| 1 | Is it time to replace propranolol with carvedilol for portal hypertension?显示文摘Beta-adrenergic receptor antagonists(β-blockers) have been well established for use in portal hypertension for more than three decades. Different Non-selective β-blockers like propranolol, nadolol, timolol, atenolol, metoprolol and carvedilol have been in clinical practice in patients with cirrhosis. Carvedilol has proven 2-4 times more potent than propranolol as a beta-receptor blocker in trials conducted testing its efficacy for heart failure. Whether the same effect extends to its potency in the reduction of portal venous pressures is a topic of on-going debate. The aim of this review is to compare the hemodynamic and clinical effects of carvedilol with propranolol, and attempt assess whether carvedilol can be used instead of propranolol in patients with cirrhosis. Carvedilol is a promising agent among the beta blockers of recent time that has shown significant effects in portal hypertension hemodynamics. It has also demonstrated an effective profile in its clinical application specifically for the prevention of variceal bleeding. Carvedilol has more potent desired physiological effects when compared to Propranolol. However, it is uncertain at the present juncture whether the improvement in hemodynamics also translates into a decreased rate of disease progression and complications when compared to propranolol. | 2 | Systematic review of oncological outcomes following laparoscopic vs open total mesorectal excision显示文摘AIM: To systematically analyze the randomized trials comparing the oncological and clinical effectiveness of laparoscopic total mesorectal excision(LTME) vs open total mesorectal excision(OTME) in the management of rectal cancer.METHODS: Published randomized, controlled trials comparing the oncological and clinical effectiveness of LTME vs OTME in the management of rectal cancer were retrieved from the standard electronic medical databases. The data of included randomized, controlled trials was extracted and then analyzed according to the principles of meta-analysis using RevMan? statistical software. The combined outcome of the binary variables was expressed as odds ratio(OR) and the combined outcome of the continuous variables waspresented in the form of standardized mean difference(SMD). RESULTS: Data from eleven randomized, controlled trials on 2143 patients were retrieved from the electronic databases. There was a trend towards the higher risk of surgical site infection(OR = 0.66; 95%CI: 0.44-1.00; z = 1.94; P < 0.05), higher risk of incomplete total mesorectal resection(OR = 0.62; 95%CI: 0.43-0.91; z = 2.49; P < 0.01) and prolonged length of hospital stay(SMD,-1.59; 95%CI:-0.86--0.25; z = 4.22; P < 0.00001) following OTME. However, the oncological outcomes like number of harvested lymph nodes, tumour recurrence and risk of positive resection margins were statistically similar in both groups. | 3 | Validating a point of care lactate meter in adult patients with sepsis presenting to the emergency department of a tertiary care hospital of a low-to middle-income country显示文摘BACKGROUND: Timely identifi cation of high blood lactate levels in septic patients may allow faster detection of those patients requiring immediate resuscitation. Point-of-care(POC) testing is being increasingly utilized in the emergency department(ED). We examined the accuracy and timesaving effect of a handheld POC lactate device for the measurement of fingertip and whole blood lactate as compared with reference laboratory blood testing in septic ED patients.METHODS: A convenience sample of adult ED patients receiving serum lactate testing were enrolled prospectively in the ED of a multidisciplinary tertiary care hospital serving the population of one of the major cities of Pakistan. Participants underwent fi ngertip POC lactate measurement with a portable device and simultaneous whole blood sampling for analysis by both the POC lactate device and standard laboratory method. Lactate measurements were compared by intraclass correlation(ICC) and Bland and Altman plots.RESULTS: Forty-three septic patients were included in the study. The fingertip POC & whole blood POC lactate measurements each correlated tightly with the reference method(ICC=0.93 & ICC=0.92, respectively). | 4 | e-antigen-negative chronic hepatitis B in Bangladesh显示文摘BACKGROUND:Bangladesh is a densely populated country where about 10 million people are chronically infected with hepatitis B virus(HBV).The aim of the present study was to evaluate the biochemical,virological and histological characteristics of HBeAg-negative chronic hepatitis B(CHB). METHODS:Patients were included in this study if they were chronically infected with HBV with detectable DNA.The patients who were co-infected with human immunodeficiency virus,hepatitis delta virus or hepatitis C virus,and previously subjected to antiviral treatment,and those with hepatocellular carcinoma were excluded.The study was conducted during the period of January 2001 to December 2007.During this period 2617 patients with CHB were studied.HBeAg-positive cases were included to compare the characteristics.Among them,237 cases underwent liver biopsy. RESULTS:2296 patients(87.7%)were male,with a mean age of 28.9±13.7 years.2375 patients(90.8%)had CHB,and 242(9.2%)were cirrhotic.HBV DNA levels were 7.6±1.5 copies/ml,ALT was 111.3±212.5 U/L,and AST was 91.5± 148.9 U/L.The number of HBeAg-negative CHB cases was 1039(39.7%).HBeAg-negative patients with a lower DNA load were older,and they had more fibrotic changes in the liver than HBeAg-positive patients.The two groups did not differ in necroinflammatory activity,but the former had lower ALT and AST values.Cirrhosis was more common in e-antigen-negative patients.CONCLUSIONS:e-antigen-negative CHB patients are older and have more hepatic fibrosis patients than HBeAg-positive patients,although they have similar necroinflammatory activity. | Nooruddin Ahmad Shahinul Alam Golam Mustafa Abul Barkat Muhammad Adnan Rahat Hasan Baig Mobin Khan | 2008 | Hepatobiliary & Pancreatic Diseases International2008,7,4: | 6 |
