1Factors influencing health-related quality of life in chronic liver disease显示文摘AIM: To investigate the factors contributing to health- related quality of life (HRQL) in chronic liver disease (CLD). METHODS: Patients with CLD and age- and sex- matched normal subjects performed the validated Thai versions of the short-form 36 (SF-36) by health survey and chronic liver disease questionnaire (CLDQ). Stepwise multiple regression analysis was used to assess the impact of disease severity, demography, causes of CLD, socioeconomic factors, and self-rating health perception on HRQL. RESULTS: Two-hundred and fifty patients with CLD and fifty normal subjects were enrolled into the study. Mean age and the numbers of low educated, unemployed, blue-collar career and poor health perception increased significantly from chronic hepatitis to Child’s Classes A to B to C. Advanced stage of CLD was related to deterioration of HRQL. Increasing age and female reduced physical health area. Low socioeconomic factors and financial burden affected multiple areas of HRQL. In overall, the positive impact of self-rating health perception on HRQL was consistently showed. CONCLUSION: Advanced stages of chronic liver disease, old age, female sex, low socioeconomic status and financial burden are important factors reducing HRQL. 2Molecular alterations in gastric cancer with special reference to the early-onset subtype显示文摘Currently, gastric cancer(GC) is one of the most frequently diagnosed neoplasms, with a global burden of 723000 deaths in 2012. It is the third leading cause of cancer-related death worldwide. There are numerous possible factors that stimulate the procarcinogenic activity of important genes. These factors include genetic susceptibility expressed in a singlenucleotide polymorphism, various acquired mutations(chromosomal instability, microsatellite instability, somatic gene mutations, epigenetic alterations) and environmental circumstances(e.g., helicobcter pylori infection, EBV infection, diet, and smoking). Most of the aforementioned pathways overlap, and authors agree that a clear-cut pathway for GC may not exist. Thus, the categorization of carcinogenic events is complicated. Lately, it has been claimed that research on early-onset gastric carcinoma(EOGC) and hereditary GC may contribute towards unravelling some part of the mystery of the GC molecular pattern because young patients are less exposed to environmental carcinogens and because carcinogenesis in this setting may be more dependent on genetic factors. The comparison of various aspects that differ and coexist in EOGCs and conventional GCs might enable scientists to: distinguish which features in the pathway of gastric carcinogenesisare modifiable, discover specific GC markers and identify a specific target. 4Evaluation of an erosion-sediment transport model for a hillslope using laboratory flume data显示文摘Climate change can escalate rainfall intensity and cause further increase in sediment transport in arid lands which in turn can adversely affect water quality. Hence, there is a strong need to predict the fate of sediments in order to provide measures for sound erosion control and water quality management. The presence of microtopography on hillslopes influences processes of runoff generation and erosion, which should be taken into account to achieve more accurate modelling results. This study presents a physically based mathematical model for erosion and sediment transport coupled to one-dimensional overland flow equations that simulate rainfall-runoff generation on the rill and interrill areas of a bare hillslope. Modelling effort at such a fine resolution considering the flow connection between interrill areas and rills is rarely verified. The developed model was applied on a set of data gathered from an experimental setup where a 650 cm×136 cm erosion flume was pre-formed with a longitudinal rill and interrill having a plane geometry and was equipped with a rainfall simulator that reproduces natural rainfall characteristics. The flume can be given both longitudinal and lateral slope directions. For calibration and validation, the model was applied on the experimental results obtained from the setup of the flume having 5% lateral and 10% longitudinal slope directions under rainfall intensities of 105 and 45 mm/h, respectively. Calibration showed that the model was able to produce good results based on the R2(0.84) and NSE(0.80) values. The model performance was further tested through validation which also produced good statistics(R2=0.83, NSE=0.72). 5Early-onset gastric cancer:Learning lessons from the young显示文摘There is by no means a clear-cut pattern of mutations contributing to gastric cancers,and gastric cancer research can be hampered by the diversity of factors that can induce gastric cancer,such as Helicobacter pylori infection,diet,ageing and other environmental factors.Tumours are unquestionably riddled with genetic changes yet we are faced with an unsolvable puzzle with respect to a temporal relationship.It is postulated that inherited genetic factors may be more important in early-onset gastric cancer (EOGC) than in gastric cancers found in older patients as they have less exposure to environmental carcinogens.EOGC,therefore,could provide a key to unravelling the genetic changes in gastric carcinogenesis.Gastric cancers occurring in young patients provide an ideal background on which to try and uncover the initiating stages of gastric carcinogenesis.This review summarizes the literature regarding EOGC and also presents evidence that these cancers have a unique molecular-genetic phenotype,distinct from conventional gastric cancer.Anya N Milne G Johan A Offerhaus 2010World Journal of Gastrointestinal Oncology2010,2,2:3
