1Portal vein thrombosis:Insight into physiopathology,diagnosis,and treatment显示文摘Portal vein thrombosis (PVT) is a relatively common complication in patients with liver cirrhosis, but might also occur in absence of an overt liver disease. Several causes, either local or systemic, might play an important role in PVT pathogenesis. Frequently, more than one risk factor could be identified; however, occasionally no single factor is discernable. Clinical examination, laboratory investigations, and imaging are helpful to provide a quick diagnosis, as prompt treatment might greatly affect a patient's outcome. In this review, we analyze the physiopathological mechanisms of PVT development, together with the hemodynamic and functional alterations related to this condition. Moreover, we describe the principal factors most frequently involved in PVT development and the recent knowledge concerning diagnostic and therapeutic procedures. 2Hepatic echinococcosis:Clinical and therapeutic aspects显示文摘Echinococcosis or hydatid disease(HD)is a zoonosis caused by the larval stages of taeniid cestodes belonging to the genus Echinococcus.Hepatic echinococcosis is a life-threatening disease,mainly differentiated into alveolar and cystic forms,associated with Echinoccus multilocularis(E.multilocularis)and Echinococcus granulosus(E.granulosus)infection,respectively.Cystic echinococcosis(CE)has a worldwide distribution,while hepatic alveolar echinococcosis(AE)is endemic in the Northern hemisphere,including North America and several Asian and European countries,like France,Germany and Austria.E.granulosus young cysts are spherical,unilocular vesicles,consisting of an internal germinal layer and an outer acellular layer.Cyst expan-sion is associated with a host immune reaction and the subsequent development of a fibrous layer,called the pericyst;old cysts typically present internal septations and daughter cysts.E.multilocularis has a tumorlike,infiltrative behavior,which is responsible for tissue destruction and finally for liver failure.The liver is the main site of HD involvement,for both alveolar and cystic hydatidosis.HD is usually asymptomatic for a long period of time,because cyst growth is commonly slow;the most frequent symptoms are fatigue and abdominal pain.Patients may also present jaundice,hepatomegaly or anaphylaxis,due to cyst leakage or rupture.HD diagnosis is usually accomplished with the combined use of ultrasonography and immunodiagnosis;furthermore,the improvement of surgical techniques,the introduction of minimally invasive treatments[such as puncture,aspiration,injection,re-aspiration(PAIR)]and more effective drugs(such as benzoimidazoles)have deeply changed life expectancy and quality of life of patients with HD.The aim of this article is to provide an up-todate review of biological,diagnostic,clinical and therapeutic aspects of hepatic echinococcosis.Giuseppe Nunnari Marilia R Pinzone Salvatore Gruttadauria Benedetto M Celesia Giordano Madeddu Giulia Malaguarnera Piero Pavone Alessandro Cappellani Bruno Cacopardo 2012World Journal of Gastroenterology2012,18,13:72
3Oxidative stress, cardiolipin and mitochondrial dysfunction in nonalcoholic fatty liver disease显示文摘Nonalcoholic fatty liver disease(NAFLD) is today considered the most common form of chronic liver disease, affecting a high proportion of the population worldwide. NAFLD encompasses a large spectrum of liver damage, ranging from simple steatosis to steatohepatitis, advanced fibrosis and cirrhosis. Obesity, hyperglycemia, type 2 diabetes and hypertriglyceridemia are the most important risk factors. The pathogenesis of NAFLD and its progression to fibrosis and chronic liver disease is still unknown. Accumulating evidence indicates that mitochondrial dysfunction plays a key role in the physiopathology of NAFLD, although the mechanisms underlying this dysfunction are still unclear. Oxidative stress is considered an important factor in producing lethal hepatocyte injury associated with NAFLD. Mitochondrial respiratory chain is the main subcellular source of reactive oxygen species(ROS), which may damage mitochondrial proteins, lipids and mitochondrial DNA. Cardiolipin, a phospholipid located at the level of the inner mitochondrial membrane, plays an important role in several reactions and processes involved in mitochondrial bioenergetics as well as in mitochondrial dependent steps of apoptosis. This phospholipid is particularly susceptible to ROS attack. Cardiolipin peroxidation has been associated with mitochondrial dysfunction in multiple tissues in several physiopathological conditions, including NAFLD. 4Current trends in management of hepatitis B virus reactivation in the biologic therapy era显示文摘Hepatitis B virus (HBV) reactivation represents an emerging cause of liver disease in patients undergoing treatment with biologic agents. In particular, the risk ofHBV reactivation is heightened by the use monoclonalantibodies, such as rituximab (anti-CD20) and alemtuzumab (anti-CD52) that cause profound and longlasting immunosuppression. Emerging data indicatethat HBV reactivation could also develop following theuse of other biologic agents, such as tumor necrosis factor (TNF)-α inhibitors. When HBV reactivation is di-agnosed, it is mandatory to suspend biologic treatmentand start antiviral agents immediately. However, preemptive antiviral therapy prior to monoclonal antibodyadministration is crucial in preventing HBV reactivationand its clinical consequences. Several lines of evidencehave shown that risk of HBV reactivation is greatlyreduced by the identifi cation of high-risk patients andthe use of prophylactic antiviral therapy. 