| 5 | Use of health-related quality of life tools in hepatobiliary surgery显示文摘BACKGROUND:The objective of this article is to review the literature and discuss the various tools used in hepatobiliary surgery for the measurement of health related quality of life (HR-QOL) and highlight various outcome variables that affect the HR-QOL among patients with common hepatobiliary disorders. DATA SOURCES: We reviewed HR-QOL articles published in the last 20 years on different hepatobiliary curative or palliative procedures in all languages. RESULTS: HR-QOL is a questionnaire tool which is utilized to assess the changes in the health status of patients after a hepatobiliary intervention. These surveys are of increasingly importance, as health care providers are challenged to justify treatment approaches and rationale for any surgical intervention. These HR-QOL tools are very helpful for the evaluation of subjective outcome of common hepatobiliary procedures like gastrointestinal quality of life index (GIQLI) for cholecystectomy, functional assessment in cancer therapy (FACT) for liver resection, short form 36 (SF-36) for liver transplantation, and quality of life questionnaire for patients with pancreatic cancer (QLQ-PAN). CONCLUSIONS: Use of validated and reliable health instruments in hepatobiliary surgery is directed at measuring the impact in a reproducible and valid fashion. Curative or palliative procedures should be offered to the patients of hepatobiliary disorders after the assessment by HR-QOL tools. | 6 | Robotic total meso-rectal excision for rectal cancer: A systematic review following the publication of the ROLARR trial显示文摘AIM To compare outcomes in patients undergoing rectal resection by robotic total meso-rectal excision(RTME) vs laparoscopic total meso-rectal excision(LTME).METHODS Standard medical electronic databases such as Pub-Med, MEDLINE, EMBASE and Scopus were searched to find relevant articles. The data retrieved from all types of included published comparative trials in pati-ents undergoing RTME vs LTME was analysed using the principles of meta-analysis. The operative, post-operative and oncological outcomes were evaluated to assess the effectiveness of both techniques of TME. The summated outcome of continuous variables was expressed as standardized mean difference(SMD) and dichotomous data was presented in odds ratio(OR).RESULTS One RCT(ROLARR trial) and 27 other comparative studies reporting the non-oncological and oncological outcomes following RTME vs LTME were included in this review. In the random effects model analysis using the statistical software Review Manager 5.3, the RTME was associated with longer operation time(SMD, 0.46; 95%CI: 0.25, 0.67; z = 4.33; P = 0.0001), early passage of first flatus(P = 0.002), lower risk of conversion(P = 0.00001) and shorter hospitalization(P = 0.01). The statistical equivalence was seen between RTME and LTME for non-oncological variables like blo-od loss, morbidity, mortality and re-operation risk. The oncological variables such as recurrence(P = 0.96), number of harvested nodes(P = 0.49) and positive circumferential resection margin risk(P = 0.53) were also comparable in both groups. The length of distal resection margins was similar in both groups. | 7 | Strength of Concrete Filled Steel Tubular Columns显示文摘Composite columns of steel and concrete have been used and studied world wide, but filled tubu- lar columns need more attention. This paper presents an experimental study on the behavior of short con- crete filled steel tubular columns (CFT) axially loaded in compression to failure. A total of 28 specimens (16 were filled with concrete and 12 were kept hollow) with different cross-sections were tested to investigate the load capacity. The length-to-diameter ratios of these columns were between 4 and 9. Parameters for the tests were tube shape and diameter-to-thickness ratio. Some of the concrete filled columns had internal bracing of #3 deformed bars. The test results are compared with the theoretical results and previous studies. The results show that the confinement effect on concrete does play a role in increasing the compressive strengths to almost 60% in some cases. | 8 | An Investigation of Landslide Susceptibility Using Logistic Regression and Statistical Index Methods in Dailekh District, Nepal显示文摘Landslide distribution and susceptibility mapping are the fundamental steps for landslide-related hazard and disaster risk management activities, especially in the Himalaya region which has resulted in a great deal of death and damage to property. To better understand the landslide condition in the Nepal Himalaya, we carried out an investigation on the landslide distribution and susceptibility using the landslide inventory data and 12 different contributing