| 2 | Upper-gastrointestinal bleeding secondary to peptic ulcer disease:Incidence and outcomes显示文摘AIM:To evaluate the incidence,surgery,mortality,and readmission of upper gastrointestinal bleeding(UGIB)secondary to peptic ulcer disease(PUD).METHODS:Administrative databases identified all hospitalizations for UGIB secondary to PUD in Alberta,Canada from 2004 to 2010(n=7079)using the International Classification of Diseases Codes(ICD-10).A subset of the data was validated using endoscopy reports.Positive predictive value and sensitivity with 95%confidence intervals(CI)were calculated.Incidence of UGIB secondary to PUD was calculated.Logistic regression was used to evaluate surgery,in-hospital mortality,and 30-d readmission to hospital with recurrent UGIB secondary to PUD.Co-variants accounted for in our logistic regression model included:age,sex,area of residence(i.e.,urban vs rural),number of Charlson comorbidities,presence of perforated PUD,undergoing upper endoscopy,year of admission,and interventional radiological attempt at controlling bleeding.A subgroup analysis(n=6356)compared outcomes of patients with gastric ulcers to those with duodenal ulcers.Adjusted estimates are presented as odds ratios(OR)with95%CI.RESULTS:The positive predictive value and sensitivity of ICD-10 coding for UGIB secondary to PUD were85.2%(95%CI:80.2%-90.2%)and 77.1%(95%CI:69.1%-85.2%),respectively.The annual incidence between 2004 and 2010 ranged from 35.4 to 41.2 per100000.Overall risk of surgery,in-hospital mortality,and 30-d readmission to hospital for UGIB secondary to PUD were 4.3%,8.5%,and 4.7%,respectively.Interventional radiology to control bleeding was performed in 0.6%of patients and 76%of these patients avoided surgical intervention.Thirty-day readmission significantly increased from 3.1%in 2004 to 5.2%in 2010(OR=1.07;95%CI:1.01-1.14).Rural residents(OR rural vs urban:2.35;95%CI:1.83-3.01)and older individuals(OR≥65 vs<65:1.57;95%CI:1.21-2.04)were at higher odds of being readmitted to hospital.Patients with duodenal ulcers had higher odds of dying(OR=1.27;95%CI:1.05-1.53),requiring surgery(OR=1.73;95%CI:1.34-2.23),and being readmitted to hospital(OR=1.54;95%CI:1.19-1.99)when compared to gastric ulcers.CONCLUSION:UGIB secondary to PUD,particularly duodenal ulcers,was associated with significant morbidity and mortality.Early readmissions increased over time and occurred more commonly in rural areas. | Samuel Quan Alexandra Frolkis Kaylee Milne Natalie Molodecky Hong Yang Elijah Dixon Chad G Ball Robert P Myers Subrata Ghosh Robert Hilsden Sander Veldhuyzen van Zanten Gilaad G Kaplan | 2014 | World Journal of Gastroenterology2014,20,46: | 21 |
| 3 | Molecular alterations in gastric cancer with special reference to the early-onset subtype显示文摘Currently, gastric cancer(GC) is one of the most frequently diagnosed neoplasms, with a global burden of 723000 deaths in 2012. It is the third leading cause of cancer-related death worldwide. There are numerous possible factors that stimulate the procarcinogenic activity of important genes. These factors include genetic susceptibility expressed in a singlenucleotide polymorphism, various acquired mutations(chromosomal instability, microsatellite instability, somatic gene mutations, epigenetic alterations) and environmental circumstances(e.g., helicobcter pylori infection, EBV infection, diet, and smoking). Most of the aforementioned pathways overlap, and authors agree that a clear-cut pathway for GC may not exist. Thus, the categorization of carcinogenic events is complicated. Lately, it has been claimed that research on early-onset gastric carcinoma(EOGC) and hereditary GC may contribute towards unravelling some part of the mystery of the GC molecular pattern because young patients are less exposed to environmental carcinogens and because carcinogenesis in this setting may be more dependent on genetic factors. The comparison of various aspects that differ and coexist in EOGCs and conventional GCs might enable scientists to: distinguish which features in the pathway of gastric carcinogenesisare modifiable, discover specific GC markers and identify a specific target. | 4 | Use of Farmers’ Indicators to Evaluate the Sustainability of Cropping Systems on Sloping Land in Yunnan Province, China显示文摘Diversity in the biophysical and socio-economic attributes of agricultural systems makes them uniquely niche based. Farmers are expert in local biophysical and socio-economic situations and can contribute in developing pragmatic indicators of agro-environmental development. During evaluation of an agricultural research project in Yunnan, China, local farmers were capable of evaluating the effects of modified technologies on existing cropping systems and discussed their attitudes to the interventions using their own indicators. Farmers’ response can be grouped into seven major aspects: i) effects on income, ii) effects on production resources, iii) effects on crop management, iv) existing local knowledge about the technology, v) availability of inputs, vi) access to information, and vii) socio-economic conditions of farming households. Farmers