10Aging and uremia:Is there cellular and molecular crossover?显示文摘Many observers have noted that the morphological changes that occur in chronic kidney disease(CKD) patients resemble those seen in the geriatric population, with strikingly similar morbidity and mortality profiles and rates of frailty in the two groups, and shared characteristics at a pathophysiological level especially in respect to the changes seen in their vascular andimmune systems. However, whilst much has been documented about the shared physical characteristics of aging and uremia, the molecular and cellular similarities between the two have received less attention. In order to bridge this perceived gap we have reviewed published research concerning the common molecular processes seen in aging subjects and CKD patients, with specific attention to altered proteostasis, mitochondrial dysfunction, post-translational protein modification, and senescence and telomere attrition. We have also sought to illustrate how the cell death and survival pathways apoptosis, necroptosis and autophagy are closely interrelated, and how an understanding of these overlapping pathways is helpful in order to appreciate the shared molecular basis behind the pathophysiology of aging and uremia. This analysis revealed many common molecular characteristics and showed similar patterns of cellular dysfunction. We conclude that the accelerated aging seen in patients with CKD is underpinned at the molecular level, and that a greater understanding of these molecular processes might eventually lead to new much needed therapeutic strategies of benefit to patients with renal disease.William E White Muhammad M Yaqoob Steven M Harwood 2015World Journal of Nephrology2015,4,1:1
11Zinc-oxide/PDMS-clad tapered fiber saturable absorber for passively mode-locked erbium-doped fiber laser显示文摘We propose and demonstrate a passively mode-locked erbium-doped fiber laser(EDFL)based on zinc-oxide/polydimethylsiloxane(ZnO/PDMS)saturable absorber(SA)that evanescently interacts with the light on a tapered fiber.The ZnO/PDMS composite is coated on the whole surface of the tapered fiber to guarantee the maximum efficiency of the SA device,with a measured insertion loss of 0.87 dB and a modulation depth of 6.4%.The proposed laser can generate soliton mode-locking operation at a threshold power of 33.07 mW.The generated output pulse yields a repetition rate and pulse width of 9.77 MHz and 1.03 ps,respectively.These results indicate that the proposed ZnO/PDMS-clad tapered fiber could be useful as an efficient,compatible,and low-cost SA device for ultrafast laser applications.F D Muhammad S A S Husin E K Ng K Y Lau C A C Abdullah M A Mahdi 2021Chinese Physics B2021,30,5:0
12Decline in subarachnoid haemorrhage volumes associated with the first wave of the COVID- 19 pandemic显示文摘Background During the COVID-19 pandemic,decreased volumes of stroke admissions and mechanical thrombectomy were reported.The study’s objective was to examine whether subarachnoid haemorrhage(SAH)hospitalisations and ruptured aneurysm coiling interventions demonstrated similar declines.Methods We conducted a cross-sectional,retrospective,observational study across 6 continents,37 countries and 140 comprehensive stroke centres.Patients with the diagnosis of SAH,aneurysmal SAH,ruptured aneurysm coiling interventions and COVID-19 were identified by prospective aneurysm databases or by International Classification of Diseases,10th Revision,codes.The 3-month cumulative volume,monthly volumes for SAH hospitalisations and ruptured aneurysm coiling procedures were compared for the period before(1 year and immediately before)and during the pandemic,defined as 1 March-31 May 2020.The prior 1-year control period(1 March-31 May 2019)was obtained to account for seasonal variation.Findings There was a significant decline in SAH hospitalisations,with 2044 admissions in the 3 months immediately before and 1585 admissions during the pandemic,representing a relative decline of 22.5%(95%CI−24.3%to−20.7%,p<0.0001).Embolisation of ruptured aneurysms declined with 1170-1035 procedures,respectively,representing an 11.5%(95%CI−13.5%to−9.8%,p=0.002)relative drop.Subgroup analysis was noted for aneurysmal SAH hospitalisation decline from 834 to 626 hospitalisations,a 24.9%relative decline(95%CI−28.0%to−22.1%,p<0.0001).A relative increase in ruptured aneurysm coiling was noted in low coiling volume hospitals of 41.1%(95%CI 32.3%to 50.6%,p=0.008)despite a decrease in SAH admissions in this tertile.Interpretation There was a relative decrease in the volume of SAH hospitalisations,aneurysmal SAH hospitalisations and ruptured aneurysm embolisations during the 13Effects of cumulative COVID-19 cases on mental health:Evidence from multi-country survey显示文摘BACKGROUND Depression and anxiety were both ranked among the top 25 leading causes of global burden of diseases in 2019 prior to the coronavirus disease 2019(COVID-19)pandemic.The pandemic affected,and in many cases threatened,the health and lives of millions of people across the globe and within the first year,global prevalence of anxiety and depression increased by 25%with the greatest influx in places highly affected by COVID-19.AIM To explore the psychological impact of the pandemic and resultant restrictions in different countries using an opportunistic sample and online questionnaire in different phases of the pandemic.METHODS A repeated,cross-sectional online international survey of adults,16 years and above,was carried out in 10 countries(United Kingdom,India,Canada,Bangladesh,Ukraine,Hong