5Subtotal gastrectomy for gastric cancer显示文摘Although a steady decline in the incidence and mortality rates of gastric carcinoma has been observed in the last century worldwide,the absolute number of new cases/year is increasing because of the aging of the population. So far,surgical resection with curative intent has been the only treatment providing hope for cure;therefore,gastric cancer surgery has become a specialized field in digestive surgery. Gastrectomy with lymph node(LN) dissection for cancer patients remains a challenging procedure which requires skilled,well-trained surgeons who are very familiar with the fast-evolving oncological principles of gastric cancer surgery. As a matter of fact,the extent of gastric resection and LN dissection depends on the size of the disease and gastric cancer surgery has become a patient and 'diseasetailored' surgery,ranging from endoscopic resection to laparoscopic assisted gastrectomy and conventional extended multivisceral resections. LN metastases are the most important prognostic factor in patients that undergo curative resection. LN dissection remains the most challenging part of the operation due to the location of LN stations around major retroperitoneal vessels and adjacent organs,which are not routinely included in the resected specimen and need to be preserved in order to avoid dangerous intra-and postoperative complications.Hence,the surgeon is the most important non-TMN prognostic factor in gastric cancer.Subtotal gastrectomy is the treatment of choice for middle and distal-third gastric cancer as it provides similar survival rates and better functional outcome compared to total gastrectomy,especially in early-stage disease with favorable prognosis.Nonetheless,the resection range for middle-third gastric cancer cases and the extent of LN dissection at early stages remains controversial.Due to the necessity of a more extended procedure at advanced stages and the trend for more conservative treatments in early gastric cancer,the indication for conventional subtotal gastrectomy depends on multiple variables.This review aims to clarify and define the actual landmarks of this procedure and the role it plays compared to the whole range of new and old treatment methods.Roberto Santoro Giuseppe M Ettorre Eugenio Santoro 2014World Journal of Gastroenterology2014,20,38:9

7Esophageal tissue engineering:A new approach for esophageal replacement显示文摘A number of congenital and acquired disorders require esophageal tissue replacement.Various surgical techniques,such as gastric and colonic interposition,are standards of treatment,but frequently complicated by stenosis and other problems.Regenerative medicine approaches facilitate the use of biological constructs to replace or regenerate normal tissue function.We review the literature of esophageal tissue engineering,discuss its implications,compare the methodologies that have been employed and suggest possible directions for the future.Medline,Embase,the Cochrane Library,National Research Register and ClinicalTrials.gov databases were searched with the following search terms:stem cell and esophagus,esophageal replacement,esophageal tissue engineering,esophageal substitution.Reference lists of papers identified were also examined and experts in this field contacted for further information.All full-text articles in English of all potentially relevant abstracts were reviewed.Tissue engineering has involved acellular scaffolds that were either transplanted with the aim of being repopulated by host cells or seeded prior to transplantation.When acellular scaffolds were used to replace patch and short tubular defects they allowed epithelial and partial muscular migration whereas when employed for long tubular defects the results were poor leading to an increased rate of stenosis and mortality.Stenting has been shown as an effective means to reduce stenotic changes and promote cell migration,whilst omental wrapping to induce vascularization of the construct has an uncertain benefit.Decellularized matrices have been recently suggested as the optimal choice for scaffolds,but smart polymers that will incorporate signalling to promote cell-scaffold interaction may provide a more reproducible and available solution.Results in animal models that have used seeded scaffolds strongly suggest that seeding of both muscle and epithelial cells on scaffolds prior to implantation is a prerequisite for complete esophageal replacement.Novel approaches need to be designed to allow for peristalsis and vascularization in the engineered esophagus.Although esophageal tissue engineering potentially offers a real alternative to conventional treatments for severe esophageal disease,important barriers remain that need to be addressed.Giorgia Totonelli Panagiotis Maghsoudlou Jonathan M Fishman Giuseppe Orlando Tahera Ansari Paul Sibbons Martin A Birchall Agostino Pierro Simon Eaton Paolo De Coppi 2012World Journal of Gastroenterology2012,18,47:5