factors in the Dailekh district, Western Nepal. Based on the evaluation of the frequency distribution of the landslide, the relationship between the landslide and the various contributing factors was determined.Then, the landslide susceptibility was calculated using logistic regression and statistical index methods along with different topographic(slope, aspect, relative relief, plan curvature, altitude, topographic wetness index) and non-topographic factors(distance from river, normalized difference vegetation index(NDVI), distance from road, precipitation, land use and land cover, and geology), and 470(70%) of total 658 landslides. The receiver operating characteristic(ROC) curve analysis using 198(30%) of total landslides showed that the prediction curve rates(area under the curve, AUC) values for two methods(logistic regression and statistical index) were 0.826, and 0.823with success rates of 0.793, and 0.811, respectively. The values of R-Index for the logistic regression and statistical index methods were83.66 and 88.54, respectively, consisting of high susceptible hazard classes. In general, this research concluded that the cohesive and coherent natural interplay of topographic and non-topographic factors strongly affects landslide occurrence, distribution, and susceptibility condition in the Nepal Himalaya region. | 9 | Exploring the potential of Sentinel-2A satellite data for aboveground biomass estimation in fragmented Himalayan subtropical pine forest显示文摘The Sentinel-2 A satellite having embedded advantage of red edge spectral bands offers multispectral imageries with improved spatial,spectral and temporal resolutions as compared to the other contemporary satellites providing medium resolution data.Our study was aimed at exploring the potential of Sentinel-2 A imagery to estimate Above Ground Biomass(AGB) of Subtropical Pine Forest in Pakistan administered Kashmir.We developed an AGB predictive model using field inventory and Sentinel 2 A based spectral and textural parameters along with topographic features derived from ALOS Digital Elevation Model(DEM).Field inventory data was collected from 108 randomly distributed plots(0.1 ha each) across the study area.The stepwise linear regression method was employed to investigate the potential relationship between field data and corresponding satellite data.Biomass and carbon mapping of the study area was carried out through established AGB estimation model with R(o.86),R2(0.74),adjusted R2(0.72) and RMSE value of 33 t/ha.Our results showed that first order textures(mean,standard deviation and variance) significantly contributed in AGB predictive modeling while only one spectral band ratio made contribution from spectral domain.Our study leads to the conclusion that Sentinel-2 A optical data is a potential source for AGB estimation in subtropical pine forest of the area of interest with added benefit of its free of cost availability,higher quality data and long-term continuity that can be utilized for biomass carbon distribution mapping in the resource constraint study area for sustainable forest management. | Mobushir Riaz KHANI Iftikhar Ahmad KHAN Muhammad Hasan Ali BAIG LIU Zheng-jia Muhammad Irfan ASHRAF | 2020 | Journal of Mountain Science2020,17,12: | 2 |

| 12 | Early vs late cholecystectomy in mild gall stone pancreatitis: Anupdated meta-analysis and review of literature显示文摘BACKGROUND Gallstone pancreatitis is one of the most common causes of acute pancreatitis.Cholecystectomy remains the definitive treatment of choice to prevent recurrence.The rate of early cholecystectomies during index admission remains low due toperceived increased risk of complications.AIMTo compare outcomes including length of stay, duration of surgery, biliarycomplications, conversion to open cholecystectomy, intra-operative, and postoperativecomplications between patients who undergo cholecystectomy duringindex admission as compared to those who undergo cholecystectomy thereafter.METHODSStatistical Method: Pooled proportions were calculated using both Mantel-Haenszel method (fixed effects model) and DerSimonian Laird method (randomeffects model).RESULTSInitial search identified 163 reference articles, of which 45 were selected andreviewed. Eighteen studies (n = 2651) that met the inclusion criteria were includedin this analysis. Median age of patients in the late group was 43.8 years while thatin the early group was 43.6. Pooled analysis showed late laparoscopiccholecystectomy group was associated with an increased length of stay by 88.96 h(95%CI: 86.31 to 91.62) as compared to early cholecystectomy group. Pooled riskdifference for biliary complications was higher by 10.76% (95%CI: 8.51 to 13.01) in the late cholecystectomy group as compared to the early cholecystectomy group.Pooled analysis showed no risk difference in intraoperative complications [riskdifference: 0.41%, (95%CI: -1.58 to 0.75)], postoperative complications [riskdifference: 0.60%, (95%CI: -2.21 to 1.00)], or conversion to open cholecystectomy[risk difference: 1.42%, (95%CI: -0.35 to 3.21)] between early and latecholecystectomy groups. Pooled analysis showed the duration of surgery to beprolonged by 39.11 min (95%CI: 37.44 to 40.77) in the late cholecystectomy groupas compared to the early group.CONCLUSIONIn patients with mild gallstone pancreatitis early cholecystectomy leads to shorterhospital stay, shorter duration of surgery, while decreasing the risk of biliarycomplications.