Kong,Pakistan,Egypt,Bahrain,Saudi Arabia).The online questionnaire was based on published approaches to understand the psychological impact of COVID-19 and the resultant restrictions.Five standardised measures were included to explore levels of depression[patient health questionnaire(PHQ-9)],anxiety[generalized anxiety disorder(GAD)assessment],impact of trauma[the impact of events scale-revised(IES-R)],loneliness(a brief loneliness scale),and social support(The Multidimensional Scale of Perceived Social support).RESULTS There were two rounds of the online survey in 10 countries with 42866 participants in Round 1 and 92260 in Round 2.The largest number of participants recruited from the United Kingdom(112985 overall).The majority of participants reported receiving no support from mental health services throughout the pandemic.This study found that the daily cumulative COVID-19 cases had a statistically significant effect on PHQ-9,GAD-7,and IES-R scores.These scores significantly increased in the second round of surveys with the ordinary least squares regression results with regression discontinuity design specification(to control lockdown effects)confirming these results.The study findings imply that participants’mental health worsened with high cumulative COVID-19 cases.CONCLUSION Whist we are still living through the impact of COVID-19,this paper focuses on its impact on mental health,discusses the possible consequences and future implications.This study revealed that daily cumulative COVID-19 cases have a significant impact on depression,anxiety,and trauma.Increasing cumulative cases influenced and impacted education,employment,socialization and finances,to name but a few.Building a database of global evidence will allow for future planning of pandemics,particularly the impact on mental health of populations considering the cultural differences.Shanaya Rathod Saseendran Pallikadavath Elizabeth Graves Mohammad M Rahman Ashlea Brooks Pranay Rathod Rachna Bhargava Muhammad Irfan Reham Aly Haifa Mohammad Saleh Al Gahtani Zahwa Salam Steven Wai Ho Chau Theone S E Paterson Brianna Turner Viktoria Gorbunova Vitaly Klymchuk Peter Phiri 2023World Journal of Psychiatry2023,13,7:0
14Deceased donor organ procurement injuries in the United States显示文摘AIM: To determine the incidence of surgical injury during deceased donor organ procurements. METHODS: Organ damage was classified into three tiers, from 1-3, with the latter rendering the organ nontransplantable. For 12 consecutive months starting in January of 2014, 36 of 58 organ procurement organization's(OPO)'s prospectively submitted quality data regarding organ damage(as reported by the transplanting surgeon and confirmed by the OPO medical director) seen on the procured organ.RESULTS: These 36 OPOs recovered 5401 of the nations' s 8504 deceased donors for calendar year 2014.A total of 19043 organs procured were prospectively analyzed. Of this total, 59 organs sustained damage making them non-transplantable(0 intestines; 4 pancreata; 5 lungs; 6 livers; 43 kidneys). 15Colorectal cancer in Arab world:A systematic review显示文摘BACKGROUND The incidence of colorectal cancer(CRC)is increasing among young individuals in the Arab world as well as in other regions of the world.AIM To explore the incidence and prevalence of CRC in the Arab world.METHODS The PubMed,Scopus,Web of Science,EBSCO and Wiley databases were searched to retrieve relevant articles irrespective of the language or the publication year.The search terms were('colon OR rectum OR sigmoid OR rectal OR colonic OR colorectal')AND('cancer OR malignancy OR malignant OR neoplasm')AND('Jordan'OR'United Arab Emirates'OR'Bahrain'OR'Tunisia'OR'Algeria'OR'Djibouti'OR'Saudi Arabia'OR'Sudan'OR'Syria'OR'Somalia'OR'Iraq'OR'Oman'OR'Palestine'OR'Qatar'OR'Comoros'OR'Kuwait'OR'Lebanon'OR'Libya'OR'Egypt'OR'Morocco'OR'Mauritania'OR'Yemen').Reviews,metaanalyses,and articles containing nonoriginal data were excluded.Retrieved articles were screened,and relevant data were extracted.Descriptive statistics were used for data analysis.RESULTS Nine studies were included.Five of the studies provided information regarding the prevalence of CRC.The prevalence of CRC was 0.72% in Saudi Arabia and 0.78% in the United Arab Emirate,while in Egypt,it ranged from 0.4% to 14%.Four studies showed information regarding the incidence.The annual incidence rate of CRC in Qatar was 7.5/100000/year.In Egypt,the crude incidence rate(CIR)in males was 3.1 for colon cancer and 1 for rectal cancer,while in females,it was 2.3 for colon cancer and 0.8 for rectal cancer.The age-standardized rate for CRC incidence in 2003 was 36.90 for males,26.50 for females,and 30.49 for both sexes in Saudi Arabia.In 2016,the CIRs in Saudi Arabia were 3.6 and 2.1 in females for colon cancer and rectal cancer,respectively,while in males,it was 3.3 and 2.8 for colon cancer and rectal cancer,respectively.One study in Egypt revealed that 25% of CRC cases occurred among individuals younger than 40 years old.CONCLUSION There is a considerable prevalence of CRC in some Arab countries.More studies are needed to explore the incidence and prevalence of CRC in the rest of the Arab world.Nahed A Makhlouf Muhammad Abdel-Gawad Aya M Mahros Sameh A Lashen Mariam Zaghloul Ahmed Eliwa Eman E Elshemy Zainab Ali-Eldin Doaa Abdeltawab Fathiya El-Raey Dalia Omran Marwa Khalaf Nancy Fanous Ahmed Shawkat Abdelmohsen Ahmed Abu-Elfatth Mohamed Abdelghani Mahmoud Farouk Mohamed Abdelaziz Mohamed Alboraie 2021World Journal of Gastrointestinal Oncology2021,13,11:0