8Determination of surface color of‘all yellow’mango cultivars using computer vision显示文摘Image processing techniques are increasingly applied in sorting applications of agricultural products.This work has assessed the use of image processing for inspecting surface color of two Thai mango cultivars.A computer vision system(CVS)was developed and experiments were conducted to monitor peel color change during the ripening process.Conversion of RGB to CIE-LAB values was done via image processing and prediction models were developed to estimate color parameters from CVS data.Performance evaluations showed insufficient prediction for L values(R2=0.42-0.58),but better results for A and B values(R2=0.90-0.95 and 0.80-0.82,respectively).Compared to the calculated color values hue angle and chroma,a yellowness index computed from intermediate XYZ values was found to be much more adept at accurately predicting peel color from CVS data.Correlations were strong for both cultivars(R2=0.93 for‘Nam Dokmai’and R2=0.95 for‘Maha Chanok’).Results from classification analysis indicated satisfactory results for classifying fruits according to ripeness based on yellowness.Success rates of true positives in the categories unripe,ripe and overripe ranged 72%-92%for‘Nam Dokmai’and 98%-100%for‘Maha Chanok’.Therefore,it was shown that the CVS was capable of producing accurate color values for the two mango cultivars investigated.The findings of this study can be incorporated for development of a robust system for quality prediction and establishment of a CVS for automatic grading and sorting of mangos.Marcus Nagle Kiatkamjon Intani Giuseppe Romano Busarakorn Mahayothee Vicha Sardsud Joachim Müller 2016International Journal of Agricultural and Biological Engineering2016,9,1:5
9Hepatitis C virus-related B cell subtypes in non Hodgkin's lymphoma显示文摘AIM:To evaluate if indolent B cell-non Hodgkin's lymphoma(B-NHL) and diffuse large B-cell lymphoma(DLBCL) in hepatitis C virus(HCV) positive patients could have different biological and clinical characteristics requiring different management strategies.METHODS:A group of 24 HCV related B-NHL patients(11 indolent,13 DLBCL) in whom the biological and clinical characteristics were described and confronted.Patients with DLBCL were managed with the standard of care of treatment.Patients with indolent HCV-related B-NHL were managed with antiviral treatment pegylated interferon plus ribavirin and their course observed.The outcomes of the different approaches were compared.RESULTS:Patients with DLBCL had a shorter duration of HCV infection and a higher prevalence of HCV genotype 1 compared to patients with indolent B-NHL in which HCV genotype 2 was the more frequent genotype.Five of the 9 patients with indolent HCV-relatedB-NHL treated with only antiviral therapy,achieved a complete response of their onco-haematological disease(55%).Seven of the 13 DLBCL patients treated with immunochemotheraphy obtained a complete response(54%).CONCLUSION:HCV genotypes and duration of HCV infection differed between B-NHL subtypes.Indolent lymphomas can be managed with antiviral treatment,while DLBCL is not affected by the HCV infection.Adriano M Pellicelli Massimo Marignani Valerio Zoli Mario Romano Aldo Morrone Lorenzo Nosotti Giuseppe Barbaro Antonio Picardi Umberto Vespasiani Gentilucci Daniele Remotti Cecilia D'Ambrosio Caterina Furlan Fabrizio Mecenate Ettore Mazzoni Ignazio Majolino Roberto Villani Arnaldo Andreoli Giorgio Barbarini 2011World Journal of Hepatology2011,3,11:4
10Neglected features of lifestyle: Their relevance in nonalcoholic fatty liver disease显示文摘AIM To investigated in non-alcoholic-fatty-liver-disease(NAFLD), with ultrasound(US)-detected fatty liver, and in a group of non-alcoholic and otherwise healthy subjects, relationship of neglected features of lifestyle with NAFLD and obesity. METHODS Five hundred and thirty-two NAFLD and 667 non-NAFLD healthy subjects, age 21-60 years were studied. Severity of liver steatosis was assessed by US bright liver score. The adherence to mediterranean diet score(AMDS) was assessed on the basis of a 1-wk recall computerized questionnaire which included a detailed physical activity reports(Baecke questionnaire). The western dietary profile score, as a simplified paradigm of unhealthy diet, a questionnaire quantifying sun exposure score and a sleep habits questionnaires provided a further comprehensive lifestyle assessment.RESULTS Body mass index(BMI), insulin resistance(HOMA), and triglycerides, poorer adherence to a mediterranean diet profile, sedentary habits, minor sun exposure and use of 'western diet' foods are greater in NAFLD. Multiplelinear regression analysis, weighted by years of age, displays BMI, HOMA and AMDS as the most powerful independent predictors of fatty liver severity; however, also the physical activity score, the western diet habit and the sun exposure score are acting inside the model with significant independent effects.CONCLUSION Articulated clinical intervention, according to our results, are justified in NAFLD and can be pursued addressing by focused intervention nutritional profile, physical exercise mainly in open-air subsets for enhancing sun exposure and healthier sleep duration and rhythm.Francesca M Trovato Giuseppe Fabio Martines Daniela Brischetto Guglielmo Trovato Daniela Catalano 2016World Journal of Hepatology2016,8,33:4
