量的药物,则小鼠的死亡率会增加至75%.用表阿霉素(doxorubicin)和顺铂(cisplatin)治疗晚期卵巢癌的临床研究是另一个例子.对病人采用晚间注射表阿霉素和早晨注射顺铂的治疗方案产生副作用的机率明显高于采用早晨注射表阿霉素和晚间注射顺铂的方案.5氟尿嘧啶(5-Fu)的抗癌疗效和副作用的产生也有很强的时间依赖性.对这种时间依赖性的机制现已有所了解.二氢嘧啶脱氢酶(DPD)是氟嘧啶代谢的限速酶,它可以将5-Fu及代谢衍生物转化成无细胞毒性的产物而排出体外.研究表明DPD在大鼠肝脏中的表达是有昼夜节律的,正是DPD在肝脏有节律地代谢、清除5-Fu及代谢衍生物才使得5-Fu的疗效和副作用有很强的时间依赖性.抗癌药物的发展是一个复杂、代价高昂和充满冒险的过程.许多化合物或衍生物被设计、合成或生产,这些备选药物可能针对细胞周期的不同阶段或机制,其中很多备选药物在层层筛选中因为毒性、特异性或有效性等问题而被弃用.如果一个备选药物被证明有效或完全无效,则取舍的结果将是完全不同的,此时请别忘记考虑用药时间的因素.我们的研究表明人体和癌症之间的平衡存在着日、月或季节的节律性变化.了解和承认这些日、月或季节性变化可帮助我们更好地预防、诊断,更安全、有效地治疗人类的癌症.在我们这个以24/7(24小时/日,7日/周)为时间周期单位的世界里,一个现代化、工业化的世界,不适当的环境污染,包括光的污染(如夜间强光照射)日渐增多.它所造成的人的生物钟节律的紊乱是显著的.遗憾的是只有少数医学科学家重视和正在从事这方面的研究.理解生物时间节律的概念可以更好地预防、诊断和治疗人类癌症.我们邀请更多的中国医学科学研究人员和机构加入到我们这个研究行列,运用时间生物学的概念,为更有效地预防癌症、更早期地诊断癌症、更好地控制和治疗癌症而努力.William J M Hrushesky Jovelyn Du-Quiton Masami Ohmori Patricia A.Wood 2005基础医学与临床2005,25,4:5
15Familial pancreatic cancer: Concept, management and issues显示文摘Familial pancreatic cancer (FPC) is broadly defined as two first-degree-relatives with pancreatic cancer (PC) and accounts for 4%-10% of PC. Several genetic syndromes, including Peutz-Jeghers syndrome, hereditary pancreatitis, hereditary breast-ovarian cancer syndrome(HBOC), Lynch syndrome, and familial adenomatous polyposis (FAP), also have increased risks of PC, but the narrowest definition of FPC excludes these known syndromes. When compared with other familial tumors, proven genetic alterations are limited to a small proportion (<20%) and the familial aggregation is usually modest. However, an ethnic deviation (Ashkenazi Jewish>Caucasian) and a younger onset are common also in FPC. In European countries, 'anticipation' is reported in FPC families, as with other hereditary syndromes; a trend toward younger age and worse prognosis is recognized in the late years. The resected pancreases of FPC kindred often show multiple pancreatic intraepithelial neoplasia (Pan IN) foci, with various K-ras mutations, similar to colorectal polyposis seen in the FAP patients. As with HBOC patients, a patient who is a BRCA mutation carrier with unresectable pancreatic cancer (accounting for 0%-19% of FPC patients) demonstrated better outcome following platinum and Poly (ADP-ribose) polymerase inhibitor treatment. Western countries have established FPC registries since the 1990 s and several surveillance projects for highrisk individuals are now ongoing to detect early PCs. Improvement in lifestyle habits, including non-smoking, is recommended for individuals at risk. 16Characteristics of nanoparticles emitted from burning of biomass fuels显示文摘The characteristics of the particles of the smoke that is emitted from the burning of biomass fuels were experimentally investigated using a laboratory-scale tube furnace and different types of biomass fuels:rubber wood,whole wood pellets and rice husks.Emitted amounts of particles,particle-bound polycyclic aromatic hydrocarbons(PAHs)and water-soluble organic carbon(WSOC)are discussed relative to the size of the emitted particles,ranging to as small as nano-size(<70 nm),and to the rate of heating rate during combustion.differential thermal analysis(DTA)and thermogravimetric analysis(TG)techniques were used to examine the effect of heating rate and biomass type on combustion behaviors relative to the characteristics of particle emissions.In the present study,more than 30%of the