concluded that environmental conditions in the experimental catchment in comparison to an adjacent untreated catchment were better in terms of soil and water losses, vegetation cover and natural resources, infrastructures and catchment management, use of environmentally-friendly technologies, and crop productivity. Success in soil and water conservation programmes depends on the efforts of the farmers and other local users and their greater involvement helps to identify more pragmatic indicators. Furthermore, it increases ownership of the programme, enhances interactions with the project scientists, increases farmers’ awareness of agro-environmental problems and their possible consequences. These development will enable scientists to develop better targeted interventions and increase the likelihood of adoption of tested technologies by local communities. | 6 | Population genetics and breeding system of Tupistra pingbianensis (Liliaceae),a naturally rare plant endemic to SW China显示文摘The levels and partitioning of genetic diversity and inbreeding depression were investigated in Tupistra pingbianensis,a narrow endemic of southeast Yunnan,China,characterized by a naturally fragmented distribution due to extreme specialization on a rare habitat type.Here genetic diversity and patterns of genetic variation within and among 11 populations were analyzed using amplified fragment length polymorphism markers with 97 individuals across its whole geographical range.High levels of genetic variation were revealed both at the species level(P 99 = 96.012%;H t = 0.302) and at the population level(P 99 = 51.41%;H s = 0.224).Strong genetic differentiation among populations was also detected(F ST = 0.2961;θⅡ = 0.281),which corresponded to results reported for typical animal-pollinated,mixed selfing,and outcrossing plant species.This result was consistent with mating patterns detected by our pollination experiments.The indirect estimate of gene flow based on θⅡ was low(N m = 0.64).Special habitat and its life history traits might play an important role in shaping the genetic diversity and the genetic structure of this species.A pollination experiment also failed to detect significant inbreeding depression upon F 1 fruit set,seed weight,and germinate rate fitness-traits.As a naturally rare species,T.pingbianensis is not seriously genetically impoverished and likely to have adapted to tolerating a high level of inbreeding early in its history,we propose this species need only periodic monitoring to ensure their continued persistence,but not intervention to remain viable. | Qin QIAO Chang-Qin ZHANG Richard Ian MILNE | 2010 | Journal of Systematics and Evolution2010,48,1: | 3 |

| 16 | An Awareness-Adoption Matrix for Strategic Decision Making in Agricultural Development Projects: A Case Study in Yunnan Province, China显示文摘Significant achievements have been made in generating sustainable agricultural technologies in developing countries. Usually, these new technologies have been more effective in increasing production and productivity than existing technologies. However, many new technologies considered ‘effective’ have not been successful in alleviating the associated sustainability problems, due to poor adoption by targeted users. Success of any novel technical intervention should be judged on the basis of how widespread adoption is in the target area by targeted users. In this context, a case study identified the factors affecting farmers’ adoption of improved technologies extended by an agricultural development project in a rural Village in Yunnan Province, China. Initial adoption/adaptation of project technologies was influenced by farmers’ awareness of the technology. Farmers were more aware of polythene mulch, contour cultivation, intercropping and tree planting technologies than others. This led to a comparatively high initial uptake of polythene mulch, contour cultivation, sweet chestnut, and intercropping technologies. Farmers had inadequate knowledge about some Project technologies, the adoption of which was particularly low. This reveals the need for increasing farmers’ awareness about the rationale for Project technologies to achieve greater adoption/adaptation of project technologies by farmers over wider areas. Farmers’ testing of the technology leads to better adoption/adaptation compared to mere awareness. However, considerable time and resources are required for testing/trying technologies. This justifies the usefulness of comparatively quick and less resource demanding options for awareness creation. Development of effective cropping technologies is important for sustainable agricultural development. The success of any agricultural development project, especially in terms of improving sustainability, depends on how widely those improved technologies are adopted/adapted by farmers in the targeted region. Therefore, farmers’ adoption of technologies should be a key criterion for judging the success of any project. The awareness-adoption matrix is a useful tool to guide research and development projects to achieve expected adoption/adaptation of technologies.