11Monitoring physiological responses to water stress in two maize varieties by infrared thermography显示文摘Water stress is one of the main causes of yield reductions in crops,especially in arid and semi-arid regions where the water supply is limited.Plant water status is frequently assessed by pre-dawn leaf water potential(ΨPD)or leaf stomata conductance(gL)measurements,in support of advanced irrigation scheduling.However,both methods are time and labour consuming.A non-invasive approach to water status detection is the use of infrared thermography(IRT).This experiment was conducted in a greenhouse on two potted maize varieties under irrigated and non-irrigated conditions,and the measurements began when the crop had reached its twelve leaf stage.In order to establish the IRT measurements for detecting the water status of maize,an IRT-based crop water stress index(CWSI)was calculated and compared with simultaneously measuredΨPD and gL data.Good correlations were found between CWSI and gL data(r2=0.71&0.81),as well between CWSI andΨPD data(r2=0.53&0.81).These results highlight the appropriateness of infrared thermal imagery to detect and differentiate between the crop water statuses of different genotypes.Shamaila Zia Klaus Sophrer Du Wenyong Wolfram Spreer Giuseppe Romano He Xiongkui Joachim Müller 2011International Journal of Agricultural and Biological Engineering2011,4,3:3
12Chronic rejection after liver transplantation:Opening the Pandora’s box显示文摘Chronic rejection(CR)of liver allografts causes damage to intrahepatic vessels and bile ducts and may lead to graft failure after liver transplantation.Although its prevalence has declined steadily with the introduction of potent immunosuppressive therapy,CR still represents an important cause of graft injury,which might be irreversible,leading to graft loss requiring re-transplantation.To date,we still do not fully appreciate the mechanisms underlying this process.In addition to T cell-mediated CR,which was initially the only recognized type of CR,recently a new form of liver allograft CR,antibody-mediated CR,has been identified.This has indeed opened an era of thriving research and renewed interest in the field.Liver biopsy is needed for a definitive diagnosis of CR,but current research is aiming to identify new non-invasive tools for predicting patients at risk for CR after liver transplantation.Moreover,the minimization or withdrawal of immunosuppressive therapy might influence the establishment of subclinical CR-related injury,which should not be disregarded.Therapies for CR may only be effective in the“early”phases,and a tailored management of the immunosuppression regimen is essential for preventing irreversible liver damage.Herein,we provide an overview of the current knowledge and research on CR,focusing on early detection,identification of non-invasive biomarkers,immunosuppressive management,re-transplantation and future perspectives of CR.Roberta Angelico Bruno Sensi Tommaso M Manzia Giuseppe Tisone Giuseppe Grassi Alessandro Signorello Martina Milana Ilaria Lenci Leonardo Baiocchi 2021World Journal of Gastroenterology2021,27,45:2

16Influence of colour changes and moisture content during banana drying on laser backscattering显示文摘Pre-drying treatments are frequently employed to preserve fruit quality.The objective of this research was to monitor colour changes of banana during drying by laser backscattering and to determine the influence of the fruit discolouration on the light distribution into banana tissue.Moreover,to examine the influence of drying on the laser backscatter,the relationship between moisture content and relative laser area of banana slices was analyzed with different degrees of colour degradation.The experiments were conducted at drying air temperature of 63℃with various pre-treatments like chilling,soaking in ascorbic/citric acid and dipping in distilled water.An untreated sample was used as a control.A laser diode emitting at 670 nm with 3 mW power was used as light source.The backscattering relative laser area was used as an indicator for the light absorption into the tissue.The high result achieved on coefficient of determination R^(2)(>0.93)confirmed linear relationship between relative laser area and moisture content.Treatment with ascorbic acid gave the best prediction of the moisture content with the standard error of 5.7 and 8.8 for the estimated intercept and slope.The results showed a significant difference of lightness(L*values)during drying according to the different treatments.As a result,colour degradation did not have a significant influence on the absorption of light at 670 nm wavelength.Giuseppe Romano Dimitrios Argyropoulos Klaus Gottschalk Emanuele Cerruto Joachim Müller 2010International Journal of Agricultural and Biological Engineering2010,3,2:2