smoke particles from the burning of biomass fuel had a mass that fell within a range of<100 nm.Particles smaller than 0.43μm contributed greatly to the total levels of toxic PAHs and WSOC.The properties of these particles were influenced by the fuel component,the combustion conditions,and the particle size.Although TG–DTA results indicated that the heating rate in a range of 10–20°C did not show a significant effect on the combustion properties,there was a slight increase in the decomposition temperature as heating rate was increased.The nano-size particles had the smallest fraction of particle mass and particle-bound PAHs,but nonetheless these particles registered the largest fraction of particle-bound WSOC.Mitsuhiko Hata Jiraporn Chomanee Thunyapat Thongyen Linfa Bao Surajit Tekasakul Perapong Tekasakul Yoshio Otani Masami Furuuchi 2014Journal of Environmental Sciences2014,26,9:5
17Impact of time from diagnosis to chemotherapy in advanced gastric cancer: A Propensity Score Matching Study to Balance Prognostic Factors显示文摘BACKGROUND It is unclear whether treatment delay affects the clinical outcomes of chemotherapy in advanced gastric cancer(A-GC).AIM To assess whether treatment delay affects the clinical outcomes of chemotherapy in A-GC.METHODS This single-center retrospective study examined consecutive patients with A-GC between April 2012 and July 2018. In total, 110 patients with stage Ⅳ A-GC who underwent chemotherapy were enrolled. We defined the wait time(WT) as the interval between diagnosis and chemotherapy initiation. We evaluated the influence of WT on overall survival(OS).RESULTS The mean OS was 303 d. The median WT was 17 d. We divided the patients into early and elective WT groups, with a 2-wk cutoff point. There were 46 and 64 patients in the early and elective WT groups, respectively. Compared with the elective WT group, the early WT group had significantly lower albumin(Alb)levels and higher neutrophil/lymphocyte ratios and C-reactive protein(CRP)levels but not a lower performance status. The elective WT group underwent more combination chemotherapy than did the early WT group. OS was different between the two groups(230 d vs 340 d, respectively). Multivariate analysis revealed that higher CRP levels, lower Alb levels and monotherapy were significantly related to a poor prognosis. 18Effects of Fuscoporia obliqua on Postprandial Glucose Excursion and Endothelial Dysfunction in Type 2 Diabetic Patients显示文摘Postprandial hyperglycemia has been reported to elicit endothelial dysfunction and provoke future cardiovascular complications. A reduction of postprandial blood glucose levels by the glucosidase inhibitor Fuscoporia obliqua was associated with a risk reduction of cardiovascular complications, but the effects of Fuscoporia obliqua on endothelial function have never been elucidated. This study is aimed to assess the efficacy of Fuscoporia obliqua on postprandial metabolic parameters and endothelial function in type 2 diabetic patients. Postprandial peak glucose (14.47±1.27 vs. 8.50±0.53 mmol/liter), plasma glucose excursion (PPGE), and change in the area under the curve (AUC) glucose after a single loading of test meal (total 450 kcal; protein 15.3%; fat 32.3%; carbohydrate 51.4%) were significantly higher in the diet-treated type 2 diabetic patients (n=14) than the age-and sex-matched controls (n=12). The peak forearm blood flow response and total reactive hyperemic flow (flow debt repayment) during reactive hyperemia, indices of resistance artery endothelial function on strain-gauge plethysmography, were unchanged before and after meal loading in the controls. But those of the diabetics were significantly decreased 120 and 240 min after the test meal. A prior administration of Fuscoporia obliqua decreased postprandial peak glucose, PPGE, and AUC glucose. The peak forearm blood flow and flow debt repayment were inversely well correlated with peak glucose, PPGE, and AUC glucose, but not with AUC insulin or the other lipid parameters. Even a single loading of the test meal was shown to impair the endothelial function in type 2 diabetic patients, and the postprandial endothelial dysfunction was improved by a prior use of Fuscoporia obliqua. 19甘蔗提取物对夏季高温环境下育肥猪生长性能的影响显示文摘旨在探究日粮添加甘蔗提取物(SCE)对夏季高温环境下育肥猪生长性能的影响。试验选择120头初始体重为(70±5.0)kg健康的'杜×长×嘉'育肥猪,随机分为2组,每组4个重复,每个重复15头,分别饲喂基础日粮(对照组)和添加SCE的试验日粮(试验组),试验组每周前3 d连续饲喂含1.5%SCE的日粮,后4 d饲喂正常日粮,在夏季高温环境下饲喂4周后结束试验。结果表明:与对照组相比,试验组末重和平均日采食量分别显著增加3.90%和7.88%(P<0.05),平均日增重增加13.79%(P=0.06),血清丙二醛含量显著降低(P<0.05),十二指肠绒毛高度与隐窝深度比值显著提高(P<0.05)。结论:在夏季高温环境下,日粮中添加SCE能提高育肥猪生长性能,改善小肠黏膜上皮形态,降低机体内脂质过氧化水平。夏阳春 李延森 沈祥星 Masami Mizu Toma Furuta 李春梅 2017畜牧与兽医2017,49,5:4
20Combination of two-hour post-endoscopic retrograde cholangiopancreatography amylase levels and cannulation times is useful for predicting post-endoscopic retrograde cholangiopancreatography pancreatitis显示文摘AIM To estimate the efficacy of 2 h post-endoscopic retrograde cholangiopancreatography(ERCP) serum amylase levels and other factors for predicting postERCP pancreatitis.METHODS This was a retrospective,single-center cohort study of consecutive patients who underwent ERCP from January 2010 to December 2013.Serum amylase levels were measured 2 h post-procedure,and patient- and procedure-related pancreatitis(PEP) risk factors wereanalyzed using a logistic model.RESULTS A total of 1520 cases(average age 72 ± 12 years,60% male) were initially enrolled in this study,and 1403 cases(725 patients) were ultimately analyzed after the exclusion of 117 cases.Fifty-five of these cases developed PEP.We established a 2 h serum amylase cutoff level of two times the upper limit of normal for predicting PEP.Multivariate analysis revealed that a cannulation time of more than 13 min [odds ratio(OR) 2.28,95%CI:1.132-4.651,P=0.0210] and 2 h amylase levels greater than the cutoff level(OR=24.1,95%CI:11.56-57.13,P<0.0001) were significant predictive factors for PEP.Forty-seven of the 55 patients who developed PEP exhibited 2 h amylase levels greater than the cutoff level(85%),and six of the remaining eight patients who developed PEP(75%) required longer cannulation times.Only 2 of the 1403 patients(0.14%) who developed PEP did not exhibit concerning 2 h amylase levels or require longer cannulation times.CONCLUSION These findings indicate that the combination of 2 h post-ERCP serum amylase levels and cannulation times represents a valuable marker for identifying patients at high risk for PEP.Shiro Hayashi Tsutomu Nishida Hiromi Shimakoshi Akiyoshi Shimoda Takahiro Amano Aya Sugimoto Kei Takahashi Kaori Mukai Tokuhiro Matsubara Masashi Yamamoto Sachiko Nakajima Koji Fukui Masami Inada 2016World Journal of Gastrointestinal Endoscopy2016,8,20:4
